Abstract. Background: Hypovitaminosis D is a vitamin deficiency that has been increasing in developed countries; it was also suggested as an emerging risk factor for developing of atherosclerosis and acute myocardial infarction. The primary source of vitamin D is its cutaneous synthesis under exposure to sunlight. It has been suggested that 30 min of sun exposure twice weekly leads to sufficient vitamin D synthesis. The residents of Trieste (Italy) are well-known for their high exposure to sunlight in all seasons. We aimed to investigate the vitamin D status in subjects with acute myocardial infarction living in this area. Methods: Vitamin D status was identified in 478 subjects diagnosed with acute myocardial infarction. Results: The median serum 25-hydroxyvitamin D concentration was 14.5 [7.8 - 22.7] ng/mL. Vitamin D deficiency and insufficiency were present in 324 (68 %) and 107 (22 %) subjects, respectively. Vitamin D deficiency was less frequent among subjects enrolled in the period from July to the end of September (p < 0.001). In a multivariate analysis vitamin D deficiency was predicted by older age (p = 0.02), female gender (p = 0.002), higher body mass index (p = 0.05), autumn/winter sampling (p < 0.001), increased parathyroid hormone (p = 0.03) and alkaline phosphatase (p = 0.003). Conclusions: We observed very high prevalence of vitamin D deficiency among subjects with myocardial infarction in all seasons of enrollment. However, it was lower in the summer when sun exposure is higher. The exposure to sunlight may be a cost-saving therapeutic strategy for the management of vitamin D deficiency.

Background: Breast milk is considered the optimal source of nutrition during infancy. Although the vitamin D concentration in human breast milk is generally considered poor for infants, vitamin D in breast milk is an important source for exclusively breastfed infants. Increases in vitamin D insufficiency and deficiency in lactating mothers may reduce vitamin D concentrations in breast milk. This study aimed to compare vitamin D and 25-hydroxyvitamin D (25OHD) concentrations in breast milk collected in 1989 and 2016–2017 and simultaneously analyze them with liquid chromatography-tandem mass spectrometry (LC-MS/MS); the association between the lifestyle of recent lactating mothers (2016–2017) and vitamin D status in human breast milk was also evaluated. Method: Lactating mothers were recruited from three regions of Japan in 1989 (n = 72) and 2016–2017 (n = 90), and milk from 3–4 months was collected in summer and winter. The samples were strictly sealed and stored at −80℃ until measurement. Breast milk vitamin D and 25OHD concentrations were analyzed by LC-MS/MS. Vitamin D intake, sun exposure, and sunscreen use of the lactating mothers in 2016–2017 were assessed. Results: Both vitamin D and 25OHD concentrations in breast milk were higher in the summer regardless of the survey year. Significantly lower vitamin D and 25OHD concentrations were observed in 2016–2017 compared with 1989 in summer, but no survey year difference was observed in winter. The stepwise multiple regression analyses identified season, daily outdoor activity, and suntan in the last 12 months as independent factors associated with vitamin D3 concentrations. Conclusion: The results suggest that low vitamin D status in recent lactating mothers may have decreased vitamin D and 25OHD concentrations in breast milk compared with the 1980s. These results are helpful for developing public health strategies to improve vitamin D status in lactating mothers and infants.

Objective: To determine whether patients with chronic kidney disease (CKD) without dialysis their stage impacts the native vitamin D levels. Methods: Patients over 18 years with chronic kidney disease stage 2-5 without dialysis treatment. They demographic, anthropometric variables, degree of sun exposure, disease etiology and laboratory variables related to bone and mineral disorders were evaluated. Study analytical cross-sectional prospective. Descriptive statistical methods for quantitative and qualitative are characterized, and analytical correlation between levels of vitamin D statistical laboratory tests related to bone and mineral disorders, sun exposure and ethnicity variables for each stage were characterized. Results: 331 patients were evaluated, with a mean age of 71 years, the mestizo majority (71%), 173 women, main etiology of CKD hypertensive nephropathy (33.2%). 21.1% of patients had normal levels of vitamin D, 70.1% insufficient, and 8.8% in deficit. Negative correlation was detected between the levels of vitamin 25(OH)D and serum creatinine, phosphorus, calcium x phosphorus product, PTH, proteins in urine 24 hours and BMI. Positive correlation for calcium and albumin. Positive statistical significance between the levels of vitamin 25(OH)D and sun exposure for 3b and 4 stages was found. Conclusions: In patients with CKD is common to detect low levels of vitamin 25(OH)D, which can contribute to the generation of secondary hyperparathyroidism.

Objective. The aim of the present study was to compare the serum levels of total antioxidant status (TAS) and 25(OH) D3 and dietary intake of multiple sclerosis (MS) patients with those of normal subjects.Method. Thirty-seven MS patients (31 women) and the same number of healthy matched controls were compared for their serum levels and dietary intake of 25(OH) D3 and TAS. Sun exposure and the intake of antioxidants and vitamin D rich foods were estimated through face-to-face interview and food frequency questionnaire.Results. Dietary intake of antioxidants and vitamin D rich foods, vitamin C, vitamin A, and folate was not significantly different between the two groups. There were also no significant differences in the mean levels of 25(OH) D3 and TAS between the study groups. Both groups had low serum levels of 25(OH) D3 and total antioxidants.Conclusion. No significant differences were detected in serum levels and dietary intake of vitamin D and antioxidants between MS patients and healthy controls. All subjects had low antioxidant status and vitamin D levels.

Multiple studies have provided evidence for an association between reduced sun exposure and increased risk of multiple sclerosis (MS), an association likely to be mediated, at least in part, by the vitamin D hormonal pathway. Herein, we examine whether the vitamin D receptor ( VDR), an integral component of this pathway, influences MS risk in a population-based sample where winter sun exposure in early childhood has been found to be an important determinant of MS risk. Three polymorphisms within the VDR gene were genotyped in 136 MS cases and 235 controls, and associations with MS and past sun exposure were examined by logistic regression. No significant univariate associations between the polymorphisms, rs11574010 ( Cdx-2A > G), rs10735810 ( Fok1T >  C), or rs731236 ( Taq1C > T) and MS risk were observed. However, a significant interaction was observed between winter sun exposure during childhood, genotype at rs11574010, and MS risk ( P = 0.012), with the ‘G’ allele conferring an increased risk of MS in the low sun exposure group (≤2 h/day). No significant interactions were observed for either rs10735810 or rs731236, after stratification by sun exposure. These data provide support for the involvement of the VDR gene in determining MS risk, an interaction likely to be dependent on past sun exposure.

Pancreatic cancer is a malignancy of poor prognosis which is mostly diagnosed at advanced stages. Current treatment modalities are very limited creating great interest for novel preventive and therapeutic options. Vitamin D seems to have a protective effect against pancreatic cancer by participating in numerous proapoptotic, antiangiogenic, anti-inflammatory, prodifferentiating, and immunomodulating mechanisms. 25-hydroxyvitamin D [25(OH)D] serum concentrations are currently the best indicator of vitamin D status. There are three main sources of vitamin D: sun exposure, diet,and dietary supplements. Sun exposure has been associated with lower incidence of pancreatic cancer in ecological studies. Increased vitamin D levels seem to protect against pancreatic cancer, but caution is needed as excessive dietary intake may have opposite results. Future studies will verify the role of vitamin D in the prevention and therapy of pancreatic cancer and will lead to guidelines on adequate sun exposure and vitamin D dietary intake.

Purpose Vitamin D deficiency is a well-recognised public health problem within the UK, with specific population groups more vulnerable to deficiency. Two pilot studies were used to explore awareness of vitamin D deficiency and attitudes towards food fortification. Design/methodology/approach A survey of 120 participants from five at-risk groups (South Asians, Blacks, Middle Eastern, Far Eastern and Caucasian older adults over 65 years) plus a group of British Caucasians who do not avoid sun exposure explored awareness of vitamin D, sun exposure knowledge and behaviour and attitudes towards food fortification. The latter group was included to provide a comparison group who were at a reduced risk of deficiency. χ2 was used to test associations between categorical variables and the study groups. The second study used three focus groups and two interviews, conducted on young South Asian females and examined knowledge and awareness of vitamin D and vitamin D-fortified foods. Findings A lack of knowledge and misconceptions were highlighted by both studies in relation to at-risk factors, including sunlight exposure (p = 0.037), dietary intakes (p = 0.0174) and darker skin pigmentation (p = 0.023), sources of vitamin D and the health benefits associated with optimal consumption. Attitudes to mandatory fortification of some foods varied significantly (p = 0.004) between the groups with acceptance rates for Blacks (68 per cent), those over 65 years (50 per cent), Middle Eastern (67 per cent) and Far Eastern (73 per cent), whereas the control (71 per cent) showed no acceptance, and South Asians gave a mixed response (48 per cent No). Focus group findings highlighted positive views towards fortification, although this was less for mandatory as opposed to voluntary fortification. Both pilot studies highlight the need for more research into this area, to create more effective public health policies. Originality/value The research presents novel insights into a topical area where there is limited research.

Abstract Objectives 1. Determine acceptability and feasibility of a phototherapy kiosk designed as a self-care intervention for low vitamin D; 2. Demonstrate that narrow spectrum ultraviolet (UV) B delivered by the kiosk is equivalent to recommended daily D3 supplementation to raise or maintain serum 25-hydroxyvitamin (OH)D levels at or above 30 ng/mL; and 3. Examine the relationship of demographic variables, including age, gender, body mass index, physical activity, ethnicity, skin type, season, and sun exposure to serum 25(OH)D levels in both treatment arms. Methods Participants were randomized to either the phototherapy kiosk (PK) for a treatment every other week or 600 IU D3 oral supplement (OS) daily, for 10 weeks. PK exposure was based on response to previous treatment and Fitzpatrick Skin Type category; subjects wore undergarments only and eye protection for treatments of 2–6 minutes duration. OS group received a one-time prescription of 70 capsules, dispensed by a research pharmacist. Primary outcome was serum 25(OH)D level. Results Final sample 88 adults; OS group n = 45 and PK group n = 43. Demographics: median age 35 years, mostly female (66%), no difference observed between groups for age, race/ethnicity, or marital status. No difference seen in skin type, birth location, sun exposure, or use of sunscreen. OS group had higher BMI and %body fat. Baseline 25(OH)D level were similar, P = 0.25. At 12 weeks median OS 25(OH)D 25.5 ng/mL and PK 30 ng/mL; P = .01. At 16 weeks, OS group 25(OH)D level 21 ng/mL and PK 27 ng/mL; P = .04. Adherence: OS 92% vs PK 100%. There were no reportable adverse events. Conclusions Use of PK every other week achieved higher serum levels of 25(OH)D than usual care vitamin D3 600 IU/day for 10 weeks, although both groups returned to baseline levels 30 days post-treatment. At a time when self-care measures are highly valued for health promotion, programmed UVB delivered via PK in the community appears to be a safe, efficacious alternative to oral D3 supplementation but requires recurring use. Results from this study suggest a need for early intervention in preventable health conditions impacting Warfighter performance and readiness, particularly pre-deployment when optimizing long-term wellness for duty in austere environments.
