The Netherlands is a state located in Western Europe bordering Germany and Belgium. The population of the country is just over 17million people. In terms of GDP, theNetherlands is among the twenty richest countries in the world, and in terms of exports, it is in the top ten. The average life expectancy in theNetherlands is 81.4 years; in the structure ofmortality, malignant neoplasms come out on top, which distinguishes the state from other European countries, where the main cause of deaths is cardiovascular diseases. The compulsory health insurance system was introduced in the country in 2006 after the medical reform. A distinctive feature of the Dutch healthcare system is its relative autonomy from the state, which performs only the function of an external controller, and all other powers belong to the municipal authorities. As a result, several private insurance companies have been admitted to health insurance in the Netherlands, which create healthy competition among themselves, thereby contributing to better quality and more affordable healthcare.

Abstract The medical service system is an important guarantee for human rights to survival, health and development of every social member, and thus it is significant to explore, interpret and explain the diachronic construction for its legislative reform. In a corpus approach, the study firstly collects the medical-related statutes from 1990 to 2021 in China to build the P.R.C. Medical Legislation Corpus (PRCMLC), and analyzes the keywords and their collocation in the exploratory, explosive and expanding phase of the medical legislative reform. Secondly, from the perspectives of sociosemiotics, the PRCMLC data is combined with the concrete medical laws and regulations for further discussion of the MSS, MIS, DSS and PHS in legislative system. Thirdly, the study explores the core legislative ideas and the relationships among the subsystems in the diachronic analysis, which provide a general overview of the legislative objects, target, participants and mechanisms in the medical reform of China.

Introduction. Dismantling of the old economic model of health care as a purely budget organization, which took place under the influence of medical reform in Ukraine, led to a new nature of economic relations, the need for work and development of hospitals as participants in a competitive environment, regional or local health services market, exacerbated the impact of opportunities and threats. Given the uncertainty and dynamism of change, this highlights the need for strategic planning primarily at the level of medical institutions that provide secondary and tertiary (specialized) care to determine priorities and focus resources on achieving their development goals, which requires attention to the scientific and methodological framework ensuring this task.Purpose. The purpose of the article is to substantiate the need for strategic planning for the development of health care facilities in the context of medical reform and to develop recommendations for its implementation in their management.Research methods. In the course of the research the following methods were used: induction and deduction, system analysis - to determine the components of strategic planning at the level of health care, PEST analysis - to identify factors influencing the development of health care and factors that indicate the need to implement strategic planning in the hospital management system, institutional and comparative analysis - to identify features of strategic planning models, SWOT-analysis, expert assessments - to assess the internal and external environment of the organization, setting goals and strategic priorities.Results. Based on the analysis of the theoretical basis, the author’s approach to the interpretation of the essential characteristics of strategic planning of health care facilities is formed. The current state of the internal and external environment of health care facilities that provide secondary care in the context of health care reform in Ukraine is assessed, problems are identified. The planning system in medical institutions of this type is analyzed. The factors of actualization of the problem of implementation of strategic planning at the present stage are revealed and the necessity of its implementation in management of health care institutions is proved. Based on empirical evaluation conducted using the methods of expert collective commission, SPEIS-analysis, SWOT-analysis, the strengths and weaknesses of modern hospitals, opportunities and threats, the degree of probability and importance of the impact of opportunities and threats on the development of medical institutions, critical issues, the mission is specified, the main strategic goals and priorities are defined. An algorithm for the implementation of strategic planning in hospital management, a system of strategies and their content characteristics is proposed.Perspectives. Further research is needed on organizational design related to the implementation of strategies of medical institutions, organization of strategies, overcoming resistance to changes related to the implementation of strategies, training staff to work in new conditions, the use of other management actions aimed at building a perfect mechanism strategic planning.

Internet of things technology began to spread to all industries of our lives; the application of medical internet of things in many hospitals highlighted its advantages and brought a lot of convenience to patients and medical staff. With the continuous progress of China’s medical reform and the continuous improvement of patients’ requirements for medical service quality, this paper discusses the application of medical internet of things in clinical nursing in ward, and the basic information collection, infusion, and mobile nursing were discussed and studied. Through the parallel control study of the laboratory itself, this paper evaluates whether the two different clinical measurement methods of medical internet of things technology and traditional technology are consistent in body temperature, pulse, respiration, and blood oxygen saturation. At the same time, it also deeply studies the value and advantages of internet of things technology in the application of other monitoring indicators in clinical nursing and analyses the problems in its application. The experimental data show that the two measurement methods with different principles can be completely replaced in clinical application, and the time efficiency of the new clinical nursing method under the medical internet of things technology in mapping body temperature, pulse, and respiration has been improved by 76.20% and 72.02%, respectively, surpassing the traditional information technology and realizing the intelligent, automatic, and standardized data acquisition method. It ensures the authenticity of data and the real-time of information flow and meets the needs of resource sharing and medical regional interconnection.

Objective We aim to estimate the total factor productivity and analyze factors related to the Chinese government’s health care expenditure in each of its provinces after its implementation of new health care reform in the period after 2009. Materials and methods We use the Malmquist DEA model to measure efficiency and apply the Tobit regression to explore factors that influence the efficiency of government health care expenditure. Data are taken from the China statistics yearbook (2004–2020). Results We find that the average TFP of China’s 31 provincial health care expenditure was lower than 1 in the period 2009–2019. We note that the average TFP was much higher after new health care reform was implemented, and note this in the eastern, central and western regions. But per capita GDP, population density and new health care reform implementation are found to have a statistically significant impact on the technical efficiency of the provincial government’s health care expenditure (P<0.05); meanwhile, region, education, urbanization and per capita provincial government health care expenditure are not found to have a statistically significant impact. Conclusion Although the implementation of the new medical reform has improved the efficiency of the government’s health expenditure, it is remains low in 31 provinces in China. In addition, the government should consider per capita GDP, population density and other factors when coordinating the allocation of health care input. Significance This study systematically analyzes the efficiency and influencing factors of the Chinese government’s health expenditure after it introduced new health care reforms. The results show that China’s new medical reform will help to improve the government’s health expenditure. The Chinese government can continue to adhere to the new medical reform policy, and should pay attention to demographic and economic factors when implementing the policy.

Under the context of rapid economic and social development, and growing demands for a better life, Chinese residents have been increasingly concerned with their health status and issues. In this study, the internal relations between the purchase of commercial insurance by residents and their health status are analyzed and studied with a polytomous logit model based on the data of Chinese General Social Survey (CGSS) in 2015. According to the research result, purchase of commercial insurance significantly improved the health status of residents, with an improving effect for rural residents apparently better than that among urban residents. In addition, purchase of commercial insurance can promote the health status of residents by increasing their household income. This research will provide an effective reference for the innovative development and medical reform of the commercial insurance of China in the future, which is theoretically and practically significant to the implementation of the Healthy China Strategy.
