Background: Whole-body plethysmography (WBP) is the gold standard for measuring lung volume, but its clinical application is limited as it requires expensive equipment and is not simple to use. Studies have shown that the single-breath helium dilution (SBHD) method, which is commonly used in clinical practice, significantly underestimates lung volume in patients with obstructive lung disease (OLD). By comparing the differences in lung volume measured using SBHD and WBP, we aimed to establish a correction equation for the SBHD method to determine the total lung volume in patients with OLD of different severities.Methods: From 628 patients with OLD simultaneously subjected to SBHD and WBP, 407 patients enrolled between January 2018 and November 2019 were in the training group and 221 enrolled between December 2019 and December 2020 were in the prospective verification cohort. The multiple linear regression equation was used for data in the training group to establish a correction equation for SBHD to determine the total lung volume, and this was validated in the prospective validation cohort.Results: There was a moderate positive correlation between total lung capacity (TLC) determined using the SBHD [TLC (SBHD)] and WBP methods [TLC (WBP)] (r = 0.701; P < 0.05), and the differences between TLC (SBHD) and TLC (WBP) (ΔTLC) were related to the severity of obstruction. As the severity of obstruction increased, the TLC was underestimated by the SBHD method. We established the following correction equation: TLC (adjusted SBHD) (L) = −0.669 + 0.756*TLC(SBHD)(L) – 0.047*FEV1FVC+0.039*height (cm)–0.009*weight(kg)(r2 = 0.753 and adjusted r2 = 0.751). Next, we validated this equation in the validation cohort. With the correction equation, no statistical difference was observed between TLC (adjusted SBHD) and TLC (WBP) among the obstruction degree groups (P > 0.05).Conclusions: The SBHD method is correlated with WBP to measure the total lung volume, but the SBHD method presents limitations in determining the total lung volume in patients with obstructive lung disease. Here, we established an effective and reliable correction equation in order to accurately assess the total lung volume of patients with OLD using the SBHD method.

Helium is the second most abundant element in the Universe after hydrogen. Considerable resources of helium-3 isotope (He3) are located mostly outside the Earth. He3 is very important for science and industry, especially for airport neutron detectors, lung tomography and helium dilution refrigerators. Besides, global warming is forcing the industry and governments to search for alternative energy sources, and He3 has the potential to be used as fuel in future nuclear fusion power plants. Unfortunately, the price of gaseous He3 has recently increased from $200 per liter to over $2750. The expected further increase in price and demands led us to present an analysis of the economic profitability for He3 separation process, which utilizes the properties of superfluid helium. This paper shows the arguments supporting the idea that extraction from natural sources is the only economically viable way of obtaining He3 isotope nowadays. The method could be relatively easily implemented into the production cycles of the low temperature natural gas purification plant.

AbstractTotal lung capacity is usually measured by a body plethysmograph or helium dilution methods. In this study an alternative approach to obtain total lung capacity of spontaneous breathing subjects is introduced. The device utilises an optoelectronic plethysmograph and a small tube, which measures mouth pressure and allows total lung capacity to be obtained, which differs less than 0.4 L from the total lung capacity of the body plethysmograph. The method shows potential to be a less burdensome method to estimate total lung capacity determination than the body plethysmograph.

Multiple-breath nitrogen washout (MBNW) and its clinical parameter lung clearance index (LCI) are gaining increasing attention for the assessment of small airway function. Measurement of LCI relies on accurate assessment of functional residual capacity (FRC). The EasyOne Pro LAB (ndd) and Exhalyzer D (EM) are two commercially available MBNW devices. The aim of the study was to compare these two devices in vitro and in vivo in healthy subjects with regard to FRC, LCI and secondary outcome parameters and to relate FRCMBNW to FRC measured by body plethysmography (pleth) and helium dilution technique. MBNW measurements were performed using a lung model (FRC between 500 and 4000 mL) in vitro and in 38 subjects aged 6–65 years followed by helium dilution and pleth in vivo using fixed and relaxed breathing techniques. In vitro accuracy within 5% of lung model FRC was 67.3% for ndd, FRC was >5% higher for EM in all tests. In vivo, FRCpleth ranged from 1.2 to 5.6 L. Mean differences (limits of agreement) between FRCpleth and FRCMBNW were −7.0%, (−23.2 to 9.2%) and 5.7% (−11.2 to 22.6%) using ndd and EM, respectively. FRCndd was consistently lower than FRCEM (−11.8% (−25.6 to 2%)). LCI was comparable between the two devices (−1.3% (−21.9 to 19.3%)). There was a difference of >10 % in LCI in 12 of 38 subjects. Using the most recent software updates, both devices show relevant deviations in FRC measurement both in vitro and in vivo and individual differences in LCI in a significant proportion of subjects. The devices are therefore not interchangeable.
