Abstract I quantify the welfare and environmental gains and losses from a policy establishing an environmental tax/subsidy for new cars in France in 2008. I estimate a structural model of demand and supply that features heterogeneity in consumer preferences to go beyond the average policy effects and analyse distributional aspects. The policy reduces average carbon emissions by 1.6% at the cost of additional emissions of local pollutants. The regulation favours middle-income individuals but has redistributive effects when combined with a tax that is proportional to income. Moreover, local pollutant emissions increase least in poor and rural areas, suggesting another redistribution channel.

Migration is one of the top debate topics in terms of the national policy agendas of middle-income countries, and Thailand is no exception. The segmentation of its labour market explains why Thailand is experiencing large-scale immigration and a simultaneous emigration of low-skilled workers. Immigration inflows from its less-developed neighbour countries -namely, Laos, Cambodia and Myanmar – pose a challenge for Thailand. Wage differentials between Thailand and other migrant-receiving countries, which are mostly more economically developed than Thailand, also stimulate emigration from there. Due to regional disparities within the country and to a lack of employment and educational opportunities in rural areas, internal migration is also common and encouraged. In this paper I first analyse the economic pros and cons of migration both to and within Thailand before formulating labour migration policies that aim to maximize beneficial outcomes while minimizing economic costs. The cost–benefit analysis of labour migration is key to addressing relevant gaps in formulating and implementing effective policies.

Subject. The article describes the findings of the authors of fundamental strategic decisions on the formation of multifunctional urban complexes, using the housing demand and supply criterion. Objectives. We undertake a comprehensive study aimed at perfecting the methodology for evaluating the options for city infrastructure development at two stages, i.e. strategic, when general targets of feasible commissioning are determined, and current, when parameters of demand for facilities are taken into account. Methods. The study employs methods of expert survey, statistical data processing, predictive and investigative analysis. Results. We explored factors of creating amenities and comfort in residential construction areas, developed an algorithm to calculate the volume of new living space commissioning on the basis of evaluating demands in the Smart City paradigm. Conclusions. The study shows the cost increase depending on the built-up area, number of floors, and the balance between the type of capacity and the number of residents in the quarter (linear relationship).

Introduction: Implementation of the Federal Public Health Promotion Project should raise awareness and develop skills of healthy nutrition in children, thus contributing to disease prevention. Our objective was to evaluate the results of pilot nutrition monitoring in school-aged children of the Sverdlovsk Region as part of the Federal Public Health Promotion Project and the National Demography Project. Results: We established that school meals were generally satisfactory: the rations complied with physiological needs of children in terms of their nutritional value, basic nutrients, energy, and distribution of calories by main meals. We noted differences in the cost and nutritional value of meals and the variety of dishes and foodstuffs used between urban and rural areas. As a rule, pupils have one or two school meals a day. Outside of school, their consumption of dairy products and fruit is limited. Conclusions: Our findings may promote the elaboration of municipal programs aimed, inter alia, at changing the amount of sugar and salt used in the manufacture of public catering products, the cost of dishes with a high content of sugar, saturated fats, and salt, and subsidies on healthy nutrition.

Bacillus Calmette–Guerin (BCG), the only available vaccine for tuberculosis (TB), has been applied for decades. The Indonesian government recently introduced a national TB disease control programme that includes several action plans, notably enhanced vaccination coverage, which can be strengthened through underpinning its favourable cost-effectiveness. We designed a Markov model to assess the cost-effectiveness of Indonesia’s current BCG vaccination programme. Incremental cost-effectiveness ratios (ICERs) were evaluated from the perspectives of both society and healthcare. The robustness of the analysis was confirmed through univariate and probabilistic sensitivity analysis (PSA). Using epidemiological data compiled for Indonesia, BCG vaccination at a price US$14 was estimated to be a cost-effective strategy in controlling TB disease. From societal and healthcare perspectives, ICERs were US$104 and US$112 per quality-adjusted life years (QALYs), respectively. The results were robust for variations of most variables in the univariate analysis. Notably, the vaccine’s effectiveness regarding disease protection, vaccination costs, and case detection rates were key drivers for cost-effectiveness. The PSA results indicated that vaccination was cost-effective even at US$175 threshold in 95% of cases, approximating the monthly GDP per capita. Our findings suggest that this strategy was highly cost-effective and merits prioritization and extension within the national TB programme. Our results may be relevant for other high endemic low- and middle-income countries.

Urban sustainability refers to building and maintaining cities that can continue to function without running out of resources. However, growing cities require more land and urban sprawl has transformed surrounding rural areas into urbanized settlements. Furthermore, the prosperity of large cities depends on the supply of both natural and human resources from rural areas, either nearby or remote. On the other hand, the use of resources of rural areas by cities may cause negative externalities to rural areas, affecting their sustainability. Therefore, a critical, but very much neglected issue, is how unban sustainability should be pursued without affecting rural sustainability. In this study, cases in Japan and China were analyzed from resources and population migration perspectives to provide evidence for the possibility that urban sustainability might have been pursued at the cost of rural unsustainability. It was intended to develop a better understanding of urban sustainability through the lens of externalities. Based on the analysis, a new framework for urban sustainability study was proposed, which consists of three new pillars. Namely, externality, vulnerability, and population instability.

ObjectivesThis study wants to assess the cost-effectiveness of unmanned aerial vehicles (UAV) equipped with automated external defibrillators (AED) in out-of-hospital cardiac arrests (OHCA). Especially in rural areas with longer response times of emergency medical services (EMS) early lay defibrillation could lead to a significant higher survival in OHCA.Participants3296 emergency medical stations in Germany.SettingRural areas in Germany.Primary and secondary outcome measuresThree UAV networks providing 80%, 90% or 100% coverage for rural areas lacking timely access to EMS (ie, time-to-defibrillation: >10 min) were developed using a location allocation analysis. For each UAV network, primary outcome was the cost-effectiveness using the incremental cost-effectiveness ratio (ICER) calculated by the ratio of financial costs to additional life years gained compared with current EMS.ResultsCurrent EMS with 3926 emergency stations was able to gain 1224 life years on annual average in the study area. The UAV network providing 100% coverage consisted of 1933 UAV with average annual costs of €43.5 million and 1845 additional life years gained on annual average (ICER: €23 568). The UAV network providing 90% coverage consisted of 1074 UAV with average annual costs of €24.2 million and 1661 additional life years gained on annual average (ICER: €14 548). The UAV network providing 80% coverage consisted of 798 UAV with average annual costs of €18.0 million and 1477 additional life years gained on annual average (ICER: €12 158).ConclusionThese results reveal the relevant life-saving potential of all modelled UAV networks. Furthermore, all analysed UAV networks could be deemed cost-effective. However, real-life applications are needed to validate the findings.

Abstract Background Determining the cost-effectiveness thresholds for healthcare interventions has been a severe challenge for policymakers, especially in low- and middle-income countries. This study aimed to estimate the cost per disability-adjusted life-year (DALY) averted for countries with different levels of Human Development Index (HDI) and Gross Domestic Product (GDP). Methods The data about DALYs, per capita health expenditure (HE), HDI, and GDP per capita were extracted for 176 countries during the years 2000 to 2016. Then we examined the trends on these variables. Panel regression analysis was performed to explore the correlation between DALY and HE per capita. The results of the regression models were used to calculate the cost per DALY averted for each country. Results Age-standardized rate (ASR) DALY (DALY per 100,000 population) had a nonlinear inverse correlation with HE per capita and a linear inverse correlation with HDI. One percent increase in HE per capita was associated with an average of 0.28, 0.24, 0.18, and 0.27% decrease on the ASR DALY in low HDI, medium HDI, high HDI, and very high HDI countries, respectively. The estimated cost per DALY averted was $998, $6522, $23,782, and $69,499 in low HDI, medium HDI, high HDI, and very high HDI countries. On average, the cost per DALY averted was 0.34 times the GDP per capita in low HDI countries. While in medium HDI, high HDI, and very high HDI countries, it was 0.67, 1.22, and 1.46 times the GDP per capita, respectively. Conclusions This study suggests that the cost-effectiveness thresholds might be less than a GDP per capita in low and medium HDI countries and between one and two GDP per capita in high and very high HDI countries.

Abstract Background: Adherence to the Mediterranean diet has been decreasing in southern Europe, which could be linked to several cultural or educational factors. Our aim is to evaluate the extent to which economic aspects may also play a role, exploring the relationship between food prices in Portugal and adherence to the Mediterranean diet. Methods: We evaluated data from the Portuguese National Food, Nutrition, and Physical Activity Survey (IAN-AF 2015-2016) (n=3,591). Diet expenditures were estimated by attributing a retail price to each food group and the diet was transposed into the Mediterranean Diet Score used in the literature. Prices were gathered from five supermarket chains (65% of the Portuguese market share). Linear regression models were used to assess the association between different adherence levels to the MD levels and dietary costs. Results: Greater adherence to the MD was associated with a 21.2% (p< 0.05) rise in total dietary cost, which accounts for more 0.59€ in mean daily costs when compared with low adherence. High adherence individuals (vs. low adherence) had higher absolute mean daily costs with fish (0.62€/+285.8%; p< 0.05), fruits (0.26€/+115.8%; p< 0.05), and vegetables (0.10€/+100.9%; p< 0.05). The analysis stratified by education and income level showed significantly higher mean daily diet cost only amongst higher income groups. Conclusions: Our findings suggest that greater adherence to the MD was positively and significantly associated with higher total dietary cost. Policies to improve population’s diet should take into consideration the cost of healthy foods, especially for large low- and middle-income families.

The cost of fuel and its availability are among the most major concerns for aircrafts and the aviation industry overall. Environmental difficulties with chemical pollutant emissions emitted by aviation machines are also connected to fuel consumption. As a result, it is crucial to examine factors that affect the overall fuel usage and consumption in the airport-based aviation industry. Several variables were investigated related to the total fuel consumed, such as dry operating weight (DOW) (KG), zero-fuel weight (ZFW), take-off weight (TOW), air distance (AIR DIST) (KM), and ground distance (GDN DIST). Analysis of the correlation between total fuel consumed as well as the extra fuel and selected variables was conducted. The results showed that the most positively associated factors with the total used fuel were the air distance (r2 = 0.86, p < 0.01), ground distance (r2 = 0.78, p < 0.01), TOW (r2 = 0.68, p < 0.01), and flight time (r2 = 0.68, p < 0.01). There was also a strong positive association between the average fuel flow (FF) and actual TOW (r2 = 0.74, p < 0.01) as well as ZFW (r2 = 0.61, p < 0.01). The generalized linear model (GLM) was utilized to assess the predictions of total energy usage after evaluating important outliers, stability of the homogeneity of variance, and the normalization of the parameter estimation. The results of multiple linear regression revealed that the most significant predictors of the total consumed fuel were the actual ZFW (p < 0.01), actual TOW (p < 0.01), and actual average FF (p < 0.05). The results interestingly confirmed that wind speed has some consequences and effects on arrival fuel usage. The result reflects that thermal and hydrodynamic economies impact on the flying fuel economy. The research has various implications for both scholars and practitioners of aviation industry.
