Epidemiologic evaluation of multiple respiratory pathogens in cats in animal shelters

Upper respiratory tract infection (URI) propagates readily within cats in shelters and often results in euthanasia of affected cats. In a case-control evaluation of 573 cats in eight shelters in California in 2001 and 2002, the prevalence of feline calicivirus (FCV) was from 13 to 36%, feline herpesvirus (FHV) was from 3 to 38%, and prevalence of Bordetella bronchiseptica, Chlamydophila felis, and Mycoplasma species was from 2 to 14%. Cats with URI tended to be housed in isolation, dehydrated, and younger than cats without URI, and infected with FHV, Mycoplasma species, FCV, or C felis. Shelters differed in the prevalence of pathogens and many cats appeared positive for infection after about 1 week of sheltering. It is helpful for shelters to understand the risk factors associated with URI in order to evaluate the costs and benefits of treatment and improve their procedures to decrease the incidence of URI within their facilities. Antiherpetics and antimycoplasmal drugs may be beneficial for individual animal care. Results document the utility of comprehensive URI surveillance and herd management for specific pathogens typical in that shelter.

Abstract Background Acute upper respiratory tract infections are a common cause of Emergency Department (ED) visits and often result in unnecessary antibiotic treatment.  Methods We conducted a randomized clinical trial to evaluate the impact of a rapid, multi-pathogen respiratory panel (RP) test versus usual care (control). Patients were eligible if they were ≥12 months old, had symptoms of upper respiratory infection or influenza like illness, and were not on antibiotics. The primary outcome was antibiotic prescription; secondary outcomes included antiviral prescription, disposition, and length of stay (ClinicalTrials.gov# NCT02957136). Results Of 191 patients enrolled, 93 (49%) received RP testing; 98 (51%) received usual care. Fifty-three (57%) RP and 7 (7%) control patients had a virus detected and reported during the ED visit (p=0.0001). Twenty (22%) RP patients and 33 (34%) usual care patients received antibiotics during the ED visit (-12% [95% CI -25%, 0.4%]; p=0.06/0.08); 9 RP patients received antibiotics despite having a virus detected. The magnitude of antibiotic reduction was greater in children (-19%) versus adults (-9%; post-hoc analysis). There was no difference in antiviral use, length of stay, or disposition. Conclusions Rapid RP testing was associated with a trend towards decreased antibiotic use, suggesting a potential benefit from more rapid viral tests in the ED. Future studies should determine if specific groups are more likely to benefit from testing and evaluate relative cost and effectiveness of broad testing, focused testing, and a combined diagnostic and antimicrobial stewardship approach.

Objectives Our objective was to perform the first multicentric study in Spain to evaluate the prevalence of feline herpesvirus-1 (FHV-1), feline calicivirus (FCV), Chlamydophila felis and Mycoplasma felis in cats with upper respiratory tract disease (URTD), conjunctivitis and/or gingivostomatitis (GS) compared with control cats; and to evaluate risk factors for these clinical conditions. Methods Conjunctival and oropharyngeal swabs were collected and a questionnaire regarding signalment, lifestyle, vaccination history and clinical signs was obtained for each cat. Swabs were tested for each pathogen by real-time PCR. Results The study population consisted of 358 cats, including 98 control cats. Among the 260 diseased cats, 127 cats presented with URTD, 149 cats had conjunctivitis, 154 cats were suffering GS; many cats presented more than one clinical condition. The prevalence observed of FHV-1, FCV, C felis and M felis was, respectively, 28.3%, 48.0%, 20.5% and 46.5% in cats with URTD; 24.2%, 43.6%, 19.5% and 38.3% in cats with conjunctivitis; and 15.6%, 58.4%, 9.1% and 37.7% in cats with GS. Prevalences in the control group were 6.1%, 15.3%, 2.0% and 20.4%, respectively. Coinfections were common among all groups of cats. Risk factors were identified for all groups. FHV-1, FCV and C felis were associated with URTD and conjunctivitis. FCV was strongly associated with GS. M felis was present in a high percentage of the population in all groups, but its role in these clinical conditions remains uncertain. Vaccination was protective for URTD and GS but not for conjunctivitis. Conclusions and relevance This epidemiological study describes, for the first time, prevalence for FHV-1, FCV, C felis and M felis in Spain. In general, the prevalences found are similar to those reported in other countries. Factors associated with disease expression were also identified, which are relevant for practitioners.

Background The risk and characteristics of upper respiratory tract (URT) bacterial infections (URT-BI) among HIV (+) patients is understudied. We analyzed factors associated with its occurrence and the spectrum of pathogens among patients routinely followed at the HIV Out-Patient Clinic in Warsaw. Methods All symptomatic HIV (+) patients with available URT swab culture were included into analyses. Patients were followed from the day of registration in the clinic until first positive URT swab culture or last clinical visit. Cox proportional hazard models were used to identify factors associated with positive URT swabs culture (those with p<0.1 in univariate included into multivariable). Results In total 474 patients were included into the analyses, 166 with positive URT swab. In general 416 (87.8%) patients were male, 342 (72.1%) were infected through MSM contact, 253 (53.4%) were on antiretroviral therapy. Median follow-up time was 3.4 (1.3-5.7) years, age 35.2 (30.6-42.6) years and CD4+ count 528 (400-685) cells/μl. The most common pathogens were S. aureus (40.4%) and S. pyogenes (13.9%) (Table 1). Patients with URT-BI were more likely to be MSM (68.5% vs 78.9%; p<0.016), have detectable viral load (20.9% vs 12.0%; p<0.0001) and CD4+ cell count <500 cells/μl (55.2% vs 39.0%; p=0.003) (Table 2). In multivariate survival analyses detectable viral load (HR3.13; 95%Cl: 2.34-4.19) and MSM (1.63;1.09-2.42) were increasing, but older age (0.63;0.58-0.69, per 5 years older) and higher CD4+ count (0.90;0.85-0.95, per 100 cells/μl) decreasing the risk of URT-BI (Table 2). Conclusions URT BI are common among HIV (+) positive patients with high CD4+ count. Similarly to general population most common patogens are S. aureus and S. pyogenes. Risk factors identified in multivariate survival analysis indicate that younger MSM patients with detectable HIV viral load are at highest risk. In clinical practice this group of patients requires special attention.

Bacteriophages - viruses infecting bacteria are the largest known group of viruses, which in their structure mainly have doublestranded genomic DNA, but among them there are also groups with double-stranded RNA and single-stranded DNA and RNA. The total population is about 1031–1032 phages, they play an essential role in the regulation of the world’s number of bacteria. The rather complex and diverse interaction of these representatives of the microcosm continues throughout the history of their existence on our planet. The question of the use of bacteriophages in the treatment of patients with various bacterial infections still remains completely unexplored. The very idea of using these microorganisms for therapeutic purposes dates back to the First World War, when the French biologist and researcher Felix d’Hérelle discovered a special type of «bacteria-eating» viruses, on the basis of which he created drugs for the treatment of patients with dysentery. To date, a fairly large clinical experience has been accumulated in the use of phage preparations in the treatment of infectious and inflammatory diseases of the respiratory, gastrointestinal, urogenital tract, as well as in the therapy and prevention of purulent-septic processes and nosocomial infections. The mucous membrane of the upper respiratory tract is the first line of defense against various respiratory pathogens. The ability of bacteriophages to attach to the surface layer of mucus - mucin, forming an antibacterial protection of the mucous membrane and thus reducing the level of colonization of mucus by bacterial pathogens, determines their effective use in the treatment and prevention of inflammatory diseases of the upper respiratory tract. Due to certain unique properties of bacteriophages, peculiarities of vital activity and interaction with a bacterial cell, their use seems to be promising for the treatment of patients with infectious diseases of the upper respiratory tract.

Abstract Background: Feline herpesvirus-1 (FHV-1) is a most common virus that cause viral rhinotracheitis and ocular diseases in domestic cats and wild felids. As other alpha-herpesviruses, acute FHV-1 infection is responsible for severe upper respiratory tract and ocular disease, followed by lifelong latency that persist the limited virus in sensory neuronal cells. While latency reactivation can result in recrudescence, leading sever ocular lesions. Hence, FHV-1 infection in cats can be considered as a good natural host model to study alpha-herpesvirus pathogenesis. Results: In this study, the FHV-1 CH-B was isolated from nasal discharge collected from a British shorthair cat in China, and was further identified via transmission electron microscopy (TEM) observation, indirect immunofluorescence assay (IFA) and genome analysis. Experimental infection of domestic cats with different dose of isolate CH-B, ranging from 104 to 107 TCID50, showed that cats inoculated with 105 TCID50 not only showed typical upper respiratory track and ocular symptoms, but also could copy the progress of disease development. Therefore, the FHV-1 infection model was established by intranasally inoculated with 105 TCID50 of FHV-1 isolate CH-B. Infected cats began to show clinical signs at days 5 post inoculated (dpi), developed severe upper respiratory tract and ocular symptoms at 10-15 dpi, began to recover at 20 dpi, and recovered almost completely by 25 dpi. During acute infection period, virus mainly replicates in turbinate, conjunctiva, cornea and sensory neuronal cells, while virus only persists in trigeminal ganglia (TG) at lifelong latency. Viremia and viral infections in lungs do not appear in FHV-1 CH-B infected cats, with only one exception. We also demonstrate that FHV-1 CH-B infection can induce severe inflammatory responses and lung, trachea, and tonsils tissues damage in cats. In addition, we found that FHV-1 infected cats can shed virus via nasal and ocular discharge, resulting FHV-1 infection in in-contact cats. Conclusions: This natural host model of FHV-1 infection will be valuable for the screen and assessment of antiviral drugs and vaccines, as well as the studies of the pathogenesis of alpha-herpesvirus infection in animals and humans.

Feline herpesvirus is widespread and initiates respiratory infections in representatives of the feline family, including in Moscow. The article shows that acute herpesvirus infection in cats occurs with upper respiratory tract syndrome, conjunctivitis, and fever. Sick cats of various breeds and contents were identified, but more often they get sick at the age of 2 months to a year. The virus is transmitted aerogenically and with secretions from the nose, mouth, eyes when sick animals come into contact with healthy ones. Various stress factors, colds, cause the reactivation of the virus also with release into the environment. Therefore, the disease is more often recorded in the cold season (homeless animals) and during the rainy season it lasts 7–10 days but a relapse or transition to a chronic form is possible. With the group keeping of cats (shelter), the disease can significantly spread and acquire the character of an epizootic. PCR is an objective assessment criterion for virus detection and differential diagnosis.
