Objectives: The aim of this study was to estimate the relationship between the financial impact of a new drug and the recommendation for reimbursement by the Australian Pharmaceutical Benefits Advisory Committee (PBAC).Methods: Data in the PBAC summary database were abstracted for decisions made between July 2005 and November 2009. Financial impact—the upper bound of the values presented in the PBAC summary database—was categorized as ≤A$0, >A$0 up to A$10 million, A$10 million up to A$30 million, and >A$30 million per year. Descriptive, logistic, survival, and recursive partitioning decision analyses were used to estimate the relationship between the financial impact of a new drug indication and the recommendation for reimbursement. Multivariable analyses controlled for other clinical and economic variables, including cost per quality-adjusted life-year gained.Results: Financial impact was a significant predictor of the recommendation for reimbursement. In the logistic analysis, the odds ratios of reimbursement for drug submissions with financial impacts ≥A$10 million to ≥A$30 million or >A$0 to <A$10 million compared with ≤A$0 were 0.12 (95 percent confidence interval [CI]: 0.03–0.51) and 0.16 (95 percent CI: 0.04–0.60), respectively. In the recursive partition decision analysis, the first split of the data was for submissions with a positive financial impact compared with those with a zero or negative financial impact.Conclusions: In Australia, financial impact on the drug budget is an important determinant of whether a new drug is recommended for reimbursement when cost-effectiveness estimates and other clinical and economic variables are controlled.

Background: Chronic active lesions with iron rims have prognostic implications in patients with multiple sclerosis. Objective: To assess the relationship between iron rims and levels of chitinase 3-like 1 (CHI3L1), neurofilament light chain (NfL) and glial fibrillary acidic protein (GFAP) in patients with a first demyelinating event. Methods: Iron rims were identified using 3T susceptibility-weighted imaging. Serum NfL and GFAP levels were measured by single-molecule array assays. CSF (cerebrospinal fluid) CHI3L1 levels were measured by enzyme-linked immunosorbent assay (ELISA). Results: Sixty-one patients were included in the study. The presence of iron rims was associated with higher T2 lesion volume and higher number of gadolinium-enhancing lesions. In univariable analysis, having ⩾2 iron rims (vs 0) was associated with increased CSF CHI3L1 levels (β = 1.41; 95% confidence interval (CI) = 1.10–1.79; p < 0.01) and serum NfL levels (β = 2.30; 95% CI = 1.47–3.60; p < 0.01). In multivariable analysis, however, only CSF CHI3L1 levels remained significantly associated with the presence of iron rim lesions (β = 1.45; 95% CI = 1.11–1.90; p < 0.01). The presence of ⩾2 iron rims was not associated with increased serum GFAP levels in univariable or multivariable analyses. Conclusion: These findings support an important contribution of activated microglia/macrophages to the pathophysiology of chronic active lesions with iron rims in patients with a first demyelinating event.

Nicotine and cannabis use are common among adolescents and may be associated with behavioral problems, poor academic outcomes and use disorders. The goals of this analysis were the following: (1) Describe the influence of friends’ nicotine and cannabis smoking and vaping on self-reported use. (2) Describe the relationship between friends’ nicotine and cannabis use on participants’ urinary biomarkers of nicotine (cotinine) and cannabis (11-nor-9-carboxy-Δ⁹tetrahydrocannabinol=THC-COOH). This is a secondary analysis of survey and biomarker data collected in adolescents aged 12–21 between April 2017 and April 2018, in Long Island, New York. Bivariate and multivariable analyses were conducted using SPSS 26. A cutoff value of ≥10 ng/mL was used to signify recent usage for urinary cotinine and THC-COOH levels. Over one-third of the 517 surveyed adolescents reported using tobacco and one-third reported using cannabis. A significant relationship between friends’ substance use and self-use was found. For both tobacco and cannabis, over 90% (p < 0.01) of participants with urinary biomarker levels above cutoff had friends who used the respective substance. Friends’ nicotine and friends’ cannabis use were each independently associated with urinary biomarker levels for those substances (for nicotine, beta = 88.29, p = 0.03; for cannabis, beta = 163.58, p = 0.03). Friends’ use of nicotine and cannabis is associated with adolescents’ intake, as well as the physiological exposure to those substances. These findings underscore the importance of including peer influence in the discussion with adolescents about tobacco and cannabis use.

Thousands of subsequent studies have examined the relationship between oil prices and various economic variables, including the stock market. This strand of the literature began with the pioneering work of Kling (1985). Since then, other financial markets, such as banking, have also received a fair share of analysis.

This paper reports on a study that examined the financial impact of an endoluminalgrafting procedure for an abdominal aortic aneurysm using the Mialhe EndoluminalAortic Stentor. Clinical outcomes were not a focus of this study. The results of the studysuggest that financial impacts of new clinical procedures can be understood andaddressed through planning and greater liaison between clinicians, codingprofessionals and clinical costing staff.

PurposeThis paper examines the relationship between the originality of a pharmaceutical innovation and its patent quality. Greater patent quality has been shown in the extant literature to enhance market value, which better enables firms to recoup research and development (R&D) expenditures incurred during the innovation process. Understanding how originality improves patent quality can assist policymakers, when determining the optimal length of pharmaceutical patent protection and/or market exclusivity.Design/methodology/approachThe relationship between originality and patent quality is empirically investigated using a tobit, as well as a zero-inflated negative binomial, estimation approach to account for prevalence of patents receiving zero forward citations. Moderating effects of joint innovation, innovation by a university researcher and innovation by an established innovator on originality are also considered.FindingsThere is a robust and positive relationship between patent originality and quality in the pharmaceutical sector. This relationship is positively moderated by joint patent ownership with a university. As such, innovators that target originality in new drug development (especially those collaborating with universities) should, according to extant literature, see greater increases in their market value.Originality/valuePolicymakers can use information on the originality of a new drug to discern the optimal length of market exclusivity needed to enable the innovator to recoup expenditures related to R&D. Better predictions of the timing for which firms can recoup R&D expenditures will equip policymakers with knowledge about the appropriate timing to introduce competition into the market, which is critical to reducing the price of pharmaceuticals to consumers.

Introduction: Drinking alcohol before sleeping (Ne-Zake) or in the morning (Mukae-Zake) is occasionally seen in Japan although it is well known that drinking alcohol have some negative effects on sleep. There have been studies which indicated that alcohol worsen sleep quality and quantity however few which investigated how alcohol before sleeping and no which noticed on how alcohol in the morning affects. The purpose of this study is to verify the relationship between Ne-Zake and/or Mukae-Zake and insomnia adjusting the influence of quality and frequency of drinking on the relationship. Hypothesis: We assessed the hypothesis that there is a significant association between Ne-Zake and/or Mukae-Zake and sleeplessness but this association can be confounded by quality and frequency of drinking. Method: Seven hundred and forty-six farmers who underwent annual medical checkups were analyzed. Ne-Zake was defined as “drinking as a sleeping aid” and Mukae-Zake was defined as “drinking in order to get calm down or to fix hungover”. Athene Insomnia Scale (AIS) was used to evaluate objective sleeplessness. AIS consists of 8 questions, which were evaluated from 0 (no sleeplessness) to 3 points (severe sleeplessness), asking the sleeping status in the previous 2 weeks. Insomnia was defined by AIS point more than 6. We conducted the multivariable logistic analysis, setting the presence of insomnia as a dependent variable and the presence of Ne-Zake and Mukae-Zake, quantity of alcohol at one time, frequency of drinking, sleeping related diseases (sleep apnea syndrome, restless leg syndrome, narcolepsy, limb movement disorder, depression), gender, and age as independent variables. Results: Among a total of 746 farmers (mean age 59.5 years; 74.1% men), insomnia, Ne-Zake, and Mukae-Zake were observed in 151 (20.2%), 140 (18.8%) and 37 (5.0%), respectively. Adjusted odds ratios (95% Confidence Intervals) of Ne-Zake and Mukae-Zake were 1.75 (1.07-2.88) and 3.01 (1.41-6.42). The association of Ne-Zake was consistent between men and women but that of Mukae-Zake was stronger in men than in women. (P-values for interaction = 0.98 and 0.03, respectively). Conclusion: Both Ne-Zake and Mukae-Zake had statistically positive significant association with insomnia independent of quantity and frequency of drinking. It may indicate that the timing of drinking alcohol has a stronger relationship with insomnia than quantity and frequency of drinking does. The feature of farmers’ lives in which they can decide their life style might give more opportunities to have Ne-Zake and/or Mukae-Zake than groups with well-scheduled working environment. This result can be utilized to improve farmers’ further physical and psychological health related to alcohol by encouraging to pay attention to not only the quantity or frequency of alcohol drinking but also the timing in a day.

Earlier postpartum cyclicity based on hormonal analysis was related to the subsequent fertility, although it has not been clarified under field conditions. This field study examined the relationship between the detection of corpus luteum (CL) at the first post partum month and subsequent reproductive performance in dairy cows. Holstein dairy cows underwent ultrasonography (Sonoace 600 with 7.5 MHz linear-array transducer; Medison Co. Ltd., Korea) at 30 (±7) days postpartum to determine the existence of CL in ovaries and then divided into 2 groups based on the existence of CL; the CL group (n = 156) or the non-CL group (n = 281). At the same time of ultrasonography, all cows were scored for body condition. Cows received the normal herd reproductive management program including oestrous detection or synchronization, or synchronization of ovulation in the breeding period. Pregnancy was determined per rectum 60 days after artificial insemination by both ultrasonography and palpation. Reproductive performance data were collected for a minimum of 210 days postpartum. Reproduction data between the CL and non-CL groups were evaluated by t-test, chi-square test, or logistic regression using an SAS program (Version 9.1: SAS Institute, Inc., Cary, NC, USA). The proportion of cows with CL in ovaries at 30 (±7) days postpartum was 35.7% of all cows. The number of days to first insemination after calving was more delayed (P < 0.0001) in the non-CL group (100.1 ± 2.7 days) than in the CL group (80.9 ± 2.9 days), while the pregnancy rate to first insemination did not differ (P > 0.05) between the CL group (44.2%) and the non-CL group (45.9%). The CL group had higher pregnancy proportions (PP) within 60 (odds ratio, OR = 2.3; P = 0.0063), 90 (OR = 1.6; P = 0.04), and 120 days (OR = 1.5; P = 0.05) postpartum than the non-CL group; however, the PP within 150 or 210 days postpartum did not differ between the 2 groups (P > 0.05). Besides, logistic analysis revealed that cows with body condition score (BCS) ≥3.00 were more likely to have CL (OR = 1.8; P = 0.015) compared with cows with BCS ≤ 2.75, while primiparous cows were less likely to have CL (OR = 0.6; P = 0.0085) compared with multiparous cows. In addition, cows that underwent ultrasonography later than 30 days postpartum were more likely to have CL (OR = 1.6; P = 0.0192) compared with cows that underwent ultrasonography earlier. In conclusion, the earlier cyclicity based on the detection of CL using ultrasonography at the first postpartum month, which might be in part due to a higher BCS, predicts an enhanced reproductive performance in dairy cows, particularly during the early and early-mid lactations. Using a large special study population and refined indicators of socio-economic status, the relationship of the live birth sex ratio to socio economic variables is examined. The analyses provide control for the possible confounding effects of race and birth order.The sex ratio is shown to be significantly and positively associated with socio-economic variables. The relationship is not linear, however, but instead is characterized by an apparent ‘diminishing returns’ curve. The major increase in sex ratio is detected as socio-economic status increases from low to moderate. No significant change occurs between moderate and higher socio-economic levels.Even after controlling for the significant socio-economic effect, the analyses detect a significant difference between the sex ratios of Whites on the one hand and Negroes and Puerto Ricans on the other.No significant association between birth order and the sex ratio can be detected in the study population.
