Introduction Food insecurity and foodborne disease are important issues in Canada, and the public health actions taken to address them can be conceptualized as factors shaping the food environment. Given emerging evidence that these two areas may interrelate, the objective of this study was to explore ways in which community food security efforts and food safety practices (and the population health issues they aim to address) may intersect in British Columbia, Canada, and interpret what this might mean for conceptualizing and attaining healthier food environments. Methods We conducted 14 key informant interviews with practitioners working in community food security and food safety in British Columbia, and used qualitative descriptive analysis to identify examples of intersections between the sectors. Results Participants identified four key ways that the two sectors intersect. They identified (1) how their daily practices to promote safe or healthy food could be helped or hindered by the activities of the other sector; (2) that historically disjointed policies that do not consider multiple health outcomes related to food may complicate the interrelationship; (3) that the relationship of these sectors is also affected by the fact that specific types of food products, such as fresh produce, can be considered both risky and beneficial; and (4) that both sectors are working towards the same goal of improved population health, albeit viewing it through slightly different lenses. Conclusion Food security and food safety connect in several ways, with implications for characterizing and improving Canadian food environments. Collaboration across separated public health areas related to food is needed when designing new programs or policies aimed at changing the way Canadians eat.

Food safety and food security are two important public health sectors within Canada, which aim to address foodborne disease and food insecurity, respectively.  While these sectors are often siloed within public health organizations, the actions of the two sectors often interact and conflict at the program level despite their common goal of improving population health.  The objective of the present study was to identify determinants that influenced the success of collaboration between practitioners of the two sectors in British Columbia, to inform Canadian food policy.  We inductively analyzed 14 interviews with practitioners working in the two sectors who had experience with successful collaboration. Data were interpreted in consultation with an inter-professional collaboration framework.  Participants identified determinants at the systemic level, including the cultural, professional, educational, legislative, and political systems, which were often considered barriers to collaboration.  Participants also identified determinants at the organizational level that influenced the success of collaboration between the sectors, including: the organization’s structure and philosophy, leadership, resources, and communication mechanisms.  Finally, participants identified interactional determinants as ways to overcome existing barriers, including: willingness to collaborate, trust, communication, mutual respect, and taking a solutions-oriented approach.  Practitioners working in food safety and food security can apply the interactional determinants identified in this study to mitigate existing barriers to collaboration and support more synergistic food policies.

A major challenge for society today is how to secure and provide plentiful, healthy, and nutritious food for all in an environmentally sustainable and safe manner, while also addressing the multiple burdens of undernutrition, overweight and obesity, stunting and wasting, and micronutrient deficiencies, particularly for the most vulnerable. There are considerable ethical questions and trade-offs that arise when attempting to address this challenge, centered around integrating nutrition into the food security paradigm. This chapter attempts to highlight three key ethical challenges: the prioritization of key actions to address the multiple burdens of malnutrition, intergenerational justice issues of nutrition-impacted epigenetics, and the consequences of people’s diet choices, not only for humanity but also for the planet.

AbstractThe objective of the present study was to evaluate intakes and serum levels of vitamin A, vitamin E, and related compounds in a cohort of maternal–infant pairs in the Midwestern USA in relation to measures of health disparities. Concentrations of carotenoids and tocopherols in maternal serum were measured using HPLC and measures of socio-economic status, including food security and food desert residence, were obtained in 180 mothers upon admission to a Midwestern Academic Medical Center labour and delivery unit. The Kruskal–Wallis and independent-samples t tests were used to compare measures between groups; logistic regression models were used to adjust for relevant confounders. P < 0·05 was considered statistically significant. The odds of vitamin A insufficiency/deficiency were 2·17 times higher for non-whites when compared with whites (95 % CI 1·16, 4·05; P = 0·01) after adjustment for relevant confounders. Similarly, the odds of being vitamin E deficient were 3·52 times higher for non-whites (95 % CI 1·51, 8·10; P = 0·003). Those with public health insurance had lower serum lutein concentrations compared with those with private health insurance (P = 0·05), and living in a food desert was associated with lower serum concentrations of β-carotene (P = 0·02), after adjustment for confounders. Subjects with low/marginal food security had higher serum levels of lutein and β-cryptoxanthin compared with those with high food security (P = 0·004 and 0·02 for lutein and β-cryptoxanthin). Diet quality may be a public health concern in economically disadvantaged populations of industrialised societies leading to nutritional disadvantages as well.

Alcohol sales data provide a more accurate indication of alcohol consumption than alternative methods such as population surveys. This information can be used to better understand epidemiological issues related to alcohol consumption, policy development and evaluation. Official sales records were collected for the 28 regional districts of British Columbia (BC) for 2002–2005, while homemade alcohol was estimated from survey data. Alcohol consumption rates were found to vary across geographic regions, by season, and with population level demographics. Government stores were the largest source of alcohol consumption in BC, accounting for 45.1% of total alcohol consumption in 2004. U-Brews/U-Vins accounted for 4.0%, private liquor stores accounted for 27.5% of the total, and homemade alcohol made up 4.3% of total alcohol consumption. Analysis also revealed that the average alcohol concentration in wines (12.53%) and coolers (6.77%) has been underestimated by Statistics Canada. The feasibility of developing this type of alcohol monitoring system is examined. Finally, implications for the development of targeted public health initiatives and future research are discussed.

BackgroundSocial marketing is a tool used in the domain of public health for prevention and public education. Because injury prevention is a priority public health issue in British Columbia, Canada, a 3-year consultation was undertaken to understand public attitudes towards preventable injuries and mount a province-wide social marketing campaign aimed at adults aged 25–55 years.MethodsPublic response to the campaign was assessed through an online survey administered to a regionally representative sample of adults within the target age group between 1 and 4 times per year on an ongoing basis since campaign launch. A linear regression model was applied to a subset of this data (n=5186 respondents) to test the association between exposure to the Preventable campaign and scores on perceived preventability of injuries as well as conscious forethought applied to injury-related behaviours.ResultsCampaign exposure was significant in both models (preventability: β=0.27, 95% CI 0.20 to 0.35; conscious thought: β=0.24, 95% CI 0.13 to 0.35), as was parental status (preventability: β=0.12, 95% CI 0.03 to 0.21; conscious thought: β=0.18, 95% CI 0.06 to 0.30). Exposure to the more recent campaign slogan was predictive of 0.47 higher score on conscious thought (95% CI 0.27 to 0.66).DiscussionThis study provides some evidence that the Preventable approach is having positive effect on attitudes and behaviours related to preventable injuries in the target population. Future work will seek to compare these data to other jurisdictions as the Preventable social marketing campaign expands to other parts of Canada.
