Effect of Urbanization on Objectively Measured Physical Activity Levels, Sedentary Time, and Indices of Adiposity in Kenyan Adolescents

Background:Urbanization affects lifestyles in the developing world but no studies have assessed the impact on objectively measured physical activity in children and adolescents from sub-Saharan Africa.Purpose:To compare objectively measured habitual physical activity, sedentary time, and indices of adiposity in adolescents from rural and urban areas of Kenya.Methods:Physical activity and sedentary time were assessed by accelerometry for 5 consecutive days in 97 (50 female and 47 male) rural and 103 (52 female and 51 male) urban adolescents (mean age 13 ± 1 years). Body Mass Index (BMI) and BMI z-scores were used to assess adiposity.Results:Rural males spent more time in moderate-to-vigorous intensity physical activity (MVPA) compared with urban males (68 ± 22 vs. 50 ± 17 min, respectively; P < .001). Similarly, Rural females spent more time in MVPA compared with urban females (62 ± 20 vs. 37 ± 20 min, respectively; P < .001). Furthermore, there were significant differences in daily sedentary time between rural and urban subjects. Residence (rural vs. urban) significantly (P < .001) influenced BMI z-score (R2 = .46).Conclusion:Rural Kenyan adolescents are significantly more physically active (and less sedentary) and have lower indices of adiposity compared with urban adolescents and this is a likely refection of the impact of urbanization on lifestyle in Kenya.

Background: Peripheral artery disease (PAD) reduces physical activity, and both PAD and lower physical activity levels have been shown to increase mortality risk. Yet, their joint contributions to mortality have not been systematically studied, especially with the objective measure of physical activity. Hypothesis: PAD and lower 1-week objectively-measured physical activity are synergistically associated with mortality. Methods: We studied 7124 Hispanic adults aged 45-74 years at baseline from the Study of Latinos who adhered to an objective measure of physical activity using wearable accelerometers. We quantified the associations of the status of PAD (ankle-brachial index [ABI] ≤0.90 [PAD], 0.91-1.39 [no PAD], and ≥1.40 [possible PAD with ankle artery calcification]) and two physical activity measures (sedentary time [tertiles] and daily physical movement counts [tertiles]) at baseline with mortality using multivariable Cox models accounting for sampling weights. Results: During a median follow-up of 7.0 years, 289 participants died. Sedentary time and daily physical movement counts were weakly correlated (r=-0.17). Both low and high ABI, longer sedentary time, and lower physical movement counts were all independently associated with increased mortality risk (marginal cells in Table ). In terms of the joint associations of PAD and physical activity with mortality, they generally demonstrated synergistic associations (e.g., hazard ratio 4.43 [95%CI, 2.44-8.03] in ABI ≤0.9 plus top tertile of sedentary time and 5.46 [2.47-12.08] in ABI ≥1.40 plus top tertile of sedentary time vs. normal ABI with shortest sedentary time) (cross-categories in Table ), with no significant interactions. Conclusions: Both low and high ABI, and lower objectively-measured physical activity levels were significantly associated with greater risk of mortality independently of each other and potential confounders. Our results suggest the importance of objectively evaluating daily physical activity levels and leg vascular condition.

Background. Regular physical activity in childhood and adolescent plays an important role in reducing the risk of cardiovascular health diseases, diabetes, and obesity in adulthood. However, little is known about physical activity levels (PA) and sedentary time among children and adolescents in Morocco. Objective. To examine gender, type of day, and age grade differences in objectively measured sedentary time, physical activity levels, and physical activity guideline attainment among children and adolescents in Morocco. Method. 172 children/adolescents (mean age = 10.92 ± 1.55 years, 49.4% are boys) were recruited for this study and wore a tri-axial accelerometer (GT3X+) for 7 consecutive days. Time spent in sedentary, PA levels, and daily steps were measured and compared according to gender, age grade, and the type of day (weekdays/weekends). Results. In weekdays children/adolescents spent more time in sedentary than weekends (p < 0.001). Boys were eight times more likely to meet the recommendation for at least 60 min of moderate to vigorous physical activity per day than girls (OR: 8.569; 95% [CI]: 4.23–17.32), p < 0.001. Conclusion. These findings highlight the need for effective and sustainable strategies and programs aiming to promote physical activity and to reduce sedentary behavior among children and adolescents in Morocco.

Background: This study examined the distribution of objectively measured physical activity (PA) and sedentary time of fifth-grade students during school, leisure time, and physical education (PE) classes. Demographic, anthropometric, and PA data were collected from 17 representative Finnish schools. Methods: To estimate the PA and sedentary time, participants (N = 592) wore wGT3X-BT ActiGraphs for 7 consecutive days. Comparisons were made between genders and different BMI groups. Results: From the study sample, 43.7% met the moderate to vigorous PA (MVPA) guidelines. Participants spent 62.2% of the day sedentary and 8.2% in moderate and vigorous activities. Boys performed more MVPA than girls, and girls were more sedentary during school days. Boys had more MVPA than girls in leisure time, but there were no differences in sedentary time. However, an examination of PA assessed during PE classes revealed no differences between boys and girls. Normal-weight boys engaged in more MVPA than overweight and obese boys. No differences were found for girls. Conclusions: The PA levels differ between different BMI groups in leisure time and during school but not during PE lessons. PA for overweight children should be targeted and compulsory PE time should be increased to achieve the PA guidelines.

Abstract Objective To study the associations of objectively measured physical activity (PA) and sedentary time (ST) with the combined outcome of incident stroke, myocardial infarction (MI) or all-cause mortality in older adults. Methods N = 3343 men and women aged 70 who participated in a health survey between 2012 and 2017 were included. Actigraph GT3X+ accelerometers were used to measure light-intensity PA (LPA), moderate-intensity PA (MPA) and ST for 1 week. Incident cases of cardiovascular disease (CVD) in terms of stroke or MI, and all-cause mortality were identified using national registers. Hazard ratios (HR) and 95% confidence intervals (CI) were estimated using multivariable-adjusted Cox regressions. Results During a mean follow-up of 2.7 years (0.1–5.6), there were 124 events of CVD or all-cause mortality. After adjusting for potential confounders and mediators, every 30-min/day increment in LPA was associated with 11% lower risk of CVD or all-cause mortality (HR 0.89, 95% CI 0.82–0.97), and every 30-min/day increment in MPA was associated with 36% lower risk (HR 0.64, 95% CI 0.48–0.84). Every 1-h/day increment in ST increased the risk of the outcomes by 33% (HR 1.33, 95% CI 1.14–1.56), although there was no significant association among participants who performed ≥ 30 min/day MPA (HR 1.11, 95% CI 0.82–1.50, P = 0.034 for interaction). None of the associations were modified by sex (P > 0.4 for all). Conclusion Objectively measured LPA and MPA are each associated with lower risk of stroke, MI or all-cause mortality in 70-year-old individuals, while ST is associated with increased risk. The greatest risk reduction is observed for MPA, which also appears to attenuate some of the increased risks associated with ST.

Summary COVID-19 disproportionately affects males especially those who are older and more socio-economically disadvantaged. This study assessed wellbeing outcomes among men’s shed members (Shedders) in Ireland at baseline (T1), 3 (T2), 6 (T3) and 12 months (T4) in response to a 10-week health promotion program ‘Sheds for Life’ (SFL). Two cohorts participated in SFL commencing in March and September 2019. This study compares the T3 findings from one cohort carried out during the COVID-19 pandemic [COVID cohort (n = 185)] with T3 findings from a comparator cohort [pre-COVID cohort (n = 195)], completed pre-COVID-19. Questionnaires assessing wellbeing [life satisfaction, mental health, loneliness, physical activity (PA), self-rated health and other lifestyle measures] were analyzed in both cohorts T1, T2 and T3. Self-rated Health and life satisfaction decreased in the COVID cohort at T3 (p &lt; 0.001), while loneliness scores increased (p &lt; 0.0005). Higher loneliness scores were correlated with lower health ratings, life satisfaction and PA during COVID-19 (p &lt; 0.001). Days PA decreased in the COVID cluster at T3 from T2 (p &lt; 0.01) with those in urban areas reporting lower activity levels than rural areas (p &lt; 0.05). Those sufficiently active at baseline managed to maintain PA during COVID-19 while those not meeting guidelines were more likely to report decreases (p &lt; 0.001). Shedders experiencing COVID-19 restrictions are at an increased risk of poorer wellbeing and increased levels of loneliness. Support and guidance are needed to safely encourage this cohort back into men’s sheds, settings that protect against loneliness and positively promote health and wellbeing. Lay summary The COVID-19 pandemic will have wide-reaching implications on wellbeing, particularly on those who are older and more vulnerable. Evidence also suggests that COVID-19 disproportionately affects males. This study aimed to understand the impact that COVID-19 has had on men in the setting of Men’s Sheds in Ireland. Two cohorts of men who were participating in a 10-week health and wellbeing program (Sheds for Life) at different stages were followed over time. At 6 months follow-up the first Cohort had not experienced COVID-19 whereas the second cohort was actively experiencing the COVID-19 pandemic. We measured wellbeing using questionnaires, comparing both groups of men for differences. We found that the men who were experiencing COVID-19 had lower self-rated health, physical activity and life satisfaction as well as higher rates of loneliness, with those who were more lonely reporting lower wellbeing scores. We also found that men in rural areas were more physically active during COVID-19 and that those were not active were more likely to become more inactive during COVID-19. This study suggests that support and guidance is needed to safely encourage this cohort back into Men’s Sheds, settings that protect against loneliness and positively promote health and wellbeing.
