AbstractPosturography is utilized to assess the influence of aging on postural control. Although this measurement is advantageous for finding group-level differences between the young and the elderly, it is unclear whether it has the potential to differentiate elderly individuals who are affected by various impacts of aging. The purpose of this study was to determine the utility of posturography to discriminate elderly individuals from young adults. We investigated the performances of the random forest classifiers constructed from center of pressure (COP) indices for discriminating standing postures between healthy elderly and young people. Postural sways in 19 young and 31 community-dwelling elderly participants were measured using force plates in 4 standing conditions: bipedal standing, standing on a narrow base, standing on foam rubber, and standing with eyes closed. We further verified the informative predictors that contributed to the prediction model. As the results, the classifier based on the COP indices for standing on foam rubber showed the best performance (accuracy: 93.4%, sensitivity: 94.4%, specificity: 93.6%, area under the curve of receiving operator characteristics: 0.95), followed by the classifier for standing with eyes closed. The informative predictors varied depending on the postural conditions. Our findings demonstrated the potential of posturography for identifying elderly postures. The evaluation of sensory re-weighting using the appropriate COP indices would be a useful clinical tool for detecting the progress of aging on postural control.

A stroke and aging process can modify the postural control. We aimed to compare the postural control of health elderly individuals to that of individuals with stroke sequelae. This cross-sectional transversal study was made with individuals capable of walking without any assistance and that were considered clinically stable. The study had 18 individuals in the group with stroke sequelae (SG) and 34 in the healthy elderly control group (CG). The participants were evaluated for the timed up and go test (TUG) and force platform. The SG showed the worst results in relation to the time of execution of the TUG and the force platform evaluation. The displacement of center of pressure was worse for both groups in the eyes-closed situation, especially in the anteroposterior direction for the CG. The GS showed worse results in the static and dynamic postural control. The healthy elderly showed more dependence on sight to maintain their static balance and there was no difference in the balance tests in relation to the side affected by the stroke.

Proprioceptive input may greatly affect postural stability. However, the proprioceptive postural strategy in elderly, middle-aged, and young people has not been investigated sufficiently. Hence, in this study, we aimed to investigate differences in proprioceptive postural strategies of elderly, middle-aged, and young people. The center of pressure displacement was determined in 23 elderly, 23 middle-aged, and 23 young people during upright stance on a balance board with their eyes closed. Vibratory stimulations at 30, 60, and 240 Hz were applied to the lumbar multifidus (LM) and gastrocnemius (GS) muscles to evaluate the contributions of different proprioceptive signals used in balance control. Compared with middle-aged and young people, elderly people showed a high dependence on postural control of the GS at 30 Hz (p-values: Young and elderly: 0.033; middle-aged and elderly: 0.001). Moreover, compared with young people, elderly people were more dependent on postural control of the LM at 240 Hz (p = 0.016). There were no significant differences with respect to the GS at 60 and 240 Hz, and with respect to the LM at 30 and 60 Hz between the elderly, young, and middle-aged people. Thus, the postural control strategy of elderly people depends on the GS at 30 Hz.

One possible factor influencing the control of upright stance is the perceived threat to one's personal safety, i.e. balance confidence. We explored this factor by examining the control of stationary stance when standing on an elevated platform under various conditions of reduced visual and vestibular inputs. Twenty-eight adults (14 male and 14 female, mean age = 23.5 years) participated in the experiment. Postural control was examined by recording the amplitude variability (RMS) and mean power frequency (MPF) of center of pressure excursions (COP) over a 2-minute interval while participants stood in a normal stance on a low (0.19 m) and a high (0.81 m) platform with toes positioned either at or away from the edge of the platform. Vision was manipulated through eyes open and eyes closed trials. Vestibular input was reduced by tilting the head into extension [1]. Anterior-posterior RMS and MPF of COP were significantly influenced by an interaction between surface height and vision. When vision was available, a significant decrease in RMS was observed during quiet standing on a high surface compared to a low surface independent of step restriction. When vision was available MPF increased when subjects were raised from a low to a high surface. The mean position of the COP was significantly influenced by an interaction between height and step restriction. Differences in RMS and MPF responses to height manipulation were observed between genders in eyes closed conditions. Vestibular input influenced postural control at both low and high levels with significant increases in RMS when vestibular input was reduced. The reciprocal changes observed in RMS and MPF suggest modifications to postural control through changes in ankle stiffness. Vision appears to play a role in increasing ankle stiffness when balance confidence is compromised.

Postural sway during stance has been found to be sensitive to moving visual scenes in young adults, children, and those with vestibular disease. The effect of visual environments on balance in elderly individuals is relatively unknown. The purpose of this study was to compare postural sway responses of healthy elderly to those of young subjects when both groups were exposed to a moving visual scene. Peak to peak, root mean squared, and mean velocity of the center of pressure were analyzed under conditions combining four moving scene amplitudes ( 2 . 5 ∘ , 5 ∘ , 7 . 5 ∘ , 10 ∘ ) and two frequencies of scene movement (0.1 Hz, 0.25 Hz). Each visual condition was tested with a fixed floor and sway referenced platform. Results showed that elderly subjects swayed more than younger subjects when experiencing a moving visual scene under all conditions. The elderly were affected more than the young by sway referencing the platform. The differences between the two age groups were greater at increased amplitudes of scene movement. These results suggest that elderly are more influenced by dynamic visual information for balance than the young, particularly when cues from the ankles are altered.

We explored if adolescents with Gilles de la Tourette syndrome (GTS) had functional postural control impairments and how these deficits are linked to a disturbance in the processing and integration of sensory information. We evaluated the displacements of the center of pressure (COP) during maximal leaning in four directions (forward, backward, rightward, leftward) and under three sensory conditions (eyes open, eyes closed, eyes closed standing on foam). GTS adolescents showed deficits in postural stability and in lateral postural adjustments but they had similar maximal COP excursion than the control group. The postural performance of the GTS group was poorer in the eyes open condition (time to phase 1 onset, max-mean COP). Moreover, they displayed a poorer ability to maintain the maximum leaning position under the eyes open condition during mediolateral leaning tasks. By contrast, during forward leaning, they showed larger min-max ranges than control subjects while standing on the foam with the eyes closed. Together, these findings support the idea that GTS produces subclinical postural control deficits. Importantly, our results suggest that postural control disorders in GTS are highly sensitive to voluntary postural leaning tasks which have high demand for multimodal sensory integration.

CONTEXT AND OBJECTIVE: Little is known about postural control among elderly individuals with osteoporosis and its relationship with falls. It has been suggested that elderly women with kyphosis and osteoporosis are at greater risk of falling. The aim of this study was to evaluate posture and postural control among elderly women with and without osteoporosis. DESIGN AND SETTING: Cross-sectional study conducted at the Physical Therapy and Electromyography Laboratory, School of Medicine, Universidade de São Paulo (USP). METHODS: Sixty-six elderly women were selected from the bone metabolism disorders clinic, Division of Rheumatology, USP, and were divided into two groups: osteoporosis and controls, according to their bone mineral density (BMD). Postural control was assessed using the Limits of Stability (LOS) test and the Modified Clinical Test of Sensory Interaction and Balance (CTSIBm) and posture, using photometry. RESULTS: The elderly women with osteoporosis swayed at higher velocity on a stable surface with opened eyes (0.30 versus 0.20 degrees/second; P = 0.038). In both groups, the center of pressure (COP) was at 30% in the LOS, but with different placements: 156° in the osteoporosis group and 178° in the controls (P = 0.045). Osteoporosis patients fell more than controls did (1.0 versus 0.0; P = 0.036). CONCLUSIONS: The postural control in elderly women with osteoporosis differed from that of the controls, with higher sway velocity and maximum displacement of COP. Despite postural abnormalities such as hyperkyphosis and forward head, the COP position was posteriorized.

Context: Insomnia is prevalent among elderly individuals and has become a public health problem receiving increasing concern. Low sleep quality is related to a number of negative health outcomes. Objective: This research aims to investigate the sleep status of community-dwelling elderly individuals (aged 60 years or older) in Shanghai, China, to explore the impact factors affecting sleep quality. Methods: One street in Shanghai city was randomly selected, and 50% of the elderly residents were randomly selected as participants for a total number of 9,616 people. Investigations were conducted by indoor surveys through home doctors and paper questionnaires. For statistical analysis, univariate and multivariate regression analysis and the Spearman correlation model were performed using SAS 9.4 to analyze the effects of the impact factors on the sleep quality of elderly individuals. Results: Sleep quality is greatly affected by physical pain in both the elderly individual and the individual’s spouse. Healthy elderly people tend to have better sleep quality, and the more chronic diseases an elderly individual suffers, the worse their sleep quality is. In addition, the physical pain of a cohabiting spouse is one of the most significant impact factors influencing sleep quality. Conclusion: We suggest that community-dwelling elderly people maintain moderate outdoor activities and a healthy mental state, which will improve their sleep quality.

Objective The purpose of this study was to evaluate the effect of hearing aids on body balance function in a strictly controlled auditory environment. Methods We recorded the findings of 10 experienced hearing aid users and 10 normal-hearing participants. All the participants were assessed using posturography under eight conditions in an acoustically shielded non-reverberant room: (1) eyes open with sound stimuli, with and without foam rubber, (2) eyes closed with sound stimuli, with and without foam rubber, (3) eyes open without sound stimuli, with and without foam rubber, and (4) eyes closed without sound stimuli, with and without foam rubber. Results The auditory cue improved the total path area and sway velocity in both the hearing aid users and normal-hearing participants. The analysis of variance showed that the interaction among eye condition, sound condition, and between-group factor was significant in the maximum displacement of the center-of-pressure in the mediolateral axis (F [1, 18] = 6.19, p = 0.02). The maximum displacement of the center-of-pressure in the mediolateral axis improved with the auditory cues in the normal-hearing participants in the eyes closed condition (5.4 cm and 4.7 cm, p < 0.01). In the hearing aid users, this difference was not significant (5.9 cm and 5.7 cm, p = 0.45). The maximum displacement of the center-of-pressure in the anteroposterior axis improved in both the hearing aid users and the normal-hearing participants.

Abstract Background: Age-related sarcopenia meaningfully increases the risks of functional limitations and mortality in the elderly. Although circulating microRNAs (c-miRNAs) are associated with aging-related cellular senescence and inflammation, the relationships between c-miRNAs and sarcopenia in the elderly remain unclear. This study investigates whether circulating myo-miRNAs and inflammation-related miRNAs are associated with sarcopenia in the elderly. Methods: This investigation recruited 77 eligible subjects (41 males and 36 females) from 597 community-dwelling older adults, and then divided into normal (n=24), dynapenic (loss of muscular function without mass, n=35), and sarcopenic groups (loss of muscular function with mass, n=18). Moreover, myo- (c-miRNA-133a and c-miRNA-486) and inflammation- (c-miRNA-21 and c-miRNA-146a) related miRNAs, as well as, inflammatory-related cytokine and peroxide levels in plasma were determined using quantitative polymerase chain reaction and ELISA, respectively. Results: Sarcopenic group exhibited lesser skeletal muscle mass index (SMI), handgrip strength, and gait speed, as well as, lower c-miR-486 and c-miR-146a levels, compared to those of normal and dynapenic groups. Moreover, c-miR-486 level was positively related to SMI (r=0.334, P=0.003), whereas c-miR-146a level was positively associated with SMI (r=0.240, P=0.035) and handgrip strength (r=0.253, P=0.027). In the receiver operating characteristic analysis for predicting sarcopenia, the area under the curve in c-miR-486 was 0.708 (95% confidence interval: 0.561-0.855, P=0.008) and c-miR-146a was 0.676 (95% CI: 0.551-0.801, P=0.024). However, no significant relationships were observed between SMI/handgrip strength/gait speed and plasma myeloperoxidase/interleukin-1𝛽/interleukin-6 levels. Conclusions: Myo-miRNA (c-miR-486) and inflammation-related miRNA (c-miR-146a) are superior to inflammatory peroxide/cytokines in plasma for serving as critical biomarkers of age-related sarcopenia.

This study investigated the quantitative scaling properties of the center of pressure (COP) as well as the spatial-temporal properties of the COP to elucidate the postural control behavior of healthy elderly (HE) adults and adults with Parkinson’s disease (PD) during quiet standing. Eighteen adults with PD and eighteen HE adults participated in this study. The COP movements were recorded while participants stood on either a firm surface or on a foam pad with their eyes either opened or closed. The sway ranges in the anterior–posterior (AP) ([Formula: see text] and medio-lateral (ML) ([Formula: see text] directions, the total length of the trajectory ([Formula: see text], sway area ([Formula: see text], and scaling exponents ([Formula: see text] from detrended fluctuation analysis were computed from the measured COP data. All temporal variables of the COP in all conditions were found to be significantly larger in the PD group than in the HE group. Low scaling exponents obtained for the PD group showed this group possessed diminished postural control ability compared to the HE group. The PD group showed unpredictable open-loop control in both the AP and ML directions. This proprioceptive control became predictable and the time scale relations decreased as the postural challenges increased. The AP and ML closed-loop control of the PD group was more predictable than that of the HE group only when proprioception was distorted using intact visual input, and the visual and proprioceptive inputs were both intact.
