AbstractThe misuse of substances is often maintained by both physical and psychological factors. Opioid-substitution medications manage physical aspects of addiction; however, difficulties with emotional regulation and avoidance perpetuate continued substance misuse. In the UK, individuals who misuse substances are often excluded from mental health services, meaning these underlying difficulties are not addressed. Acceptance and Commitment Therapy (ACT) seeks to reduce emotional avoidance. A hermeneutic single-case efficacy design was used to evaluate the effects of ACT within drugs and alcohol service. Quantitative and qualitative data was critically analysed to understand factors involved in identified changes. Analysis recognised the client progressed towards two of three of their goals, related to motivation and anxiety. Their psychological flexibility also increased. ACT processes played a key role in this; however, the therapeutic relationship and psychopharmacological factors were also noted. Study limitations and clinical and research implications are discussed.

Obstetric emergencies are the leading cause of maternal and child mortality worldwide. According to WHO, about 15% of all pregnant women will develop obstetric emergencies that will require special skills to manage. A Nigerian woman has a 1 in 22 lifetime risk of dying during pregnancy, childbirth or postpartum/post-abortion whereas most other developed countries have a risk of 1 in 4900. With figures so high, there is need for prompt response to arrest the problem. A good knowledge/awareness of pregnant women on the predisposing risk factors for occurrence of obstetric emergencies better equips them with appropriate steps to take in preventing the occurrence of such. In this study, the awareness of pregnant women on the risk factors of obstetric emergencies was assessed. Just about 37% of the women knew the concept of obstetric emergencies but when asked the specific types, a good number were aware of all the types. The most popular type was postpartum hemorrhage with 43.5% knowledge. The risk factors most recognized included poor antenatal care, age, infection, drugs and alcohol. 96% of the population however did not think they were at risk of developing any obstetric emergency.

AimsTo gather information on useful medications to treat visual snow syndrome (VSS) as well as to validate an instrument to assess its clinical severity and the course of the disorder over time.MethodsFour hundred patients with VSS were included in this web-based prospective questionnaire study. All subjects completed a treatment questionnaire and a clinical diary. The first allowed evaluation of the effects of previous medications on visual snow, while the second measured VSS symptoms daily over the course of 30 days.ResultsPatients commonly reported previous use of medications such as antidepressants, antiepileptics, antibiotics and benzodiazepines. However, none of these drug classes was beneficial for the majority of patients. Recreational drugs and alcohol worsened visual snow symptoms in several reports. Vitamins and benzodiazepines had high therapeutic ratios, although in most cases they did not change the course of VSS.The monthly diary confirmed that the static in VSS is a consistent symptom over time. It also showed that indoor and fluorescent lights have a worse effect on symptoms when compared with natural outdoor lighting.ConclusionsThe study confirms clinical experience that medications are generally ineffective in VSS, with the exception of vitamins and perhaps benzodiazepines, which could be beneficial in some patients. The 30-day diary represents a useful tool to measure symptom progression over time, which could be used in future trials on VSS.

The liver is a requisite organ in the body, maintaining various physiological functions. Liver damage or toxicity is due to consuming certain chemicals, dietary supplements, drugs, and alcohol at high doses in day-to-day life. Several phytoconstituents acts as hepatoprotective agents. Research advances in analytical techniques and knowledge of active components have allowed research scientists to study the relationship between the phytoconstituents and their activity on the human liver. The review article aims to compile data on promising active constituents that act as lead molecules for future natural products-based drug discovery. The online database, including Web of Science, PubMed, Scopus, and Science Direct, was explored for papers and studied the hepatoprotective activity. Some phytoconstituents act against the chemicals such as CCl4 , paracetamol, alcohol, oxidative stress-induced hepatotoxicity, etc.
