The main theme of this dialogue is Nell’s slow movement toward death and her frustration at the delay. Her physical status is declining and she is visibly deteriorating. Since it is difficult for her to travel by car, we decided to meet in her home. She is frightened by her appearance changes and declares she looks like a beetle, with a bloated body and sticklike appendages. She longs for a witness to her bodily changes and yet is reluctant to ask her spouse Al to look at her. She is disappointed there is nowhere she can find the comfort of someone’s witness to her physical changes. She has met with her medical oncologist and come away frustrated because he has said death is not imminent and yet she is ready. Even so, Nell’s sense of humor is intact and she laughs about completing her income taxes: “Now I can die!” She has had an initial negative encounter with hospice and expresses her concern about their competence. She finds comfort in guided imagery introduced to her by her dear friend Mary, geographically distant but regularly present via phone call.

The traditional cuts in Tchaikovsky's Violin Concerto include nearly half of a developmental passage in the first movement and a series of shorter cuts in the finale. The first-movement cut comes after the second tutti, a triumphant thematic culmination that rings false on two levels, since it is a "polonaise" in 4/4, and since there has been no first tutti. The developmental passage that follows seems to confirm Tchaikovsky's self-confessed weakness in handling large-scale forms, but may arguably constitute an "anti-development" that sets up a mincing violin variation, which, in falling between two versions of the orchestral tutti and rescuing the larger group from its developmental ineptitude, models a homosexuality closeted within the aristocracy (this structural grouping reappears in the original slow movement). The various instances of "passing" in this movement (the faux polonaise, the "second tutti," the "anti-development," and a gradually emergent octatonic element in the violin climax just before these events) relate to Tchaikovsky's own "passing" dilemma, both as a homosexual and as a Russian nationalist working within Germanic forms; his specific treatment reflects the fact that the concerto was composed between his disastrous marriage and his later affiliation with the imperial court. A particular marker for Tchaikovsky's musical "passing" is the blended octatonic passage in the main theme of the finale——which, however, forms the core of the series of traditional cuts in that movement.

Dementia is a condition caused by and associated with separate physical changes in the brain. The signs and symptoms of dementia are very similar across the diverse types, and it is difficult to diagnose the category by behavioral symptoms alone. Diagnostic criteria have relied on a constellation of signs and symptoms, but it is critical to understand the neuroanatomical differences among the dementias for a more precise diagnosis and subsequent management. With this regard, this review aims to explore the neuroanatomical aspects of dementia to better understand the nature of distinctive subtypes, signs, and symptoms. This is a review of English language literature published from 1996 to the present day of peer-reviewed academic and medical journal articles that report on older people with dementia. This review examines typical neuroanatomical aspects of dementia and reinforces the importance of a thorough understanding of the neuroanatomical characteristics of the different types of dementia and the differential diagnosis of them.

AbstractNew communication technologies allow patients to communicate with their physicians from anywhere using computer or smartphone. Adding video to the mere phone call optimizes the personal contact between patient and physicians regardless of distance. Legal and reimbursements requirements must be taken into account, especially only certified software products must be used. In addition, patient consent is needed and confidentiality must be assured. The video patient consultation can be reimbursed by the health insurance companies. As with all new technologies, the introduction of these video consultations faced some challenges. Although patients and physicians have expressed great interest in this technology, it has been rarely used so far. The current COVID crisis increased the need for video consultations resulting in an increasing use of video patient consultation. It can be expected that this demand will still exists after the COVID crisis.

The article explores the role of antropoetonyms in the reader’s “horizon of expectation” formation. As a kind of “text in the text”, antropoetonyms are concentrating a large amount of information on a minor part of the text, reflecting the main theme of the work. As a “text” this class of poetonyms performs a number of functions: transmission and storage of information, generation of new meanings, the function of “cultural memory”, which explains the readers’ “horizon of expectations”. In analyzing the context of the literary work we should consider the function of antropoetonyms in vertical context (the link between artistic and other texts, and the groundwork system of culture), as well as in the context of the horizontal one (times’ connection realized in the communication chain from the word to the text; the author’s intention). In this aspect, the role of antropoetonyms in the structure of the literary text is extremely significant because antropoetonyms convey an associative nature, generating a complex mechanism of allusions. It’s an open fact that they always transmit information about the preceding text and suggest a double decoding. On the one hand, the recipient decodes this information, on the other – accepts this as a sort of hidden, “secret” sense.
