Children After Fontan have Strength and Body Composition Similar to Healthy Peers and Can Successfully Participate in Daily Moderate-to-Vigorous Physical Activity

Introduction: This cross-sectional study evaluated the healthy, active lifestyle capacity (daily physical activity, strength, flexibility, body composition) of children after Fontan, which was hypothesized to be lower than healthy peers. Methods: Participants (n=64, 25 female) were 9 ± 2 years of age. Fontan completion occurred at 3 ± 1 years of age. Canadian Health Measures Survey protocols assessed aerobic endurance (walking up/down steps at set pace), strength (handgrip dynamometry), flexibility (sit and reach), body composition (body mass index) and daily moderate-to-vigorous physical activity (7-day accelerometry). Participant versus published norm differences were evaluated with t-tests. Linear regression evaluated associations with age/gender/demographic factors. Results: Children after Fontan had strength scores similar (mean difference 1 kg) to their peers, were less likely to be obese (mean difference of body mass index = 1.1 ± 2.5, p=.001) and performed 50 minutes of moderate-to-vigorous activity per day. Estimated maximal aerobic endurance (mean difference = 21 ± 3 ml/kg/min or 61% of expected) and flexibility (mean difference = 9 ± 8 cm or 64% of expected) were lower than peers(p<.001). Participants performed fewer minutes of daily activity (mean difference from normal 12 ± 17 minutes/day, p<.001), but almost all (60/63) demonstrated the capacity for at least 20 minutes per day. Daily activity was higher with Fontan completion at a younger age (4 ± 2 mins/year) and for those taking antithrombotic medication (7 ± 18 and 22 ± 17 fewer minutes/day for those taking/not taking antithrombotics, respectively). Conclusions: Children after Fontan demonstrate the capacity to successfully perform the daily physical activity associated with optimal health. They have similar levels of strength and good body composition. We recommend that children after Fontan be counselled to expect that they can successfully participate in physically active peer play.

ObjectivesPhysical activity may be important in deterring the obesity epidemic. This study aimed to determine whether objectively measured physical activity in first year of upper secondary high school predicted changes in body composition over 2 years of follow-up in a cohort of Norwegian adolescents (n=431).DesignA longitudinal study of adolescents (mean age of 16 (SD 0.4) at baseline, 60.3% girls) participating in the Fit Futures studies 1 (2010–2011) and 2 (2012–2013).SettingAll eight upper secondary high schools in two municipalities in Northern Norway.ParticipantsStudents participating in both studies and under the age of 18 at baseline and with valid measurement of physical activity at baseline and body composition in both surveys.Primary and secondary outcomesChange in objectively measured body mass index and waist circumference and change in dual-energy X-ray absorptiometry measured fat mass index, lean mass index (LMI) and appendicular LMI (aLMI) between baseline and follow-up.ResultsAt baseline, boys had significantly higher physical activity volume (p=0.01) and spent on average of 6.4 (95% CI 2.1 to 10.6) more minutes in moderate-to-vigorous physical activity (MVPA) than girls (p<0.01). In girls, multivariate regression analyses showed that more sedentary time was negatively associated with changes in LMI (p<0.01) and aLMI (p<0.05), whereas more light activity had opposite effects on these measures (p<0.01 and p<0.05, respectively). No significant associations between measures of baseline physical activity and changes in body composition parameters were observed in boys.ConclusionsIn this cohort of Norwegian adolescents, sedentary and light physical activity was associated with changes in LMI and aLMI in girls, but not boys. Minutes spent in MVPA in first year of upper secondary high school was not associated with changes in measures of body composition in neither sex after 2 years.

BACKGROUND Mobile health (mHealth) is currently among the supporting elements that may contribute to an improvement in health markers by helping people adopt healthier lifestyles. mHealth interventions have been widely reported to achieve greater weight loss than other approaches, but their effect on body composition remains unclear. OBJECTIVE This study aimed to assess the short-term (3 months) effectiveness of a mobile app and a smart band for losing weight and changing body composition in sedentary Spanish adults who are overweight or obese. METHODS A randomized controlled, multicenter clinical trial was conducted involving the participation of 440 subjects from primary care centers, with 231 subjects in the intervention group (IG; counselling with smartphone app and smart band) and 209 in the control group (CG; counselling only). Both groups were counselled about healthy diet and physical activity. For the 3-month intervention period, the IG was trained to use a smartphone app that involved self-monitoring and tailored feedback, as well as a smart band that recorded daily physical activity (Mi Band 2, Xiaomi). Body composition was measured using the InBody 230 bioimpedance device (InBody Co., Ltd), and physical activity was measured using the International Physical Activity Questionnaire. RESULTS The mHealth intervention produced a greater loss of body weight (–1.97 kg, 95% CI –2.39 to –1.54) relative to standard counselling at 3 months (–1.13 kg, 95% CI –1.56 to –0.69). Comparing groups, the IG achieved a weight loss of 0.84 kg more than the CG at 3 months. The IG showed a decrease in body fat mass (BFM; –1.84 kg, 95% CI –2.48 to –1.20), percentage of body fat (PBF; –1.22%, 95% CI –1.82% to 0.62%), and BMI (–0.77 kg/m<sup>2</sup>, 95% CI –0.96 to 0.57). No significant changes were observed in any of these parameters in men; among women, there was a significant decrease in BMI in the IG compared with the CG. When subjects were grouped according to baseline BMI, the overweight group experienced a change in BFM of –1.18 kg (95% CI –2.30 to –0.06) and BMI of –0.47 kg/m<sup>2</sup> (95% CI –0.80 to –0.13), whereas the obese group only experienced a change in BMI of –0.53 kg/m<sup>2</sup> (95% CI –0.86 to –0.19). When the data were analyzed according to physical activity, the moderate-vigorous physical activity group showed significant changes in BFM of –1.03 kg (95% CI –1.74 to –0.33), PBF of –0.76% (95% CI –1.32% to –0.20%), and BMI of –0.5 kg/m<sup>2</sup> (95% CI –0.83 to –0.19). CONCLUSIONS The results from this multicenter, randomized controlled clinical trial study show that compared with standard counselling alone, adding a self-reported app and a smart band obtained beneficial results in terms of weight loss and a reduction in BFM and PBF in female subjects with a BMI less than 30 kg/m<sup>2</sup> and a moderate-vigorous physical activity level. Nevertheless, further studies are needed to ensure that this profile benefits more than others from this intervention and to investigate modifications of this intervention to achieve a global effect. Introduction: Physical activity is a key component of a healthy lifestyle for youths with visual impairments (i.e., blindness or low vision). This study aims to examine the habitual physical activity and sedentary time of Chinese children and adolescents with visual impairments and to determine whether age, gender, and body composition significantly influence their behavior. Methods: A purposeful sample method was utilized to recruit 137 Chinese youths with visual impairments from a special school for individuals with visual and hearing impairments. A total of 72 students with visual impairments (aged 7–17 years; M age = 13.4) participated in this study. ActiGraph GT3X accelerometers were used to measure the habitual physical activity and sedentary time. Results: Youths with visual impairments in this study spent an average of 287.86 (standard deviation [ SD] = 45.67) minutes of light physical activity, 146.79 ( SD = 56.95) minutes of moderate physical activity, 18.47 ( SD = 15.15) minutes of vigorous physical activity, 165.26 ( SD = 69.83) minutes of moderate-to-vigorous-intensity physical activity, and 979.51 ( SD = 109.10) minutes of sedentary time per day. Children with visual impairments in elementary school engaged in more habitual physical activity and had less sedentary time than did students in middle and high school. Differences in gender and body composition with regard to time spent being active or sedentary were insignificant. Discussion: Findings demonstrated that Chinese youths with visual impairments in this study had sufficient health-enhancing physical activity and were habitually active. Future research is needed to identify the factors that account for the high habitual physical activity levels of Chinese youths with visual impairments. Implications for practitioners: Transition planning for students with visual impairments to facilitate active habitual physical activity participation when children grow up is warranted.

Objectives: US Army reserve soldiers and active-duty soldiers differ in their daily work demands and supporting resources, yet research on reservists’ health and fitness is lacking. The objectives of this study were to (1) determine whether physical test failure rates and health behaviors differed between active-duty soldiers and reserve soldiers and (2) establish which demographic and health behavioral factors were associated with failing physical tests. Methods: We analyzed a sample of 239 329 US Army active-duty and reserve soldiers surveyed from September 2013 through March 2015 using the Global Assessment Tool. We extracted data on soldier demographic characteristics and health behaviors, as well as Body Composition Test (BCT) and Army Physical Fitness Test (APFT) results. We compared the 2 groups using the active-to-reserve adjusted odds ratio (aOR) for each variable. We used logistic regression models to determine which variables were associated with failing these tests. Results: The odds of failing the BCT (aOR = 0.76; 95% confidence interval [CI], 0.73-0.78) or the APFT (aOR = 0.31; 95% CI, 0.30-0.32) were lower among active-duty soldiers than among reservists, and the odds of doing high levels of high-intensity interval training (aOR = 1.47; 95% CI, 1.42-1.51), resistance training (aOR = 1.45; 95% CI, 1.42-1.48), and vigorous physical activity (aOR = 2.92; 95% CI, 2.86-2.98) were higher among active-duty soldiers than among reservists. The odds of using tobacco (aOR = 1.37; 95% CI, 1.35-1.40), binge drinking alcohol (aOR = 1.11; 95% CI, 1.09-1.13), having insomnia (aOR = 1.46; 95% CI, 1.43-1.48) or mild depression (aOR = 1.50; 95% CI, 1.48-1.53), and sustaining a physical activity–related injury (aOR = 2.52; 95% CI, 2.47-2.57) were higher among active-duty soldiers than among reservists. Conclusions: Policy makers and military leaders could use this information to implement health screenings and tailor health-promotion, intervention, and treatment programs.

Background. Increasing evidence suggests that vigorous physical activity (VPA) in youth may yield greater health benefits than moderate (MPA) or moderate-to-vigorous physical activity (MVPA). The purpose of this review was to assess the relationship between PA intensity and body composition, cardiorespiratory fitness (CRF), and cardiometabolic (CM) biomarkers in youth. Methods. We conducted a systematic review of observational studies examining PA intensity and selected health outcomes in youth aged 6 to 18 years. Forty-five articles were selected for final review. Results. VPA was more strongly associated with reduced body fat and central adiposity compared with MPA and/or MVPA. Additionally, VPA was more strongly associated with increased CRF when compared with lower intensities. Findings were inconclusive between all PA intensity levels and CM biomarkers, and several significant relationships observed for VPA were attenuated when controlling for CRF. Conclusions. A potential VPA dose is identified as yielding favorable health benefits in adiposity and fitness. While CM biomarkers were not consistently associated with PA intensity level, the literature suggests VPA may yield health benefits above those received from MPA for reduced adiposity and improved CRF. This review highlights the need for longitudinal observational and experimental studies to determine optimal VPA dose for CM health in youth.
