: Autism is a neurodevelopmental disorder belonging to the autism spectrum disorder (ASD). In ASDs, the individuals show substantial impairments in social communication, repetitive behaviours, and sensory behaviours deficits in the early stages of their life. Globally, the prevalence of autism is estimated to be less than 1%, especially in high-income countries. In recent decades, there has been a drastic increase in the incidence of ASD, which has put ASD into the category of epidemics. Presently, two US Food and Drug Administration-approved drugs, aripiprazole and risperidone are used to treat symptoms of agitation and irritability in autistic children. However, to date, no medication has been found to treat the core symptoms of ASD. The adverse side effects of conventional medicine and limited treatment options have led families and parents of autistic children to turn to complementary and alternative medicine (CAM) treatments, which are perceived as relatively safe compared to conventional medicine. Recently, N,N-dimethylglycine (DMG), a dietary supplement, has emerged as a useful supplement to improve the mental and physical state of children with ASD. The current review discusses ASD, the prevalence of ASD, CAM approach and efficacy of CAM treatment in children with ASD. Moreover, it highlights the chemistry, pharmacological effect, and clinical studies of DMG, highlighting its potential for improving the lifestyle of children with ASD.

The effects of perinatal complications and quality of environment were studied in a cohort of 1955 births and a selected sample of 1956 births in the Kauai Pregnancy Study. The 10-year follow-up of the 866 children utilized records, teachers' ratings, and group tests as screening devices and diagnostic examinations by psychologists, pediatricians, and allied disciplines. Independent environmental ratings of socioeconomic status, educational stimulation, and emotional support were based on home interviews. Differences found between children with and without perinatal stress centered on a small group of survivors of severe complications who had a significantly higher proportion of major physical handicaps, predominantly of the central nervous and sensory systems, a higher proportion of I.Q.'s below 85 and lower mean scores on the factors V, R, N, and P of the Primary Mental Abilities Test. No differences were found between children with and without perinatal complications in the proportion of poor grades and the incidence of language, perceptual, and emotional problems, except for those in institutions. Environmental ratings showed a significant association with intellectual, achievement, and emotional problems at age 10. Many more children were affected by unfavorable environment than by severe perinatal stress.

We investigated the self-concepts of Tourette Syndrome patients and their mothers. The subjects were 30 children with Tourette Syndrome and their mothers, and 30 children matched on age, sex, and socioeconomic status and their mothers. The results indicated that, while the children did not differ on self-concept, the mothers of the Tourette Syndrome patients had lower self-concepts than the mothers of the controls, suggesting that family members should be considered in the treatment of the Tourette Syndrome children. In addition, severity of illness was positively correlated with the children's reports of behavior disturbance and dysphoric mood, although not with over-all self-concept. These results indicate that in more severe cases of Tourette Syndrome, children may experience emotional problems requiring therapeutic intervention.

AbstractEmotional and behavioral problems (EBPs) frequently occur in young autistic children. Discrepancies between parents and other informants are common but can lead to uncertainty in formulation, diagnosis and care planning. This study aimed to explore child and informant characteristics are associated with reported child EBPs across settings. Participants were 83 4–8-year-old autistic children and their parents and teachers in the Autism Spectrum Treatment and Resilience (ASTAR) study. Questionnaires of child EBPs were completed by parents and teachers, and self-reported parenting stress and wellbeing measures were obtained. An observation of parent–child/researcher-child interaction was also completed. Parents reported more EBPs than teachers and parent-teacher agreement was low, particularly for emotional problems. Greater parenting stress and being verbal was associated with more parent- but not teacher-reported EBPs. More observed behaviors that challenge were displayed by minimally verbal children. More parenting stress could be associated with the presence of more EBPs in the home; alternatively, parenting stress may confound reports. It is essential for assessments of EBPs in autistic children to take a multi-informant approach. Better understanding of the associations between informant characteristics and informant discrepancies of EBPs is needed.

Backgrounds and aims Early symbolic play abilities are closely related to long-term language development for both autistic and non-autistic children, but few studies have explored these relations for different dimensions of pretence and of language. The current study explores carer-reported measures of solitary symbolic play, object substitution and peer role play abilities at age 3, and their respective relations with parent-reported semantics, syntax and narrative abilities at age 7 for both autistic and non-autistic children. Methods We conducted secondary data analyses exploring links between different aspects of pretence and of language on the Longitudinal Study of Australian Children population cohort. We identified 92 autistic children and used propensity score matching to match them with 92 non-autistic children based on demographic and developmental information such as non-verbal IQ and socioeconomic status. We explored concurrent and longitudinal relations using correlation and regression models. Results: Both correlational and hierarchical regression analyses confirmed the significant effects of age 3 symbolic play abilities in facilitating age 7 semantics, syntax and narrative abilities for autistic children. We found that object substitution held most prominent influence, followed by peer role play and solitary symbolic play. In contrast, for non-autistic children, none of the age 3 symbolic play abilities were significant predictors, whereas socioeconomic status at birth and age 3 language abilities held significant influences on their age 7 semantics, syntax and narrative abilities. Conclusion: We discuss the implications of our findings for play interventions targeting language outcomes.

Background Behavior and emotional problems in a mentally re-tarded child can inhibit the educational process of the child.Objectives The aims of this study were to find out the prevalenceof behavior and emotional problems in children with mental retar-dation as reported by parents and to assess associated risk fac-tors consisting of mother’s psychopathology, marital discord, malesex, child’s age, socioeconomic status, and family size.Methods This cross sectional study was carried out on 63 chil-dren with mental retardation in a school for retarded children, SLBC Asih Budi I, from January until March 2003. Parents were inter-viewed at school and home using special questionnaires, the ChildBehavior Checklist (CBCL) and Symptom Checklist-90 (SCL-90).Results The prevalence of behavior and emotional problems inthis study was 52%. The majority of behavior and emotional prob-lems were anxious/depressed (18%) and withdrawn (16%). Malesex and age group of 12-18 year-old were not risk factors of be-havior and emotional problems (OR=0.49, p=0.190 and OR=1.14,p=0.94, respectively) while socioeconomic status, family size, andmother’s psychopathology were risk factors (OR= 4.08, p=0.008;OR=4.17, p=0.014; OR=9.28, p=0.018; respectively). There wasa correlation between behavior and emotional problems of chil-dren and marital discord.Conclusion The prevalence of behavior and emotional problemswas 52% in which the majority was internalizing disorder. Risk fac-tors for behavior and emotional problems were mother’s psycho-pathology, four or more children in the family, low socioeconomicstatus, and marital discord

Abstract Self-reported studies alone cannot be used to deduce the significance of adolescent problems. Therefore, the current study focuses on parents’ and class teachers’ perspectives of school-going adolescents’ emotional and behavioural problems. The objectives are to apprehend the prevalence of adolescent problems, comparisons on gender, school-area; and its association with socio-demographic details. A cross-sectional questionnaire-based study was conducted among 19 schools from government-private and rural-urban schools across Kollam District, Kerala. Malayalam/English version of the Strength and Difficulties Questionnaire (SDQ) was administered among a sample of 600 parents and 60 class teachers of school-going adolescents. Gender correlation with parents’ report found emotional problems (P<0.01), conduct problems (P<0.01), hyperactivity (P<0.001), peer problems (P<0.001) whereas in teachers’ report found relationship with emotional problems (P<0.05) and hyperactivity (P<0.001). Multivariate test results reveal that there is a statistically significant difference in emotional and behavioural problems of school-going adolescents based on school area (government-urban, government-rural, private-urban and private-rural). Multiple linear regression analysis on parents reports is significantly predicted with gender (P<0.01), urban-rural settings (P<0.001) and socioeconomic status (P<0.01). Teachers’ report has significantly been predicted with urban-rural settings (P<0.01) and socioeconomic status (P<0.001). Parents’ reports revealed a less overall prevalence of emotional and behavioural problems than teachers’ reports. Female adolescents tend to have more emotional problems and males were found to be more hyperactive. A significant difference was found with the school-area settings and significant association with socio-demographic details of adolescents. More attention is needed for the protection of adolescents’ mental health and fills mental health gaps in services.

Autism diagnosis is moving from the identification of common inherited genetic variants to a systems biology approach. The aims of the study were to explore metabolic perturbations in autism, to investigate whether the severity of autism core symptoms may be associated with specific metabolic signatures; and to examine whether the urine metabolome discriminates severe from mild-to-moderate restricted, repetitive, and stereotyped behaviors. We enrolled 57 children aged 2–11 years; thirty-one with idiopathic autism and twenty-six neurotypical (NT), matched for age and ethnicity. The urine metabolome was investigated by gas chromatography-mass spectrometry (GC-MS). The urinary metabolome of autistic children was largely distinguishable from that of NT children; food selectivity induced further significant metabolic differences. Severe autism spectrum disorder core deficits were marked by high levels of metabolites resulting from diet, gut dysbiosis, oxidative stress, tryptophan metabolism, mitochondrial dysfunction. The hierarchical clustering algorithm generated two metabolic clusters in autistic children: 85–90% of children with mild-to-moderate abnormal behaviors fell in cluster II. Our results open up new perspectives for the more general understanding of the correlation between the clinical phenotype of autistic children and their urine metabolome. Adipic acid, palmitic acid, and 3-(3-hydroxyphenyl)-3-hydroxypropanoic acid can be proposed as candidate biomarkers of autism severity.
