Background: Dengue is a viral disease transmitted by mosquito to humans and becomes a problem for the health of Indonesian people. Dengue Hemorrhagic Fever occurs due to several epidemiological factors. The purpose of this study is to determine the clinical characteristics of adult patients with Dengue Hemorrhagic Fever at Prof. dr. Sulianti Saroso Sunter Infectious Hospital 2018. Method: This Study was an observational descriptive study by taking medical record samples in 49 adult patients Dengue Hemorrhagic Fever in 2018. Results: From the 49 cases of Dengue Haemorrhagic Fever, the highest fender category was male (65,3%), occurred in the 18-25 year age category (38,8%), and the average patient worked as a Private Employee (55,1%). Most patients were hospitalized for the less than 7 days (83,7%). Clinical manifestations were fever (95,9%), nausea (85,7%) and headache (71,4%). Physical examination found was fever (100%) and hepatomegaly (12,2%). Laboratory tests showed normal hematocrit values (71,4%), thrombocytopenia values (87,8%), normal leukocyte values (49%), positive IgG serology tests (83,7%), IgM serology tests (71,4%), Positive IgM and IgG Serology test (69,3%), and NS1 Serology test (2%). Management provided is the administration of intravenous infus fluid drops (100%), PPI (87,8%), H2 Antagonist (4,1%), and Antipiretik (83,7%). The outcome patient is recovered and no complications were found. Conclusion: Dengue Hemorrhagic fever occurs in the age range of 18-25 years with dominant clinical characteristic of fever and thrombocytopenia. And, the most given treatment is Intravena Fluid therapy.

Plasma leakage is one crucial point of dengue hemorrhagic fever (DHF) that differentiates it from dengue fever (DF). DHF has to meet 4 criteria which are 2 – 7 days of acute fever, hemorrhagic manifestation, thrombocytopenia (≤100.000 cells/mm3) and evidence of plasma leakage. Plasma leakage consists of increasing hematocrit ≥20%, hypoalbuminemia or evidence of pleural effusion or ascites. Often doctors only base their DHF diagnosis on the presence of thrombocytopenia. This study analyzed the presence of plasma leakage between adult and pediatric patients with a DHF diagnosis in RSUD Dr. Soetomo in order to make the diagnosis and healthcare services better in the future. This was a retrospective study which used medical records of DHF patients admitted from January to June 2014. 78 cases were included, 24 adult patients (31%) and 54 pediatric patients (69%). 29/78 (37%) patients had no evidence of plasma leakage. No adult patients had ascites whereas 11/54 (20%) pediatric patients presented with ascites. No adult patients had pleural effusion whereas 25/54 (53%) pediatric patients did. Most adult patients that had serum albumin checked had normal albumin levels (12/14 [86%]) while only 14/28 (52%) pediatric patients had normal albumin level. 5/22 (23%) adult patients versus 32/53 (60%) pediatric patients showed hematocrit increments ≥20%. Patients admitted with dengue virus infection may currently be often misclassified as DHF because there are no plasma leakage manifestation in some patients.. There are significant differences in plasma leakage manifestations between adult and pediatric patients which poses a theory that pediatric patients are more susceptible to have plasma leakage manifestations than adult patients.

The clinical manifestation of dengue virus infection is often not clear, varies widely from mild to severe. Exposure of dengue virus which serotype is different from a previous infection is a risk factor for the severe manifestation of dengue virus infection. Dengue hemorrhagic fever is classified into four degrees of severity based on clinical manifestations and laboratory results. Real-time RT-PCR Dengue can detect dengue virus serotype in early dengue virus infection. The aimed of this study was to prove the correlation between dengue virus serotype and degree of severity in adult patients. This study was a cross-sectional observational design done in February until July 2016. Subjects consisted of 100 dengue virus infection patients. Serum of the patients was examined using Real-time RT-PCR Dengue (Simplexa™ Dengue). It was shown that from 46 patients with DENV-3 serotype was 63%, DENV-2 serotype 17.4%, DENV-1 serotypes 17.4% and mixed infection of DENV-1 and DENV-3 serotype 2.2%. There was not any DENV-4 serotype. Dengue Hemorrhagic Fever (DHF) stage I was 47.8%, DHF stage II was 30.4%, DHF stage III was 10.9% and Dengue Fever was 10.9%. There was not any DHF stage IV. There was not enough evidence that DENV-3 correlated with the degree of severity (p= 0.510). Based on this research, DENV-3 serotype was the dominant serotype prevalent at the Dr. Soetomo Hospital. There was no correlation between viral dengue serotype and severity in dengue adult patients in this study.

Kata Kunci : Demam Berdarah, Maya Index, pelatihan, pengendalian vektor   ABSTRACT Background: Dengue Hemorrhagic Fever (DHF) caused by Dengue virus could cause death. The most appropriate prevention of Dengue is eradication of mosquito nests (PSN). This study aims to determine the effect of Dengue vector control training on elementary students towards Maya Index in Majalengka. Method: This study used quasi-experimental design (pretest-posttest control group design). A total of 4 schools were selected as intervention groups and 4 other schools as controls. The subjects were students in grades IV-VI consisting of 171 students in the intervention group and 163 in the control group. The instrument used was periodic larva monitoring form. Results: The number of houses with a high Maya Index category in the intervention group decreased from 27.5% to 9.4%. There was a decrease in the high BRI category in the intervention group from 20.5% to 1.8%. In the control group, there was no decrease in the high BRI category (22.1%), on the contrary, there was a decrease in the low category from 34.4% to 3.7%. There was no decrease in HRI rates both of intervention or control groups. Conclusion: Dengue Fever vector control training could decrease the value of BRI and Maya Index, but does not affect the value of HRI. The absence of changes in HRI  indicate that environmental hygiene and sanitation are factors that influence the probability of dengue fever occurrence. Keywords: Dengue Hemorrhagic Fever, Maya Index, training, vector control

The diagnosis process is the first step to knowing a type of disease. This type of disease caused by mosquitoes is one of the major viruses (MAVY), dengue hemorrhagic fever (DHF) and malaria. Sometimes not everyone can find the virus that is carried by this mosquito, usually children who are susceptible to this virus because the immune system that has not been built perfectly is perfect. To know for sure which virus is infected by mosquitoes, it can diagnose by seeing symptoms perceived symptoms. Expert systems are one of the most used artificial intelligence techniques today because expert systems can act as consultations. In this case the authors make a system to start a diagnosis process with variable centered intelligent rule system (VCIRS) methods through perceived symptoms.
