Abstract Background and aims: High levels of non-HDL and atherogenic cholesterol can induce inflammation, and as risk factor for cardiovascular diseases. This study was to evaluate the effects of fiber-rich snacks on non-HDL cholesterol, atherogenic index, and Creactive protein (CRP) levels in type 2 diabetes patients (T2DM). Material and Methods: Twenty T2DM patients, were recruited from the Policlinic of Endocrine, Dr. Sardjito General Hospital, Yogyakarta, Indonesia. The subjects received daily 32g fiber-rich snacks made of Dioscorea esculenta, arrowroot, cassava and pumpkin for 4 weeks. Fasting non-HDL cholesterol, atherogenic index, CRP and HbA1c levels were measured before and after intervention. Paired t-test was used to evaluate the results. Results: The fiber-rich snack intervention in T2DM patients significantly reduced levels of non-HDL cholesterol and CRP levels (p<0.05), but the decreased the atherogenic index was not significant (p>0.05). The intervention also significantly reduced the CRP levels (p<0.05) but did not affect HbA1c levels. Body weight, body mass index (BMI), waist circumference decreased significantly after consuming the snacks (p<0.05). Conclusions: This study showed fiber-rich snack has a positive effect in improving non-HDL cholesterol, atherogenic index and CRP levels but does not affect HbA1c levels in T2DM patients.

Abstract Background: Food insecurity can increase risks of health and nutritional problems, leading to difficulties in self-care and poor glycemic control in diabetic patients. Purpose: This study assessed food insecurity and its association with diabetes control and self-care in type 2 diabetes patients.Methods: In this cross sectional study, 148 adults with type 2 diabetes participated. Food insecurity and self-care were determined using the 18-item USDA household food security status questionnaire and self-care (SDSCA) questionnaire. Data were analyzed using logistic and linear regression tests (SPSS 20 software).Results: Thirty-seven percent of the participants were food-insecure and significantly more likely than food-secure participants to have poor glycemic control (OR=3.02; CI: 1.45-2.65). No significant association was found between food-insecurity and overall self-care score.Conclusion: Food-insecurity was directly associated with poor glycemic control in type 2 diabetes patients. Since economic status was significantly better in the food-secure group, it can be postulated that financial problems will eventually lead to poor glycemic control.Policy strategies to increaseaccess to diabetes-appropriate foods mayreduce socioeconomic inequalities in glycemiccontrol.

Abstract This study aims to assess vitamin D deficiency-induced dyslipidemia and cardiovascular disease (CVD) risk in poor glycemic control among type 2 diabetes mellitus (T2DM) patients. This study was carried out among 455 T2DM patients involving poor glycemic control (n = 247) and good glycemic control (n = 208). Fasting plasma glucose (FPG) and HbA1c were measured to assess glycemic control. Cardiac risk ratio, atherogenic index plasma, and atherogenic coefficient were calculated to assess and compare the CVD risk in different groups. Patients with poor control had a significantly higher level of total cholesterol (TC), triglyceride (TG), and non-high-density lipoprotein lipase cholesterol (non-HDL-C), atherogenic variables, and lower level of high-density lipoprotein lipase cholesterol (HDL-C) as compared to patients with good glycemic control. We also observed significant negative correlation of vitamin D with lipid markers and atherogenic variables in poor glycemic control diabetic population. The serum vitamin D levels were inversely associated with HbA1c, FPG, TG, TC, and non-HDL-C. Furthermore, hypercholesterolemia, hypertriglyceridemia, and elevated non-HDL-C were the independent risks in hypovitaminosis D population. Vitamin D deficiency in poor glycemic control is likely to develop dyslipidemia as compared to vitamin D insufficient and sufficient groups. Thus, vitamin D supplementation and an increase in exposure to sunlight may reduce the risk of cardiovascular complications in diabetes.
