Objective: To determine mean platelet volume in patients of psoriasis Study Design: Cross sectional study Place and duration of study: Dermatology Department, Pak Emirates Military Hospital Rawalpindi from August 2017 to January 2018. Methodology: Approval from the hospital ethical review committee was taken and a total of 100 patients were enrolled by non-probability consecutive sampling. After an informed written consent, name, gender, age, hospital record number, and phone number of each individual were noted. At the time of enrollment, all the individuals were subjected to complete blood count, bleeding time, fasting blood sugar, fasting lipid profile, renal and liver function tests. Patients with abnormal parameters were excluded from the study. A two ml venous blood sample was drawn from each patient and sent in CP bottle to the laboratory of Armed Forces Institute of Pathology for blood complete analysis by hematology analyzer machine. Data analysis was done using SPSS version 22. Results: In this study mean age was 40 ± 10.40 years. 54% patients were male and 46% patients were female. Mean platelet volume in patients presenting with psoriasis was 9.10fL with SD ± 2.12. Conclusion: Our study concludes that the mean platelet volume in patients presenting with psoriasis was 9.10fL with SD ± 2.12 which is in the normal range. Therefore, mean platelet volume is not affected by psoriasis. Key words: Mean platelet volume, psoriasis, severity, cross-sectional study

The hyperactivity of platelet had been seen in patients with metabolic syndrome which can be caused by several factors, such as:insulin resistance, obesity, dyslipidemia and hypertension. The hyperactivity of platelet leads to its aggregation that can be increased therisk of cardiovascular disease. This study is aimed to know the platelet aggregation and mean platelet volume in patients with metabolicsyndrome and obesity by determination 30 patients were choosen for this cross sectional study, those whom attended to the laboratoryand policlinic at H. Adam Malik Medan Hospital, between May 2013 until August 2013. The diagnosis used of metabolic syndromecriteria established by the International Diabetic Federation 2005. From the 30 patients with 15 metabolic syndrome and 15 obesity,four patients were excluded because of their tryglyceride were more than 200 mg/dL. There is no significant differences between theplatelet aggregation with the agonist adenosin difosfat (ADP) in patient with metabolic syndrome and obesity. And there is no significantdifferences of the mean platelet volume values between the metabolic syndrome (9.6±0.93) and the obesity group (9.73±0.74), with pvalue 0.846. Based on this study there is no significant differences between the platelet aggregation and the Mean Platelet Volume values(MPV) in the metabolic syndrome and the obese group

Objective: To study the morphological variations of median nerve observed in sick persons during carpal tunnel release operation. Study Design: Cross Sectional study. Setting: Qazi Hussain Ahmad Medical Complex, Nowshera. Period: One year (November, 2019 till October, 2020). Material & Methods: This Cross-sectional study was conducted after obtaining ethical approval from the Institutional Ethical Review Board (IERB), NMC in which people were tracked with mild to severe carpal tunnel symptoms for a year. Less than 24 hours after their arrival, the victims were operated on under local anesthesia in Qazi Hussain Ahmad Medical Complex, Nowshera, Pakistan. Each surgical method viewed the nerve. Branches around the carpal tunnel were seen and documented on a spreadsheet. 150 were planned. The statistics were in percentages. Results: In 150 instances, 102 (68%) were female and 48 (32%) were male. 138 (92%) individuals showed normal median nerve morphology, whereas 12 (8%) had aberrant branching. The transverse carpel ligament had significant splitting. There was a persisting median in three of the 12 instances. Conclusion: The top division of the median nerve showed altered branching morphology. As with the persistent median artery, recurring median artery raises the chance of median nerve dysfunction.

Childhood diarrheal disease is the second leading cause of death globally. All over the world, children below five years of age experience, on average, 3.2 episodes of diarrhea per year. Objectives: To explore an association of maternal education with childhood diarrhea in Pakistan. Study Design: Cross sectional study. Methodology: This study was conducted from Aug 2016 to Jan 2017 in Combined Military Hospital (CMH), Peshawar following approval from ethical review board. Total children under study were 363 aged 2 to 5 years, amid whom 121 were acute diarrhea cases and 242 were healthy controls leading to cases control ratio as 1:2. Data collection was done through semi structured questionnaire asked from mothers. Statistical analysis: Data was analyzed by SPSS software, version 22. To compare the association of maternal education with acute diarrhea, chi square test was applied at 5% level of significance. Statistical significance was taken at p value <0.05. Results: In present study, there was significant association between formal education of mothers and acute diarrhea with a highly statistically significant association (p < 0.001) observed showing increased proportion of childhood diarrhea, with increasing levels of education in mothers. Conclusion: We concluded that a significant increase in childhood diarrhea with higher levels of maternal education refutes the commonly observed finding. We need to further probe the underlying causes to minimize this high observed morbidity. Key Words: Maternal Education, Child Health and Safety and Acute Childhood Diarrhea

Introduction: Teledermatology provides virtual consultation to patients using telecommunication technology. Using this method dermatologists can diagnose a condition with the help of pictures of the lesions and short history. During the COVID-19 pandemic, practicing this method has become more relevant. Providing teleconsultations to patients with fungal skin infections can prevent inadvertent use of topical corticosteroids. The objective of this study was to find the prevalence of fungal infections among teledermatology consultations done in a tertiary care hospital. Methods: It was a descriptive cross-sectional study where the store and forward and real-time methods were used between February 2020 to July 2020. Ethical clearance was taken from the institutional review board (reference number: 245). Data of those patients who wanted consultations from this department were sent by medical officers deployed in military hospitals that are under the central army hospital located in Kathmandu. Convenient sampling was used. The collected data was entered and analyzed in the Statistical Package of Social Sciences version 20. Point estimate at 95% Confidence Interval was calculated along with frequency and percentage for binary data. Results: A total of 451 (33.45%) (30.93-35.97 at 95% Confidence Interval) were diagnosed with fungal infections out of 1348 patients who were enrolled for the study. About 361 (80%) of the patients suffering from fungal infections belonged to the Terai region and 90 (20%) belonged to mountainous areas. Conclusions: The prevalence of fungal infection among teledermatology consultation was lower than the findings from a similar international study.

Objective. To study whether mean platelet volume (MPV) and splenomegaly could be used as subclinical inflammatory markers in children with familial Mediterranean fever (FMF) at the attack-free period.Patients and Methods. The study included ninety-seven children with FMF. MPV was carried out within 4 hours of blood sampling according to standard laboratory practice. Splenomegaly was determined by abdominal ultrasound (USG).Results. High MPV was detected in 84.45% of our studied patients and was significantly higher in FMF patients with splenomegaly than in patients without splenomegaly. There was a statistically significant correlation between MPV and splenic span (P=0.045).Conclusion. Elevated MPV and its significant correlation with splenic span in FMF children during the attack-free periods support the use of MPV and splenomegaly as useful markers of the subclinical inflammation in FMF patients at the attack-free period.

Background: Mean platelet volume, as well as platelet count, are an index of haemostasis and its dysfunction i.e. thrombosis. Aim of this study is to investigate the association between mean platelet volume (MPV) & acute ischemic stroke. Methods: This retrospective cross-sectional study included 200 patients admitted to the emergency & general opd (medicine & pediatric) of a tertiary care hospital in JLN Medical College Ajmer from Aug jan 2020 to Nov 2020. The data were evaluated according to MPV levels, National Institutes of Health Stroke Scale (NIHSS) scores. The patients were divided into groups based on MPV level ≤7.5, 7.6-10.0,10.1-12.5,≥12.6. Results: In our study, a signicant association was found between severity of stroke (according to NIHSS criteria) and MPV in male and female populations (p<0.0001 and p<0.0001 respectively). A signicant difference was found between MPV values of all 3 categories ie patients of severe stroke had higher value of MPV than those of mild and moderate stroke and patients of moderate stroke had higher values than those of mild stroke. Conclusion: This is a simple and cost effective test which can be easily done at any primary care centre and can serve as a useful marker to differentiate ischemic stroke from hemorrhagic stroke.

Objective: To assess the impact of age on outcome of COVID-19 patients at a tertiary care hospital. Study Design: Cross sectional study. Place and Duration of Study: Combined Military Hospital Malir, Karachi, from Apr to Jun 2020. Methodology: This was a retrospective cross sectional study conducted at Combined Military Hospital Malir Karachi. About 120 positive cases of SARS COV-2 were studied, including males and females. Age range was 21-85 years. Patients were divided into groups according to age, group A (21-40 years), group B (41-55 years), group C (56-85 years). Presenting complaints and associated co morbidities were also analyzed. Mild cases were managed conservatively. Moderate to severe oxygen dependent cases were managed in intensive care units. Results: The mean age of patients was 40.24 years with 16.25 ± SD. Co- morbidities were noted in 67 (55.83%) patients, highest in group C, hypertension being the most common. Ten patients could not survive, due respiratory failure complicated by adult respiratory distress syndrome (ARDS). Bilateral infiltrates >50% observed in (14%) out of (n=120) patients. One hundred and ten patients were discharged with uneventful recovery. Conclusion: Mortality was highest among elderly patients with comorbidities. Therefore, international standing operative measures (SOPs) to be followed. Meticulous monitoring of vital signs and oxygen saturation in mild cases is the bare minimum requirement and timely appropriate treatment escalation of moderate to severe cases in intensive care units is the key to success.

Abstract Objective: To study the frequency and diversity of fungi involved in onychomycosis. Method: The cross-sectional study was conducted from February 2018 to February 2019 at the Department of Microbiology, Basic Medical Sciences Institute, Jinnah Postgraduate Medical Centre, Karachi, and the University of Karachi. Specimens were taken from patients suffering with onychomycosis attending the Dermatology Department of Jinnah Postgraduate Medical Centre and were processed for the isolation of fungi. Specimens were processed for the diagnosis by potassium hydroxide mount for microscopic evaluation and mycological culture to determine the type of fungus involved in onychomycosis. Fungi were isolated using Sabouraud Dextrose Agar with antibiotics cycloheximide and chloramphenicol, and without antibiotics. Plates were observed for the growth periodically until 4th week. Appearance of any colony was studied further for identification on the basis of cultural characteristics and microscopy. Precise identification of Candida species was done. Results: Of the 230 samples, 85(36.96%) were from males and 145(63.04%) from females. Potassium hydroxide mount showed fungal element in 134(58.26%) specimens, while in 111(48.26%) patients the fungal culture was positive. Nine (3.9%) cases were culture-positive but potassium hydroxide-negative, while among the 134(58.26%) potassium hydroxide-positive cases, 32(23.9%) were culture-negative.
