Optic Disc Features in Familial Retinal Arteriolar Tortuosity

Abstract Purpose Describing optic disc appearance in familial retinal arteriolar tortuosity (fRAT) using multimodal imaging and raising awareness of peripapillary arterial changes due to this disorder. Methods A cross-sectional study was performed in four consecutive patients of two non-related families. Detailed ophthalmological examination was performed and supported by medical and family history and multimodal imaging. Results In all subjects, increased tortuosity of second- and third-order retinal arteries in superior and deeper vascular plexus was documented. Furthermore, tortuosity in the peripapillary circle of Zinn-Haller was found. In addition, retinal vessel oximetry confirmed tortuosity only of the arterial vessels. Conclusion The present data suggests that a blurry bordered, hyperemic optic disc in the presence of abnormally tortuous arteriolar vessels and asymptomatically or oligosymptomatically spontaneously resolved hemorrhages could be associated with a fRAT. This finding could be linked to peripapillary arterial vessel tortuosity.

Abstract Objective To evaluate retinal oxygen metabolism by retinal oximetry for ocular and CNS diseases in a cross-sectional study of sarcoidosis. Methods Overall 201 eyes from 103 biopsy-verified sarcoidosis patients were included and divided into four groups depending on the organ affection: (i) sarcoidosis without ocular or CNS affection, (ii) ocular sarcoidosis, (iii) CNS sarcoidosis, and (iv) combined ocular and CNS sarcoidosis. Retinal oximetry was obtained and analysed, with the mean retinal arteriolar and venular saturation as well as arteriovenous difference as principal outcomes. Comparison between groups was done in a multi linear regression model adjusted for age, sex, duration of sarcoidosis, best corrected visual acuity and retinal oximetry quality. Results Mean (s.d.) age was 50.5 (13.4) (95% CI: 47.9, 53.1) years and 52.2% were males. Eyes of the combined Ocular/CNS group had a higher retinal arteriovenous difference than eyes of the Non-ocular/no-CNS group (42.1% vs 37.7%, P = 0.012) but did not differ between other groups. Eyes in the four groups (Non-ocular/no-CNS, Ocular, CNS and Ocular/CNS) did not differ according to retinal arterial (94.5%, 93.5%, 93.5% and 94.5%, respectively) or venular (57.5%, 56.4%, 55.0% and 52.5%, respectively) oxygen saturation. Conclusions The results of this study suggest that eyes of sarcoidosis patients with combined ocular and CNS affection have an altered oxygen metabolism indicating a subclinical eye affection that is not recognized by conventional screening methods.

BackgroundGlaucoma may cause physiological and behavioural circadian misalignment because of the loss of intrinsically photosensitive retinal ganglion cells, the primary receptors of environmental light. Although studies have suggested a high prevalence of depression in patients with glaucoma, it is unclear whether the association is independent of the light exposure profiles as an important confounding factor.MethodsIn this cross-sectional study of a community-based cohort of 770 elderly individuals (mean age, 70.9 years), glaucomatous optic discs were assessed using fundus photographs and depressive symptoms were assessed using the short version of the Geriatric Depression Scale (GDS). Daytime and night-time ambient light exposures were objectively measured for 2 days.ResultsDepressive symptoms (GDS score ≥6) were observed in 114 participants (prevalence, 14.8%) and glaucomatous optic discs were detected in 40 participants (prevalence, 5.2%). The prevalence of depressive symptoms was significantly higher in the group with glaucomatous optic disc than in the group without it (30.0% vs 14.0%, respectively; p=0.005). Multivariable logistic regression analysis adjusted for potential confounding factors, including daytime and night-time light exposures, revealed that the OR for depressive symptoms was significantly higher in the group with glaucomatous optic disc than in the group without it (OR 2.45, 95% CI 1.18 to 5.08; p=0.016).ConclusionsIn this general elderly population, glaucomatous optic disc was significantly associated with higher prevalence of depressive symptoms independent of a number of potential confounding factors, including daily light exposure profiles.

Introduction: Pigmentary Glaucoma (PG) and Pigment Dispersion Syndrome (PDS) are two different spectrums of a single disease. Since the disease is seen in younger population and is rapidly progressive blinding disease, therefore early diagnosis and treatment will reduce the burden of the disease and improve the quality of life. Aim: To evaluate clinical characteristics of PDS and PG patients in eastern part of Uttar Pradesh. Materials and Methods: This was a two years (1st January 2018 to 31st December 2019) hospital‑based retrospective cross‑sectional study of patients who attended the glaucoma clinic. Diagnosis of PDS was made when they had normal optic disc, normal visual field {with or without increased Intra Ocular Pressure (IOP)} and at least two of the following three signs were found clinically: Krukenberg spindle, homogenous moderate‑to‑heavy (≥Spaeth 2+) Trabecular Meshwork (TM) pigmentation, and any degree of zonular and/or lenticular pigment granule dusting. Patients with PDS were diagnosed with PG, if they had two or more of the following findings: initial IOP >21 mmHg, glaucomatous optic nerve damage or glaucomatous visual field loss. Various parameters such as influence of demographics, IOP, Best‑Corrected Visual Acuity (BCVA), Central Corneal Thickness (CCT), Mean Deviation (MD), Visual Field Index (VFI %), spherical equivalent and clinical finding of anterior segment of study patients were analysed. Mean, standard deviation and percentage were calculated using GraphPad Instat version 3.0. Results: Among 40 patients, nine eyes of the six patients had myopia of ‑0.5D or greater, with mean refractive error of ‑3.55±4.72 spherical equivalent. The average baseline IOP in study patients (PDS+PG), was 30.21±11.42 mmHg. Twenty four (60%) patients, either in one or both eyes had glaucoma, secondary to PDS at the initial diagnosis. Thirty three (82.5%) patients had Krukenberg spindles. Homogeneous TM pigmentation was seen in all patients. Typical spoke‑like radial Iris Transillumination Defects (ITDs) were not observed in any of the patients except in one patient, who had isolated short slit‑like trans‑illumination defects in iris crypts. Conclusion: PDS patients with normal optic disc and visual field and raised IOP, should be started prophylactic treatment and needs to be monitored more closely. Thus, the finding of PDS in Indians should alert the ophthalmologist to look for glaucoma during the initial examination.

There is no consensus on whether amblyopia affects the retinal vascular plexus and morphology. Previous studies focused on the differences between amblyopic patients and normal controls without evaluating amblyopic eyes after patching. To evaluate differences in the superficial vascular density of amblyopic eyes, normal eyes, and amblyopic eyes reaching normal BCVA after patch therapy, OCTA was used. All patients underwent a comprehensive ophthalmological examination, including visual acuity, refraction, ocular motility tests, and anterior and posterior segment examination. OCTA was performed by an expert physician using the Zeiss Cirrus 5000-HD-OCT Angioplex (Carl Zeiss, Meditec, Inc., Dublin, OH, USA). OCTA scans were performed using a 3 × 3 mm2 and 6 × 6 mm2 fovea-centered image setting. The mean outer macular vessel density in the previously amblyopic group was 19.15 ± 0.51%. This was statistically significantly higher than in both the amblyopic group (18.70 ± 1.14%) and the normal controls (18.18 ± 1.40%) (p = 0.014). The previously amblyopic group also significantly differed from both normal controls and amblyopic eyes with regards to the inner (p = 0.011), outer (p = 0.006), and full (p = 0.003) macular perfusion. Finally, linear regression analysis revealed that BCVA was linearly correlated to outer perfusion in amblyopic (p = 0.003) and ex amblyopic eyes (p < 0.001). Considering the cross-sectional nature of our study, from our results, we can only hypothesize a possible correlation between light stimulation and retinal vasculature development. However, further longitudinal studies are needed to support this hypothesis.

Introduction. The aim of our observational cross-sectional study was to evaluate the association between visual function and anatomical characteristics of LMH, considering in particular different subtypes of LMH and their features. Materials and Methods. This observational clinical study has been conducted in the Ophthalmology Clinic, ASST-Sette Laghi, University of Insubria of Varese-Como, Italy. Included patients underwent a complete ophthalmological examination, as well as MP1 microperimetry evaluation and optical coherence tomography (OCT). Two experienced masked observers evaluated OCT imaging in order to assess the integrity of the photoreceptor layer (interdigitation zone and ellipsoid zone: IZ/EZ) and the external limiting membrane (ELM). Results. Twenty-five patients affected by an LMH were evaluated. Eighteen eyes of 18 patients met the study criteria and were included. Based on morphological and functional data, LMHs were divided into two subgroups: tractional (tLMH) and degenerative (dLMH). We identified 11 tLMHs and seven dLMHs. Functional parameters showed a significative difference in visual acuity and retinal sensitivity between the two groups, respectively: (sample median and the interquartile range) 0.0 (0.0; 0.09) LogMAR vs 0.15 (0.09; 0.52) LogMAR and 16.2 (14.2; 17.7) dB vs 10.0 (7.5; 11.8) dB (p<0.05). Fixation was predominantly central in 90.9% of tLMH vs 71.4% of dLMH and stable in 81.8% tLMH vs 42.9% dLMH, but the differences were not statistically significant. Tractional and degenerative LMHs showed no significant differences in central foveal thickness. Conversely, LMH depth and horizontal diameters appeared different between the two groups. Tractional LMH showed a greater depth 257 (205; 278) μm vs 190 (169; 249) μm, whereas degenerative LMH showed a greater horizontal diameter 653 (455; 750) μm vs 429 (314; 620) μm (p<0.05). IZ/EZ line was unaffected in 81.8% of tLMHs eyes versus 14.3% of dLMHs eyes (p<0.05). Visual acuity and retinal sensitivity were higher in eyes with integrity of both IZ/EZ and ELM compared to those with a disruption of one or both layers (p<0.05). Conclusion. Two different subtypes of LMH showed peculiar functional aspects due to their morphological features. Tractional LMHs revealed higher visual acuity and retinal sensibility due to the relative preservation of the outer retinal layers compared to degenerative LMHs. Moreover, we underlined the importance of microperimetry to better identify functional defects in macular degenerative pathologies.

Abstract Background: The aim of this study was to investigate the correlation between optic disc deformation and retinal vasculature in high myopia.Methods: One hundred and thirty eyes with non-pathological high myopia were included in this cross-sectional study. The optic disc tilt ratio, and horizontal and vertical disc diameters were analyzed using fundus color photography. A 3 × 3 mm2 grid and a 4.5 × 4.5 mm2 grid were used to scan parafoveal and peripapillary regions, respectively, using optical coherence tomography angiography. Vessel flow density (VFD) and fractal dimension of the retina and the foveal avascular zone (FAZ) were analyzed and quantified using en face projection images. Optic disc parameters that were associated with vascular changes were determined using multiple linear regression analysis.Results: Results from the multivariate analysis revealed that the tilt ratio (β = -2.291, p = 0.025) was negatively correlated with three sectors in the deep layer. Age was negatively correlated with the VFD of the retina (β = -2.161; p < 0.034). Additionally, FAZ was not significantly correlated with any factors in the current study. Further, there was a positive correlation between retinal nerve fiber layer thickness and VFD of the optic nerve head (β = -2.146, p = 0.035).Conclusions: Overall, our results demonstrated that optic disc deformation was negatively correlated with the retinal microvasculature in non-pathological high myopia. Therefore, optic disc deformation may be used to predict the retinal vasculature in high myopia.

Objective: Raising awareness on corneal donation among doctors and medical students is an important step to attain more local corneal donations. The objective of the study was to evaluate the knowledge and attitude of medical students regarding corneal donations. Method: A cross-sectional study was conducted among 5th and final year’s medical students from August 2019 to October 2019. Non-probability convenience sampling was employed. A self-administered questionnaire that evaluated the aspects of knowledge and attitudes about corneal donation was used after informed consent was taken. SPSS 20 was used for statistical analysis. Results: The study participants had a mean score of 3.24+1.86 on the knowledge aspect. Eighty percent correctly recognized that people who die with AIDS cannot donate. The most chosen reason for eyes donation was that pledged corneas can give vision to a blind person (n=66, 47%). Religious beliefs were a major obstacle for eye donation (n=62, 44%). The internet was identified by 79 (56%) of the participants to be the most common source of information. No significant difference was found in the knowledge level of the 5th year (3.31 + 1.84) and final year (3.17 + 1.89) medical students (p=0.65). Conclusion: The study showed a low level of knowledge of students about corneal donations. While raising awareness among medical students is important to increase donation rates, media and internet are not enough for improving the student’s knowledge. Information about the basics of corneal donation is important to fill gaps in knowledge. Keywords: Knowledge, attitude, corneal donation, Medical students

Background/Aims: Optic disc hemorrhage (DH) is an important glaucoma risk factor, and occurs in a wide intraocular pressure (IOP) range. We sought to characterize distinct clinical subtypes of patients with high- (HTDH) and low-tension DHs (LTDH). Methods: In this cross-sectional study, treated glaucomatous patients with DHs from two Glaucoma Services were consecutively enrolled. Disc photographs were evaluated for the presence of DH by two glaucoma specialists. After inclusion, patients were classified on HTDH (IOP≥16mmHg) and LTDH (IOP<16mmHg; median split). Clinical and ocular data from the time of DH detection were compared between groups. Results: One hundred thirty-three DH patients were included (LTDH=66 eyes; HTDH=67 eyes). Patients with LTDH were more often women than those with HTDH (77% vs 42%; p=0.030). There was also a trend for a higher prevalence of Asian descendants (24% vs 9%; p=0.058) and symptoms suggestive of vascular dysregulation (34% vs 14%; p=0.057) in LTDH patients. Eyes with LTDH also had worse visual field (VF) mean deviation index (p=0.037), higher prevalence of normal-tension glaucoma (NTG) diagnosis (46% vs 17%; p<0.001) and tended to have thinner central corneas (p=0.066). Conclusion: Patients developing DHs with treated IOPs in the low teens seem to more frequently fit in a profile represented by women, NTG diagnosis and greater VF loss. The presence of symptoms suggestive of vascular dysregulation and race also seem to differ between these two clinical subtypes. A closer optic disc surveillance is recommended for patients with the LTDH subtype, as they may develop DHs despite seemingly well-controlled IOP.
