AbstractDefence cases with an escalatory structure, in which the levels of violence between aggressor and defender start out as minor and then become major, even lethal, raise sharp problems for defence theory, and for our understanding of the conditions of defence: proportionality, necessity, and imminence. It is argued here that defenders are not morally required to withdraw from participation in these cases, and that defensive escalations do not offend against any of the conditions of defence, on an adequate understanding of them. No plausible interpretation of proportionality or necessity excludes defensive permissions in escalatory cases. Moreover, the structure of escalatory defensive cases also sheds useful light on permissible responses to conditional deadly threats. This is because we can analyse conditional deadly threats as disguised cases of escalation. Because defensive resistance is permissible in escalatory cases, it is also permissible in cases involving conditional deadly threats.

Abstract “Food for Thought” provides an opportunity for review of pertinent topics to add to updates in areas of concern for professionals involved with feeding and swallowing issues in infants and children. Given the frequency with which speech-language pathologists (SLPs) make decisions to alter feedings when young infants demonstrate silent aspiration on videofluoroscopic swallow studies (VFSS), the need for increased understanding about cough and its development/maturation is a high priority. In addition, understanding of the role(s) of laryngeal chemoreflexes (LCRs), relationships (or lack of relationships) between cough and esophagitis, gastroesophageal reflux (GER), and chronic salivary aspiration is critical. Decision making regarding management must take into account multiple systems and their interactions in order to provide safe feeding for all children to meet nutrition and hydration needs without being at risk for pulmonary problems. The responsibility is huge and should encourage all to search the literature so that clinical practice is as evidence-based as possible; this often requires adequate understanding of developmentally appropriate neurophysiology and function.

Reading comprehension is a reading activity that aims to obtain adequate understanding and interpretation of the meanings contained in written symbols. the reading process is divided into three stages, such as pre-reading, reading, and post-reading. But the reality is that many of the students still experience difficulties in reading comprehension learning. Therefore, researchers applied an Accelerated Learning approach to improve students' reading comprehension skills. The results showed, the assessment of the process of reading students' skills for each assessment indicator was in the value range of 78.35% to 87.35% which was included in the good and very good category. While the results of the assessment of the overall reading skills process were 82.32% with a very good category. Based on the results of the study it can be concluded that learning based on the accelerated learning approach can enhance reading comprehension skills of students in class V of the Primary school of 011 Air Emas, Singingi District.

Time is a neglected subject in recent, especially analytically minded reflections on the good life. The article highlights the fundamental role of time and temporality for an adequate understanding of the good life. Time functions both as an external factor with which we have to reckon in our practical deliberations and as an internal structure of living our lives. It is argued that striving for a good life also means striving for being in harmony with the time of one's life. The exploration of this idea allows to link analytical with phenomenological approaches to time and good life.

Michael Heinrich, one of the leading Marx scholars, provides a general introduction into Das Kapital with emphasis on the latest interpretations of it. The circumstances surrounding its writing and publication are shown to have interfered with an adequate appreciation of it. The formal structure and organization of the first volume are obstacles to readers and demand much from their education and intellect. The article summarizes the basic trajectories of Marx’s criticisms of political economy, including the critique of naturalizing social forms arising under capitalism and Marx’s original monetary theory of value. The author disentangles Marx’s Das Kapital from views mistakenly ascribed to it, such as the idea that value is determined solely by labor and the prediction of pauperization of the masses. First, Marx’s theory of value goes well beyond explaining prices under capitalism. Second, his main prophecy concerned the inevitable growth of inequality between the masters of capital and the employed classes and did not forecast impoverishment. The paper also points out that the sequence of publication of different volumes of Das Kapital caused lacunae in interpreting Marx’s oeuvre. For instance Engels’ efforts made the third volume more accessible to readers but also obscured the overall pattern of Marx’s thinking. the article shows that Das Kapital was a dynamic and fluctuating project to such an extent that Marx himself several times revisited his views of the causes of economic crises and falling profits and also intended to deal extensively with ecological issues. Reaching an adequate understanding of the theory contained in Das Kapital cannot depend on the manuscripts of those volumes alone. Marx’s notebooks, which have only recently published, are an indispensable aid to understanding it.

There is an argument for the existence of God from the incompleteness of nature that is vaguely present in Plantinga’s recent work. This argument, which rests on the metaphysical implications of quantum physics and the philosophical deficiency of necessitarian conceptions of physical law, deserves to be given a clear formulation. The goal is to demonstrate, via a suitably articulated principle of sufficient reason, that divine action in an occasionalist mode is needed (and hence God’s existence is required) to bring causal closure to nature and render it ontologically functional. The best explanation for quantum phenomena and the most adequate understanding of general providence turns out to rest on an ontic structural realism in physics that is grounded in the immaterialist metaphysics of theistic idealism.

This chapter investigates the syntactic properties and the pragmatic behaviour of verb-initial declarative clauses in the history of German. The focus is on OHG because in this period, verb-initial declaratives represent a frequent, well-known alternative to canonical verb-second main clauses. It is argued that verb-initial declaratives are native in origin, and that they are derivable under a special interpretation of the verb-second rule. The main part of the chapter deals with the pragmatic properties of verb-initial declaratives in OHG, summarizing the various attempts at explaining the distribution of these orders and showing that further research is needed to arrive at a more adequate understanding of their function in the discourse. The chapter closes up with the discussion of the later development of verb-initial declaratives in German, sketching the controversial treatments of this question in the literature on German diachronic syntax.

Abstract Molecularly targeted therapy with MEK inhibitors is increasingly being incorporated into the treatment of pediatric low-grade gliomas (LGGs). Trametinib is an orally available MEK1/2 inhibitor that has demonstrated tumor control in LGGs with BRAF alterations. Safe expansion of MEK inhibitor therapy within the pediatric patient population demands adequate understanding of and surveillance for potential MEK-inhibitor specific toxicities, especially among young children. Hyponatremia has been reported in adult patients receiving BRAF/MEK inhibitor combination treatment as well as in two pediatric patients with known diabetes insipidus treated with trametinib monotherapy. To our knowledge, single-agent trametinib has not previously been reported to be associated with hyponatremia in children in the absence of an underlying endocrinopathy. We present a case of hyponatremia associated with trametinib use in an infant with progressive LGG without known endocrine dysfunction, which recurred after significant dose reduction. Therapy with an alternative MEK1/2 inhibitor, binimetinib, provided excellent tumor response without hyponatremia. Hyponatremia is a rare but serious side effect of trametinib, even without underlying pituitary dysfunction. Infants and patients lacking the ability to quickly regulate fluid intake in response to osmolality changes are at particular risk of suffering severe consequences from hyponatremia and should be monitored closely with initiation of trametinib. Switching to a different drug within the same class may offer an alternative to significant dose reduction or discontinuation due to this toxicity.

White dwarf stars provide important boundary conditions for the understanding of stellar evolution. An adequate understanding of even these simple stars is impossible without detailed knowledge of their interiors. PG1346+082, an interacting binary white dwarf system, provides a unique opportunity to view the interior of one degenerate as it is brought to light in the accretion disk of the second star as the primary strips material from its less massive companion (see Wood et at. 1987).PG1346+082 is a photometric variable with a four magnitude variation over a four to five day quasi-period. A fast Fourier transform (FFT) of the light curve shows a complex, time-dependent structure of harmonics. PG1346+082 exhibits flickering – the signature of mass transfer. The optical spectra of the system contain weak emission features during minimum and broad absorption at all other times. This could be attributed to pressure broadening in the atmosphere of a compact object, or to a combination of pressure broadening and doppler broadening in a disk surrounding the compact accretor. No hydrogen lines are observed and the spectra are dominated by neutral helium. The spectra also display variable asymmetric line profiles.

Abstract Objectives This study was aimed to describe a far lateral approach for microsurgical resection of a transverse ligament cyst, with emphasis on the microsurgical anatomy and technique. Design A far lateral craniotomy is performed in the lateral decubitus position. After opening the dura laterally, dural sutures are placed for retraction. A stitch placed through the dentate ligament is advantageous to rotate the spinal cord to allow access to the ventral cyst. The cyst is marsupirlized and mass effect on the spinal cord is relieved. Photographs of the region are borrowed from Dr Rhoton's laboratory to illustrate the microsurgical anatomy. Participants The first author performed the surgery and edited the video. Chart review and literature review were performed by the other authors. Outcome Measures Outcome was assessed with postoperative neurological function. Results The patient was discharged home after an uneventful hospital course. At short-term follow-up, the patient had a significant improvement in postoperative strength. Conclusion The far lateral approach provides an adequate corridor to the ventrolateral brainstem in combination with utilization of the dentate ligament to reach ventral cysts compressing the spinal cord. An adequate understanding of the relevant microsurgical anatomy is a key to safe surgery in this region.The link to the video can be found at: https://youtu.be/5MGVPO2Q2pI.

Changes in land-use can have an important effect on rates of erosion and denudation. In order to avoid accelerating erosion rates, decision makers in land-use planning require adequate information on the contemporary geomorphological processes and hydrology of areas where modifications in the land-use pattern are envisaged. The first phase in the acquisition of the information is the reconnaissance survey, which shows where and how the erosional status of an area is likely to be affected, and also acts as a foundation on which to base more detailed work in the future.The Solitude area is an active erosional system, dominated by mass movement and fluvial processes. The rate of removal of material is likely to be increased by modifications in land-use which, from other points of view, would be considered entirely suitable for the area. Therefore, if accelerated denudation is to be avoided, projects involving land-use changes should be implemented only by those having an adequate understanding of the mechanism of geomorphological processes operating in the area.
