Drug Management and Measures of Pharmacy Department under the New Medical Reform

  PRB is a health service provided to people with chronic diseases. The implementation of PRB has been runningsince 2014, but until now it is still not optimal, one of which is the procurement and availability of medicines. The aim of the study was to study drug management for PRB patients. The research was conducted in Surabaya 2018. This is descriptive research with cross-sectional design. Data collection by in-depth interviews with pharmacy department managers in two FKTP units and pharmacies in Surabaya. Data were analyzed descriptively. The results of the study show that FKTP doesn’t buy medicine with e-purchasing, because the drug is given by the pharmacy according to the BPJS mapping list. The pharmacy has many obstacles to ordering drugs with e-purchase, so the order is done conventionally. The pharmacy orders drugs in several ways using the Order Letter, calling PBF and ordering via the WhatsApp (WA) application. The Guidelines for Procurement of Medicines with E-Purchasing Procedures Based on E-Catalogs already exist, but socialization must continue to be carried out, especially at the level of Puskesmas and pharmacies. Periodic evaluations must be carried out so that problems and defi ciencies that occur in the fi eld can be immediately resolved.Cooperation and good intentions are needed between various parties so that all involved can benefi t from this program, especially PRB patients. Abstrak Program Rujuk Balik (PRB) adalah pelayanan kesehatan yang diberikan kepada penderita penyakit kronis. PRBsudah berjalan sejak tahun 2014, namun masih belum optimal, salah satunya perihal pengelolaan obat. Tujuan penelitian adalah mengkaji pengelolaan obat untuk pasien PRB. Penelitian dilakukan di Surabaya tahun 2018. Jenis penelitian adalah deskriptif dengan desain potong lintang. Pengumpulan data dilakukan dengan wawancara mendalam kepada pengelola bagian farmasi di 2 unit Puskesmas dan 2 Apotek di Surabaya. Data dianalisis secara deskriptif. Hasil penelitian menunjukkan bahwa Puskesmas tidak melakukan pengadaan obat secara e-purchasing, karena obat diberikan oleh apotek sesuai daftar mapping BPJS. Apotek mempunyai banyak kendala dalam melakukan pemesanan obat dengan e-purchase, sehingga pemesanan dilakukan secara konvensional. Apotek melakukan pemesanan obat dengan beberapa cara yaitu menggunakan Surat Pemesanan (SP), menelpon PBF (Perusahaan Besar Farmasi) dan melalui aplikasi WhatsApp (WA).Petunjuk pelaksanaan pengadaan obat dengan prosedur E-Purchasing, berdasarkan E-Catalogue, sudah ada namun sosialisasi harus terus dilakukan terutama di tingkat Puskesmas dan apotek. Evaluasi berkala harus dilakukan agar permasalahan dan kekurangan yang terjadi di lapangan dapat segera diselesaikan. Perlunya kerja sama dan komitmen antar berbagai pihak sehingga semua yang terlibat dapat merasakan manfaat akan program ini, terutama pasien PRB.

Planning and procurement which are parts of management drug supply give the biggest effect on drug inventory and hospital’s cost. This study aimed to describe planning and procurement systems in Pharmacy Department of Muntilan Regional Public Hospital in year 2015–2016. Study was conducted by comparing the real drug management supply indicators with standard indicators. This study was conducted on December 2016 untill February 2017. Data was collected using retrospective approach for secondary data including data of finance, drug procurement and use; as well as primary data collected by interview with hospital director, and heads of pharmacy and finance department. Data was analyzed using descriptive technique. The result showed that drug management had not been fully in accordance with the standards. It was indicated from 7 measured indicators, one of them had been in accordance with the standard which was percentage of available fund compared with cost planned. The other 6 indicators had not been in accordance with the standard: percentage of drug procurement with fund allocation, percentage of drug item planned compared with the using, percentage total quantities of a drug item with the using, the procurement frequency of each drug item, frequency uncompleted of orderlist/contract, frequency of delayed rate in payment by hospital, and indicators that can’t be measured are proportion of the quantities of products actually recieved out of total quantities planned, and percentage of quantities used out of total quantities available for consumption after deduction of buffer stock.

The article is devoted to the issues of informatization of health care in the context of medical reform in Ukraine. It was emphasized that the service component of health care reform should open free access to information for patients, in connection with which the eHealth system (electronic health care system) is being introduced in Ukraine. eHealth provides the exchange of medical information and the implementation of the program of medical guarantees of the population. It is substantiated that in order to effectively implement the state information policy in the medical field it is necessary to develop and adopt a legal act at the level of law, for example, the Law of Ukraine “On the functioning of the electronic health care system in Ukraine”. The task of such an act should be to determine the subjects of information policy in this area, the powers of individual bodies and non-governmental organizations or economic entities involved in the development and operation of electronic health care system. An important element of regulation should be to address the issue of cybersecurity when using eHealth and to determine the responsibility of specific entities for possible violations or threats to the system. It is determined that eHealth should cover all areas of medical services, including “military” and “departmental medicine”. It is emphasized that an important element of the reliable functioning of eHealth should be the training of medical staff to work with databases. At the same time, the provision on the possibility of providing “cloud” services related to the functioning of the electronic health care system should be taken into account during the creation of the draft law “On cloud services”. The possibility of using “cloud” storage of medical data and requirements for the use of “cloud” information services provided from territories of a jurisdiction other than the national one requires is assessment.

Drugs are an important component of health services that are the needs of the community. There is still a lack ofcommunity knowledge of medicines and management especially for family members, hence an educational effort as well asimprovement of quality of drug management in family through training program and mentoring by pharmacist. Thiscommunity service activity aims to determine the knowledge and attitude of the community in managing drugs in the familyand improve the quality of drug management by the community in the family. The activity was conducted in SidasariWetanKubangkangkung Village Kawunganten Cilacap. The workshop on drug management in family was conducted by pharmaciststo the 33 participants. Data collection was done by using questionnaire and observation through home visit. The result of theactivity shows that the increase of knowledge and attitude of the society in managing drugs in the familywere 10% and 7%,respectively. Workshop activities and mentoring by pharmacists can provide benefits and behavioral changes in family drugsmanagement.
