Recommendations from LGBTQ+ adults for increased inclusion within physical activity: a qualitative content analysis

Abstract For decades, physical activity contexts have been inherently exclusionary toward LGBTQ+ participation through their perpetuation of practices and systems that support sexuality- and gender-based discrimination. Progress toward LGBTQ+ inclusivity within physical activity has been severely limited by a lack of actionable and practical suggestions. The purpose of this study was to garner an extensive account of suggestions for inclusivity from LGBTQ+ adults. Using an online cross-sectional survey, LGBTQ+ adults (N = 766) were asked the following open-ended question, “in what ways do you think physical activity could be altered to be more inclusive of LGBTQ+ participation?” The resulting texts were coded using inductive qualitative content analysis. All coding was subject to critical peer review. Participants’ suggestions have been organized and presented under two overarching points of improvement: (a) creation of safe(r) spaces and (b) challenging the gender binary. Participants (n = 558; 72.8%) outlined several components integral to the creation and maintenance of safe(r) spaces such as: (i) LGBTQ+ memberships, (ii) inclusivity training for fitness facility staff, (iii) informative advertisement of LGBTQ+ inclusion, (iv) antidiscrimination policies, and (v) diverse representation. Suggestions for challenging the gender binary (n = 483; 63.1%) called for the creation of single stalls or gender-neutral locker rooms, as well as for the questioning of gender-based stereotypes and binary divisions of gender within physical activity (e.g., using skill level and experience to divide sports teams as opposed to gender). The findings of this study represent a multitude of practical suggestions for LGBTQ+ inclusivity that can be applied to a myriad of physical activity contexts.

ObjectivesThe aims were to identify patients with rheumatoid arthritis (RA) who had stopped drinking alcohol and compare them with patients drinking alcohol, and to explore reasons for stopping drinking alcohol.DesignA sequential explanatory mixed methods design was used.SettingSix rheumatology clinics in Southern Sweden Better Anti-Rheumatic FarmacOTherapy cohort.ParticipantsA total of 1509 patients completed the questions about alcohol and were included in the study. 86 of these had stopped drinking alcohol and 72 responded to the open question and their answers were analysed with qualitative content analysis.Outcome measuresThe quantitative data were from a cross-sectional survey assessing disease severity, physical function (Health Assessment Questionnaire, HAQ) and health-related quality of life (EuroQol five dimensions, EQ5D), pain, fatigue, patient global assessment (PatGA) and lifestyle factors, for example, alcohol. The questions assessing alcohol included an open question ‘Why have you stopped drinking alcohol?’ResultsThe patients who stopped drinking alcohol were older (median (min-max) 69 (36–90) vs 66 (23–95), p=0.011), had worse HAQ (1.00 (0–2.75) vs 0.50 (0–3.00), p<0.001), worse EQ5D (0.69 (−0.02–1.00) vs 0.76 (−0.58–1.00), p<0.001) worse PatGA (5 (0–10) vs 3 (0–10), p<0.001), more pain (5 (0–10) vs 3 (0–10), p<0.001) and more fatigue (6 (0–10) vs 4 (0–10), p<0.001 compared with patients drinking alcohol. The qualitative content analysis revealed five categories describing reasons for patients with RA to stop drinking alcohol: illness and treatment; health and well-being; work and family; faith and belief; and dependences and abuse.ConclusionsThe patients who had stopped drinking had worse physical functioning and higher levels in pain-related variables. Most stopped drinking due to their illness or a desire to improve health.

It seemed useful to us to broaden this questioning to other specialties, by proposing a study to all residents in the same Faculty. The objective of our study was to describe residents career aspirations and possible barriers according to gender.Methods – Declarative cross-sectional survey, using questionnaires sent by email to the specialty residents of the Faculty of Medicine of Lille (France). An analysis by specialty group (i.e. medicine, surgery, obstetrics & gynecology and anesthesia & resuscitation) and a comparison of the results according to gender were performed. Results – Of the 1,384 specialty residents currently in training, 462 answered the questionnaires (33.38%), among whom 289 women and 173 men (average age = 27.08 years +/- 0.091). 17 women (5.9%) were currently considering a university hospital career versus 37 men (21.4%) (p = 0.001). Gender analysis made it possible to identify obstacles to engaging in a university career: lacking female model, more frequent doubting in the ability to lead this type of career among women (61.6%) than men (35.3%) (p <0.001), and gender discrimination felt in the workplace for 51.6% of women (versus 7.5% of men, p <0.001). Subgroup analysis showed specificities related to each specialty. Conclusions – Few residents plan to embark upon a university hospital career, let alone female residents. There are specificities specific to each specialty and marked gender differences regarding career aspirations. Many obstacles have been identified as obstacles to access to university hospital positions for women. It is important to develop strategies to remove these barriers and enable women to pursue such university careers. Trial registration – Not applicable (no intervention)

Background:Pedometer is the most convenient and economical objective tool which is progressively being used to assess physical activity in adolescents. The purpose of this study was to describe the overall and gender stratified physical activity levels and to determine minimal no. of reliable days for collecting pedometer data among Pakistani adolescents. Methods:Descriptive cross-sectional study was conducted in four schools of Karachi during 2017. A total of 265 adolescents (age 13-16 years) wore unsealed Ymax Digi Walker SW-200 pedometer for a duration of 7 days including both weekends and weekdays. Data were analyzed using SPSS version 21. Independent t test and ANOVA were used for gender based stratified physical activity levels whereas, Cronbach’s alpha and Intra class correlation coefficient (ICC) were used to ascertain the reliability of pedometer data. Results:The average recorded step counts per day were 10,010 (SD: ± 4926). However, boys took 12,513 (SD: ± 5365) steps / day than girls 7,809 (SD: ± 3168) and higher steps were recorded from Thursday to Saturday. The reliability of pedometer monitoring increases with increase in number of days. The maximum pedometer reliability ? = 0.91 was observed for 7 days. Conclusion:This study concludes that adolescents are more physical active, with slightly higher reported activity among boys compared to girls. It also proposes that at least 7 days pedometer data should be collected for the reliable estimation of adolescent’s physical activity. The resulting data provides key information regarding pedometer-determined physical activity which may be useful for formulating strategies and other interventions to increase level of adolescents physical activity.

Sexual and gender issues have been a problem in Africa and in Nigeria in particular where females were considered less important than their male counterparts in their families, cooperatives, businesses, academic matters among others. Gender-Based Violence (GBV) is deeply rooted in many cultural and traditional values. It is regarded as a normal attitude, remains hidden and tacitly condoned. It has devastating health impacts on the women, as women are mostly controlled and dominated by their partners in a relationship and must never complain of sexual harassment and in the case of childlessness; the woman must accept it as her fault. This study was located in Nsukka Local Government area of Enugu State. A cross sectional survey of 200 study participants was conducted. The instruments used for data collection were structured 194 questionnaire and6 in-depth interview guide. The data was analyzed using Statistical Package for Social Sciences (SPSS), frequency distribution tables and chi-square (χ²) for hypotheses. The findings from the study stated that sexual/psychological, physical violence and preference of male child are the major forms of GBV in Nsukka, Enugu State. Majority of the respondents (55.7%) indicated that Spiritual manipulation and illiteracy/ignorance are the major causes of GBV. The study also concluded and recommended that mass sensitization/awareness, empowerment, advocacy by social workers, through domestication of the Convention to Eliminate All Forms of Discrimination against Women (CEDAW), will help curb GBV as indicated by 42.7% of the respondents.

Background Chhaupadi is a deeply rooted tradition and a centuries-old harmful religio-cultural practice. Chhaupadi is common in some parts of Karnali and Sudurpaschim Provinces of western Nepal, where women and girls are considered impure, unclean, and untouchable in the menstrual period or immediately following childbirth. In Chhaupadi practice, women and girls are isolated from a range of daily household chores, social events and forbidden from touching other people and objects. Chhaupadi tradition banishes women and girls into menstruation huts’, or Chhau huts or livestock sheds to live and sleep. These practices are guided by existing harmful beliefs and practices in western Nepal, resulting in poor menstrual hygiene and poor physical and mental health outcomes. This study examined the magnitude of Chhaupadi practice and reviewed the existing policies for Chhaupadi eradication in Nepal. Methods We used both quantitative survey and qualitative content analysis of the available policies. First, a quantitative cross-sectional survey assessed the prevalence of Chhaupadi among 221 adolescent girls in Mangalsen Municipality of Achham district. Second, the contents of prevailing policies on Chhaupadi eradication were analysed qualitatively using the policy cube framework. Results The current survey revealed that most adolescent girls (84%) practised Chhaupadi in their most recent menstruation. The Chhaupadi practice was high if the girls were aged 15–17 years, born to an illiterate mother, and belonged to a nuclear family. Out of the girls practising Chhaupadi, most (86%) reported social and household activities restrictions. The policy content analysis of identified higher-level policy documents (constitution, acts, and regulations) have provisioned financial resources, ensured independent monitoring mechanisms, and had judiciary remedial measures. However, middle (policies and plans) and lower-level (directives) documents lacked adequate budgetary commitment and independent monitoring mechanisms. Conclusion Chhaupadi remains prevalent in western Nepal and has several impacts to the health of adolescent girls. Existing policy mechanisms lack multilevel (individual, family, community, subnational and national) interventions, including financial and monitoring systems for Chhaupadi eradication. Eradicating Chhaupadi practice requires a robust multilevel implementation mechanism at the national and sub-national levels, including adequate financing and accountable systems up to the community level.
