In the last decade, there has been a rapid growth in the presence of clowns in hospitals, particularly in pediatric settings. The proliferation of clowns in health care settings has resulted in varying levels of professionalism and accountability. For this reason, there is a need to examine various forms of clowning, in particular therapeutic clowning in pediatric settings. The purpose of this article is to address what therapeutic clowning is and to describe the extent to which it can provide a complementary form of health care. In an attempt to apply theory to practice, the article will draw upon the experiences of a therapeutic clown within a pediatric setting while providing a historical and theoretical account of how clowns came to be in hospitals. Toward this end, a proposed model of therapeutic clowning will be offered which can be adapted for a variety of settings where children require specialized forms of play in order to enhance their coping, development and adjustment to life changes. Finally, current research on clowning in children's hospitals will be reviewed including a summary of findings from surveys administered at the Hospital for Sick Children.

PurposeTo satisfy donors and reduce public procurement acquisition prices, Benin has implemented and amended its first public procurement code guided by top-down principles of good governance.Design/methodology/approachThis study aims to measure the impact of the code and its amendment on public procurement acquisition prices of health-care equipment from 1995 to 2010.FindingsA segmented linear regression analysis was performed using interrupted time-series data. The analysis shows that the code and its amendment did not reduce acquisition prices, indicating the limited impact of the code. The authors recommend the implementation of bottom-up processes in establishing the public procurement system, and the development of a reference pricelist of the most widely used health-care equipment, as possible solutions for improving the effectiveness of the code.

RESUMOObjetivo: investigar o conhecimento do enfermeiro nas unidades de atenção primária à saúde sobre Diabetes Mellitus. Método: estudo qualitativo e realizado com 13 enfermeiros a partir de instrumento de caracterização sociodemográfica e cultural e questões relacionadas ao conhecimento sobre a Diabetes Mellitus. Os dados foram organizados e analisados pela técnica de Análise de Conteúdo, na modalidade Análise Temática. Resultados: da análise, emergiram o tema central <<Distanciamento da teoria à prática do cuidado às pessoas com diabetes>> e os subtemas <<Déficit de conhecimento da doença>>; <<Déficit de conhecimento do tratamento>>; <<Déficit de conhecimento sobre a conservação da insulina e o gerenciamento de resíduos>>; <<Déficit de conhecimentos sobre os cuidados>>; <<Déficit de conhecimentos sobre os direitos da pessoa com Diabetes Mellitus>>. Conclusão: os resultados apontam para o distanciamento da teoria à prática do cuidado às pessoas com Diabetes Mellitus. Identificam, assim, a necessidade da educação permanente nos serviços de saúde. Purpose The purpose of this paper is to study the links between contextual changes, contract arrangements and resultant problems when changes in outsourcing regulatory requirements are applied to complex pre-hospital services previously characterized by relational contracting. Design/methodology/approach The study deployed a qualitative design based on interviews with key informants and extensive studies of documents. It is a longitudinal study of a procurement process taking place in a regional health authority covering the period 2006 to 2017. Findings A complex and longitudinal public procurement process where pre-hospital (ambulance) services are transformed from relational and outsourced governance to more formal arrangements based on legal and transactional controls, is described in detail. After several years, the process collapsed due to challenges following public scrutiny, legal actions and administrative staff resignations. The public body lacked procurement competencies and the learning process following the regulations was lengthy. In the end, the services were in-sourced. Research limitations/implications This study is based on one case and it should, therefore, not be generalized without limitations. Practical implications One practical implication of this study is that transactional contracts are not optimal when core and complex services are produced in inter-organizational settings. In public sector health-care contexts, the role of informal and social controls based on relational exchanges are particularly applicable. Social implications Acute health-care services essential to citizens’ security and health imply high asset specificity, frequency and uncertainty. Such transactions should according to theory be produced in-house because of high agency costs in the procurement process. Originality/value The paper contributes to the understanding of how the public procurement process can itself be complex, as managerial challenges and solutions vary along several dimensions and are contingent upon external factors. In particular, the study increases knowledge of why the design and implementation of outsourcing models may create problems that impede and obstruct control in a particular public sector context.

Summary Objective: To bridge the divide between health informatics ‘bench research’ and the application of informatics in clinical and health care settings. Method: Identifying weak points in translational activities, i.e. in the process from health informatics research outcomes to IT system design and information management in clinical practice. Results and Conclusions: The creation of a new peer-reviewed journal, designed to cultivate broad readership across health care, is suggested in order to communicate on informatics topics of translational interest and on the application of informatics principals. Such an applied informatics journal may appeal to practicing physicians, healthcare administrators and CIOs as well as medical informaticians. In a globalizing world with eHealth initiatives spanning across borders, such a journal should be an international effort. Close ties to the International Medical Informatics Association (IMIA) and to the journal Methods of Information in Medicine are suggested.

There is no area of life that coronavirus disease would not have been effected by, not without the field of public procurement. Besides the emergency situation, governments need to secure supply of essential goods and services, not only related to health care, but also in order to maintain public services and take care of wellbeing. Citizens’ expectations towards the performance of public procurement (efficiency, effectiveness and economical) meanwhile did not change, and are at least on the same level as pre-COVID”. The aim of this study is to evaluate the high level general impact of Covid19 on public procurement and its performance. The outcome of the study is varied. There is no significant change in the total figures of the issued notices, also no change in the issuing authorities, so general picture shows stable trend. Indicators of evaluating the performance, include using less economical tenders type increase, which can lead to performance loss, but meanwhile indications of using the most economic tender criteria has been increased, which indicate better performance.
