BACKGROUND Transgender children and adolescents (ie, those who experience incongruence between assigned sex at birth and internal gender identity) are poorly understood and an understudied population in the United States. Since 2008, medical care for transgender youth has generally followed guidelines developed by professional consensus, given the paucity of empirical research, particularly in the US setting. OBJECTIVE The objective of this research was to provide evidence-based data to inform clinical care for transgender youth. The study aims (1) to evaluate the impact of gonadotropin-releasing hormone agonists administered for puberty suppression on mental health, psychological well-being, and metabolic and physiologic parameters including bone health in a cohort of children and adolescents (Tanner stages 2-4) with gender dysphoria, comparing baseline and follow-up assessments, and (2) to determine the impact of gender-affirming hormones (eg, estradiol and testosterone) administered for phenotypic gender transition on mental health, psychological well-being, and metabolic and physiologic parameters in a cohort of adolescents with gender dysphoria, comparing baseline and follow-up assessments. METHODS The study uses a longitudinal observational design to examine the outcomes of existing medical treatment protocols for gender dysphoria in two distinct cohorts: youth initiating puberty suppression and youth pursuing a phenotypic gender transition. Data on routine anthropometric and physiologic parameters are collected through chart abstraction, questionnaires, and research interviews in the 24-month study period. Audio computer-assisted self-interview and individual interview survey instruments are used to collect demographic, mental health, psychosocial, and behavioral data from parents and youth in the blocker cohort and only from youth in the gender-affirming hormone cohort at baseline and 6, 12, 18, and 24 months. RESULTS Participant recruitment commenced in July 2016, and enrollment was completed in September 2018. A total of 90 participants were enrolled in the blocker cohort and 301 participants were enrolled in the gender-affirming hormone cohort. Findings based on baseline data are expected to be submitted for publication in 2019. CONCLUSIONS This longitudinal, observational study is collecting critical data on the existing models of care for transgender youth that have been used in clinical settings for close to a decade, although with limited empirical research to support them. This research is a direct response to the Institute of Medicine report calling for such studies as well as the needs of clinicians and patients. Results from this study have the potential to significantly impact the medical and mental health services provided to transgender youth by making available rigorous scientific evidence on the impact and safety of early treatment based on the sexual development stage. Ultimately, we aim to understand if early medical intervention reduces the health disparities well known to disproportionately affect transgender individuals across their lifespan. This article investigates the impact of COVID-19 on the health and lived experiences of the transgender community in India. In particular, the study analyses how COVID-19 affects the lives of the transgender community in terms of their interaction with the government policymaking and identity negotiation, livelihoods, access to health resources, availability of gender transition services and status of mental health. The analysis shows that the COVID-19 pandemic and the government strategies to curb the spread of infection have exacerbated the challenges faced by the transgender community in India and threatened their survival. As disease outbreaks traditionally perpetuate gender inequities and increase vulnerabilities borne by marginalised groups, the analysis has implications for purposefully creating appropriate platforms for participation of the transgender community and designing and delivering suitable programs and services to enhance their welling and human dignity.

This article first examines the author’s positionality with reference to the historical and inter-generational transmission of Asian trauma, the contemporary plight of North Koreans, and the betrayal of anatomically-female individuals (including those who are sexual minority/gender-variant) within Asian heteropatriarchal systems. An analysis of the relevance of empirical research on low-income trans/gender-variant people of color is then discussed, along with an examination of HIV and health disparities in relation to the socio-economic positioning of low-income trans/gender-variant people of color and sexual minority women, and how social contexts often gives rise to gender identity, including transmasculine identities. What next follows is an appeal to feminist and queer/trans studies to truly integrate those located on the lowest socio-economic echelons. The final section interrogates concepts of health, well-being, and happiness and how an incorporation of the most highly disenfranchised/marginalized communities and populations challenges us to consider more expansive visions of social transformation.

Drawing on transgender, queer and feminist theoretical perspectives, I critically analyze two children’s picture books featuring transgender and gender variant characters. With these critical theoretical perspectives in mind, this discourse analysis examines the ways the books, both visually and textually, depict gender embodiment and the experiences of the characters. Using questions derived from these theoretical lenses, I analyze concepts of power, normalcy, difference, the gender binary, gender fluidity, intelligibility and unintelligibility. These concepts contribute to the dominant discourse of ‘the gaze’, seen in varying ways in the books. Children’s story books largely underrepresent the experiences of transgender characters, particularly books outlining, and explaining, a social gender transition. The majority of picture books with LGBTQ+ themes focus on same sex families and feature boys in dresses, thus centralize around disrupting the constraints of masculinity. I conclude this paper with recommendations for selecting, reading, and discussing books with transgender and gender variant protagonists. The central themes outlined in the academic literature illustrate that ‘the gaze’ and regulation of knowledge have a significant impact on what is visible in children’s books. This may ultimately affect children’s understanding, and appreciation, of gender variance and, hence, social gender transitions in early childhood.

A dominant media narrative of “getting better” over time is often projected onto LGBTQ people’s personal life experiences. In this research study, I examine this narrative’s role in transgender people’s emotional well-being throughout transition. A “getting better” narrative was pervasive in my qualitative analysis of 240 Tumblr transition blogs and 20 interviews with bloggers, signaling that it impacted people’s self-concept both as presented on social media and when talking about their experiences. This narrative causes undue emotional harm given contrast between one’s post-transition reality, which may involve distress (despite greater congruence between one’s body and identity), and a dominant cultural expectation of happiness. I argue that an intersectional approach to understanding trans people’s emotional well-being—by considering multiple salient identity facets and life transitions—makes trans lives more livable by complicating the cultural imperative to feel better, and to present a unilaterally positive self-image online, post-transition. Even though trans people on average feel better after gender transition, everyday realities are often in contrast to the dominant narrative’s positioning of gender transition as a process with a single, simple goal of feeling better. Challenging the “getting better” narrative gives trans people the freedom to live and exist in their post-transition identities, whether or not they feel “better.”

Abstract A recent spate of legislation such as South Dakota's HB 1057, known as the Vulnerable Child Protection Act, seeks to criminalize medical treatment for gender transition in minors under sixteen. This essay argues that these laws do not safeguard children's health but are part of a broader attack on transgender rights that uses the protection of children as a powerful pretense to scapegoat a minority. It suggests that the analyses and insights of the field of transgender studies could inform, enrich, and reconfigure current clinical and public-policy debates around gender-variant children. This essay also aims at drawing the attention of supporters of transgender children to aspects of current medical treatments and their potential implications for young people that might get lost in this explosive political climate.

Purpose Hearing loss self-management refers to the knowledge and skills people use to manage the effects of hearing loss on all aspects of their daily lives. The purpose of this study was to investigate the relationship between self-reported hearing loss self-management and hearing aid benefit and satisfaction. Method Thirty-seven adults with hearing loss, all of whom were current users of bilateral hearing aids, participated in this observational study. The participants completed self-report inventories probing their hearing loss self-management and hearing aid benefit and satisfaction. Correlation analysis was used to investigate the relationship between individual domains of hearing loss self-management and hearing aid benefit and satisfaction. Results Participants who reported better self-management of the effects of their hearing loss on their emotional well-being and social participation were more likely to report less aided listening difficulty in noisy and reverberant environments and greater satisfaction with the effect of their hearing aids on their self-image. Participants who reported better self-management in the areas of adhering to treatment, participating in shared decision making, accessing services and resources, attending appointments, and monitoring for changes in their hearing and functional status were more likely to report greater satisfaction with the sound quality and performance of their hearing aids. Conclusion Study findings highlight the potential for using information about a patient's hearing loss self-management in different domains as part of clinical decision making and management planning.

Because of the increasing number of transgender people requesting speech-language pathology services, because having gender-incongruent voice and communication has major negative impacts on an individual's social participation and well-being, and because voice and communication training is supported by an improving evidence-base, it is becoming more common for universities to include transgender-specific theoretical and clinical components in their speech-language pathology programs. This paper describes the theoretical and clinical education provided to speech-language pathology students at La Trobe University in Australia, with a particular focus on the voice and communication training program offered by the La Trobe Communication Clinic. Further research is required to determine the outcomes of the clinic's training program in terms of student confidence and competence as well as the effectiveness of training for transgender clients.

Current research on feeding outcomes after discharge from the neonatal intensive care unit (NICU) suggests a need to critically look at the early underpinnings of persistent feeding problems in extremely preterm infants. Concepts of dynamic systems theory and sensitive care-giving are used to describe the specialized needs of this fragile population related to the emergence of safe and successful feeding and swallowing. Focusing on the infant as a co-regulatory partner and embracing a framework of an infant-driven, versus volume-driven, feeding approach are highlighted as best supporting the preterm infant's developmental strivings and long-term well-being.
