PECULIARITIES OF THE MEDICAL AND BIOLOGICAL EMERGENCY MANAGEMENT PROCESS IN A REGION WITH UNSTABLE NATURAL AND CLIMATE

The solution of the problem of determining the features of the process of emergency management of medical and biological nature in the region with unstable natural and climatic conditions in the conditions of constantly changing information load on the crisis management bodies is considered. Within the framework of the set task the modern tendencies of change of natural and climatic conditions in Ukraine and their influence on dynamics of distribution of medical and biological danger are analyzed. To consider the natural and climatic features of medical and biological emergency management on the example of the Cherkasy region as a region with unstable natural and climatic conditions. The obtained results allow, using innovative organizational and technical methods of prevention of emergencies of a similar nature, to further form a modern method of counteracting emergencies of medical and biological nature, supplementing these methods with an effective system of crisis management. It is determined that on the territory of Ukraine there is a sharp and heterogeneous change of weather and climatic conditions, which leads to potential negative consequences, as a man-made ecological nature, namely: thermal overload of a number of territories; increase of flooding areas; reduction of areas and violation of the species composition of green areas; increase of natural hydrometeorological phenomena; reduction of quantity and deterioration of drinking water quality; disruption of the normal functioning of energy systems and medical and biological nature, namely the increase in the number of infectious diseases and allergic manifestations, their modification and uncharacteristic growth and change of distribution areas; secondly, the nature of changes in weather and climatic characteristics in the Cherkasy region of Ukraine and its corresponding territorial location in relation to global climatic world processes defines the latter as a region with unstable weather and climatic conditions that territorially inhomogeneously affect the spread of medical and biological emergencies. In the framework of this task, the current state of the issue on the formation of the mathematical apparatus of methods of counteracting emergencies of medical and biological nature is analyzed, taking into account the specifics of the spread of the COVID-19 epidemic. The main parameters of modeling the process of choosing a management strategy for the prevention of medical and biological emergency within the project-system management, which allowed to determine the criteria for project-system management of medical and biological emergency in a constantly changing information load on crisis management. The scheme of organizational structure of the module of management of system of resource-critical management of process of the prevention of an emergency situation of medico-biological character within the limits of interaction of links of the center of crisis management is developed; The obtained results allow, using innovative organizational and technical methods of prevention of emergencies of this nature, to further form a modern method of counteracting emergencies of medical and biological nature, supplementing the above methods with an effective system of crisis management. The proposed author's approach to system-project management of the process of formation and processing of information messages in emergencies of medical and biological nature causes, first, the creation of a fundamentally new scheme of branching and relationships within individual crisis management centers at all levels of subordination and prevention system emergencies in general; secondly, the introduction into the functional field of the system of emergency prevention of medical and biological nature of the module of resource-crisis management of information message, as an element of system-project management, requires a radical revision of approaches to training specialists (managers) in the relevant field. skills and knowledge in the organization and management of complex cyclical information and communication projects Key words: emergency situation, mathematical model, medical and biological danger, anti-crisis management.

Emergency situation was declared in Estonia in the middle of March; the country was placed under partial lockdown. Libraries had to close. One of the first steps taken by the National Library of Estonia in these exceptional circumstances was to establish a crisis management committee to monitor the overall situation and decide how the library should proceed. The library was able to react quickly and continue with basic services, providing them online or contact-free. New services were introduced, making home lending and reference enquiry more efficient and convenient. The library reopened on 18 May. Looking back, we are happy to have not just survived but managed the crisis as an opportunity. This was reflected in increased numbers of new registered readers and usage. The emergency situation clearly indicated one of the most important requirements for libraries – the need to be ready to react fast and adapt to new situations.

After the Fukushima accident, initiatives emerged offering the public the possibility to realise measurements of the radioactivity in the environment with various devices and to share data and experiences through collaborative tools. The objective of the OpenRadiation project is to offer the public the opportunity to perform measurements of the radioactivity using connected dosimeters on smartphones. The challenge is to operate such a system on a sustainable basis in normal situations and in order to be useful in an emergency situation. In normal situations, this project is based on a collaborative approach including pedagogical activities. In case of emergency situation, data from the field will be available in “real time” providing an opportunity for the emergency management and the communication with the public. The practical objectives are to develop i) a website centralising measurements using various dosimeters, providing dose rate maps with raw and filtered data and offering dedicated areas for specific projects and exchanges about data and ii) a dosimetric app using a connected dosimeter. This project is conducted within a partnership between organisms’ representative of the scientific community and associations to create links with the public.

The article analyzes the Bulgarian administrative legal framework on emergencies (state of emergency, crisis management and overcoming, emergency situation and emergency epidemic situation). It indicates the temporary restrictions of the right of free movement of the citizens provided in the legislation. The article discusses the competence of the central executive authorities, interdepartmental bodies and territorial authorities (regional governors and mayors) to deal with a pandemic. The article discusses also the legal basis and justifications for the introduction of the curfew by some of the regional governors and mayors during the state of emergency, caused by SARS-CoV-2 (COVID-19).

The number of opioid overdoses (ODs) has risen in recent years and has become more complex due to the co-involvement of both prescription and illicit opioid drug use. Provisional programs for take-home naloxone (a medication designed to rapidly reverse opiate OD symptoms) kits have been distributed to combat this potentially fatal epidemic. Although there is strong evidence to support the efficacy of naloxone in the reversal of opiate OD, there is limited evidence to support the efficacy of take-home naloxone (THN) kits. The purpose of this evidence-to-practice review was to summarize a systematic review on the efficacy of THN programs. The authors aimed to include studies of THN programs that both trained opioid users in OD prevention and reported on OD outcomes. The Bradford Hill criteria (strength of association, temporality, consistency, specificity, dose-response relationship, biological plausibility, coherence, experimental evidence, and analogy) and five additional criteria (measure cost-effectiveness, absence of negative consequences, feasibility of implementation/expansion/ coverage, unanticipated benefits, and special populations) was used as dependent variables to determine the impact of public health intervention where randomized control trials (RCTs) are not ethically feasible or operationally practical. All 22 studies included provided empirical support using the Bradford Hill Criteria for community based THN programs. Despite being unable to deduce whether death would have occurred without the administration of THN, the studies combined accounted for an estimated 2316 successful opioid OD reversals. Thus, there is a strong association between THN administration and overdose survival. Additionally, there was a low rate of adverse events: withdrawal symptoms (2.8%), vomiting (2.2%), agitation (2.1%), seizures (0.1%). Consequently, we recommend that athletic trainers include opioid crisis management equipment and procedures in a site-specific policies manual. Clinical relevance is highly dependent on patient population and geographic location, considering 90% of reversed ODs were heroin induced. Application to individuals in organized sport is minimal, but nonetheless, individuals who are prescribed opioids for pain management should be candidates for THN programs. Athletic trainers and guardians of minors prescribed opioid medications should be educated on dispensing medication, best practices for opioid crisis management, and distribution of naloxone/THN.

Social media has redefined crisis management in the recent years. Extraction of situation awareness information from social media sites such as Twitter, Facebook, Instagram, etc. is a non-trivial task once the required framework is established. Unfortunately, most public safety authorities are still suspicious of using social media in engaging and disseminating information. This chapter reports on how social media can be effectively used in the field of emergency management along with the opportunities and challenges put forth. The chapter starts with a discussion on the functions of social media and its trustworthiness. It provides a description of the framework for disaster management system and the methodology to be adopted. The methodology consists of volunteer classification, methods of data collection, challenges faced, event detection, and data characterization with currently available disaster management tools. The chapter concludes with the division between practice and research and moves toward envisioning how social media may be used as a resource in emergency management.

At the present level, the production, transportation and primary processing of oil products is associated with the problem of environmental pollution. The ingress of oil products into the natural environment leads to negative consequences and requires the enterprise to incur additional costs to eliminate the emergency situation and to take environmental protection measures to restore oil-contaminated territories. To restore contaminated territories on the basis of field surveys, which are based on geodetic work, remediation projects are developed. Remediation is carried out on the basis of a project, the reliability of which is taken into account during field geodetic work on determining the site area. In the course of the calculations, it was found that the use of tacheometric survey can significantly increase the cost of field work. In most cases, to ensure the development of a remediation project, it is enough to use domestic GPS receivers. The use of this class of devices will reduce both the timing of field work and their cost. It should be borne in mind that the accuracy of domestic GPS receivers allows using them in determining sites with an area of more than 0.5 ha.
