Objectives. Many patients receive benzodiazepines or Z-drugs during hospitalization due to sleeping problems. In a pilot study, we aimed to find out whether, and to what degree, a multi-faceted intervention can reduce the use of these drugs, especially in older patients and those without a psychiatric or neurological disorder. The results of this pilot study should inform the design of a randomised controlled trial (RCT). Methods. In a quasi-experimental design, we implemented the intervention in a German hospital with the support of the hospital director, medical and nursing staff and employee representatives. We compared prescription data for sleep-inducing drugs before and after the intervention by Fisher's exact test and used Odds-Ratios (ORs) with their 95 % confidence intervals (CI) as measure of effect size. Results. The data from 960 patients aged 65 and older before intervention and 1049 patients after in-tervention were analysed. Before intervention, 483 (50.3%) of the patients received sleep-inducing drugs at some time during their hospital stay. After the intervention, 381 (36.3%) patients received a sleep-inducing drug, resulting in an OR of 0.56 (95% CI: 0.47 to 0.68; p <0.001). The reduction was particularly pronounced in patients without a psychiatric or neurological disorder (from 45.0% to 28.8%). Especially benzodiazepines were significantly reduced (from 24.3% to 8.5%; OR: 0.31 (0.23-0.4); <0.001). Conclusions. A multi-faceted intervention to change the practice of the use of sleep-inducing drugs in one hospital was successful in terms of drug reduction, particularly for benzodiazepines. The interven-tion was effective especially for target persons, i.e. those without a psychiatric or neurological disease. Being aware of the magnitude of the change and the role of important stakeholders could help researchers, hospital physicians and hospital pharmacists to design a large RCT, including control hospitals, to evaluate the success of a multi-faceted intervention on a scientifically sound basis.

ObjectiveNurses are vital in providing and improving quality of care. To enhance the quality improvement (QI) competencies of nurses, hospitals in the Netherlands run developmental programmes generally led by internal policy advisors (IPAs). In this study, we identify the roles IPAs play during these programmes to enhance the development of nurses’ QI competencies and studied how these roles influenced nurses and management.DesignAn exploratory ethnographical study comprising observations, informal conversations, semistructured interviews, focus groups and a strategy evaluation meeting.SettingA teaching hospital in an urban region in the Netherlands.ParticipantsIPAs (n=7) in collaboration with four teams of nurses (n=131), team managers (n=4), senior managers (n=4) and the hospital director (n=1).ResultsWe identified five distinct advisory roles that IPAs perform in the hospital programme: gatekeeper, connector, converter, reflector and implementer. In describing these roles, we provide insights into how IPAs help nurses to develop QI competencies. The IPA’s professional background was a driving force for nurses’ QI role development. However, QI development was threatened if IPAs lost sight of different stakeholders’ interests and consequently lost their credibility. QI role development among nurses was also threatened if the IPA took on all responsibility instead of delegating it timely to managers and nurses.ConclusionsWe have shown how IPAs’ professional background and advisory knowledge connect organisational, managerial and professional aims and interests to enhance professionalisation of nurses.

Abstract Background and Aims The lethality and cost of chronic kidney disease (CKD) management are high in Senegal. The aim of this study was to evaluate the access in dialysis at Senegalese public’s hospitals. Method This was a retrospective cohort during 4 years (2014-2018) from the hemodialysis registry waiting list at Aristide Le Dantec University Hospital. This registry is composed by: a registration form (social survey and doctor's visa); a written letter addressed to hospital director and the national identity card. From this registry, telephone calls were made to collect data relating to mortality and access to dialysis. Results seven hundred fifty-one (751) files were collected. The mean age of the patients was 48.12 ± 15.28 years with a sex ratio of 1.02. The socioeconomic level was low in 85.40% (521/610) and average in 13.61% (83/610). The geographic origin was rural in 11.15%, semi-urban in 07.54% and urban in 81.31%. Ten patients (1.64%) had medical care coverage. On call, 49.70% (373/751) were died before accessing to public dialysis, 29.70% (223/751) had accessed public dialysis and 04.00% (30/751) didn’t yet need dialysis. Hundred twenty-one (16.10%) were unreachable and 0.50% (4/751) was unknown. Conclusion The lethality of CKD was high. Access to dialysis in public hospital remains problematic in Senegal despite its democratization. Additional efforts are needed for effective management of all patients at dialysis stage.

Sir Winston Churchill once said, “Never let a good crisis go to waste.” This was when, after the ravages of World War II, he sought to form the United Nations. This pandemic has affected our daily lives in unimaginable ways, much like the World War did, almost 80 years ago. The onslaught of daily challenges required us to change our strategies, modify our course, and even abandon some plans. It did not, however, extinguish our quench for knowledge and the desire to share it with others. I am truly proud of this COVID-19 special issue of Acta Medica Philippina, which stands as a testament to the unsinkable research spirit of the Philippine General Hospital (PGH) and the University of the Philippines as a whole. Ironically, it is in this unusual period that PGH has achieved its highest publication rate. The vital lessons that the articles in this issue impart to its readers, culled from the seemingly desperate situation we are all in, would surely be instrumental for us to emerge out of this crisis better and wiser.     Gerardo D. Legaspi, MD Director Philippine General Hospital

The proverbial "walking while chewing gum" statement has been once again proven by our colleagues in PGH. Being able to publish quality research articles amidst the challenges brought about by the turn of events, is testament to not only the grit that we have developed in its wards but also to the commitment in pursuing new knowledge. Prior to the pandemic, PGH has shown a continuous trend of increasing annual publication. From 8% in 2015, it has steadily  risen to 42%. The investment in focused attention guided by a clear  agenda, extensive funding and protected time for research obviously is paying off handsomely. It is my fervent hope that we will be able to bring our research output to the next level which is health policy formulation based on the knowledge that we have generated.   Gerardo D. The determination of the No Smoking Area is an effort to protect the community against the risk of health problems due to the environment contaminated with cigarette smoke. The purpose of this study was to determine the implementation of the No Smoking Area policy at the Undata Regional General Hospital (RSUD), Palu, especially in the aspects of communication, resources, disposition. , and bureaucratic structures. This type of research is qualitative research with a case study approach. Determination of informants is done through purposive sampling technique, namely the researcher uses his judgment in selecting informants to obtain in-depth information according to the research objectives. the concept of communication is good enough. The KTR Task Force at Undata Hospital uses print and electronic media. In the resource variable, the results are not good enough. The KTR Task Force at Undata Hospital is still lacking in terms of the number, namely only 15 people previously numbered 30 people. The disposition variables obtained in this study are good enough. The appointment of the KTR task force at Undata Hospital is directly selected by the hospital director, but the supervisory officers still lack incentives. In the bureaucratic structure variable, the results were quite good. The most authorized person in implementing the implementation of the KTR policy implementation at the Undata Hospital Palu is the Task Force coordinator. The suggestion in this research is that the KTR Task Force at Undata Hospital should be added and the Undata Hospital should create a smoking area.
