Multiple Roles in an Applied Setting: Trainee Sport Psychologist, Coach, and Researcher

This is a follow-up article to an action research study that explored the effects of an imagery intervention on an elite rugby union player conducted over a 14-week period during the competitive season (Evans, Jones, & Mullen, 2004). A key feature of the study was that the same individual fulfilled multiple roles, specifically those of trainee sport psychologist, coach, and researcher. The aim of this article is to explore, from a trainee sport psychologist’s perspective, some of the issues that resulted from fulfilling multiple roles, both in the context of the study and in professional practice generally. The issues that emerged were consistent with the dual-role literature and involved role conflict surrounding areas of responsibility, scientific evidence versus social validity, confidentiality versus public statement, and the interpersonal welfare of both athlete and coach-sport psychologist (Ellickson & Brown, 1990). The findings highlighted (a) the importance of establishing ground rules (and planning), (b) the intensified emotional demands placed on the multirole practitioner, (c) the importance of involving a critical friend or outside agent, and (d) the potential for role conflict and the threat to objectivity.

ABSTRACTThis study aimed to identify the facilitators and barriers to as well as the feasibility of Implementing Personalized Citizen Assistance for social participation (IPCA) in older adults with disabilities living in the community. The IPCA is a three-hour, weekly follow-up achieved by trained and supervised non-professional assistants that aims to increase engagement in social and leisure activities of people living with disabilities. An action research study was conducted with 16 people from the community. The adaptability of the IPCA, the presence of scientific evidence, the acknowledgement of a need for such an intervention, as well as expertise and collaboration, were the main facilitators in implementation of the IPCA. Meanwhile, funding, associated with an unfavorable political and economic context, was a barrier. Overall, the majority of the participants perceived that the IPCA could be offered in the community by volunteers. This new knowledge will facilitate the implementation of IPCA or other similar interventions.

Objectives The objective of this follow-up study was to investigate associations between individual, occupational and work environment factors and burnout among both uniformed and non-uniformed personnel working in the Danish Prison and Probation Service. Methods The participants (N = 4808) with client contact received a questionnaire in 2010 and again in 2011. In 2010, 2843 participants responded to the questionnaire (59.1%), and in 2011, 1741 responded to the questionnaire, yielding a response rate of 61.2% of the baseline population, and 36.2% of the invited population. Burnout and work characteristics were measured with validated scales from the Copenhagen Psychosocial Questionnaire, and data was analysed by logistic regression. Results Risk factors with the highest impact on burnout were work environmental factors: quantitative demands, emotional demands, involvement in and meaning of work. Role conflict, role clarity, social support and demands for hiding emotions had borderline significance. Besides cohabitation, there was no association between individual factors and burnout or between occupational characteristics and burnout. Moreover, there was no association between exposure to threats and violence and burnout. Conclusions: Efforts for preventing burnout ought to be concentrated on reducing the quantitative job-demands, on easing and improving staff–inmate relationships, but also on involvement in and meaning of work. Most work in prison is invisible and the overall goals are in conflict with each other. Management must provide solutions to problems of role conflict and support groups for social support. There is a risk of burnout among both uniformed personnel and non-uniformed personnel working in both open and closed facilities.

Background: Although foam sclerotherapy of varicose tributaries is common in daily practice, scientific evidence for the optimal sclerosant-concentration and session-frequency is still low. This study aimed to increase the knowledge on foam sclerotherapy of varicose tributaries and to evaluate the efficacy and safety of foam sclerotherapy with 0.5 % polidocanol in tributaries with 3-6 mm in diameter. Patients and methods: Analysis of 110 legs in 76 patients. Injections were given every second or third day. A maximum of 1 injection / leg and a volume of 2ml / injection were administered per session. Controls were performed approximately 6 months and 12 months after the start of therapy. Results: 110 legs (CEAP C2-C4) were followed up for a period of 14.2 ± 4.2 months. Reflux was eliminated after 3.4 ± 2.7 injections per leg. Insufficient tributaries were detected in 23.2 % after 6.2 ± 0.9 months and in 48.2 % after 14.2 ± 4.2 months, respectively. Only 30.9 % (34 / 110) of the legs required additional therapy. In 6.4 % vein surgery was performed, in 24.5 % similar sclerotherapy was repeated. Significantly fewer sclerotherapy-sessions were required compared to the initial treatment (mean: 2.3 ± 1.4, p = 0.0054). During the whole study period thrombophlebitis (8.2 %), hyperpigmentation (14.5 %), induration in the treated region (9.1 %), pain in the treated leg (7.3 %) and migraine (0.9 %) occurred. One patient with a history of thrombosis developed thrombosis of a muscle vein (0.9 %). After one year there were just hyperpigmentation (8.2 %) and induration (1.8 %) left. No severe adverse effect occurred. Conclusions: Foam sclerotherapy with injections of 0.5 % polidocanol every 2nd or 3rd day, is a safe procedure for varicose tributaries. The evaluation of efficacy is difficult, as it can hardly be said whether the detected tributaries in the controls are recurrent veins or have recently developed in the follow-up period. The low number of retreated legs indicates a high efficacy and satisfaction of the patients.

Abstract Background Two large-scale phase 3 clinical trials (ZOE-50 [NCT01165177] and ZOE-70 [NCT01165229]) demonstrated that, in adults ≥ 50 years of age followed over a mean period of 3.1 and 3.7 years respectively, the adjuvanted recombinant zoster vaccine (RZV) was efficacious against herpes zoster (HZ), highly immunogenic and had a clinically acceptable safety profile. In this extension study (ZOSTER-049 [NCT02723773]), RZV-induced immunogenicity persistence and long-term vaccine efficacy (VE) against HZ were evaluated; we report interim results after at least 2 years of follow-up (starting and ending ≈5.1 and 7.1 years, respectively, after initial vaccination during the parent studies). Methods The study design is detailed in Figure 1. Primary objective: VE against HZ over the ZOSTER-049 study. Secondary objectives: VE against HZ from 1 month post-dose 2 in ZOE-50/-70 until the end of observation for year (Y)2 of ZOSTER-049, persistence of vaccine-induced humoral immunogenicity (HI) in terms of anti-gE antibody concentrations (by ELISA) and cell-mediated immune (CMI) response in terms of frequency of gE-specific CD4+ T-cells (by intracellular cytokine staining). ZOSTER-049 study procedures, timing, endpoints and cohorts Results Of the 7,413 participants enrolled in ZOSTER-049, 7,277 were included in the VE analysis (Figure 2) and 6,972 reached Y2 of this study. The overall VE against HZ during at least 2 years of follow-up in ZOSTER-049 was 84.0% (95% confidence interval [CI]: 75.9–89.8%). From 1 month post-dose 2 in the ZOE-50/-70 studies until the end of observation for Y2 of ZOSTER-049, the overall VE was 90.9% (95% CI: 88.2–93.2%). Anti-gE antibody concentrations persisted ≈6 times above pre-vaccination levels up to Y8 after vaccination (Figure 3A) and the frequency of gE-specific CD4[2+] T-cells remained above baseline from Y6 to Y8 after vaccination (i.e. until the end of observation for Y2 of ZOSTER-049) (Figure 3B). To the Editor.— In October 1991, the Centers for Disease Control decreased the blood lead level PbB) from 25 to 10 µg/dL and designated it as abnormal because of "overwhelming and compelling scientific evidence"1 that 10 µg/dL is associated with adverse neurobehavioral development. We have evidence to the contrary, obtained in a long-term follow-up of severely lead-poisoned children whom we treated before 1972 for PbBs between 80 and 470 µg/dL (mean, 150.3 ± 77.1 µg/dL); their mean age was 28 months.

ABSTRACT The success of joint replacement surgery has been responsible for raising patients' expectations regarding the procedure. Many of these procedures are currently designed not only to relive the pain caused by arthrosis, but also to enable patients to achieve functional recovery and to engage in some degree of physical activity and sports. However, as physical exercise causes an increase in forces exercised through the articular prosthesis, it can be an important risk factor for its early failure. Scientific literature on sports after arthroplasty is limited to small-scale retrospective studies with short-term follow-up, which are mostly insufficient to evaluate articular prosthesis durability. This article presents a review of the literature on sports in the context of hip, knee, shoulder and intervertebral disc arthroplasty, and puts forward general recommendations based on the current scientific evidence. ABSTRACT Background  Residents and practicing physicians displaying signs of stress is common. It is unclear whether stress during residency persists into professional practice or is associated with future burnout. Objective  We assessed the persistence of stress after residency and its correlation with burnout in professional practice. We hypothesized that stress would linger and be correlated with future burnout. Methods  A prospective cohort study was conducted over 10 years using survey instruments with existing validity evidence. Residents over 3 academic years (2003–2005) were surveyed to measure stress in residency. Ten years later, these residents were sought out for a second survey measuring current stress and burnout in professional practice. Results  From 2003 to 2005, 143 of 155 residents participated in the initial assessment (92% response rate). Of those, 21 were excluded in 2015 due to lack of contact information; follow-up surveys were distributed to 122 participants, and 81 responses were received (66% response rate and 57% of original participants). Emotional distress in residency correlated with emotional distress in professional practice (correlation coefficient = 0.45, P < .0001), emotional exhaustion (correlation coefficient = 0.30, P = .007), and depersonalization (correlation coefficient = 0.25, P = .029). Multivariate linear regression showed that emotional distress in residency was associated with future emotional distress (β estimate = 0.57, P = .005) and depersonalization (β estimate = 2.29, P = .028). Conclusions  We showed emotional distress as a resident persists into individuals' professional practice 10 years later and has an association with burnout in practice.

Plateau iris syndrome (PIS) is a frequent cause of angle closure. Argon laser peripheral iridoplasty (ALPI) has been proposed in PIS to widen the iridocorneal angle. The objective of the present study was to perform a systematic review of the available studies evaluating the efficacy of ALPI on intraocular pressure (IOP), iridocorneal angle opening and the number of medications in patients with chronic angle-closure associated with PIS. One prospective and seven retrospective studies with a minimum 1 month of follow-up were included. Although ALPI seemed to lower IOP, to decrease the number of topical antiglaucoma medications and widen the iridocorneal angle shortly after the procedure, there is no current evidence of long-term efficacy. To date, there is no robust scientific evidence to advocate ALPI as a treatment for chronic angle-closure caused by PIS.

A Hiperbilirrubinemia neonatal é uma condição clínica caraterizada pelo acúmulo de bilirrubina no organismo do neonato podendo acarretar graves sequelas quando não tratada. Um dos meios terapêuticos consiste fototerapia, um tratamento que demanda cuidados específicos para prevenir iatrogenias. A enfermagem desempenha relevante papel nesses cuidados, pois permanece 24 horas no seguimento dos casos prevenindo complicações e contribuindo para a eficácia da terapêutica. Portanto, nesse estudo o objetivo foi investigar, através de revisão integrativa da literatura, as evidências científicas sobre os cuidados de enfermagem ao recém nascido em fototerapia. Para tal, foram pesquisadas as bases de dados: BDENF, LILACS, MEDLINE e SCIELO. Na formulação da estratégia de busca foram utilizados os descritores em saúde combinados entre si através dos operadores booleanos AND e OR. Foram encontrados quarenta artigos que após uso de critérios de inclusão e exclusão pré-definidos, permaneceram quatro que compuseram a amostra. A revisão revelou que os cuidados de enfermagem influenciam positivamente na adesão e resultado do tratamento.Descritores: Assistência de Enfermagem, Hiperbilirrubinemia Neonatal, Fototerapia. Nursing care for newborn in phototherapy, what the evidence reveals: integrative reviewAbstract: Neonatal hyperbilirubinemia is a clinical condition characterized by the accumulation of bilirubin in the neonate's organism and can cause serious sequelae when not treated. One of the therapeutic means consists of phototherapy, a treatment that requires specific care to prevent iatrogenesis. Nursing plays an important role in this care, as it remains 24 hours in the follow-up of cases, preventing complications and contributing to the effectiveness of therapy. Therefore, in this study the objective was to investigate, through an integrative literature review, the scientific evidence on nursing care for newborns undergoing phototherapy. For this, the databases were searched: BDENF, LILACS, MEDLINE and SCIELO. In the formulation of the search strategy, health descriptors combined with each other through the Boolean operators AND and OR were used. Forty articles were found that, after using pre-defined inclusion and exclusion criteria, four remained in the sample.
