ObjectiveEpidemiologists will be better prepared to serve as a practical resource within their communities and spheres of influence by taking the time to examine data sources behind and implications of news stories and studies that are being widely circulated.IntroductionIt can be difficult to distinguish between truth, half-truth, fiction, and misinformation as we watch the news, read headlines, and scroll through various social media feeds. Fortunately, epidemiologists have the tools needed to serve as a practical resource for colleagues, partners, and communities. The Scrutinizer Challenge is an opportunity for epidemiologists to tackle at least one news story or study a month that is relevant to public health. The goal is that we would do the research necessary to examine data sources and implications of news stories and studies. This process can help us deliver consistent and reliable messages to share with colleagues, partners, and communities. It also provides an opportunity for epidemiologists that practice in different settings to consolidate resources and develop working relationships that may be needed to more thoroughly examine issues.MethodsThe Scrutinizer Challenge project was launched in January 2018 and introduced to Texas Public Health Association (TPHA) Epidemiology Section members. Participants were asked to select a headline or study to scrutinize. They were provided a guidance document with 10-25 questions to help identify and determine the credibility of data sources, compare these sources to claims being made, and assess overall implications of the news story or headline. Lastly, participants were asked to submit an actionable summary or end product that could be shared with colleagues, a local partner, or the general public. Scrutinizer Challenge project submissions were shared in the Epidemiology Section Newsletter or distributed to members as an educational resource.ResultsThree Scrutinizer Challenges were submitted between January 2018 and July 2018. News stories and study topics that were scrutinized addressed maternal mortality and morbidity in the United States, social media and population-level behavior change, and supplemental vitamins and minerals for disease prevention and treatment. The actionable summaries that were submitted were aimed at healthcare providers, researchers, and the general public. Limited but positive feedback was provided for each submission. Sources were found to be mostly credible for each news story or study, however, 2 out of 3 headlines did not support the claims made in the news story or study.ConclusionsThe Scrutinizer Challenges that have been submitted so far indicate that headlines can make incomplete or inaccurate claims even when credible sources are provided. This preliminary finding supports the need for epidemiologists to serve as a practical resource in their spheres of influence and communities, so that they can help cut through the hype and share reliable messages.

BACKGROUND The coronavirus disease 2019 (COVID-19) has continued to spread in the US and globally. Closely monitoring public engagement and perception of COVID-19 and preventive measures using social media data could provide important information for understanding the progress of current interventions and planning future programs. OBJECTIVE To measure the public’s behaviors and perceptions regarding COVID-19 and its daily life effects during the recent 5 months of the pandemic. METHODS Natural language processing (NLP) algorithms were used to identify COVID-19 related and unrelated topics in over 300 million online data sources from June 15 to November 15, 2020. Posts in the sample were geotagged, and sensitivity and specificity were both calculated to validate the classification of posts. The prevalence of discussion regarding these topics was measured over this time period and compared to daily case rates in the US. RESULTS The final sample size included 9,065,733 posts, 70% of which were sourced from the US. In October and November, discussion including mentions of COVID-19 and related health behaviors did not increase as it had from June to September, despite an increase in COVID-19 daily cases in the US beginning in October. Additionally, counter to reports from March and April, discussion was more focused on daily life topics (69%), compared with COVID-19 in general (37%) and COVID-19 public health measures (20%). CONCLUSIONS There was a decline in COVID-19-related social media discussion sourced mainly from the US, even as COVID-19 cases in the US have increased to the highest rate since the beginning of the pandemic. Targeted public health messaging may be needed to ensure engagement in public health prevention measures until a vaccine is widely available to the public.

Introduction. Hookah smoking is becoming increasingly popular worldwide, especially among young adults. The growth of social media has also enabled sharing of opinions, experiences, and marketing related to hookah via user-generated content. Aim. To evaluate the portrayal and promotion of hookah on Instagram and to highlight public health challenges created by this content. Method. A qualitative content analysis was performed on a sample of 301 Instagram posts made with the hashtags #hookah or #shisha in October 2016. Results. Of the 301 coded posts, 279 were relevant to hookah. These posts were most commonly made by hookah lounges/bars (29.2%) and individual users (27.6%). Posters were most commonly from Russia (38.5%), the United States (18.6%), and Germany (10.7%). A total of 63.8% of the posts were promotional in nature; 19.7% of posts contained hashtags that expressed hookah identity or community (e.g., #hookahlife); 35.5% of posts associated nightlife activities with hookah smoking and 33.0% portrayed hookah smoking as a social activity. Only one post (0.4%) mentioned that hookah might have negative health effects. Likes were more common on posts depicting novel rather than traditional hookah designs ( p = .027). Conclusions. Images from Instagram may help in understanding health behaviors such as hookah smoking. Our results suggest that domestic and international hookah promotion is common on Instagram. Instagram posts cast hookah in an overwhelmingly positive light and may normalize hookah use through associations with nightlife, community, and hookah identity. Policy makers and researchers should explore approaches for reducing the volume of promotional posts and tailoring counter-marketing to the specific themes presented on social media.

BACKGROUND Health authorities can minimize the impact of an emergent infectious disease outbreak through effective and timely risk communication, which can build trust and adherence to subsequent behavioral messaging. Monitoring the psychological impacts of an outbreak, as well as public adherence to such messaging is also important for minimizing long term effects of an outbreak. OBJECTIVE We used social media data to identify human behaviors relevant to COVID-19 transmission and the perceived impacts of COVID-19 on individuals as a first step toward real time monitoring of public perceptions to inform public health communications. METHODS We develop a coding schema for 6 categories and 11 subcategories, which includes both a wide number of behaviors, as well codes focused on the impacts of the pandemic (e.g., economic and mental health impacts). We use this to develop training data and develop supervised learning classifiers for classes with sufficient labels. Classifiers that perform adequately are applied to our remaining corpus and temporal and geospatial trends are assessed. We compare the classified patterns to ground truth mobility data and actual COVID-19 confirmed cases to assess the signal achieved here. RESULTS We apply our labeling schema to ~7200 tweets. The worst performing classifiers have F1 scores of only 0.18-0.28 when trying to identify tweets about monitoring symptoms and testing. Classifiers about social distancing, however, are much stronger with F1 scores of 0.64-0.66. We applied the social distancing classifiers to over 228 million tweets. We show temporal patterns consistent with real-world events, and show correlations of up to -0.5 between social distancing signals on Twitter and ground-truth mobility throughout the United States. CONCLUSIONS Behaviors discussed on Twitter are exceptionally varied. Twitter can provide useful information for parameterizing models that incorporate human behavior as well as informing public health communication strategies by describing awareness of and compliance with suggested behaviors. The majority of research concerning public health crises and social media platforms has focused on analyzing the accuracy of information within social media posts. The current exploratory study explored social media users’ specific social media behaviors and experiences during the early weeks of the COVID-19 pandemic and whether these behaviors and experiences related to anxiety, depression, and stress. Data were collected March 21–31, 2020 from adults in the United States (<em>N</em> = 564) through snowball sampling on social media sites and Prime Panels. Online surveys included questions regarding social media use during the pandemic and the Depression Anxiety and Stress Scales (DASS). Forward stepwise modeling procedures were used to build three models for anxiety, stress, and depression. Participants who actively engaged with COVID-19 social media content were more likely to experience higher anxiety. Those who had emotional experiences via social media and used social media to connect during the pandemic were susceptible to higher levels of stress and depression. The current study suggests that during the pandemic specific behaviors and experiences via social media were related to anxiety, stress, and depression. Thus, limiting time spent on social media during public health crises may protect the mental health of individuals.

BACKGROUND Smartphone-based apps designed and deployed to mitigate the ongoing COVID-19 pandemic are poised to become an infrastructure for post-pandemic public health surveillance. Yet people frequently identify deep-seated privacy concerns about such apps, invoking rationalizations such as contributing to ‘the greater good’ to justify their privacy-related discomfort. We adopt a future-oriented lens and consider participant perceptions of the potential routinization of such apps as a general public health surveillance infrastructure. This work focuses on the need to temper the surveillant achievement of public health with consideration for potential colonization of public health by the exploitative mechanisms of surveillance capitalism. OBJECTIVE This study develops an understanding of people’s perceptions of the potential routinization of apps as an infrastructure for public health surveillance after the COVID-19 pandemic has ended. METHODS We conducted scenario-based interviews (n = 19) with adults in the United States in order to understand how people perceive the short- and long-term privacy concerns associated with a fictional smart-thermometer app deployed to mitigate the ‘outbreak of a contagious disease.’ The scenario indicated that the app would continue functioning ‘after the disease outbreak as dissipated.’ We analyzed participant interviews using reflexive thematic analysis (TA). RESULTS Participants contextualized their perceptions of the app in a core trade-off between public health and personal privacy. They further evidenced the widespread expectation that data collected through health-surveillant apps would be shared with unknown third parties for financial gain. This expectation suggests a perceived alignment between health surveillant technologies and the broader economics of surveillance capitalism. Because of such expectations, participants routinely rationalized the use of the fictional app, which they viewed as always already privacy-invasive, by invoking ‘the greater good.’ We uncover that ‘the greater good’ is multi-faceted and self-contradictory, evidencing participants’ worry that health surveillance apps will contribute to an expansion of exploitative forms of surveillance. CONCLUSIONS While apps may be an effective means of pandemic-mitigation and preparedness, such apps are not exclusively beneficial in their outcomes. The potential routinization of apps as an infrastructure of general public health surveillance fosters end-user exploitation. Through its alignment with surveillance capitalism, such exploitation potentially erodes patient trust in the health care systems and providers that care for them. The inroads to such exploitation are present in participants’ manifestation of digital resignation, hyperbolic scaling, expectation of an infrastructure that works ‘too well,’ and generalized privacy fatalism.

AbstractMask wearing has been advocated by public health officials as a way to reduce the spread of COVID-19. In the United States, policies on mask wearing have varied from state to state over the course of the pandemic. Even as more and more government leaders encourage or even mandate mask wearing, many citizens still resist the notion. Our research examines mask wearing policy and adherence in association with COVID-19 case rates. We used state-level data on mask wearing policy for the general public and on proportion of residents who stated they always wear masks in public. For all 50 states and the District of Columbia (DC), these data were abstracted by month for April ⍰ September 2020 to measure their impact on COVID-19 rates in the subsequent month (May ⍰ October 2020). Monthly COVID-19 case rates (number of cases per capita over two weeks) >200 per 100,000 residents were considered high. Fourteen of the 15 states with no mask wearing policy for the general public through September reported a high COVID-19 rate. Of the 8 states with at least 75% mask adherence, none reported a high COVID-19 rate. States with the lowest levels of mask adherence were most likely to have high COVID-19 rates in the subsequent month, independent of mask policy or demographic factors. Mean COVID-19 rates for states with at least 75% mask adherence in the preceding month was 109.26 per 100,000 compared to 249.99 per 100,000 for those with less adherence. Our analysis suggests high adherence to mask wearing could be a key factor in reducing the spread of COVID-19. This association between high mask adherence and reduced COVID-19 rates should influence policy makers and public health officials to focus on ways to improve mask adherence across the population in order to mitigate the spread of COVID-19.

BACKGROUND Social media has become a major resource for observing and understanding public opinions using infodemiology and infoveillance methods, especially during emergencies such as disease outbreaks. For public health agencies, understanding the driving forces of web-based discussions will help deliver more effective and efficient information to general users on social media and the web. OBJECTIVE The study aimed to identify the major contributors that drove overall Zika-related tweeting dynamics during the 2016 epidemic. In total, 3 hypothetical drivers were proposed: (1) the underlying Zika epidemic quantified as a time series of case counts; (2) sporadic but critical real-world events such as the 2016 Rio Olympics and World Health Organization’s Public Health Emergency of International Concern (PHEIC) announcement, and (3) a few influential users’ tweeting activities. METHODS All tweets and retweets (RTs) containing the keyword Zika posted in 2016 were collected via the Gnip application programming interface (API). We developed an analytical pipeline, EventPeriscope, to identify co-occurring trending events with Zika and quantify the strength of these events. We also retrieved Zika case data and identified the top influencers of the Zika discussion on Twitter. The influence of 3 potential drivers was examined via a multivariate time series analysis, signal processing, a content analysis, and text mining techniques. RESULTS Zika-related tweeting dynamics were not significantly correlated with the underlying Zika epidemic in the United States in any of the four quarters in 2016 nor in the entire year. Instead, peaks of Zika-related tweeting activity were strongly associated with a few critical real-world events, both planned, such as the Rio Olympics, and unplanned, such as the PHEIC announcement. The Rio Olympics was mentioned in &gt;15% of all Zika-related tweets and PHEIC occurred in 27% of Zika-related tweets around their respective peaks. In addition, the overall tweeting dynamics of the top 100 most actively tweeting users on the Zika topic, the top 100 users receiving most RTs, and the top 100 users mentioned were the most highly correlated to and preceded the overall tweeting dynamics, making these groups of users the potential drivers of tweeting dynamics. The top 100 users who retweeted the most were not critical in driving the overall tweeting dynamics. There were very few overlaps among these different groups of potentially influential users. CONCLUSIONS Using our proposed analytical workflow, EventPeriscope, we identified that Zika discussion dynamics on Twitter were decoupled from the actual disease epidemic in the United States but were closely related to and highly influenced by certain sporadic real-world events as well as by a few influential users. This study provided a methodology framework and insights to better understand the driving forces of web-based public discourse during health emergencies. Therefore, health agencies could deliver more effective and efficient web-based communications in emerging crises.

BACKGROUND Media coverage and scholarly research have reported that Asian people who reside in the United States have been the targets of racially motivated incidents during the COVID-19 pandemic. OBJECTIVE This study aimed to examine the types of discrimination and worries experienced by Asians and Asian Americans living in the United States during the pandemic, as well as factors that were associated with everyday discrimination experience and concerns about future discrimination that the Asian community may face. METHODS A cross-sectional online survey was conducted. A total of 235 people who identified themselves as Asian or Asian American and resided in the United States completed the questionnaire. RESULTS Our study suggested that up to a third of Asians surveyed had experienced some type of discrimination. Pooling the responses “very often,” “often,” and “sometimes,” the percentages for each experienced discrimination type ranged between 14%-34%. In total, 49%-58% of respondents expressed concerns about discrimination in the future. The most frequently experienced discrimination types, as indicated by responses “very often” and “often,” were “people act as if they think you are dangerous” (25/235, 11%) and “being treated with less courtesy or respect” (24/235, 10%). About 14% (32/235) of individuals reported very often, often, or sometimes being threatened or harassed. In addition, social media use was significantly associated with a higher likelihood of experiencing discrimination (β=.18, <i>P</i>=.01) and having concerns about future episodes of discrimination the community may face (β=.20, <i>P</i>=.005). Use of print media was also positively associated with experiencing discrimination (β=.31, <i>P</i>&lt;.001). CONCLUSIONS Our study provided important empirical evidence regarding the various types of discrimination Asians residing in the United States experienced or worried about during the COVID-19 pandemic. The relationship between media sources and the perception of racial biases in this group was also identified. We noted the role of social media in reinforcing the perception of discrimination experience and concerns about future discrimination among Asians during this outbreak. Our results indicate several practical implications for public health agencies. To reduce discrimination against Asians during the pandemic, official sources and public health professionals should be cognizant of the possible impacts of stigmatizing cues in media reports on activating racial biases. Furthermore, Asians or Asian Americans could also be informed that using social media to obtain COVID-19 information is associated with an increase in concerns about future discrimination, and thus they may consider approaching this media source with caution.
