Clinical trial with continuous low dosage (0·5 mg) chlormadinone acetate

Chlormadinone acetate (0·5 mg) was given continuously to 200 patients for 1512 cycles in an investigation lasting a little over 2 years. Only a small number of the women stayed in the trial for the whole period. The women were in the reproductive age group and of proved fertility. Lack of cycle control was the chief drawback. However, the majority of the cycles were of normal duration, intensity and flow. Breakthrough bleeding occurred in 3·8% of the total cycles. Of the patients 49% had side effects in 18·6% of the cycles. They were common in the first 3 months but were mild. No pregnancy occurred due to method failure. Six pregnancies occurred due to tablet omission on 2 or more days. A heavy drop-out rate of 78·5% was found. The majority of the patients who dropped out did so for reasons not related to the drug; only 14% of the patients dropped out due to side effects. Vaginal cytohormonal study, premenstrual endometrial biopsies and postcoital tests were carried out during the trial to study the mode of action of the drug.

Background: Heavy menstrual bleeding (HMB) is one of the commonest presenting complaints in reproductive age group. Although combined oral contraceptives (COCs) are commonly used in such patients, combined hormones by intravaginal route has been found acceptable and effective. Aim of the study is to compare the efficacy and side effects of combined intravaginal hormonal ring (IHR) with COCs in control of HMB in these patients.Methods: Hundred women with HMB fulfilling inclusion criteria were randomized into two equal groups and treated with either IHR or COCs for three cycles. Each cycle consisted of three weeks of IHR/COC use followed by 1-week ring-free/non-hormonal pills period. Outcome measures were change in PBAC score (pictorial blood loss assessment chart), hemoglobin rise, side effects and overall patient satisfaction.Results: The percentage reduction in PBAC score, the duration of menses and increase in hemoglobin levels were statistically significant at the end of study in each group. The PBAC score reduction was 87.37% vs 61.52%, menses duration was 4.24±0.74 versus 5.16±1.67, and hemoglobin increase was 3.16 (95% CI:0.142-1.412) and 1.24 (95% CI:1.048-1.640) in the IHR versus COC group. However, the intergroup reduction of mean PBAC score was not statistically significant. Significantly more ring users were satisfied and elected to continue with treatment.Conclusions: Both the IHR and COCs are effective treatments for HMB in reproductive age group. IHR may be an attractive option for HMB due to better compliance and lesser systemic side-effects.

Background: India with 1.35 billion people is the second most populated country in the world next to China. Total fertility rate of India is 2.2 and the current total fertility rate of Jammu and Kashmir is 1.7 which is still lagging behind various states. This study was conducted to investigate the knowledge, attitude and practices of contraception in women of reproductive age in this set up.Methods: The present cross-sectional study was carried out in patients attending OPD (outpatient department) of Obstetrics and Gynecology at ASCOMS, Jammu, Jammu and Kashmir, India from 1st September 2017 to 31st August 2018. 200 married women aged between 18-49 years were included in this study. Socio-demographic characteristics of the women, their knowledge, attitude and practices on contraception were evaluated with the help of predesigned questionnaire.Results: Out of 200 women, maximum respondents belong to age group of 21-25 years (75.6%). Most of the contraceptive non-users belongs to age below 20 years (62.5%). Majority of respondents were Hindu (70.37%), maximum studied up to secondary level education (84.61%), 70.30% were housewives and 56.58% belongs to middle class. In 45% of respondents, media was the main source of information. Preferred spacing method was condom in 85% of cases. 68% women had knowledge of female sterilization. According to most women, family planning methods are meant for limitations of birth (43%) and 36% meant for spacing of birth. 80% women had positive attitude towards contraceptive usage. 50% experienced side effects with the use of contraceptives. The most common side effect was menstrual irregularities in 25% of cases. Most common reason for not using contraceptive methods among non-respondents were lack of knowledge in 50% of cases.Conclusions: The study showed that inspite of having good knowledge, utilization of contraceptives were less because of large family norm, religious myth, cultural and political barriers. Ignorance regarding use and side effects of various contraceptive methods is another reason for inadequate practice of family planning methods.

Background. Iodine deficiency disorder is a major public health problem in Ethiopia that is more common in women of reproductive age. However, it is not well addressed and there is a lack of information on its prevalence and associated factors in women of reproductive age group. Therefore, the objective of this study was to assess goiter prevalence and associated factors among women of reproductive age in the Demba Gofa woreda, Gamo Gofa Zone, Southwest Ethiopia. Methods. A community-based cross-sectional study was used among 584 randomly selected women in the reproductive age group from February 05 to April 20, 2016. A simple random sampling technique was used to select the study kebeles, and a systematic random sampling technique was used to select the study samples. Data were collected through a pretested questionnaire, and the goiter examination was done clinically for each participant. The collected data were coded and entered into a computer for statistical analysis using EpiData version 3.2 and analyzed using SPSS version 20. Variables with a P value ≤0.25 in bivariate logistic regression analysis were entered into multivariate logistic regression analysis, and finally, variables with a P value <0.05 in multivariate logistic regression were considered significantly associated with the dependent variable. Results. The total goiter rate was 43%, 95% CI = 39.2–46.9. Cassava consumption (AOR: 2.02, 95% CI: 1.03–4), salt wash before use (AOR: 3.14, 95% CI: 1.1–11.3), salt use after >2 months of purchase (AOR: 11, 95% CI: 5–26), family history of goiter (AOR: 4.6, 95% CI: 1.4–15.8), and poor knowledge of iodized salt (AOR: 2.7, 95% CI: 1.4–5.5) were significant factors associated with goiter. Conclusion. Iodine deficiency was found to be severe in women of reproductive age in the study area. This showed that women of reproductive age, especially during pregnancy, are exposed to iodine deficiency and its adverse effects at delivery. Thus, they need urgent supplementation with iodine, improved access to foods rich in iodine, and intake of iodized salt. Additionally, health education should focus on the importance of iodized salt, the proper method of use, and the prevention of iodine deficiency, which are highly recommended to minimize the problem.

Abstract BackgroundThe aim of the preconception care (PCC) is to promote the health of women in the reproductive age before the conception in order to reduce preventable poor pregnancy outcomes. Although there are several published primary studies from Sub Saharan African (SSA) countries on PCC, they do not quantify the extent of PCC Utilisation, Knowledge level about PCC and the association between them among women in the reproductive age group in this region. This systematic review and meta-analysis aimed to estimate the pooled utilisation of PCC, Pooled knowledge level about PCC and their association among women in the reproductive age group in Sub Saharan Africa.MethodsDatabases including PubMed, Science Direct, Hinari, Google Scholar and Cochrane library were systematically searched for literature. Additionally, the references of appended articles were checked for further possible sources. The Cochrane Q test statistics and I2 tests were used to assess the heterogeneity of the included studies. A random-effect meta-analysis model was used to estimate pooled the prevalence of PCC, knowledge level of PCC and their correlation among reproductive aged women in Sub-Saharan African countries.ResultsOf the identified 1593 articles, 20 studies were included in the final analysis. The pooled utilisation of PCC and good knowledge level towards PCC among women of reproductive age group in Sub Saharan Africa were found to be 24.05% (95% CI: 16.61, 31.49) and 33.27% (95% CI: 24.78, 41.77), respectively. Women in the reproductive age group with good knowledge level were two times more likely to utilize the PCC than the women with poor knowledge level in Sub Saharan Africa (OR: 2.35, 95% CI: 1.16, 4.76). ConclusionIn SSA Countries, the utilisation of PCC and knowledge towards PCC were low. Additionally, the current meta-analysis found good knowledge level to be significantly associated with utilisation of PCC among women in the reproductive age. These findings indicate that, it is imperative to launch programs to uplift knowledge level about PCC utilisation among women in reproductive age group in SSA countries.
