Background: Vatavyadhis has been mentioned as one of the Ashtomahagada due to its notorious nature. Being a Vatavyadhi, located in Marma Asthi and Sandhi and its occurrence in old age makes it Kashta sadhya.Osteoarthritis is most common degenerative,disabling disorder in majority of people in 5-6 th decade of life.There is only conservstive treatment giving short term relief in pain and surgical intervention with lots of side effects..Sushruta has mentioned Agnikarma as best treatment for vata Kaphaj Vyadhis.Charaka, has mentioned Basti made up of Tikta Dravya, Ghrita and Kshira in Asthidhatu Dushti,. Keeping all these things in view Agnikarma and Panchatikta guggulu were selected for the treatment of Janusandhigatavata. Aim and objective: To evaluate the efficacy of Agnikarma and Panchatikta Guggulu in management of Janusandhigatavata ( osteoarthritis of knee joint). Materials & Methods: Total 43 diagnosed cases of osteoarthritis were registered out of which 41 patient completed the study .In group A 4 sittings of Agnikarma was done in 21 patients with Panchadhatu Shalaka.In group B 4 sittings of Agnikarma was done in 20 patients with Panchadhatu Shalaka along with Panchatikta Guggulu orally for 1 month. Result: The result was assessed by using wilcoxan, paired t test, Mann Whitney and unpaired t test.Significant relief was observed in both groups in all parameters. Clinically and percentage wise group B had shown better results. Conclusion: Study was concluded that Agnikarma alone has a definite role in reducing the knee joint pain and tenderness but addition of Panchatikta Guggulu found convincing results in stiffness, swelling and (ROM) of knee joint.

Abstract Background This study was designed to probe into the effect of cisplatin combined with capecitabine on nasopharyngeal carcinoma (NPC). Methods A total of 136 NPC patients treated for the first time in our hospital from January 2016 to March 2017 were collected and divided into two groups: A and B. Among them, 66 in group A were treated with cisplatin intravenous drip, while 70 in group B were treated with capecitabine on the basis of group A. The efficacy, toxic and side effects, and quality of life of the two groups were observed. Results The short-term efficacy of group B was better than that of group A (p<0.05). The toxic and side effects of group B were lower than that of group A (p<0.05). The quality of life in group B was higher than that in group A (p<0.05). Conclusions Cisplatin combined with capecitabine-induced chemotherapy for local NPC can improve the quality of life and reduce the toxic and side effects.

Objectives: This present study was done to compare the tolerability and efficacy of tramadol versus aceclofenac in terms of WOMAC Osteoarthritis Index and Visual Analogue Scale of patients with osteoarthritis of knee joint. Methods: A detail history, clinical examinations and relevant investigations were performed to all cases of osteoarthritis of knee joint. Group A patients were advised to receive tramadol 75 mg twice daily for 8 weeks, and group B patients were advised to receive aceclofenac 100 mg twice daily for 8 weeks. And we had evaluated the WOMAC Osteoarthritis Index and VAS score of first day and last day visit of patients with osteoarthritis of knee joint. Results: Data was analyzed by using SPSS (version 26) software. Paired sample statistical methods were used. Mean and standard deviation was observed and mean differences were compared. P value was taken less than or equal to 0.05 (p≤0.05) for significant differences. Conclusions: This present study concluded that the WOMAC Osteoarthritis Index score and VAS score of patients who had received aceclofenac medication had statistically significant differences as well as greater mean differences than patients who received tramadol. Hence, aceclofenac is effective drugs than tramadol in terms of efficacy and tolerability of patients with osteoarthritis of knee joint. As per Ayurveda, Janusandhigata Vata is one among Vatavyadhi and can be compared with Osteoarthritis of Knee Joint in Modern Medicine with respect to the similarity of symptoms. Aims and Objectives: To evaluate and compare the effect of Grihadhumadi and Kottamchukkadi Upanaha in Janusandhigata Vata. Methods: Single blind randomized comparative clinical study allocated into 2 equal groups as A and B. Procedure: In Group A, Grihadhumadi Upanaha Sweda done for 7 days and in Group B, Kottamchukkadi Upanaha Sweda done for 7 days. Patients were examined as per the assessment criteria on 1st day before treatment, 8th day after completion of the treatment and 15th day as part of follow up. Observations and Results: Group A showed statistically highly significant effect in most of the criteria’s like Swelling, Stiffness, Tenderness, Visual Analogue Scale (VAS), Range Of Movements (ROM) and WOMAC INDEX. Group B showed statistically highly significant effect in Pain and Crepitation.

Objective:  To compare short term outcome of combined surgical and medical management versus medical management alone in caries spine.Material and Methods:  This study was conducted from August 2015 to August 2018, on 68 patients randomly divided into two groups of 34 each. All were diagnosed cases of Caries spine based on history, clinical examination, ESR and imaging appearances. Group A underwent surgical intervention along with Anti-Tuberculous Treatment (ATT) while Group B received medical treatment (ATT) alone.  Results:  There were 37 (54.41%) males and 31 (45.59%) females with mean age of 34.84 ± 10.6 years. The thoracic spine was the commonest site in 33 (48.5%) patients, followed by lumbar in 20 (20.8%), dorso-lumbar in seven (10.3%) and cervical in four (5.88%) patients. The ESR fell from 85mm/hr to 24.46mm/hr in Group A and to 41.92mm/hr in Group B (p = 0.0124). Overall improvement in Frankel grade was seen in 25 (73.5%) patients in Group A and 12 (35.3%) in group B. In group A, improvement seen from grade A in two (8%), grade B in three (12%), grade C in 12(48%), Grade D in seven (28%) patients, (p = 0.000) while eight (23.5%) patients remained same and only one (2.5%) deteriorated from baseline neurological status. In Group B, 16 (47%) patients remained same and six (17.6%) deteriorated. Conclusion:  Surgery combined with antituberculous therapy was found to be beneficial in patients suffering from caries spine and to be recommended to patients desiring rapid recovery

A clinical trial was conducted to evaluate the efficacy of different treatment modalities in Janu Sandhigatavata. The Lakshanas of Janu Sandhigatavata as per classics are Shotha, Shoola, Prasaranaa kunchanayovedana, and Atopa are like the symptoms of Osteoarthritis of knee joint i.e., pain, swelling, crepitation and joint restriction. 30 patients suffering from Janu Sandhigatavata (single or both knees), who fulfilled the inclusion and exclusion criteria were selected for the study. 30 Patients were randomly allocated equally into two groups, Group A and Group B. Group A patients received Vruddhadarvadi Upanaha sweda for 1 Hour and Group B patients received Wax therapy for 20min, for 7 days. Both the groups received Placebos (capsules filled with rice flour) for 14 days during the follow-up period. The data of both the groups were analysed using Wilcoxon Signed Rank Test and Mann- Whitney U Test. The efficacy was statistically significant in both the Groups as p value <0.001 in most of the symptoms. Upanaha Sweda and Wax therapy, both the treatments shown equal effectiveness in relieving the pain, swelling, joint restriction and difficulty in walking. While comparison between the Groups, Group A i.e. Upanaha group showed marked reduction in symptoms. The final evaluation proved that the efficacy of Group A was moderately significant as compared to Group B.

Abstract Background Studies have reported that exercise stretching therapy has a certain effect on improving knee joint pain and knee joint function.However, few studies have compared the effects of post-exercise stretching and non-stretching on strain of knee joint movement system. The purpose of this study is to observe the influence of whether people have stretched after exercise on knee joint pain and iliotibial band tension,and to compare the difference between the stretched group and the unstretched group after exercise for further discussion.Methods Collecting data analysis of 71 people who love sports in the First People's Hospital of Xiaoshan District from December 2019 to October 2020, including the non-stretching group after exercise (group A, n=40) and post-exercise stretching Group (Group B, n=31), compare the differences in knee pain and iliotibial band tension between the two groups and analyze the relationship between this difference and stretching.Results The incidence of knee pain and iliotibial band tension in group A was significantly higher than that in group B (P<0.05). The risk of knee pain in group A was 2.940 times greater than that in group B. The incidence of iliotibial band tension in group A was 4.190 times higher than that in group B. Conclusions 1. People who do not stretch after exercise are at higher risk of knee pain than those who stretch. 2. People who do not stretch are more likely to develop iliotibial band tension than those who stretch. 3. Stretching after exercise helps to relieve the stiffness of the iliotibial band, maintain the stability of the knee joint, and reduce the occurrence of knee pain.

Objective: To study the effect of short-term use of oral contraceptive pills on intra-ocular pressures of women of childbearing age.   Methods: It was a comparative observational study, conducted at Arif memorial teaching hospital and Allied hospital Faisalabad for a period of six months. Hundred female subjects were divided into two groups of 50 each. Group A, included females, who had been taking oral contraceptive pills (OCP) for more than 6 months and less than 36 months. Group B, included 50 age-matched controls, who had never used OCP. Ophthalmic and systemic history was taken. Careful Slit lamp examination was performed and intraocular pressures (IOP) were measured using Goldman Applanation tonometer. Fundus examination was done to rule out any posterior segment disease. After collection of data, we analyzed and compared the intra ocular pressures between the two groups by using ANOVA in SPSS version 21.   Results: Average duration of using OCP was 14.9 months. There was no significant difference of Cup to Disc ratios between the two groups (p= 0.109). However, significant difference was noted between the IOP of OCP group and controls. (p=0.000). Conclusion: OCP significantly increase IOP even when used for short time period.

The possibility of using only tractography as a preoperative diagnosis of anaplastic glioma is limited due to its inability to show the exact functional location of the tumor; therefore, the combination of tractography and fMRI seems to be a more promising complex diagnostic method. In neurooncology, complete resection without or with minimal neurological deficit is the goal of surgical intervention. The aim of the study was to investigate the advantages and limitations of the use of tractography and fMRI in the treatment of anaplastic glioma compared to standard CT or MRI. The study involved 48 patients who were divided into two groups based on the use of fMRI and tractography: group A (24 patients) and without it — group B (24 patients). The groups were compared in terms of age, sex, histological subtype of anaplastic glioma, degree of resection, postoperative complications, and dynamics of neurological disorders. The combination of fMRI and tractography is the best preoperative diagnosis, it is safe and allows localizing neural pathways, preserve them during surgery, and reduce postoperative neurological deficits.

The relevance of the topic under consideration lies in the need to analyze the effectiveness of short-term methods of psychotherapy to provide prompt psychotherapeutic assistance in dealing with anxiety states and reactions in employees of medical institutions, including during the period of increased stress associated with the spread of the coronavirus infection. The study was conducted on a sample of 60 medical professionals. The experimental group of 30 people was divided into two groups of 15 people (group A and group B) and a control group of 30 people. According to the results of the study, the positive effect of group methods of short-term psychotherapy on reducing the level of anxiety states and reactions in medical professionals was revealed. It was also found that the psychocorrective influence on medical professionals by means of techniques of eye movement desensitization and reprocessing was more effective than by the techniques of cognitive behavioral therapy.

Cranial cruciate ligament rupture (CCrLR) is a common condition found in the small animal routine, being correlated to traumas, obesity, genetic factors, and primary osteoarthritis (OA) in dogs. Affected animals show articular instability that, if not corrected surgically, may cause secondary OA and loss of limb function. The aim of this study was to compare short-term results of the intra-articular technique for knee stabilization after CCrLR using a surgical button associated with polyester yarn (Group A), the surgical button associated with nylon yarn (Group B), and surgical toggle associated with polyester yarn (Group C). Eighteen dogs presenting CCrLR, weight varying from 5 to 35 kg, and different sex and breed were divided into three groups of six individuals. OA radiographic grade, pre- and post-operative lameness, surgical time, and the macroscopic aspect of cartilage were assessed. The intra-articular technique was performed by passing a suture through two tunnels, drilled in the femoral condyle and tibial crest to stabilize the knee joint. Twelve animals presented a decreased lameness and normal limb function after 15 days. On the other hand, four dogs from Group B presented complications: two dogs had suture rupture after 30 days and other two showed muscular contracture with decreased range of motion, followed by loss of limb function. In Group A, one dog showed suture rupture after 15 days and other had suture infection after 30 days. In Group C, dogs recovered normal limb function without complications. Therefore, surgical toggle associated with polyester yarn was better than the other studied materials.
