Serum concentrations of adenosine deaminase were determined in 223 febrile patients. In 62, we discovered extrapulmonary tuberculosis. Serum levels of immunoglobulin G were monitored in 287 febrile patients, and 68 had extra-pulmonary tuberculosis. Serum concentrations of adenosine deaminase were significantly higher in patients with tuberculosis compared to other patients with fever of unknown origin. Serum concentrations declined during antituberculosis therapy. A correlation with the localization of infection was not found. Levels of immunoglobulin G were higher in patients with tuberculosis. Both tests had high sensitivity and specificity and could therefore be used for screening extrapulmonary tuberculosis; however, they can only be interpreted adequately following a full clinical investigation.

AbstractHuman cystic echinococcosis is a widespread, chronic, endemic, helminthic zoonosis caused by larval tapeworms of the species Echinococcus granulosus. At present, there is no rational and effective therapy for patients with echinococcosis. The present study evaluated whether the combination of alkaloids from Sophora moorcroftiana seeds (SMSa2) and Bacillus Calmette–Guérin (BCG) was effective in the treatment of experimental echinococcosis. After 20 weeks of secondary infection with protoscoleces, mice were randomly allocated to five groups and treated for 6 weeks by daily intragastric administration of albendazole (ABZ, 100 mg/kg), SMSa2 (100 mg/kg), BCG (abdominal subcutaneous injection at 5 × 106 CFU), SMSa2 + BCG (100 mg/kg SMSa2 and 5 × 106 CFU BCG) or normal saline (untreated group), respectively. The results indicated a significant reduction in the weight of hydatid cysts in the SMSa2 + BCG group compared with the untreated, SMSa2 and BCG groups. The rate of inhibition of hydatid cyst growth in the SMSa2 + BCG group (76.1%) was obviously increased compared with that in the SMSa2 (25.7%) and BCG (26.6%) groups, respectively. Compared with the untreated control, the SMSa2 + BCG group showed a non-significant increase in serum interleukin-4 (IL-4). Furthermore, the serum levels of interferon-γ (IFN-γ) between the untreated and SMSa2 + BCG groups were not statistically different. Therefore, the combination of alkaloids from S. moorcroftiana seeds and BCG can reduce cyst burden and is an effective therapeutic regimen against echinococcosis.

Background: Vitamin D has an essential role in enhancing the immune system to fight and destroy bacteria. People with vitamin D deficiency are highly liable to develop active pulmonary and extrapulmonary tuberculosis. Objective: : To evaluate the serum level of vitamin D among patients with pulmonary and extrapulmonary tuberculosis. Patients and Methods: A case-control prospective study was conducted among patients with pulmonary and extrapulmonary tuberculosis and asymptomatic persons (controls). The study included 80 participants, 50 were TB patients, while 30 were controls. Vitamin D 1, 25(OH) assay was done for all. Results: Vitamin D assay of the included subjects revealed that 52 patients were vitamin D deficient, 7 patients had insufficient vitamin D and 21 had normal serum levels. Of the extrapulmonary TB patients, 35 (100%) had vitamin D deficiency, in pulmonary TB patients, 15 (100%) were vitamin D deficient. While 2 study subjects (6.7%) in the control group were deficient of vitamin D, 7 (23.3%) had insufficient vitamin D and 21cases had normal vitamin D levels. There was a statistically significant relationship between pulmonary and extrapulmonary TB cases and vitamin D deficiency (P< 0.001). Conclusion: Vitamin D deficiency was found among pulmonary and extrapulmonary tuberculosis patients especially among female patients and patients in rural residence. The current study needs additional work to evaluate the value of adding vitamin D to the TB treatment regimen.

Repeated administration of methyl alcohol to Rhesus monkeys (Maccaca mulata) by intragastric tube resulted in ultrastructural abnormalities of hepatocytes, which persisted in one animal twelve weeks after discontinuation of the methyl alcohol regime. With dosages ranging between 3.0 to 6.0 gms. of methanol per kg. of body weight, the serum levels attained within a few hours averaged approximately 475 mg. per cent.
