Influence of the Basal Metabolic Profile on the Evolution of the Pediatric Patient with Obesity

Childhood obesity is a problem of growing importance globally. It is associated with significant health problems. Knowing how to treat it effectively would improve the quality of life of these children. The aim of this chapter is to study how basal metabolism influences the somatometric evolution of the child and adolescent population with obesity in a pediatric endocrinology clinic. Study childhood obesity in a tertiary hospital by means of a multichannel impedanceometry study. All the patients had a basal metabolism lower than the calculated theoretical ideal. In overall terms, weight reduction is not achieved in this pediatric population. However, it is observed a decrease in fat content in the medium term (1-3 years). Bioelectrical impedanceometry measurement is a simple method in clinical practice to evaluate the energy consumption and the body composition. Knowing the body composition of these children would help to intervene more effectively to help control obesity and its health consequences.

Purpose: To study how basal metabolism influences the somatometric evolution of the child and adolescent population with obesity in a pediatric endocrinology clinic. Material and Methods: Study of the entire population diagnosed with childhood obesity in a tertiary hospital by means of a multichannel impedanceometry study, TANITA BF 430. Results: 100 patients were selected by chance and studied from a database with 1400 records. Most of the patients who attend to these consultations for obesity are girls, between 8 and 11 years old. All the patients studied had a basal metabolism lower than the calculated theoretical ideal. The group of boys at the beginning of their follow-up showed a higher basal metabolism and more similar than the group of girls. Girls had a higher amount of fat mass compared to boys at the beginning of their follow up. After their first year of follow-up, both groups achieved a decrease in the percentage of fat mass, double in the case of boys compared to girls. After three years of the study, it was observed that 54% of the boys achieved a reduction in their FM compared to 36% of the girls. In overall terms, weight reduction is not achieved in this pediatric population. However it is observed a decrease in fat content in the medium term (1-3 years). The reduction of this fat mass could be a protection factor against cardiovascular diseases in adults. Given the same adherence to nutritional programs, it is believed that physical exercise rates influence this reduction. It was also observed that the most relevant data in the evolution of obesity in these patients is the basal metabolism that they present at the beginning of their follow-up. Discussion: Childhood obesity is a problem of increasing importance in our society. Understanding its characteristics would allow different strategies to be taken for a better treatment and diagnosis of these cases. Boys, in general, achieve a greater reduction in fat mass in the same follow-up time. Due to sexual dimorphism, or a higher rate of physical activity. Bioelectrical impedanceometry measurement is a simple, cheap and easy to use method in clinical practice to evaluate the energy consumption and the body composition of the patient. In “healthy child” health programs and even in schools, an impedanceometry machine could be purchased to focus physical activity efforts on those boys and girls with a lower basal metabolism, adding a greater consumption factor such as physical exercise. Thus, it would be possible to increase the requirements and the basal metabolism of the patient would increase, favoring the loss of fat mass. Regarding the use of conventional scales, in this type of patients, if we only look at the weight variable, a bias could be created since it would not be noticed that there has been a decrease in fat content at the cost of an increase in muscle mass. Conclusion: We consider that it should be recommendable to supply with a multichannel impedanceometry every pediatric endocrine consultation room as it is a non-invasive, and easy-to-use test that can provide a great amount of information about the evolution of these patients, since we believe that focusing efforts on those boys and girls who have a worse basal metabolism could contribute to improving the efficiency and effectiveness of the scarce health resources that we have.

The interrelations of body size, body composition and basal metabolism were studied in 69 albino rats ranging in age from 18–174 days. The decline in metabolic rate with age was more rapid than would be predicted from the weight0.75 rule which eliminates the influence of body size in interspecific measurements. Body composition analyses indicated that the increase with age in metabolically inert fat and bone minerals was approximately balanced by a corresponding decrease in metabolically inert extracellular fluid, so that ‘active tissue mass’ was virtually unchanged. Calculations based on data in the literature indicate that about one-half the decline in metabolic rate with age may be due to the corresponding decrease in the relative weight of the viscera. The remainder of the decline in metabolic rate must be due to factors other than changes in the chemical or histological composition of the body.

Összefoglaló. Bevezetés: A szociális szférában dolgozók túlterheltsége jelentős, és munkakörülményeik többnyire kedvezőtlenek. Bizonyított, hogy az életcélok meghatározzák a karrier, az egészségtudatosság és az életminőség területeit, de ez utóbbi a szociális dolgozók mintáján még nem került bizonyításra. Célkitűzés: A kutatás célja, hogy bemutassa egy szociális és egészségügyi szolgáltatóintézmény női dolgozóinak (n = 127) tápláltsági és fittségi állapotát és életminőségét, valamint az életcélok meghatározó szerepét ezekre a mutatókra. Módszer: Kérdőív, teszt, testösszetétel-vizsgálat és fittségiállapot-felmérés segítségével gyűjtöttünk adatokat, amelyeket különbözőségvizsgálattal és regresszióelemzéssel elemeztünk. Eredmények: A minta tápláltsági mutatója túlsúlyra utal, míg a fittségi állapot, az életminőség és az életcélok még elfogadható tartományban vannak, de a szélsőértékek jelentősek. A magasabb életcélokkal rendelkezők jobb fittségi mutatókkal és életminőséggel jellemezhetők. Az ülőmunkát és a fizikai munkát végzők között minimális különbség volt kimutatható a tápláltsági mutatók, a fittség és az életminőség tekintetében. A táppénzt igénybe vevők gyengébb fittségi és életminőség-mutatókkal rendelkeznek, mint akik nem voltak betegszabadságon. Although the purpose in life is proved to determine the areas of career, health awareness, and the quality of life, the latter has not yet been confirmed among social workers. Objective: The purpose of this research is to demonstrate body composition and fitness status as well as the quality of life of female employees (n = 127) at a social institution, furthermore the decisive role purpose of life plays in these indicators. Method: Data were collected through questionnaires, tests, body composition analyses, and fitness status tests, and were assessed by t-test, analysis of variance and regression analysis. Results: Body composition of the participants indicates a generally overweight status, while fitness status, quality of life, and purpose of life are within the acceptable range, however, all with notable range values. Those with a higher-level purpose of life demonstrate better fitness indicators and higher quality of life. In terms of body composition, fitness, and quality of life, only a minor difference was found between those who do intellectual and physical type of work. Those who took sick leave demonstrated a lower level of fitness and quality of life compared to those who did not take any sick leave. Body composition and fitness status have proven to deteriorate with age. Conclusion: Previous research findings can be confirmed by stating that neither the body composition and fitness indicators, nor the quality-of-life level of social workers are optimal. The determining role of purpose in life has been proven in substantial aspects in this study. One of the focus areas of enhancing purpose in life is the workplace, where major positive impacts can be achieved in terms of the physical and mental health, well-being, and work performance. Childhood obesity is a global problem, with its prevalence in the developed world increasing fourfold in recent decades. Obesity in children is defined by the Body Mass Index and is responsible for many physical and psychological problems, as well as for reduced perceived quality of life. In recent years, it has become increasingly worrying that diseases affecting adult populations, such as diseases of the cardiorespiratory system, also affect children, as well as the health of adults who have been obese children. The involvement of physiotherapists in the treatment of obesity in children and adolescents can lead to a more understandable assessment and improved care and treatment of obesity.

Nutritional deficiencies (malnutrition, cachexia, sarcopenia, and unfavorable changes in the body composition) developing as a side effect of radiotherapy (RT) currently represents a significant but still inaccurately studied clinical problem in cancer patients. The incidence of malnutrition observed in head and neck cancer (HNC) patients in oncological radiology departments can reach 80%. The presence of malnutrition, sarcopenia, and cachexia is associated with an unfavorable prognosis of the disease, higher mortality, and deterioration of the quality of life. Therefore, it is necessary to identify patients with a high risk of both metabolic syndromes. However, the number of studies investigating potential predictive markers for the mentioned purposes is still significantly limited. This literature review summarizes the incidence of nutritional deficiencies in HNC patients prior to therapy and after the commencement of RT, and presents recent perspectives for the prediction of unfavorable nutritional changes developing as a result of applied RT.

Diabetes is recognized to lead to a series of metabolic abnormalities in the body. These alterations will affect balance, body composition, and cardiovascular performance, heightening the risk of various medical complications. The problem is that the primary instruction for those with diabetes is to monitor glucose levels, paying very insufficient consideration to other fundamental factors and health profiles that could influence the quality of life in these individuals. Identify body composition, balance, and cardiovascular components in controlled type two diabetics compared to an age-matched control group. 15 participants with controlled type II diabetes without peripheral neuropathy (CT2DM) and 18 non-diabetic subjects control (CG) were recruited. Both groups had an age average of roughly 56 years old. The CT2DM subjects had an average A1c level of 6.7+/-0.5%. Body composition, cardiovascular, and balance data were collected, analyzed, and compared among groups. Minimal alterations in balance components, body composition and cardiovascular factors were identified in the CT2DM group aside from higher SBP values and decreased BBS scores contrasted to the CG group. We can attribute the analogous outcomes in both groups to CT2DM participants managing their diabetes effectively. Our examination has prompted us to establish that the cardiovascular and balance components in middle-aged Latinx-Hispanic participants with CT2DM are proportionate to CG subjects because of effectiveness in dealing with diabetes. Further, we encourage establishing more age appropriate and complex assessment tools to identify early adaptations caused by diabetes.

BACKGROUND: Kleinfelter syndrome (KS) is a common genetic disease characterized by hypergonadotropic hypogonadism. The high risk of developing of metabolic disorders in patients with KS is be explained by the presence of androgen deficiency, which leads to a decrease in the amount of lean soft mass and an increase of the adipose tissue content. The basal metabolic rate (BMR) is determined by the amount of lean soft mass, and its reduction can contribute to weight gain and the progression of metabolic disorders in KS. AIM: Body composition, assessment of basal metabolism and metabolic profile in adolescents with KS. MATERIALS AND METHODS: The study included 28 adolescents with KS, comparable in age and stage of sexual maturation. All patients were divided into two groups depending on the presence of clinical laboratory signs of hypogonadism. Patients passed through the evaluation of metabolic profile, bioelectrical impedance analysis for the body composition and chamber-based indirect calorimetry for the evaluation of BMR. RESULTS: Normal indicants of body composition were determined in 20 (71.4%) adolescents, excess of adipose mass in 6 (21.4%) patients, and deficit of adipose mass in 7.2% of cases. Among 6 patients with excess of adipose mass, three were obese or overweight, while the rest had normal SDS body mass index (BMI). 64.3% of adolescents showed normal indicants of energy exchange at rest, 6 (21.4%) a decrease of BMR, 4 (14.3%) - an increase of BMR. A comparative analysis of patient groups did not reveal statistically significant differences in BMR adjusted to a lean soft mass (OO/TM) (p=0.36). Insulin resistance was detected in 11 patients (39.3%), and the incidence of dyslipidemia was 3.6%. The groups did not significantly differ in terms of glycemia levels in the fasted state and lipid profile indicants. CONCLUSION: Most adolescents with KS have normal indicants of body composition and basal metabolism, as well as a low frequency of metabolic disorders, regardless of the level of total testosterone in blood. In some patients with normal SDS BMI, excess of adipose mass is detected. The lack of correlation between the level of testosterone and the intensity of BMR may indicate a slight effect of androgen deficiency on energy exchange at rest in adolescents with KS.

Background. Although twin gestation is well recognized to be associated with impaired fetal and postnatal growth, specific data about body composition of twins in the first month of life are scarce.Objective. The aim of this study was to compare the body composition of twins, evaluated with air-displacement plethysmography, to that of singletons of similar gestational age and adequacy of growth, during the first month of life. We tested the hypothesis that the quality of growth would be similar.Methods. Anthropometric and air-displacement plethysmography measurements were performed in 18 pairs of twins and in 36 singleton neonates, longitudinally, from birth to the 30th day of life. Each twin was matched to a singleton infant of similar gestational age and birth weightz-score.Results. With regard to anthropometric measures, the only difference was a lower weight in twins versus singletons on the 15th day of life. With regard to body composition, we did not find any difference between groups at any time point. Fat mass increased significantly from day 1 to day 30 in both twins and singletons.Conclusion. In terms of body composition, twins do not differ from singletons of similar gestational age and weight, either at birth or in early postnatal life.

Childhood obesity can lead to adulthood obesity with adverse effects. Since body composition and physical fitness differ depending on the obesity degree, a systemic analysis could help classify that degree. We used three study designs based on the obesity degree (body mass index [BMI] as a reference) for our objectives. First, we identified the relationship between body composition and physical fitness. Second, we determined the effects of exercise on body composition and physical fitness. Third, we performed a path analysis of the impact of exercise on body composition and physical fitness, and verified those effects among the groups. In study 1, 164 10-year-old subjects were divided into four groups: 33 in the normal weight (NO), 34 in overweight (OV), 54 in obesity (OB), and 43 in the severe obesity (SOB) group. In study 2, 101 participants from study 1 who wished to participate in the exercise program were divided into four groups (same criteria). The exercise program (three times a week for 60 min, for 16 weeks) consisted of sports and reinforcement exercises of increasing intensity. Body composition was measured by body weight, percentage of body fat (%BF), muscle mass, skeletal muscle mass (SMM), and body mass index (BMI). In contrast, physical fitness was measured by muscular strength, flexibility, muscular endurance, agility, and balance. As a result, all body composition variables were higher in the SOB group than in the other groups. Physical fitness, muscular strength and balance, and agility were highest in the SOB, NO, and OV groups, respectively. Pearson’s correlation revealed that muscular strength was associated with height and body weight across all groups. Agility showed a negative correlation with %BF in the NO, OB, and SOB groups. SMM was positively correlated in the OB and SOB groups. After the exercise intervention, BMI and the %BF of the SOB group were significantly reduced (p < 0.01, and p < 0.001, respectively), while SMM presented a significant increase (p < 0.001). Height also showed a significant increase in all groups (p < 0.001). Among physical fitness variables, muscular strength, flexibility, muscular endurance, and balance showed a significant increase in all groups, while a significant increase in power was observed in only the OB and SOB groups. As for the effects of the body composition on physical fitness after exercise intervention, the greatest impact was observed for balance, muscular strength and agility, and muscular endurance in NO, OV, and OB groups, respectively. In conclusion, the body composition, physical fitness relationship, and the effects of exercise intervention on them differed depending on the obesity degree. Furthermore, the results varied according to the obesity degree. Thus, our study highlights the importance of creating particular exercise programs for the effective prevention and treatment of childhood obesity considering the obesity degree.
