Purpose: The purpose of the study is to analyze the results of surgical treatment of patients with brain and torso injury for 5 years in a single regional trauma center.Methods: We analyzed multiple trauma patients who underwent brain surgery and torso surgery for chest or abdominal injury simultaneously or sequentially among all 14,175 trauma patients who visited Dankook University Hospital Regional Trauma Center from January 2015 to December 2019.Results: A total of 25 patients underwent brain surgery and chest or abdominal surgery, with an average age of 55.4 years, 17 men and eight women. As a result of surgical treatment, there were 14 patients who underwent the surgery on the same day (resuscitative surgery), of which five patients underwent surgery simultaneously, four patients underwent brain surgery first, and one patient underwent chest surgery first, four patients underwent abdominal surgery first. Among the 25 treated patients, the 10 patients died, which the cause of death was five severe brain injuries and four hemorrhagic shocks.Conclusions: In multiple damaged patients require both torso surgery and head surgery, poor prognosis was associated with low initial Glasgow Coma Scale and high Injury Severity Score. On the other hand, patients had good prognosis when blood pressure was maintained and operation for traumatic brain injury was performed first. At the same time, patients who had operation on head and torso simultaneously had extremely low survival rates. This may be associated with secondary brain injury due to low perfusion pressure or continuous hypotension and the traumatic coagulopathy caused by massive bleeding.

ABSTRACT Objectives To systematically evaluate the effect of the surgery-first approach (SFA) on oral health-related quality of life (OHRQoL) in patients with dentofacial deformities. Materials and Methods An electronic database search and hand search of selected journals and references were carried out. Studies investigating the OHRQoL of patients receiving SFA with or without a control group were included. The risk of bias was assessed by the Cochrane risk of bias tool in randomized clinical trials (RCTs) and the Newcastle-Ottawa Scale in non-RCTs. Results A total of seven articles met the eligible criteria and were included, of which six were cohort studies and one was an RCT, and six assessed the OHRQoL of the SFA with conventional orthodontic–surgical treatment (COST) as a control and one without. A total of 214 patients were examined, with sample sizes in studies ranging from 9 to 50. A total of 3 articles successfully measured the OHRQoL both before and after treatment in both the SFA and conventional orthodontic–surgical treatment groups. A total of six cohort studies were classified as low to moderate risk of bias, and the RCT was classified as high. Conclusions The SFA could improve the OHRQoL of patients with dentofacial deformities similar to conventional orthodontic–surgical treatment at the end of complete treatment. In addition, it increases OHRQoL immediately at the beginning of treatment without a deterioration.

Orthognathic surgery and orthodontic treatment are required for patients with dentofacial deformities to obtain an ideal facial esthetic with good functioning. Recently, characterized by the surgery-first approach, an integrated orthodontic–surgical treatment has been introduced as an emerging solution to dentofacial deformity treatment. The surgery-first approach is regarded to have less treatment time and quicker enhancement of a facial profile than the conventional orthodontic–surgical treatment. Moreover, the recent advances in computing and imaging have allowed the adoption of 3-dimensional (3D) virtual planning protocols in orthognathic surgery as well as digital orthodontic treatment, which enables a paradigm shift when realizing virtual planning properly. These techniques then allow the surgeon and orthodontist to collaborate, plan, and simulate the dentofacial deformity treatment before performing the whole procedure. Along this line, in this research article, we present an integrated treatment method for the realization of an effective deformity treatment. Specifically, we implemented the integrated 3D technique by combining it with the surgery-first orthognathic approach (SFOA) as a novel treatment method for the patients. The outcomes from the combined treatments of the patients with dentofacial deformity, in practice, have demonstrated that our proposed 3D technique in orthognathics and orthodontics using clear aligner therapy (e.g., Invisalign) can enhance the satisfactory level of the patient since the start of treatment then improve their quality of life. As a result, the combined techniques realize the novel integrated treatment method using 3D technology with the use of 3D imaging and modeling as a promising development trend of dentistry, which fits into the context of Dentistry 4.0 as a key enabler to the concept of sustainable dentistry development.

Displacement of the abomasum (DA) is an increasingly common disease in dairy cattle in intensive production. This study presents surgical treatment of DA using Ljubljana method in years 2005 and 2006. Slovenian Black and White dairy cows (n = 30) with left DA in 19 cows, right DA in four cows, anterior DA in two cows and abomasal volvulus in five cows were used. The surgical treatment was successful in 28 cows, 2 had to be euthanized due to complications associated with left DA and abomasal volvulus. One month after the surgery all 28 cows were productive. Omentopexy using Ljubljana method can be used as a clinical procedure as well as procedure performed in ambulatory practice.

The recent development of the scanning electron microscope has added great impetus to the study of ultrastructural details of normal human ossicles. A thorough description of the ultrastructure of the human ossicles is required in order to determine changes associated with disease processes following medical or surgical treatment.Human stapes crura were obtained at the time of surgery for clinical otosclerosis and from human cadaver material. The specimens to be examined by the scanning electron microscope were fixed immediately in the operating room in a cold phosphate buffered 2% gluteraldehyde solution, washed with Ringers, post fixed in cold 1% osmic acid and dehydrated in graded alcohol. Specimens were transferred from alcohol to a series of increasing concentrations of ethyl alcohol and amyl acetate. The tissue was then critical point dried, secured to aluminum stubs and coated with gold, approximately 150A thick on a rotating stage in a vacuum evaporator. The specimens were then studied with the Kent-Cambridge S4-10 Scanning Electron Microscope at an accelerating voltage of 20KV.
