Nutritional status and determinants of malnutrition in institutionalized elderly people, provisional data

Nutrition is an important factor for health, physical and cognitive function, vitality, quality of life and longevity. Elderly people oft en have low food intake, induced by multifactorial causes, as a result, malnutrition is an independent risk factor for increased morbidity and mortality, regardless of the underlying pathologies (chronic organ failure, cognitive impairment, cancer, cardiovascular disease, type 2 diabetes etc.) including overweight or obese subjects.The prevalence of malnutrition varies from 4% to 10% in the elderly population from the community, from 15% to 38% for institutionalized elderly people and from 30% to 70% for hospitalized ones. The purpose of this study was to assess the nutritional status of institutionalized elderly people and establish correlations between the determinants of malnutrition and fragility syndrome.Th e epidemiological study is part of the project 20.80009.8007.25 “Fragility: diagnosis and prophylaxis in relation to medicopsychosocial problems of the vulnerable elderly”, included 50 patients institutionalized in the Republican Asylum for the Disabled and Retired in Chisinau.All persons were examined by the complex geriatric evaluation, approved by the order of the Ministry of Health no. 619 of 07.09.2010. According to the results, both malnutrition and the risk of malnutrition were high in institutionalized elderly people.Among the determinants of malnutrition, the highest association had the number of concomitant diseases, decreased functionality of the elderly, the presence of depression syndrome and cognitive disorders. At the same time, the malnutrition syndrome correlated with the fragility criteria but also with the degree of severity of frailty of the institutionalized elderly people.

Abstract. Type 2 Diabetes Mellitus is mostly elderly. In the aging process, glucose tolerance decreases. The most common problem occurs in patients with DM is to maintain ideal body weight. Some elderly people have problems with excessive weight and some malnutrition. The condition of malnutrition in elderly with type 2 diabetes has an effect on physical and psychological function, both of these factors are important in the satisfaction of quality of life. The quality of life of DM patients is a feeling of satisfaction and happiness will live in general, especially with diabetes mellitus. The aims of this study was to determine the relationship between nutritional status and functional capacity with the quality of life of elderly people with diabetes mellitus type 2. The design in this study is correlational with cross sectional approach. Sampling technique of accidental sampling. Identify elderly people with type 2 diabetes in accordance with the inclusion criteria, then Data collection is done by weighing weight and measuring the height of elderly, filling the quality of life questionnaire, and functional capacity. Data analysis using Spearman Rank. The results showed that there was a significant relationship between nutritional status and functional capacity with quality of life of elderly people with type 2 diabetes, the correlation of each with r = 0,000 and r = 0.003. Expected For health workers to provide motivation to patients to maintain a good lifestyle so as to minimize complications and quality of life of the elderly either. For families to support and facilitate healthy lifestyles in elderly people with type 2 diabetes to achieve the most nutritional status and functional status according to age and ability. Keywords: elderly people with type 2 DM, functional capacity, nutritional status, quality of life.

Objetivo: analisar as características do idoso institucionalizado apontando correlações. Método: trata-se de um estudo quantitativo, descritivo, analítico, tipo exploratório, com idosos que vivem em instituições de longa permanência para idosos. Empregaram-se questionários: sociodemográfico; Mini Mental; índice de Barthel; avaliação autorreferida de fragilidade em idosos; Family Apgar e Perfil de Saúde de Nothingham. Resultados: revela-se que as seis instituições avaliadas eram de caráter filantrópico; foram estudados 135 idosos; a média de idade foi de 76,4 anos e o tempo médio de institucionalização, de quatro anos; de acordo com os questionários, a maioria apresentou pontuação que sugere declínio cognitivo e fragilidade, era independente para as atividades básicas de vida diária, possuía acentuada disfunção familiar e apresentou perceptível perda da sensação da qualidade de vida; a maior parte dos entrevistados mudou para a instituição sem vontade própria. Constatou-se que a preservação da cognição e o maior grau de instrução se correlacionam a uma menor satisfação e percepção de qualidade de vida. Conclusão: apresentaram-se, por este estudo, alguns aspectos próprios do idoso institucionalizado e sua percepção de saúde. Se comprobó que la preservación de la cognición y el mayor nivel de educación se correlacionan con una menor satisfacción y percepción de la calidad de vida. Background: The population aging in Brazil is characterized by the accumulation of progressive disabilities in their functional activities and daily life. To consider the elderlies in his/her multiple dimensions turns out to be a critical issue to improve their assistance to an institutionalized level, improving their health and quality of life. To perform a mini-overall evaluation of institutionalized elderly person to trace their profile in relation to the socio-demographic, functional capacity, nutritional status and cognition. Methods: descriptive, cross-sectional study of quantitative approach conducted in facilities for the aged people in Natal, RN. Results: 63.3% of the elderly were female, with ages varying from 61 to 103 years. 60.0% were single; 56.7% were literate. 66.7% had no children and 55% lived with their families before the institutionalization. As for the institutionalization time, 63.3% resided in the institution for four years. As what concerns the aspects of health, 73.3 of the elderly presented hearing difficulties, 90% make use of medicines predominantly to diabetes and hypertension. There is clear evidence that the institutionalization has been harmful to the elderly as with regards basic activities of daily living, nutritional status and cognitive aspects. Conclusion: the progressive disability in functional activities of daily life interferes directly in the quality of life, increasing dependency and minimizing the autonomy of these individuals. It is necessary to effective implementation of public policies directed to the institutionalized elderly from the perspective of effective actions for improved attention and assistance. Loneliness has always been associated and reported as a risk factor of malnutrition in the elderly. People over 80 who live alone have deserved this study to detect their situation and potential for action to improve their quality of life. Objectives: To determinate the nutritional status of people over 80 living alone in the area of “La Laguna”, Cádiz. Material and method: In PIAMLA`80 program analyse those parameters and their possible correlations in a group of 342 elderly living alone over 80 years old. Different parameters were measured: Integral Geriatric Evaluation, Barthel, Lawton-Brody, Lobo, Gijon, specific analytical blood chemistry and MNA. Results: In a population of 984 people, were selected 342 initially, but finally the group decreases to 247 people. The mean Barthel was 80.42 points, 5.76 Lawton and Gijon from 11.3 Lobo 26.48. The MNA for the whole population was 24.25/30 detecting only a risk age group in women of 85-95. Correlation between nutrition and the biochemical test values showed positive for haemoglobin (0.19), total protein (0.26), Fe (0.32) and albumin (0.46). Conclusions: In our research we have not detected malnutrition in any age group or gender. The use of nutrition test MNA and its MINI version must be generalized as an accurate, clear, quick and easy tool to use.

Physiological changes in elderly individuals (EI) can contribute to nutritional deterioration and comorbidities that reduce their quality of life. Factors such as diet can modulate some of these effects. The aim was to evaluate the functionality of foods added with Brosimum alicastrum Sw. seed flour in EI. EI (n = 23) living in nursing home conditions agreed to participate. A control stage was carried out (30 days) and subsequently, an intervention stage (30 days) was realized in which a muffin and a beverage, designed for EI, were added to the participants’ their usual diet. In both stages, anthropometric parameters, body composition, nutritional status, dietary intake, sarcopenic status, cognitive and affective states, biometric parameters, and total phenolic compounds (TPC), and antioxidant capacity in foods and plasma of EI were determined. The results showed that the consumption of the foods improved the energy intake and preserved the muscle reserves of the EI. The EI gained body weight (+1.1 kg), increased their protein (+18.6 g/day; 1.5 g/kg BW/day), dietary fiber (+13.4 g/day), iron (+4.4 mg/day), zinc (+1.8 mg/day), folic acid (+83.4 µg/day) consumption while reducing their cholesterol (−66 mg/day) and sodium (−319.5 mg/day) consumption. LDL-C lipoproteins reduced (14.8%) and urea (33.1%) and BUN (33.3%) increased. The TPC increased (7.8%) in the plasma, particularly in women (10.7%). The foods improve the EI nutritional status, and this has a cardiovascular protective effect that can benefit the health of the EI.

This article deals with the issue of self-preservation of elderly people when learning to work on a computer from the point of view of information and psychological security. The necessity of solving new socio-psychological problems is indicated: to develop the ability to give a correct assessment of a specific information threat, to prevent the negative impact of the information environment, to resist negative factors, and to contribute to improving the quality of life. The purpose of counseling elderly people is to develop their ability to understand the problem of the negative impact of the information environment on psychological and physiological health, to be able to apply methods to ensure information and psychological security. Consulting and educational work with the elderly and employees of social institutions engaged in professional activities directly related to the above category should be aimed at preventing cases of fraudulent activities. Consulting work with them should not only meet the general principles of building psychological contact, but also cause an understanding of the importance of compliance with security in the digital sphere. Elderly people develop the ability to assess correctly a specific information threat and prevent the negative impact of the information environment by providing adequate resistance to negative factors and contributing to improving their quality of life. It is increasingly difficult for older people to navigate threats, unmotivated anxiety increases, depressiveness appears and becomes fixed, and asocial personality traits are formed. At the present time, it is necessary to consider the formation of information and computer literacy as a mechanism to counteract the negative impact of the information environment and a necessary condition for developing an effective strategy for the social adaptation of elderly people.

Background – Elderly people face many psychological, physical and socio economic morbidities due to ageing. Institutional settings have been opened for those elderly people who are neither able to take care of themselves nor is there any person to look after them. Objectives- The present research work was conducted to study the quality of life, loneliness and psychological distress of the elderly males and females living in institutions and non-institutional settings in urban Jammu district. Methods –The study was conducted using purposive sampling on 40 elderly living in institutional settings and 40 elderly living in non- institutional settings in the age range of 60-80 years. Tools used – GHQ  by Gautam, Nuhawan and Kamal ,UCLA loneliness scale(Russell, 1996)  and WHO (QOL-BREF, 1998),  was used. Results – The results revealed significant differences on loneliness and quality of life between those living in institutional settings and those living with their families.

Lower urinary tract symptoms, including urgency, urgency incontinence, pollakiuria and nocturia are common in the elderly people and has significant impact on quality of life. Pharmacological treatment is recommended when conservative management, such as fluid intake normalization or bladder training, does not alleviate symptoms. Antimuscarinics were often used as the I line treatment for OAB, as these drugs block the activity of the muscarinic M2/M3 receptors in the bladder and provide a significant clinical benefit for OAB symptoms in the elderly people, but their side effects are common, often leading to treatment discontinuation. Mirabegron, a β3-adrenoceptor agonist, shows similar efficacy to antimuscarinic drugs without the risk of anticholinergic effects.

Certain medicines are considered potentially inappropriate (PIM) for elderly people as they increase the risk of adverse drug events (ADE) and because safer alternative therapies are available on the market. In this context, in order to identify the instruments that assess the quality of medical prescriptions for elderly and to determine which drugs are considered PIM, a bibliographic survey was conducted in PUBMED, LILACS and PAHO databases, in February and March/2010. The search strategy included the use of health descriptors and a manual search in the references cited by selected papers. During the period of data collection, 15 instruments were identified. In 2012, with the publication of the update of Beers criteria, this instrument was included in the study. We identified 163 PIM of 25 therapeutic classes, of which 125 (76.7%) are marketed in Brazil. Of these, 31 (24.8%) are essential medicines (RENAME 2012), of which 13 have safer therapeutic equivalents and 19 (15.2%) are over-the-counter drugs. Data suggest the need for inclusion of safer alternatives for the elderly in the national list of essential medicines and the pharmaceutical care for early detection of ADE in this age group, in order to contribute to the safe use of medicines.
