The prevalence of cancer-related sexual dysfunction and body image problems varies according to diagnosis and treatment, but they affect at least half of those treated for pelvic malignancies or breast cancers and greater than a quarter of patients with other cancers. This chapter on psychosexual and psychoeducational issues outlines the impact of cancer treatment on psychosexual well-being (e.g., body image and sexual dysfunction) in males and females. It describes mechanisms to identify psychosexual problems and how to assess the potential needs of patients according to cancer site. It provides recommendations for effective evidence-based interventions to manage psychosexual problems according to presenting symptoms.

Objective: To explore and identify factors attributing towards the rising divorce rate in Pakistan and its consequences. Materials and methods: In-depth semi structured interviews were conducted from 17 divorced participants, five out of which were males. The participants were chosen from Karachi, Lahore, Peshawar and Islamabad. Interviews were conducted via email, telephone and face-to-face meeting. Results: Findings from the interviews highlighted partner violence, sexual dysfunction, in-laws interference and temperamental incompatibility were the most common causes of divorce. Moreover, consequences of divorce included depression and loneliness, and financial and accommodation issues. It was further found that it was difficult for the divorcees to judge and trust the new person requesting marriage proposal. Another major problem found was the child custody in which usually the males have to give up their children to their mothers. Finally, it was found that children too have to go through psychological issues after their parents separate. Conclusions: Divorce is an unwanted and undesired act of termination of marriage which brings a flood of financial, social and personal issues to divorcees. Divorce has adverse consequences on both males and females such as financial crisis, psychological issues and emotional breakdown. Children suffer emotionally in the absence of either of parents.

Following a bite from a juvenile Russell’s viper (Daboia russelii), a priapism (painful erection) developed rapidly in a 16-year-old male and only subsided after administration of antivenom 3 hours later. Potential mechanisms for this snakebite-induced priapism are unclear but likely due to venom toxins causing nitric oxide (NO) release and subsequent vasodilation of endothelium in the corpus cavernosum, although the possible involvement of other mechanisms cannot be ruled out. We strongly believe that this unusual case report may lead to further scientific research in order to improve the clinical understanding of the pathophysiology of envenomation due to Russell’s viper bites. Although it is too early to speculate, further research may also discover the possibilities of developing venom-based candidate molecules to treat sexual dysfunction in males and females.

Background: Sexual dysfunction is common in multiple sclerosis (MS) but reliable and valid measurement in this population is needed. Objective: The objective of this research is to re-validate the Multiple Sclerosis Intimacy and Sexuality Questionnaire-19 in a large US sample. Methods: A total of 6300 MS patients from the NARCOMS registry completed the MSISQ-19. Unforced principal component analysis utilizing oblique rotation with Kaiser Normalization validated its construct validity. Results: The scree plot supported a three-component solution, with 63% of total variance explained. The components mirrored the original validation study measuring primary, secondary, and tertiary sexual dysfunction. PCA suggested the scale could be shortened to 15 items, which were found to apply equally well to males and females (with one primary item specific for each sex). The components were moderately intercorrelated (Pearson rs ranged from 0.5 to 0.67). The secondary subscale correlated most highly with self-reported disability ( r (6081) = 0.44, p < 0.001), whereas the tertiary subscale correlated most highly with psychological distress ( r (5992) = –.37, p < 0.001). Cronbach’s alpha for the total scale (0.92) and the subscales (primary, 0.87; secondary, 0.82; tertiary, 0.91) demonstrated good reliability. Conclusion: The revised 15-item MSISQ is a reliable and valid measure of sexual dysfunction in men and women with MS.

Ultrastructural defects of ciliary structure have been known to cause recurrent sino-respiratory infection concurrent with Kartagener's syndrome. (1,2,3) These defects are also known to cause infertility in both males and females. (4) Overall, the defects are defined as the Immotile, or Dyskinetic Cilia Syndrome (DCS). Several ultrastructural findings have been described, including decreased number of cilia, multidirection orientation, fused and compound cilia, membrane blebs, excess matrix in the axoneme, missing outer tubular doublets, translocated doublets, defective radial spokes and dynein arms. A rare but noteworthy ultrastructural finding in DCS is the predominance of microvilli-like structures on the luminal surface of the respiratory epithelium. (5,6) These permanent surface modifications of the apical respiratory epithelium no longer resemble cilia but reflect the ultrastructure of stereocilia, similar to that found in the epidydimal epithelium. Like microvilli, stereocilia are devoid of microtubular ultrastructure in comparison with true cilia.
