We assessed the associations between theAPOA5  −1131T>C polymorphism and lipid parameters and other risk factors of the metabolic syndrome in Korean subjects. A total of 2,901 participants from 20 oriental medical hospitals in Korea were enrolled between 2006 and 2011. According to the modified National Cholesterol Education Program Adult Treatment Panel III definitions, subjects were classified into the metabolic syndrome group and control group. TheAPOA5  −1131T>C genotype was significantly associated with serum high-density lipoprotein cholesterol levels (effect = − 1.700 mg/dL,P=6.550-E07) in the total study population after adjustment for differences in age and gender. The association of theAPOA5  −1131T>C genotype with serum log-transformed triglyceride was also significant in an additive genetic model (effect = 0.056 mg/dL,P=2.286E-19). After adjustment for age and gender, we determined that the odds ratio for the occurrence of the metabolic syndrome was 1.322 for C-allele carriers in the additive model (95% CI = [1.165 − 1.501],P=1.48E-05). In the current study, we demonstrated that theAPOA5  −1131T>C polymorphism is associated with the metabolic syndrome because of its remarkable effect on serum triglyceride levels in Korean subjects.

SummaryGrowing evidence suggests that metabolic syndrome (MetS) has both genetic and environmental bases. We estimated the heritability of the MetS and its components in the families from the Tehran Lipid and Glucose Study (TLGS). We investigated 904 nuclear families in TLGS with two biological parents and at least one offspring (1565 parents and 2448 children), aged 3–90 years, for whom MetS information was available and had at least two members of family with MetS. Variance component methods were used to estimate age and sex adjusted heritability of metabolic syndrome score (MSS) and MetS components using SOLAR software. The heritability of waist circumference (WC), HDL-cholesterol (HDL-C), triglycerides (TGs), fasting blood sugar (FBS), systolic blood pressure (SBP) and diastolic blood pressure (DBP) as continuous traits after adjusting for age and gender were 27, 46, 36, 29, 25, 26 and 15%, respectively, and MSS had a heritability of 15%. When MetS components were analysed as discrete traits, the estimates of age and gender adjusted heritability for MetS, abdominal obesity, low HDL-C, high TG, high FBS and high blood pressure (BP) were 22, 40, 34, 38 and 23%, respectively (P < 0·05). Three factors were extracted from the six continuous traits of the MetS including factor I (BP), factor II (lipids) and factor III (obesity and FBS). Heritability estimation for these three factors were 7, 13 (P < 0·05) and 2%, respectively. The highest heritability was for HDL-C and TG. The results strongly encourage efforts to identify the underlying susceptibility genes.

Background. Panchakarma (biopurification methods) is one of the modes of ayurveda to treat disorders of the body. Virechana karma (therapeutic purgation), one among the Panchakarma, is a purification process that is commonly used to treat metabolic disorders like obesity and diabetes mellitus. Hence this study was planned to provide evidence through animal experiments. Methods. Albino rats were subject to Virechana karma (therapeutic purgation) to evaluate the influence of therapy and its mechanism over fructose-induced metabolic syndrome. Results. Results show that Virechana is effective in the management of the metabolic syndrome with decrease in the fecal fat content, fasting blood glucose, serum triglyceride, and reduced fatty changes in liver, heart, and kidney in comparison with the positive control group. Conclusion. Experimental evaluation showed decrease in fatty acid in the storage like liver, kidney, heart, and muscle adipose tissue can indirectly increase the insulin sensitivity in insulin receptor present at skeletal muscles.

<p><strong>Background and Aim.</strong> The Metabolic Syndrome (MetS) has been shown to be independently associated with increased risk for incident heart failure (HF) and coronary artery disease. The aim of this study was to investigate the additional effect of MetS on LA dysfunction in a group of patients with HF and preserved ejection fraction (HFpEF) and its relationship with symptoms.</p><p><strong>Methods. </strong>This study included 194 consecutive patients (age 62 ± 9 years) with stable HFpEF. LV dimensions, ejection fraction (EF), mitral annulus peak systolic excursion (MAPSE), myocardial velocities (s’, e’ and a’), LA dimensions and volumes were measured. Total LA emptying fraction (LA EF) was measured by Simpson rule volumes. Based on the NCEP-ATP III criteria, patients were divided into two groups; MetS (n=95) and non-MetS (n=108) and were compared with 34 age and gender matched controls.</p><p><strong>Results. </strong>Age and gender were not different between patients and control neither between MetS and non-Met. LV dimensions, EF and longitudinal function indices were also not different. The<strong> </strong>MetS patients had higher LV mass index (p=0.038), lower septal and lateral e’ (p=0.003 and p=0.001, respectively) velocities, larger LA minimal volume (p=0.007) and lower LA EF (p&lt;0.001) compared with the non-MetS patients. Age, LA EF and MetS independently predicted the NYHA class.</p><strong>Conclusions. </strong>Despite no difference in LV systolic function, patients with HFpEF and MetS have worse LA emptying fraction, compared with HFpEF and non-MetS patients. In addition, LA reservoir function impairment and MetS independently predict patients limiting symptoms, thus add to a better understanding of HFpEF.

Because first graders are critical for excess weight gain, we assessed components of the metabolic syndrome (MetS) using the pediatric definition of the International Diabetes Federation (IDF). We compared four MetS components as defined by the IDF with age- and gender-specific components in 2228 first graders at the age of 6. The growth curves were derived from 22113 children and adolescents who participated in the PEP Family Heart Study. The aim was to determine in first graders precise values of waist circumference (WC), blood pressure (BP), triglycerides (TG), and HDL-Cholesterol (HDL-C) based on growth curves that were developed for a large German population of youths and to assess the prevalence in terms of both definitions at this critical age. The prevalence of high blood pressure for age was 13% compared with only 2% according to IDF. Because of this considerable divergence, we propose to define MetS components based on national growth curves.

The goal of this study is to investigate the relationship between the serum level of uric acid (UA) and metabolic syndrome (MetS) according to age and gender. The study included 395 patients (200 women and 195 men), with a mean age of 53.6�18 years. Hyperucemia (HU) was defined by a serum level UA �7mg/dL in men and � 6mg/dL in women (according to the EULAR guide), and MetS was defined according to the criteria of the International Diabetes Federation (IDF). The prevalence of hyperuricemia in the study lot was 17.21%, and MetS was 35.44%. In women, the prevalence of HU and MetS increases with age: 13.88% of women over 65 years show HU, and 44.44% of them comprise the elements of MetS. In men, HU and MetS do not vary significantly depending on age. MetS has a higher prevalence in HU patients than in HU-free patients. HU prevalence in MetS patients was 32.65% in women versus 46.15% in men. In conclusions, the prevalence of HU and MetS differs significantly according to gender and age. The prevalence of MetS was higher in male subjects of medium age and HU did not vary significantly depending on age. In women, the prevalence of HU and MetS was higher in those over 65 years. Hyperuricemia in middle-aged female patients can predict the MetS development.

Background: Fetuin-A also known as α-2-Heremans-Schmid glycoprotein, is a multifunctional plasma glycoprotein. In developing countries like India, the metabolic syndrome (MetS) is on an exponential rise. The factors that characterize MetS are also associated with the atherosclerotic process, in which an important role is played by serum fetuin-A levels. The aim of present study is an assessment of serum fetuin-A level and its association with other components of MetS in age matched healthy controls and MetS subjects.Methods: Total eighty (N=80) subjects (40 MetS cases and 40 age and gender matched controls) were included based on inclusion and exclusion criteria. The fasting venous samples were collected to measure the fasting glucose, lipid profile, glycated hemoglobin, insulin and fetuin-A levels. The serum fetuin levels were estimated by ELISA kit.Results: The serum fetuin-A levels were significantly higher in MetS cases as compared to the control group (p value<0.001). Other components like insulin, HbA1c and HOMA-IR levels were raised in MetS compared to controls. In correlation analysis the serum fetuin-A levels were positively correlated with fasting insulin levels (r=0.36, p=0.02), fasting glucose (r=0.39, p=0.01) and triglycerides (r=0.34, p=0.03). Also, in receiver operating characteristic curve analysis the AUC for fetuin-A is 0.76 (95% CI: 0.65-0.86) was observed.Conclusions: We found that serum fetuin-A levels were strongly and independently associated with MetS. Our finding suggests that fetuin-A could be a useful marker in clinical practice in the future for the early diagnosis of MetS.

Background: The metabolic syndrome is a constellation of cardiometabolic risk factors that tend to cluster together in affected individuals more often than predicted by chance. The presence of the metabolic syndrome substantially increases the risk of developing type 2 diabetes and cardiovascular disease, and is associated with a range of adverse clinical outcomes, many of which are closely associated with aging. Current estimates suggest that approximately 20 - 25% of the world’s population is affected by the metabolic syndrome. The prevalence of the metabolic syndrome rises with age and more than 45% of people aged over 60 years have the metabolic syndrome. Recent studies show that low vitamin D status is very common in the world and this is a risk factor of metabolic syndrome. Objective: (1) Plasma 25-hydroxyvitamin D concentration in subjects with metabolic syndrome. (2) Cut off value of plasma 25-hydroxyvitamin D concentration for predicting metabolic syndrome. Material and method: A cross-sectional study with control group on 318 adult subjects for health examinations at International Medical Center at Hue Central Hospital, including 139 subjects with metabolic syndrome and control group of 179 healthy subjects. Metabolic syndrome was defined according to the IDF, NHLBI, AHA, WHF, IAS, IASO (2009). Plasma hydroxyvitamin D concentration was measured using chemiluminescent microparticle immunoassay. Reciever operating characteristic (ROC) curve were generated to assess sensitivity and specificity for different cut off value of 25-hydroxyvitamin D concentration for predicting metabolic syndrome. Results: Plasma 25-hydroxyvitamin D concentration in subjects with metabolic syndrome was 26.4 ng/ml, incidence of plasma 25-hydroxyvitamin D deficiency (59.7%) was significantly higher than in control group (23.5%) (p < 0.001). The optimal cut off point for 25-OH-D concentration for predictor of metabolic syndrome as 26.4 ng/ml (AUC=0.657, sensitivity=53.4%, specificity=71.6%). Conclusion: In 139 subjects with metabolic syndrome, the plasma 25-hydroxyvitamin D concentration was 26.4 ng/ml and the incidence of 25-hydroxyvitamin D deficiency in the metabolic syndrome group was 59.7%. The optimal cut off point for plasma 25-hydroxyvitamin D concentration for predictor of metabolic syndrome as 26.4 ng/ml. Key words: Metabolic syndrome, 25-hydroxyvitamin D
