Idiopathic scoliosis, as many authors think, being idiopathic in terms of not fully clear etiology of the disease, due to the most complicated long-term studies, has a certain theory of the pathogenesis of scoliosis, which includes the disharmony of the longitudinal growth of the spine and spinal cord and the formation of vertebral torsion. The application of various versions of modern vertebral instrumentation for 20 years made it possible to develop a differentiated approach to the choice of surgical intervention, taking into account the patient’s age and the peculiarities of the deformed spine. Nevertheless, the use of current diagnostic methods indicated that not all the details of the deformed vertebral structure have been studied, in particular the apical ones, that are usually more changed being at the apex of the deformity. The application of multislice computed tomography (MSCT) and current statistical methods made it possible to reveal the structural features of the apical vertebrae in the group of adolescent scoliosis (14–18 years old), that is important for increasing the effectiveness of preoperative planning and improving the results of idiopathic scoliosis treatment being one of the most difficult problems of the spine surgery.

Internal snapping hip syndrome, or psoas tendonitis, is a recognised cause of nonarthritic hip pain. The majority of patients are treated conservatively; however, occasionally patients require surgical intervention. The two surgical options for iliopsoas tendinopathy are step lengthening of the iliopsoas tendon or releasing the tendon at the lesser trochanter. Although unusual, refractory snapping usually occurs soon after tenotomy. We report a case of a 47-year-old active female with internal snapping and pain following an open psoas tenotomy. Postoperatively she was symptom free for 13 years. An MRI arthrogram revealed reformation of a pseudo iliopsoas tendon reinserting into the lesser trochanter. The pain and snapping resolved after repeat iliopsoas tendon release. Reformation of tendons is an uncommon sequela of tenotomies. However the lack of long-term studies makes it difficult to calculate prevalence rates. Tendon reformation should be included in the differential diagnosis of failed tenotomy procedures after a period of symptom relief.

Objective. Аnalysis of the treatment results in patients, suffering ureteric calculi, using the contact ureterolithotripsy procedure. Маterials and methods. In 104 patients with ureteric calculi, in whom ureteroscopy with further laser contact ureterolithotripsy was conducted, symptoms of the disease were analyzed, as well as the diagnostic methods value, peculiarities and efficacy of surgical treatment. Results. In all the patients endoscopic extraction of calculi have been succeeded. Calculus does not cause significant macroscopic changes in ureter, if symptoms, characteristic for ureterolithiasis, persists up to one week. Local edema is formed, if a calculus persists in ureter longer than a week. Long-term (more than two months) local ureteric persistence of calculi enhances the risk of its intramucosal intraureteric «ingrowth» greatly. Conclusion. The contact ureterolithotripsy constitutes a highly effective and miniinvasive method of surgical intervention in ureterolithiasis, guaranteeing high level of postoperative “stone free rate”. Prolongation of the calculi ureteric insertion time without a process of a stone discharge causes the ureteric wall changes, complicating performance of miniinvasive interventions (the contact ureterolithotripsy and a distant shock-wave lithotripsy).

Arabinoxylans are an essential component of dietary fiber, and their health-promoting properties are determined mainly by the content and structural features their biopolymers. Rye bread is particularly rich in these compounds; their unique features in the context of content and chemical structure of rye arabinoxylans make it a valuable component od daily diet. Long-term studies have shown the positive effect of these compounds in the aspect of prevention of civilization diseases such as type 2 diabetes, obesity, and cardiovascular diseases. Among the description of the physicochemical properties and diversity of arabinoxylans, the article contains a collection of the most important reports regarding the health-promoting effects of these polymers, as well as their metabolism in the human body.

Job insecurity has been high on the policy and research agenda since the 1980s: there has always been cause for concern about job loss, though those causes may vary across context and time. Job insecurity is particularly prevalent among employees with a more precarious profile, in particular employees in blue-collar positions or on temporary contracts, and among employees in jobs of lower quality. Job insecurity has typically been advanced as a stressor and a cause for imbalance in the employment relationship, which has led to the hypothesis that job insecurity induces strain (e.g., poorer health and well-being), poorer attitudes vis-à-vis the job and the organization (e.g., poorer organizational commitment), and poorer performance. This hypothesis has found overall support. In addition, job insecurity also threatens one’s identity, and this has been related to more conservative social attitudes and behaviors, for example, in terms of voting intentions and behavior. Finally, job insecurity affects outcomes beyond the current job and the organization: it affects other stakeholders, for example, labor unions and families, and it has scarring effects in the long term. Studies have also attempted to identify moderators that could buffer the relationship between job insecurity and outcomes; these mostly concern personal, job, and organizational resources. Other studies have sought to explain differences between countries in terms of both structural features and cultural values.

Objective:to assess whether computed tomography (CT) data can  be used for metal transitional spine fixation in patients with thoracic  and thoracolumbar scoliosis at the stage of surgical intervention planning.Material and methods.Thirty-nine patients with grades 3 and 4  (according to V.D. Chaklin) or type I–III (according to the  classification proposed by L. Lenke) thoracic and thoracolumbar idiopathic scoliosis were examined. All the patients  underwent a comprehensive radiographic and CT examinations. The  radiomorphometric features of the spine were used to plan surgical interventions.Results.The protocol was clarified for preoperative scoliosis examination using CT data on cervicothoracic and  thoracolumbar vertebrae. A personalized approach to planning an  operation on the basis of CT examination permits one to eliminate  destabilization of the supporting elements of the corrective system  at the most. Reducing the risk of postoperative losses of correction  contributes to the use of multi-rod constructs.Conclusion.CT that enables a comprehensive assessment of existing pathology and clarification of the nature of impending surgery should be carried out in patients with thoracic  and thoracolumbar idiopathic scoliosis at the stage of preoperative  planning for metal transitional spine fixation.

These two cases of successful treatment of a rare vascular disorder – cystic adventitial disease (CAD). Case 1: CAD of the popliteal artery with narrowing of the popliteal artery and intermittent claudication. Case 2: CAD of iliac vein with irreducible femoral hernia formation and iliac vein subocclusion. We described preoperative planning, surgical intervention and postoperative management in our cases. Surgery after careful preoperative planning is only one effective method of treatment of CAD and has good long-term results.

SummaryBackground: Compression stockings are widely used in patients with varicose veins. Methods: Based on published literature three main points are discussed: 1. the rationale of compression therapy in primary varicose veins, 2. the prescription of compression stockings in daily practice, 3. studies required in the future. Results: The main objective of prescribing compression stockings for patients with varicose veins is to improve subjective leg complaints and to prevent swelling after sitting and standing. No convincing data are available concerning prevention of progression or of complications. In daily practice varicose veins are the most common indication to prescribe compression stockings. The compliance depends on the severity of the disorder and is rather poor in less severe stages. Long-term studies are needed to proof the cost-effectiveness of compression stockings concerning subjective symptoms and objective signs of varicose veins adjusted to their clinical severity. Conclusion: Compression stockings in primary varicose veins are able to improve leg complaints and to prevent swelling.
