Drug rash with eosinophilia and systemic symptoms (DRESS) syndrome, is a type of hypersensitivity reaction, mainly due to anti-epileptic drugs. Lamotrigine is a newer broad spectrum antiepileptic that can be used in both children and adults. Lamotrigine induced DRESS syndrome is a rare case with incidence of 1/1000 to 1/10000. This syndrome manifests as rash, fever, tender lymphadenopathies, hepatitis, and eosinophilia. This case highlights the need for early diagnosis and treatment of such a rare and life threatening event because early detection of the condition leads to early recovery of the patient. Further re-exposure to the drug should be avoided.

ABSTRACT Eugenol is a phenolic compound commonly used in dentistry as an analgesic and anti-inflammatory drug. Zinc oxide eugenol is an important root canal filling material in primary teeth with excellent documented success. Its cytotoxic effect is documented in some cases due to production of zinc eugenolate which is highly unstable and induces type IV hypersensitivity reactions and even generalized anaphylactic symptoms. But, the case reports documenting immediate hypersensitivity reaction to eugenol are very rare. This was a case of type I immediate hypersensitivity reaction to eugenol in a 3 years old pediatric patient. During the full mouth rehabilitation under general anesthesia, hard indurated swelling of lower lip was observed at the time of obturation of mandibular primary teeth. The procedure was stopped immediately suspecting an allergic response which was later confirmed to be eugenol allergy by patch test. Ondansetron a selective 5-HT3 receptor antagonist has been widely used as a prophylactic antiemetic for chemotherapy induced and anaesthesia related nausea and vomiting. Anaphylaxis and anaphylactoid reaction rarely occur in less than 1% of the patients and may lead to potentially life-threatening events. This study is one such rare case report of hypersensitivity reaction to intravenous ondansetron in a perioperative setting. Familiarity about anaphylactoid reactions to intravenous ondansetron among the health practitioners would help them to bring about a rational approach to decrease its incidence.

<p class="abstract">Erythema multiforme (EM) is a hypersensitivity reaction to different antigenic stimuli, commoner being infection followed by drugs. It occurs predominantly in younger age group. EM has benign course but frequent recurrences are common especially secondary to HSV infection. Erythema multiforme present as classical cutaneous target/ targetoid lesions and mucosal bullae/erosions. Though almost 70% of patients suffer from mucosal involvement, isolated mucosal affection is very rare. Fuchs syndrome/ectodermosis pluriorificialis is a rare variant characterized by severe involvement of two or three mucosal sites in the absence of skin lesions commonly triggered by Mycoplasma pneumoniae. Here we are reporting a rare case of mucosal erythema multiforme secondary to drug and highlighting the importance of distinguishing it from other similarly presenting mucosal disorders.</p>
