Auricular pseudocysts (APs) are benign intercartilaginous cysts at the auricle. Various therapeutic options have been used to treat APs, but they frequently recur in usual clinical practice. Here, we report a case of AP successfully treated using a silicon cast. A 65-year-old male presented with a cyst on his right auricle. Although a local injection of steroid was administered, the cyst had recurred. A dental silicon cast was applied after removal of the contents of the cyst by suction. There was no recurrence of the cyst 6 years after that treatment. Dental silicon casts are a novel technique for an appropriate fixation over auricles in the treatment of APs and had an effective therapeutic result in our case.

Collection of medicinal parts for therapeutic preparations is known as Dravyasangrahana. For collecting different openions about dravyasangrahana, literatures about dravyasangrahana in different ayurvedic texts Charakasamhitha,Susruthasamhitha,Ashtaangahridaya, Shaarngdharasamhitha and Raajanighantu were reviewed. Ayurveda details many factors to be considered during drug collection. To attain desired therapeutic result it is mandatory to collect the drug having optimum rasaveeryaadi qualities. In Ayurvedic literature, drug collection has been mentioned according to different criteria like desa, bhoomi, kaala, nature of dravya collected and person collecting drug. Basic concepts of Ayurveda like tridosha and panchamahaabhootha were given importance in deciding drugs on the basis of quality of desa and bhoomi. Season of collection, properties of drug, method of collection, veerya of drug also have given importance. There are some references in shaarngdharasamhitha regarding collection of drugs like cares to be taken when drugs in wet and dry state are collected.

Abstract Introduction When monitoring heparin, anti-Xa assays are susceptible to interference from apixaban taken before admission and can result in inappropriate dose adjustments that can negatively affect patient care. Methods We derived a novel assay, termed corrected heparin (CH), using quantified values from a chromogenic anti-Xa assay with heparin calibrators before and after heparinase treatment to eliminate any interference from apixaban within the patient sample. We retrospectively assessed 469 specimens from 72 patients at our institution who had their unfractionated heparin infusion monitored using the CH assay because of known apixaban use. These patients were included in the study if they had detectable apixaban levels (>0.1 IU/mL by anti-Xa). Results The analytical performance of the assay was evaluated, and precision was found to be 8.8% within 1 day and 13.3% over multiple days, with acceptable linearity (R2 = 0.997). Evaluation of clinical performance was compared with the partial thromboplastin time (PTT), showing a lack of correlation similar to comparisons between the PTT and anti-Xa assay (Blood Coagul Fibrinolysis 1993;4:635–8). The mean time to a therapeutic result in this cohort was 10 hours and 10 minutes. The CH assay was used to determine how long the apixaban was detected by the anti-Xa assay. The majority of patients (80%) still had measurable anti-Xa assay interference from apixaban at 24 hours after the last apixaban dose. Conclusions We have developed and evaluated an assay capable of quantifying heparin in the presence of apixaban. This assay showed acceptable performance in both analytical and clinical performance.

ABSTRACT Adult patients with anterior open bite and hyperdivergent retrognathic phenotype demand complex treatments, as premolar extractions, molar intrusion or orthognathic surgery. In the present clinical case, a young adult patient without significant growth, with Class I and anterior open bite, was treated with four premolar extractions. The therapeutic result shows good intercuspation, good facial esthetic, good function balance, and stability in a two-year post-fixed treatment follow-up.

Febuxostat used for the treatment of patients with arthritis littered with Hyperuricemia and is utilized in its Chronic Management. a number of the internal organ adverse effects have diode to the employment of febuxostat replaced with with allopurinol drug. Febuxostat, associate degree compound accelerator matter, achieves its therapeutic result by decreasing humour acid. At therapeutic concentrations it is not expected that Febuxostat will inhibit different enzymes which are involved in purine and pyrimidine synthesis and their metabolism. Metabolism of Febuxostat is done by conjugation via uridine diphosphate glucuronosyltransferase (UGT) enzymes and also by UGT1A1, UGT1A3, UGT1A9, and UGT2B7 and reaction via hemoprotein P450 (CYP) enzymes as well as CYP1A2, 2C8 and 2C9 and non-P450 enzymes. Febuxostat is eliminated primarily through each viscus and excretory organ pathways. Febuxostat could cause heart issues that may result in coronary failure, could cause issues within the blood vessels that visit your brain. this could conjointly result in stroke, urarthritis flare-ups and Liver injury. Some facet effects of febuxostat could occur that sometimes don't would like medical attention. We conclude from our review, vessel Death urarthritis patients with established vessel (CV) illness treated with febuxostat had a better rate of CV death compared to those treated with allopurinol drug in a very CV outcomes study.

Purpose: to study the efficacy and tolerability of the domestic drug cromoviz in the treatment of allergic conjunctivitis. The state of the organ of vision in 60 patients (120 eyes) with allergic conjunctivitis was studied. Depending on the therapy, the patients were divided into two groups of homogeneous clinical manifestations. At the same time, the patients of the main group (30 patients) were instilled with the drug Cromoviz (Uzbekistan), 2 drops 4 times a day for 4 weeks. Patients of the control group (30 patients) were instilled with Aycrol according to the same scheme. The obtained research results showed that the use of the domestic drug cromoviz against the background of basic treatment is expressed in a decrease in subjective complaints of patients and a significant clinical effect in 95.9% of cases. The revealed economic efficiency of the drug action indicates the achievement of the maximum level of therapeutic result at an acceptable price for the patient and therapeutic-prophylactic institution. Cases of side effects and intolerance to the domestic drug cromoviz were not identified in our studies.

Background: Nanomedicines are capable of disease diagnosis, drug delivery, and in monitoring the therapeutic result to provide appropriate tasks towards research goals. The best therapeutic pattern can be achieved by developing a theranostic nanomedicine, which is an emerging field. It has the advantage of loading phytoconstituents as drugs and is useful for both imaging and therapeutic function. Methods: Nowadays, the design of a novel drug delivery system of the herbal constituent is usually done through the nanotechnology approach. This technique increases the biological activity and counters the puzzles associated with plant medicines. Traditional medicine integration with nanocarriers as an NDDS is very essential in the management of chronic diseases such as hypertension, diabetes, and cancer. Results: The nanotechnology combination with plant science is a green revolution with a practical approach for decreasing the therapeutic side effects. The object of the study is to review herbal nanomedicine with an enhanced therapeutic profile and less toxicity. Conclusion: The development of herbal theranostic nanoformulation is very useful for the treatment of different diseases.

The article describes a case with a 22-year-old female patient, who had been in a “common-law marriage” with a 29-year-old man during 7 months. When she sought medical advice the patient informed that she remained a virgin. She associated it with tough upbringing and a hard set that one must not live a sex life before marriage. From 6 to 16 years of her life the patient and her mother were members of the international religious organization “Jehovah’s Witnesses”, where the above set exists. Besides, her mother told the patient that one could engage in sexual intercourses only after registration of marriage, and those girls who allowed themselves to do it before marriage were prostitutes. The patient reported that an attempt to make coitus caused “a spasm of muscles in my vagina” and a severe pain, and for that reason the man could not “enter” her. Before such an attempt the following thoughts appeared: “It is not allowed before marriage, because it will be regarded as adultery. God will be against it”. But a strong desire to make coitus existed. She made a point that she was afraid of a severe pain and haemorrhage that might appear in coitus (she saw twice some porn, where defloration was made hard, resulting in fixation of some fear in her). She believed that the above was still producing its effect on her. At first I thought that the patient had vaginismus of non-organic origin caused by psychogenic inhibiting effects and coitophobia. But later, having drawn an analogy with contraction of muscles during orgasm, I concluded that she did not have the above contraction while attempting to make coitus, but an obstacle to perform it was caused by a pain that developed at attempt of introjection and made her squeeze her legs together, thereby imitating the result of contraction of muscles involved in vaginismus. Therefore the diagnosis of “dyspareunia of non-organic origin, the phobic variant” was made. The following treatment was provided: different cognitive techniques, hypnosuggestive therapy (2 sessions). Recommendations for optimizing the performance of coitus were given. An immediate therapeutic result was achieved with complete resolution of the above problem.
