A Systematic Review of Child and Adolescent Physical Activity by Schoolyard Location

The authors conducted a systematic review of children’s and adolescent’s physical activity by schoolyard location. PubMed and Web of Science were searched and articles were selected that included 3- to 17-year-olds and specifically examined and reported physical activity by schoolyard location. The primary outcomes of interest were the percentage of total time or observation intervals spent in each location and percentage of time or observation intervals in each location being sedentary or participating in moderate to vigorous physical activity. Included studies (N = 24) focused on preschoolers (n = 6), children (n = 11), adolescents (n = 2), or children and adolescents (n = 5) and primarily used direct observation (n = 17). Fields, fixed equipment, and blacktop were all important locations for physical activity participation, but there were differences by age group and sex. More research is needed that uses consistent methodology and accounts for other factors such as time of year, provided equipment, and differences in schoolyard designs.

Background and Objective: The benefits of physical activity (PA) for children and adolescents with disabilities are well documented, and children and adolescents with visual impairments (VI) engage in less PA than their sighted peers. Two reviews have summarized studies on PA of children and adolescents with VI, but no systematic review with semi-quantitative assessment has been conducted to specifically identify the correlates of their PA. This review aims to systematically summarize the existing literature, which investigated the correlates of PA of children and adolescents with VI until 2019 and identify variables that contribute to their PA participation. Methods: A systematic search using Academic Search Premier (ASP), Education Resources Information Center (ERIC), Education Source (ES), PsycINFO, Psychology and Behavioral Sciences Collection (PBSC), MEDLINE, Scopus, and Web of Science (WOS) was conducted in September 2019 to identify studies examining the correlates of PA in children and adolescents with VI aged 5 to 17 years. Two researchers independently screened studies, assessed their methodological quality and extracted relevant data. The correlates of PA among children and adolescents with VI were synthesized and further assessed semi-quantitatively. Results: A total of 17 articles identified correlates of PA in children and adolescents with VI. Out of 21 variables identified from the reviewed studies, 3 were consistently associated with PA of children and adolescents with VI. Body mass index (BMI)/obesity, percent of body fat, and visual impairment level were consistently and negatively associated with PA of children and adolescents with VI. Gender and age were identified as having inconsistent relationships with PA in children and adolescents with VI. The level of parental education was identified to have “no association” with children and adolescents with VI. Conclusions: This review can aid in developing effective interventions to improve the PA of children and adolescents with VI and propose directions for future research.

Background: The health benefits of physical activity (PA) participation are well-documented. Little was known about the PA levels of students with autism spectrum disorder (ASD) and their typically developing (TD) peers in inclusive schools. This study aimed to synthesize available studies examining PA levels of children and adolescents with and without ASD and its associated factors that affected their PA participation during inclusive schools applying the social–relational model of disability (SRMD).Methods: Eight databases were searched including CINAHL Complete, SPORTDiscus with Full Text, PubMed, Embase, Web of Science, Eric, APA PsycINFO, and Scopus from inception through May 2021 to identify related studies. Two researchers independently screened studies, assessed methodological quality, and summarized relevant data. The McMaster Critical Reviewer Form for quantitative studies was used to evaluate the methodological quality of the included articles.Results: A total of seven articles were included in this systematic review. Overall, meta-analysis results indicated that children and adolescents with ASD had a moderately decreased PA levels compared with their TD peers [SMD = −0.585, 95% CI (−0.774, −0.425), p < 0.01]. Individual-, social-, and environmental-level factors that influence PA levels in children and adolescents with ASD were identified from the perspective of SRMD.Conclusion: This review indicates that children and adolescents with ASD have lower PA levels than their TD peers in inclusive schools and multilevel factors affect their PA.

A systematic review of the prevalence of Active Play in Brazilian children and adolescents was performed. Only fully available scientific papers that measured Active Play or leisure time physical activity independently from other types of physical activity in Brazilian children and/or adolescents were considered for inclusion. The search for potential articles was performed on the following electronic databases: Pubmed/Medline, Web of Science, Bireme, Scielo and Scopus. Initially, 63 papers met the eligibility criteria. However, after deeply analyzed, seven studies remained and were included in the present review. The overall prevalence of Active Play was 36%, in which varied from 27.2% to 79.3%. Boys presented a prevalence of 47%, ranging from 39,1% to 79.9%, while the prevalence in girls reached 26%, varying from 13.9% to 78.7%. Although the prevalence of Active Play in Brazil in not satisfactory, the potential to present a reliable data regarding this theme should be highlighted. In agreement with some other studies, boys presented a higher prevalence of physical activity than girls. Also, any socioeconomic pattern regarding the prevalence of Active Play in children and adolescents was observed when the results from this study were compared to other studies developed in other countries from several socioeconomic status. The Active Play should be further investigated individually, as a pivotal component of physical activity, as well as its impact on physical inactivity-related comorbidities.

Background: This systematic review determines the levels of physical activity (PA) during school hours in children and adolescents. Methods: Studies carried out from January 1987 to December 2019 were retrieved from four databases (Web of Science, Pubmed, Scopus and SportDiscus). Results: Twenty-nine studies were included in this systematic review. Most of them used accelerometers and showed that male and female children accumulated a mean of between 14 and 68 min of moderate-to-vigorous PA (MVPA) during school hours (3–22% of this daily segment), and male and female adolescents accumulated a mean of between 13 and 28 min of MVPA during this daily segment (3–8% of the school hours). Less than a quarter of children and adolescents reached the recommended 30 min of MVPA during school hours, with notable differences between sexes. Conclusions: These results suggest that the levels of PA during school hours are not enough, and consequently, schools should develop strategies for helping children and adolescents reach the school PA recommendation.

Background:In patients with JIA, growth impairment and variance in body composition are well-known long-term complications that may be associated with prolonged drug therapy (e.g. glucocorticoids) as well as impaired physical and psychosocial well-being. An increased accumulation of body fat represents a significant risk factor for metabolic abnormalities and a modifiable variable for a number of comorbidities. Recently, evidence has emerged in favour of the potential negative influence of overweight on the course of the disease and treatment response [1].Objectives:The study aimed a) to estimate the prevalence of underweight, overweight and obesity in children and adolescents with JIA compared to the general population, and b) to investigate correlates of patients’ weight status.Methods:A cross-sectional analysis of physicians’ recorded body weights and heights of patients with JIA enrolled in the NPRD in the year 2019 was performed. Underweight (BMI <10th), overweight (BMI >90th) and obesity (BMI >97th) were defined according to age- and sex-specific percentiles used in the German reference system. For comparison with data from the general population [2], sex- and age-matched pairs of 3-17-year-old patients and controls were generated. A multinomial logistic regression analysis was performed to examine the association between weight status and patients’ clinical and self-reported outcomes.Results:In total, data from 6.515 children and adolescents with JIA (age 11.2 ± 4.1 years, disease duration 4.9 ± 3.8 years, 67% girls, 40% persistent oligoarthritis) were included. Of these, 3.334 (age 5.9 ± 2.1 years, 52.5% girls) could be considered for matched-pair analysis. Compared with the general population, patients underweight, overweight and obesity rates were 10.6% (vs. 8.1%), 8.8% (vs. 8.5%) and 6.1% (vs. 5.7%), respectively. No significant sex differences were found in either group. Largest difference in prevalence was registered for underweight, specifically in the age group 3-6 years (12.9% patients vs. 5.9% controls). Similar to the general population, higher rates of overweight were observed in adolescent patients than in affected children (19.1% age group 11-13 vs. 8.4% age group 3-6). While the highest underweight prevalence was registered in patients with RF+ polyarthritis (16%), patients with Enthesitis-related arthritis (22%), psoriatic arthritis (21%) and systemic JIA (20%) showed the highest overweight rates (including obesity). Younger age (OR = 0.51, 95% CI = 0.31-0.83), more frequent physical activity (OR = 0.92, 95% CI = 0.85-0.99) and high parental vocational education (OR = 0.39, 95% CI = 0.18-0.80) were independently associated with a lower likelihood of being overweight/obese.Conclusion:The overall prevalence of underweight, overweight and obesity in children and adolescents with JIA is comparable to that found in the general population. Behavioural health promotion, including regular physical activity, as part of the treatment strategy in JIA should preventively already begin at preschool age and necessarily be made accessible to patients of all educational levels.References:[1]Giani T et al. Wearable activity trackers (wearables) embed numerous behaviour change techniques (BCTs) that have previously been shown to increase adult physical activity (PA). With few children and adolescents achieving PA guidelines, it is crucial to explore ways to increase their PA. This systematic review examined the acceptability, feasibility, and effectiveness of wearables and their potential mechanisms of action for increasing PA in 5 to 19-year-olds. A systematic search of six databases was conducted, including data from the start date of each database to December 2019 (PROSPERO registration: CRD42020164506). Thirty-three studies were included. Most studies (70%) included only adolescents (10 to 19 years). There was some—but largely mixed—evidence that wearables increase steps and moderate-to-vigorous-intensity PA and reduce sedentary behaviour. There were no apparent differences in effectiveness based on the number of BCTs used and between studies using a wearable alone or as part of a multi-component intervention. Qualitative findings suggested wearables increased motivation to be physically active via self-monitoring, goal setting, feedback, and competition. However, children and adolescents reported technical difficulties and a novelty effect when using wearables, which may impact wearables’ long-term use. More rigorous and long-term studies investigating the acceptability, feasibility, and effectiveness of wearables in 5 to 19-year-olds are warranted.

ObjectiveTo explore whether physical activity is inversely associated with the onset of depression, we quantified the cumulative association of customary physical activity with incident depression and with an increase in subclinical depressive symptoms over time as reported from prospective observational studies.DesignSystematic review and meta-analysis.Data sourcesMEDLINE, PsycINFO, PsycARTICLES and CINAHL Complete databases, supplemented by Google Scholar.Eligibility criteriaProspective cohort studies in adults, published prior to January 2020, reporting associations between physical activity and depression.Study appraisal and synthesisMultilevel random-effects meta-analysis was performed adjusting for study and cohort or region. Mixed-model meta-regression of putative modifiers.ResultsSearches yielded 111 reports including over 3 million adults sampled from 11 nations in five continents. Odds of incident cases of depression or an increase in subclinical depressive symptoms were reduced after exposure to physical activity (OR, 95% CI) in crude (0.69, 0.63 to 0.75; I2=93.7) and adjusted (0.79, 0.75 to 0.82; I2=87.6) analyses. Results were materially the same for incident depression and subclinical symptoms. Odds were lower after moderate or vigorous physical activity that met public health guidelines than after light physical activity. These odds were also lower when exposure to physical activity increased over time during a study period compared with the odds when physical activity was captured as a single baseline measure of exposure.ConclusionCustomary and increasing levels of moderate-to-vigorous physical activity in observational studies are inversely associated with incident depression and the onset of subclinical depressive symptoms among adults regardless of global region, gender, age or follow-up period.
