Purpose Problems in health-care leadership are serious in West Sumatra, Indonesia, especially in hospitals, which are controlled locally. The purpose of this paper is to present the experience of three hospitals in balancing the conflicting demands of the national health-care system and the traditional model of leadership in the local community. Design/methodology/approach Three case studies of the hospital leadership dynamic in West Sumatra were developed from in-depth interviews with directors, senior administrators and a representative selection of employees in various professional categories. Findings An analysis of findings shows that traditional views about leadership remain strong in the community and color the expectations of hospital staff. Hospital directors, however, are bound by the modern management practices of the national system. This conflict has intensified since regional autonomy which emphasizes the local culture much more than in the past. Research limitations/implications The research was carried out in one Indonesian province and was limited to three hospitals of different types. Practical implications The findings elucidate a potential underlying cause of problems in hospital management in Indonesia and may inform culturally appropriate ways of addressing them. Originality/value The social and cultural contexts of management have not been rigorously studied in Indonesia. The relationship between local and national culture reported here likely has a similar effect in other parts of the country.

Purpose This paper aims to explore avenue where suppliers and manufacturers are aligned with health-care providers to improve supply chain visibility. Supply chain finance is explored to link suppliers/manufacturers with health-care providers. Design/methodology/approach Existing literature on supply chain visibility in health care forms a basis to achieve the study purpose. Alignment calls also for financial health where supply chain partners’ working capital is readily available to execute joint supply chain plan. Findings There is a disjoint in supply chain alliance between suppliers/manufacturers and providers where providers are unable to trace the origin of supplies. Quality care suffers and cost of care rises as providers search for supplies on an emergency basis. This paper provides a framework where solution can be formulated. Research limitations/implications Suppliers/manufactures form a direct strategic alliance with providers where product visibility enables health-care providers with a better patient management with lower cost of supplies. Inventory management and logistics cost will be lowered as better planning/forecasting is in place. This paper does not call for testing any hypothesis. Perhaps, next move along this line will be to investigate financial health of supply chain partners based on supplier relationship management practices. Originality/value This paper proposes health-care supply chain as an alternative solution to achieve the following twin purposes: controlling the cost while improving quality of care through supply chain finance. As far as we know, this study is the first attempt to achieve the goals.

Purpose Health-care (HC) is one of the most polluting industries and recognised as the second energy-intensive sector, emitting 8 per cent out of total 40 per cent of total carbon emissions. Integrating sustainability to facilities management operations is imperative and could significantly contribute to reducing energy consumption, waste and day-to-day operational costs of buildings. The integration of sustainability into FM practices depends on factors such as facility type, organisational scale, business sector and organisation characteristics. This paper aims to explore the SFM practice with a specific focus on HC-specific FM services and respective sustainable practice that could be integrated into FM operations. Design/methodology/approach A Delphi survey was administered to ten experts in two rounds, who are specialised in FM and sustainable practices in the HC sector. Data gathered from the survey were analysed using the Relative Importance Index to identify the most significant FM services and sustainable practices. Findings The study found 9 significant FM services and 49 sustainable in HC. The top three significant FM services include “building services (BS)”, “space planning (SP)” and “quality management (QM)”. Further, “identifying applications for energy-saving measures” and “ensure onsite, off-site storage and transport of wastes” were found as the topmost significant sustainable practices. The relevancy of these identified sustainable practices to the principles of sustainability was determined. The results showed that 22, 18 and 09 sustainable practices were relevant to environmental, social and economic principles of sustainability, respectively. Originality/value There is a dearth of literature that integrates sustainable FM practices in HC sector, and this study fulfills this research gap. The study is novel in offering a framework to integrate sustainability into FM practice in HC sector.

Purpose This paper aims to identify barriers toward the adoption of blockchain (BC) technology in Indian health-care industry and also examines the significant issues of BC applications in health-care industry. Design/methodology/approach The barriers of the study are identified by two phases including the review of literature and semistructured interviews with hospital staff and administration operating in India. The experts (N = 15) are being taken from top-level management, IT experts and patients from the hospitals. The study implemented integrated total interpretative structural modeling-FUZZY-Cross-impact matrix multiplication applied to classification (TISM-FUZZY-MICMAC) methods for identifying the interrelationship among the barriers. Findings A total of 15 barriers have been determined in the Indian health-care industry through discussion with the selected experts. TISM is applied to develop multilevel structure for BC barriers. Further, FUZZY-MICMAC has been used to compute driving and dependent barriers. The findings suggest that low awareness related to legal issues and low support from high level of management have maximum driving power. Research limitations/implications The present study applies multicriterion approach to identify the limited barriers in BC adoption in health care. Future studies may develop the relationship and mark down the steps for implementation of BC in health-care setting of a developing economy. Empirical study can be conducted to verify the results along with selected case studies. Practical implications The present study identifies the BC adoption barriers in health-care industry. The study examines the pertinent issues in context to major support required, bottlenecks in adoption, key benefits of adoption planning and activities. The technology adoption practices are expected to provide applications such as distributed, secured medical and clinical data and patient centric systems that will enhance the efficiency of the health-care industry. Originality/value The study is among few primary studies that identify and analyze the BC adoption in health-care industry.

Purpose – The focused factory is one of the concepts that decision-makers have adopted for improving health care delivery. However, disorganized definitions of focus have led to findings that cannot be utilized systematically. The purpose of this paper is to discuss strategic options to focus health care operations. Design/methodology/approach – First the literature on focus in health care is reviewed revealing conceptual challenges. Second, a definition of focus in terms of demand and requisite variety is defined, and the mechanisms of focus are explicated. A classification of five focus strategies that follow the original idea to reduce variety in products and markets is presented. Finally, the paper examines managerial possibilities linked to the focus strategies. Findings – The paper proposes a framework of five customer-oriented focus strategies which aim at reducing variety in different characteristics of care pathways: population; urgency and severity; illnesses and symptoms; care practices and processes; and care outcomes. Research limitations/implications – Empirical research is needed to evaluate the costs and benefits of the five strategies and about system-level effects of focused units on competition and coordination. Practical implications – Focus is an enabling condition that needs to be exploited using specific demand and supply management practices. It is essential to understand how focus mechanisms differ between strategies, and to select focus that fits with organization’s strategy and key performance indicators. Originality/value – Compared to previous more resource-oriented approaches, this study provides theoretically solid and practically relevant customer-oriented framework for focusing in health care.

Purpose – The purpose of this paper is to demonstrate that there was a non-Eurocentric approach being attempted at Borobudur in the 1970s and 1980s. From the case of Borobudur, this research aims to provide lessons for better management practices by clarifying the development of the buffer zone concept and its potential impact on communities. Design/methodology/approach – The paper will examine an empirical literature study using topographic maps, contracts between Governments of Indonesia and Japan, documents concerning the Park Project and local newspapers as well as a sequence of one-to-one and group interviews with the key persons who were involved in the Park Project including villagers at Borobudur. Findings – The development of the Borobudur Archaeological Park began in 1979 and marks an important shift in thinking about buffer zones from a monument-centric approach to the consideration of the wider context and approaches to community participation in the context of international heritage management. Originality/value – There has not yet been a detailed study concerning the progression of the Borobudur Park establishment and implementation of the buffer zone concept per se, and its impact on local community members. Furthermore, crucial management planning documents for the establishment of the Borobudur Archaeological Park have yet to be analysed by scholars. One of the documents is entitled the Updated Former Plans and Schematic Design for Borobudur and Prambanan National Archaeological Parks Project (Updated Plan). The Updated Plan is in principle not disclosed to the public and can only be viewed with permission of the Indonesian authorities thereby lessening opportunities for research to be undertaken on how the Japan International Cooperation Agency Master Plan was modified and the Park Project was executed in the 1970s and 1980s. The author opportunely received permission to access to the Updated Plan by the authorities on 23 November 2012, which made this study possible to pursue.

PurposeIn the field of critical heritage studies, it has often been argued that a more inclusive and participatory heritage management approach neutralises differences and can contribute to a more contemporary, plural, democratic and inclusive notion of heritage. Yet, the needs and aspirations of those assumed being excluded from heritage making are not always taken into account, because the analysis and critique often focussed on the dominant heritage discourses, organisations and institutions. This paper conceptualises heritage from below and explores and reconceptualises how subdominant notions of heritage relate to dominant, institutionalised conceptualisations of heritage.Design/methodology/approachBased on a case study of the Mobarak mosque in The Hague, the authors present the multiplicity of subdominant conceptualisations of heritage, the ways heritage is (expected to be) recognised and represented by the community and the complexity of issues of social inclusion/exclusion.FindingsThe authors conclude that inclusive and/or participatory heritage management practices are inclusive in name only when the needs and aspirations of those seemingly being “excluded” are not fully understood and taken into account.Originality/valueA binary heritage/non-heritage framework, the authors argue, is limited to understand matter that matters. Also the authors find that the assumption that there is a growing desire among local community groups to include their histories and related materialities as heritage in the dominant heritage discourse should be challenged.

Purpose This paper aims to gain insights about information management practices in public health-care organizations in Kuwait and offer recommendations to improve these practices. Design/methodology/approach This study involves secondary analysis of quantitative and qualitative accreditation-related data pertaining to the compliance with the Information Management standard at seven public tertiary health-care facilities over two accreditation cycles. Findings Overall, organizations improved their compliance with the Information Management standard. However, issues exist with effectively and efficiently transmitting data, aggregating clinical and administrative data and using the information for both strategic planning and quality improvement initiatives. Research limitations/implications The analysed data set does not provide information about the improvements done between the accreditation cycles. Caution should be applied before assuming generalizability of the results, considering the context and social constructs around the health-care system is essential. Practical implications Compliance with predetermined criteria through accreditation can improve information management practices. Without proper management of information at health-care facilities, achieving safe and effective patient care is futile. The role of health information technology (IT) should not be sidelined; robust health IT solutions can help support good information management practices thereby improving care quality and aiding health-care reform. Originality/value Concerning information management, health-care organizations providing focused services have clear advantages over organizations providing general care services. Considering the type of care organization (general vs specialized) can provide insights into how information management practices can affect the operations of the organization.

Purpose Youth with autism spectrum disorder (ASD) often have co-occurring health care needs and are likely to come into contact with several health care professionals over their lives. At the hospital, youth with ASD may require specialized supports to optimize health care experiences and for medical services to be delivered safely. At present, there is a limited understanding of how to best support this patient population. The purpose of this paper is to develop, implement, and evaluate an online training module for hospital staff about ASD. Design/methodology/approach To evaluate participants’ perceived utility of the learning tool, a post-module survey was administered. Findings In all, 102 health care professionals and other hospital staff completed the training and evaluation measure. Majority of participants had prior ASD-focused education (66 percent) and had experience working with at least 20 youths with ASD (57 percent). Majority of participants (88 percent) perceived the information from the module to be helpful in their daily work and reported that they learned something new (63 percent). Participants were interested in receiving additional ASD online module training opportunities on topics including: hands-on behavior management strategies, in-hospital resources, guidance on treatment adherence, and ASD training geared specifically to protection services staff. Originality/value The results from this evaluation have important practice implications for hospital staff working with patients with ASD and their families. Evidence-based strategies were easily accessible for staff and the module can be feasibly built upon and expanded as well as disseminated beyond the current hospital setting.

Purpose The purpose of this paper is to outline several critical risk theories and explore their application to risk concerns in mental health care. This will contribute to the on-going debate about risk management practices and the impact these might have on recovery and social inclusion. Notably, while risks like suicide can be therapeutically addressed, risk management may involve paternalistic practices that exclude the participation needed for recovery. Design/methodology/approach A viewpoint of key risk theories will be presented to provide a critical eye about some clinical risk concerns in mental health care. Implications for recovery and social inclusion will then be discussed alongside direction for practice and research. Findings Clinical concerns seemed to involve difficulties with uncertainty, holding onto expertise, and the othering of patients through risk. These concerns suggest the patient voice might become lost, particularly within the backdrop of clinical fears about blame. Alternatively, a relational approach to risk management could have merit, while patient expertise may develop understanding in how to improve risk management practices. Originality/value Clinical concerns appear more than managing potential harms; it can involve appraising behaviours around societal norms, explaining to an extent why mental illness might be addressed in terms of risks. While the points raised in the paper support existing findings about risk management, the underlying reasons drawing on the critical risk theories are less explored.

Purpose This paper aims to discuss the effects of COVID-19 on hotel marketing and management practices and outlines a three-pronged research agenda to stimulate knowledge development in the hotel sector. Design/methodology/approach This paper is based on an overview of the relevant literature on hotel marketing and management and the hotel guest behavior. The authors also investigated hospitality service trends to propose a research agenda. Findings This paper presents a research agenda from three dimensions – artificial intelligence (AI) and robotics, hygiene and cleanliness and health and health care. First, different types of AI (mechanical, thinking and feeling) might open up distinct research streams at the intersection of health crises and hotel management, in light of the COVID-19 pandemic. Additionally, this paper recommends that researchers move beyond typical perspectives on the antecedents and outcomes of hotel hygiene and cleanliness to delve into guests’ perceptions of the cleanliness of specific hotel surfaces. Furthermore, a more in-depth analysis is warranted about the evolving relationship between hotels and the health-care sector. Practical implications The recommended research areas are intended to advance the knowledge base to help hotels recover from the COVID-19 pandemic. The suggested research streams are expected to provide actionable insights to promote the development and sustainability of the hotel sector. Originality/value This paper appears to be a frontier study, critically examining possible effects of the COVID-19 pandemic on hotel marketing and management practices and how hoteliers may respond to such challenges to recover after this pandemic.
