Background: Performance improvement in hospitals requires an appropriate model for performance evaluation. Because organizational goals cannot be achieved without a comprehensive model for evaluating and reviewing programs, organizations cannot apply effective management of programs without regard to the results of their activities. Objectives: The purpose of this study is to introduce a new model for improving the performance of hospital information systems using Six Sigma method in Kermanshah province hospitals. Methods: This is a qualitative research based on content analysis and Delphi method. The data are collected through semi-structured interviews and the participants are 19 experts in hospital performance systems which were selected by snowball sampling. After transcribing the interviews, three steps were done using content analysis: firstly, using line-by-line coding (or open coding) words and segments of interviews were coded, secondly, each code was conceptualized and categorized into some axial codes based on focused coding, and finally, selective coding was done in which categories were identified based on axial codes’ similarity, conceptual relationship and common characteristics. Results: The results of the coding of the interviews showed that variables such as reduction of medical errors, improvement of hospital quality of services, promotion of hospital efficiency and productivity, quick and easy access to information, increased patient satisfaction and safety, management and cost reduction, and time management and control influences the performance of hospital information systems. Conclusions: For improving hospital information systems, hospital managers and system developers should not only pay attention to variables which are controlled by the software and are least affected by the human resources of the organization; such as: cost control, quality control, information control and time control but also human based variables such as specialization of the staffs, their motivation, reduce errors and etc., which are sometimes neglected.

The objective of this study is to identify factors influencing unsafe use of hospital information systems in Malaysian government hospitals. Semi-structured interviews with 31 medical doctors in three Malaysian government hospitals implementing total hospital information systems were conducted between March and May 2015. A thematic qualitative analysis was performed on the resultant data to deduce the relevant themes. A total of five themes emerged as the factors influencing unsafe use of a hospital information system: (1) knowledge, (2) system quality, (3) task stressor, (4) organization resources, and (5) teamwork. These qualitative findings highlight that factors influencing unsafe use of a hospital information system originate from multidimensional sociotechnical aspects. Unsafe use of a hospital information system could possibly lead to the incidence of errors and thus raises safety risks to the patients. Hence, multiple interventions (e.g. technology systems and teamwork) are required in shaping high-quality hospital information system use.

Background: One of the ways through which the quality of the hospital services can be improved is the accreditation, and most of the countries have launched their own accreditation system as a strategy to improve services. The aim of this research was to investigate the way of implementing the accreditation programs at Shahid Motahhari educational center in Marvdasht city. Methods: The current study was qualitative with conventional content analysis which was conducted by using the semi-structured interviews with eight officials and employees of Marvdasht Shahid Motahhari Medical Education Center in 2017. Purposeful sampling was done in this study and the next stage; the snowball sampling method was used to select the desired samples. Data were gathered to reach the data saturation point, and conventional content analysis was done to analyze the research data. Results: Two main themes and nineteen sub-themes were obtained through the nine categories. Self-confidence in the team, sense of superiority and achievement, all units’ coordination, accurate and scientific coherent supervision and management, and considering the book of standards as a guiding factor are among the strengths of this research.Poor management of programs, lack of proper context while running the program, lack of proper training, lack of financial and human resources, lack of commitment and interest in implementing the program among different groups were main identified barriers of the program. Conclusion: The effective factors on the implementation of the accreditation standards in the healthcarecenters should be identified. Besides, planning effective education, continued monitoring and assessing, creating suitable contexts of financial and human resources,consolidating the strengths and using the available potentials can be effective factors in line with the implemetation of accreditation standards.   Keywords: Accreditation, Hospital, Qualitative study, Evaluation

Abstract Background: Nurses as the main human resource in coping with COVID-19 face many challenges and problems. Therefore, the present study aimed to explain the experiences, challenges, and adaptation strategies of nurses in taking care of patients with COVID-19 with a qualitative approach.Methods: The present study was conducted with a qualitative conventional content analysis approach among 30 nurses working in wards for COVID-19 patients, in Tehran hospitals. Access to participants and data collection was done through purposive sampling, snowball sampling and semi-structured interviews. Data were analyzed using qualitative content analysis (conventional) and MAXQDA-18 software. Guba and Lincoln criteria were observed to evaluate the quality of research results.Results: From data analysis, 2 main categories and 16 subcategories were obtained, including (1) experiences and challenges (lack of protective equipment, high work pressure, marginalized physical health, problems related to the use of protective equipment, getting excluded, lack of supportive work environment, problems related to patients, psychological problems, fear, marginalized personal and family life and the challenge of communication with patients' families) (2) Strategies for adapting to working conditions (performing religious-spiritual activities, creating an empathetic atmosphere in the workplace, spiritualizing their work , trying to convince the family and gain their support, strengthening sense of self-worth and responsibility).Conclusion: The working conditions for nurses can be improved through providing adequate protective equipment, providing a suitable work environment and more social and financial support, more attention to the physical and mental health of nurses and considering appropriate mechanisms for nurses to communicate with their families and patients' families.

Hospital Information Systems [HIS] is developed to support healthcare organizations in providing efficient, quality, and safe healthcare services. The objective of this study is to identify and describe doctors’ perspective on the impact of HIS use in the examination rooms and wards on quality of care and patient safety. Semi-structured interviews were carried out with thirty one doctors from three Malaysian government hospitals. Thematic qualitative analysis was performed by using ATLAS.ti to deduce the relevant themes. HIS were commonly believed to improve quality of care and patient safety in terms of : [1] accessibility of patients’ record, [2] efficient patient-care, [3] well-structured report viewing, [4] less missing patients’ records, [5] legibility of patients’ records, and [6] safety features. In conclusion, the use of HIS in examination rooms and wards suggests to improve the quality of care and patient safety.

Abstract Background Disasters may result in mass casualties and an imbalance between health care demands and supplies. This imbalance necessitates the prioritization of the victims based on the severity of their condition. Contributing factors and their effect on decision-making is a challenging issue in disaster triage. The present study seeks to address criteria for ethical decision-making in the prioritization of patients in disaster triage. Methods This conventional content analysis study was conducted in 2017. Subjects were selected from among Iranian experts using purposeful and snowball sampling methods. Data were collected using semi-structured interviews and were analyzed by the content analysis. Results Efficient and effective triage and priority-oriented triage were the main categories. These categories summarized a number of medical and nonmedical factors that should be considered in the prioritization of the victims in disaster triage. Conclusion A combination of measures should be considered to maximize the benefits of the prioritization of causalities in disasters. None of these measures alone would suffice to explain all aspects of ethical decision-making in disaster triage. Further investigations are needed to elaborate on these criteria in decision-making.

The objectives of this study were to investigate the degree to which hospital information systems are aligned with hospital strategies and how they impact on nurses. Semi-structured interviews were conducted with nine hospital staff from a range of disciplines in a large public hospital in Australia. The interviews revealed that the focus of hospital information systems on administration has meant that applications to support the patient?s clinical journey have been deemed unaffordable. A consequence is that the historic use of the nurses? station as a source of verbal information about patients has continued, with negative impacts on nurses? perceived role and status, and the possible exacerbation of the clash between administrative and clinical cultures. We conclude that hospitals need clear strategies, with alignment of information systems. To do otherwise can undermine culture and morale, while disrupting workflows and patients? clinical journeys.

Abstract Introduction: Cardiovascular diseases are the most common cause of death in the world. Due to the rate of emergencies and disasters in the country, This study was conducted to investigate the challenges of providing health services to cardiovascular patients in emergencies and disasters in Iran.Methods: This conventional content analysis study was conducted in 2020. Subjects were selected from among Iranian experts using purposeful and snowball sampling methods. Data were collected using semi-structured interviews and were analyzed by the content analysis.Results:. After analyzing the data, in the Pre-emergency Phase (lack of training on medication and nutrition, and lack of training vulnerable groups, lack of databases of cardiovascular patients, and lack of identification of patients before disasters), The Emergency Response Phase (lack of sleep and rest patterns, lack of health forces, lack of blood pressure control, lack of proper nutrition, increased medication needs, lack of mental health interventions) and the Post-emergency phase (lack of planning, lack of management of patients' mental problems). Conclusion: Developing strategies for planning, training, providing resources, and mental health during the 3 phases of emergencies management cycle for specific groups such as cardiovascular patients together with empowering these patients in the event of disasters is one of the key strategies which can be used after curbed emergences’ and disasters to reduce the rate of mortality.

Abstract Background: The petrochemical industry is hazardous, in part because of the inherently dangerous nature of the work conducted, and incidents frequently result in significant financial and social losses. The most common immediate cause of incidents and injuries in this industry is unsafe worker behaviour. Identifying the factors encouraging unsafe work behaviours is the first step in taking action to discourage them. The aim of this study was to (a) explore workers', supervisors' and safety managers' attitudes and perceptions of safety in a petrochemical company in Iran, and (b) identify the factors that discourage safe work behaviours. Methods: A qualitative study was conducted by applying the steps described by Graneheim and Lundman (2004). Twenty participants were recruited from an Iranian petrochemical company using a multi-stage approach, with initial purposive sampling followed by snowball sampling to enhance recruitment. Individual face-to-face and semi-structured interviews were conducted to gain an in-depth understanding of factors acting as barriers to safe behaviour. The interviews were recorded and transcribed in Persian and then translated into English. Conventional content analysis was performed. Results: The main themes emerging from the interviews were: (i) poor direct safety management and supervision; (ii) unsafe workplace conditions; (iii) workers' perceptions, skills and training; and (iv) broader organisational factors. Conclusions: The findings give insights into practical organisational measures that can be implemented by management to promote workers’ commitment to safety and engage in safe behaviours in their workplace.
