Abstract This study aimed to investigate the occurrence of tonsillectomy in asthmatic children using a control group with a comparable frequency of a preoperative history of asthma. Asthmatic children ≤15 years old were collected from the Korean Health Insurance Review and Assessment Service - National Sample Cohort (HIRA-NSC) from 2002 through 2013. In study I, asthmatic children who had undergone a tonsillectomy (n = 2,326) and control I participants (n = 9,304) were selected and matched 1:4 for age, sex, income, and region of residence but not a preoperative history of asthma. In study II, a preoperative history of asthma was additionally matched for between the tonsillectomy (n = 2,280) and the new control II participants (n = 9,120). The margin of equivalence of difference (control-tonsillectomy) for asthma was set at −0.05 to 0.05 per year. In addition, repeated measures ANOVA was performed for tonsillectomy according to yearly changes in asthma, status asthmaticus, and admission. In study I, the preoperative frequencies of asthma, status asthmaticus, and admission were higher in the tonsillectomy group than in the control group (P ≤ 0.001). The frequencies of postoperative asthma, status asthmaticus, and admission were lower in the tonsillectomy group than in the control I group for 3 years. In study II, the frequencies of postoperative 1-, 2-, and 3-year asthma and admission were not lower in the tonsillectomy group than in the control II group. Tonsillectomy did not further reduce the frequency of asthma in patients who underwent this procedure compared to the control group when a preoperative history of asthma history was equally matched between the two groups.

Ocular and ophthalmological adverse effects may be seen in β-thalassemia major (BTM) patients treated with regular blood transfusions and iron-chelating agents. We hypothesized that olfactory dysfunction may be present in this population. In this study, we aimed to investigate olfactory dysfunction in patients with BTM and determine the etiological factors. A total of 43 patients with BTM were included in the study. Forty-three patients without any nasal complaints, history of facial trauma, or nasal surgery were included as the controls. All participants had nasal endoscopy. The iron-chelating agents used, their duration of use, as well as hemoglobin and ferritin levels of the BTM patients were recorded. Sniffin’ Sticks test (SST) was used to assess olfactory functions, and BTM and control groups were compared for the results. The correlations of SST scores with the other study parameters were analyzed. Eight (18.6%) of 43 patients in the BTM group had hyposmia while none of the patients in the control group had hyposmia ( P < .001). Older age, low-hemoglobin level, and longer use of deferoxamine were found to be correlated with olfactory dysfunction. Olfactory dysfunction may be seen in BTM patients treated with iron-chelating agents. The results of this study suggest that screening for olfactory function may be needed in routine follow-up of BTM patients.

Introduction: The Human Papilloma Virus (HPV) is the most prevalent sexually transmissible disease. The treatment of Condyloma Accuminatum is a great challenge because of the high recurrence rate and of the lack of any drug efficient in its elimination. Objective: To assess the efficacy of the treatment with Imuno BCG (Bacillus Calmette-Guérin) for bearers of condylomas recurrent for more than 2 years. Patients and Methods: Patients with age between 18 and 60 years, having a history of more than 2 years of genital warts, were included, attended as from 01-08-2011; bearers of serious diseases, immunodeficiency or users of immune-suppressors, being excluded. The procedure adopted began with a biopsy of the wart(s) with local anesthetic, followed by the electro-coagulation of all the warts and bases of the biopsies, a solution with 80 mg of Imuno BCG dissolved in 2 ml of saline solution 0.9% was applied to all the genital area, including the cauterized areas. The area was covered with plastic for 2 hours and afterwards washed with water. The local application of Imuno BCG was repeated for 8 consecutive weeks. At the end of the second month the cases were re-assessed. When there was clinical recurrence a new procedure with the same drug was undertaken, but with 3 weekly applications made by the patient himself for 8 weeks. Result: Sixteen patients completed 2-year follow-up. The cure rate was of 62.5%. Of the 10 patients cured, 6 (37.5%) used one series of Imuno BCG, 1 (6.25%) used 2 series, and 3 (18.75%) used 3 series. Of the 6 patients who were not cured, there was a reduction in the number of cauterizations from 5.5 times (over an average period of the disease of 51.6 months) to 2.4 cauterizations (over an average follow-up period of 52.3 months) after the use Imuno BCG. The collateral effects of the use of Imuno BCG were insignificant. Conclusion: Topical BCG is a good option for the treatment of recurrent condylomas, with minimal collateral effect. It may be used even on PPD-negative patients. However, this result must be confirmed with larger sample populations and control-group studies.

The aim of this research is to investigate the effect of the Dealing with Homophobia Psycho-Education Program on homophobia levels of psychological counselor candidates. This research was conducted with a mixed model and utilized quantitative and qualitative methods. 2 (experimental and control groups) x 3 (pre-test, post-test, follow-up) research design which is a type of quasi-experimental design was used and content analysis was applied to the data obtained via interviews. The study was conducted with a total of 24 psychological counselor candidates, 12 in the experimental group and, 12 in the control group. The Homophobia Scale was used to determine the homophobia levels of psychological counselor candidates. The Dealing with Homophobia Psycho-Education Program was developed by the researcher. In order to determine the effectiveness of the program, The Two-Way Analysis of Variance with Repeated Measures was used. As a result of the research, it was determined that the Dealing with Homophobia Psycho-Education Program is effective in decreasing the levels of homophobia of the psychological counselor candidates. Also, according to the participant&#39;s views, it was determined that the psycho-education program was effective in decreasing homophobia and caused changes in attitudes. The importance and necessity of using the program on psychological counselor education are discussed.

ObjectiveTo evaluate the risk of spine fracture in patients with mood disorder using a nationwide cohort.DesignA longitudinal follow-up study.SettingClaims data for the population ≥20 years of age were collected from 2002 to 2013 for the Korean National Health Insurance Service-National Sample Cohort.ParticipantsA total of 60 140 individuals with mood disorder were matched with 240 560 individuals (control group) for age, sex, income, region of residence and osteoporosis.InterventionsIn both the mood disorder and control groups, the history of spine fracture was evaluated. The International Classification of Diseases 10th Revision codes for mood disorder (F31–F39) and spine fracture (S220 and S320) were included.Primary and secondary outcome measuresThe univariable and multivariable HRs and 95% CIs of spine fracture for patients with mood disorder were analysed using a stratified Cox proportional hazards model. Subgroup analyses were conducted according to the history of osteoporosis, age and sex.ResultsApproximately 3.3% (2011/60 140) of patients in the mood disorder group and 2.8% (6795/240 560) of individuals in the control group had spine fracture (p<0.001). The mood disorder group demonstrated a higher adjusted HR for spine fracture than the control group (multivariable HR=1.10, 95% CI 1.04 to 1.15, p<0.001). The participants without osteoporosis showed a higher HR of mood disorder for spine fracture than the control participants (multivariable HR=1.25, 95% CI 1.14 to 1.37, p<0.001). According to age and sex, this result was consistent in subgroups of women aged 20–39 and 40–59 years and men aged ≥60 years.ConclusionThe risk of spine fracture was increased in patients with mood disorder. The potential risk of spine fracture needs to be evaluated when managing patients with mood disorder.

Aims: Most people in the community believe that substance abuse is masculine, while women are progressing alongside men in this area, with statistics indicating an increasing number of infected women. The purpose of this study was to determine the effectiveness of enriching couples relationships on tolerance of distressed women who were drug abusers Methods & Materials: This is a quasi-experimental study with pre-test, post-test and follow-up test with control group. Initially, 40 women were selected from the study and 20 were randomly assigned to the experimental group and 20 to the control group. Subjects were then assessed using a distress tolerance test (Simmons & Gahr, 2005). Experimental group participants participated in enrichment sessions based on Doba, Graham, Britz and Minatra (2009) choice theory and control group subjects remained on the waiting list. A follow-up test was taken after three months. Multivariate analysis of variance with repeated measures was used to analyze the data. The above analyzes were performed with SPSS V. 24 software Findings: The results indicated that there was a significant difference between the mean of pre-test, posttest and follow-up scores of distress tolerance scales in the experimental and control groups. (P<0.05). The mean distress tolerance in the experimental group increased from 34.533 to 42.80 in the pre-test phase and reached 42.533 in the follow-up phase. Also, the significant interaction between the stages with the experimental group in the mentioned variable indicates that in the post-test and follow-up stages, the mean of the experimental groups was significantly higher than the control group. Conclusion: Due to the increasing trend of addiction in women, proper education is one of the essentials. Therefore, it is recommended to take effective steps to improve couple distress tolerance training by enriching couple relationships

Background: The children’s acute and chronic psychological and physical diseases cause their mothers’ mental health. Therefore, the objective of the present study was to investigate the effect of spirituality therapy on social stigma and worry in the mothers of the children with autism. Methods: It was a quasi-experimental study with pretest, posttest, control group and two-month follow-up period design. The statistical population included the mothers of the children with autism visiting the autism centers in the city of Isfahan in the last three months of the lunar year 1396. Convenient sampling method and random replacement were used in the present study in a way that 40 mothers were selected from the ones with children with autism and they were randomly replaced into experimental and control groups (20 in each group). The experimental group received eight ninety-minute sessions of spirituality therapy interventions during two months. The applied questionnaires included Social Stigma Questionnaire and Worry Questionnaire. The data of the study were analyzed through repeated measures ANOVA and SPSS 23. Results: Results showed that spirituality therapy has influenced social stigma (f = 18.81, P < 0.0001) and worry (f = 24.84, P < 0.0001) in the mothers of the children with autism. Conclusion: According to the findings of the present study, it can be concluded that spirituality therapy using techniques such as knowing values and deep belief in God can decrease social stigma and worry in the mothers of the children with autism.

Objetivos: Comparar la efectividad del entrenamiento de estrategias e intervenciones cognitivo-conductuales basadas en la atención plena en el estrés competitivo de los francotiradores (es decir, nivel de cortisol salival) con un seguimiento de dos meses. Métodos:Veinticuatro francotiradores profesionales varones jóvenes con antecedentes de disparos durante al menos 3 años se dividieron aleatoriamente en tres grupos: intervención cognitivo-conductual (n=8), intervención basada en la atención plena (n=8) y grupo de control (n=8). Los participantes de los grupos experimentales participaron en seis sesiones de formación durante seis semanas (75-90 minutos cada sesión). La concentración de cortisol en saliva se midió en reposo, antes y después de la prueba y el seguimiento. Resultados:ANOVA con medidas repetidas mostró un efecto principal para el grupo (F2.21=3.787, sig=0.039) y etapa (F3.21=52.529, sig=0.001) y también el efecto interactivo entre los grupos en la etapa (F6, 42=3.700, sig=0.005). El ANOVA de medidas repetidas mostró que la intervención cognitivo-conductual a nivel de cortisol salival tuvo un efecto en el postest, mientras que la intervención de mindfulness tuvo efectos positivos en el postest y los tiempos de seguimiento. No hubo diferencias significativas entre los grupos de atención plena y cognitivo-conductual en la prueba posterior. Además, los resultados del tiempo de seguimiento mostraron que hubo una diferencia significativa solo entre la atención plena y el grupo de control. Aplicaciones prácticas:Los resultados indican que la práctica de habilidades psicológicas, especialmente la atención plena, se puede utilizar para reducir el estrés previo a la competencia. Methods: Twenty four young male professional shooters who had a history of shooting for at least 3 years were randomly assigned into three groups: cognitive-behavioral intervention (n=8), mindfulness based intervention (n=8) and control group (n=8). Participants in the experimental groups participated in six training sessions during six weeks (75-90 minutes each session). Salivary cortisol concentration was measured at rest, pre- and post-test and follow-up times. Results: ANOVA with repeated measures showed a main effect for group (F2,21 = 3.787, sig = 0.039) and stage (F3,21 = 52.529, sig = 0.001) and also the interactive effect between the groups in the stage (F6,42 = 3.700, sig = 0.005). ANOVA with repeated measures showed that cognitive-behavioral intervention on salivary cortisol level had effect in the post-test, while mindfulness intervention had positive effects on the post-test and follow-up times. ANOVA also revealed that there was no significant difference between the mindfulness and cognitive-behavioral groups in the post-test time. In addition, the results of the follow-up time showed that there was significant difference only between mindfulness and control group. Practical Applications: Results indicate that the practice of psychological skills, especially mindfulness can be used to reduce the pre-competition stress. Resultados: ANOVA com medidas repetidas mostrou um efeito principal para o grupo (F2,21 = 3.787, sig = 0.039) e o estágio (F3,21 = 52.529, sig = 0.001) e também o efeito interactivo entre os grupos no estágio (F6,42 = 3.700, sig = 0.005). A ANOVA com medidas repetidas mostrou que a intervenção cognitivo-comportamental ao nível do cortisol salivar teve efeito no pós-teste, enquanto que a intervenção de atenção teve efeitos positivos no pós-teste e nos tempos de seguimento. ANOVA também revelou que não havia diferença significativa entre o nível de atenção e os grupos cognitivo-comportamentais no período pós-teste. AimsTo evaluate the safety and efficacy of corneal cross-linking (CXL) followed by photorefractive keratectomy (PRK) for refractive correction in patients with bilateral asymmetric topography.MethodsForty-four patients (88 eyes) were enrolled in this prospective randomised clinical trial. CXL with subsequent PRK after 6 months was performed in one eye (study group), and PRK alone was performed in contralateral eyes (control group). Patients were followed for 24 months after PRK. Outcome measures investigated included visual acuity (VA), refraction, aberrometry, topography, pachymetry and endothelial cell count. Groups were compared with linear mixed regression and repeated measures logistic regression. Multiple comparison adjustment with the Holm procedure was performed.ResultsAt baseline, the logMAR VA (best spectacle corrected) in study and control groups was 0.12±0.13 (mean±SD) and 0.08D±0.14, respectively, and axial inferior–superior index (IS) (topographic IS) in study and control groups were 0.59D±0.31D and 0.58D±0.32D, respectively. After 24 months, a mean under correction of −0.50D was observed in both groups. Change from baseline in logMAR VA in study and control groups was 0.00D±0.08D and −0.02D±0.10D, respectively. Frequency of haze at 30 months in study and control group eyes was, respectively, 18.2% and 4.6% (p=0.05). There was no statistical difference between groups in spherical aberration and coma after adjustment for multiple comparisons.ConclusionsNon-simultaneous CXL followed by PRK may be performed safely, and refractive results over a 2-year follow-up are highly similar in virgin and previously cross-linked corneas. Despite using mitomycin C, corneal haze can be significantly higher in the first year after PRK in eyes pretreated with CXL.

Objectives: Molar pregnancy is the gestational trophoblastic disease and impact on the women’s health. It has several complications such as toxicity, infection, bleeding. Molar pregnancy also has high risk of choriocarcinoma which can be dead. Aim: To assess the risks of molar pregnancy. Materials and Methods: The case control study included 76 molar pregnancies and 228 pregnancies in control group at Hue Central Hospital. Results: The average age was 32.7 ± 6.7, the miximum age was 17 years old and the maximum was 46 years old. The history of abortion, miscarriage in molar group and control group acounted for 10.5% and 3.9% respectively, with the risk was higher 2.8 times; 95% CI = 1.1-7.7 (p<0.05). The history of molar pregnancy in molar pregnancy group was 9.2% and the molar pregnancy risk was 11.4 times higher than control group (95% CI = 2.3-56.4). The women having ≥ 4 times births accounted for 7.9% in molar group and 2.2% in control group, with the risk was higher 3.8 times, 95% CI= 1.1-12.9 (p<0.05). The molar risk of women < 20 and >40 years old in molar groups had 2.4 times higher than (95% CI = 1.1 to 5.2)h than control group. Low living standard was 7.9% in molar group and 1.3% in the control group with OR= 6.2; 95% CI= 1.5-25.6. Curettage twice accounted for 87.5%, there were 16 case need to curettage three times. There was no case of uterine perforation and infection after curettage. Conclusion: The high risk molar pregnancy women need a better management. Pregnant women should be antenatal cared regularly to dectect early molar pregnancy. It is nessecery to monitor and avoid the dangerous complications occuring during the pregnancy.
