Purpose.Aim of the study is to evaluate the efficacy of the endoscopic (pneumatic dilation) versus surgical (Heller myotomy) treatment in patients affected by esophageal achalasia using barium X-ray examination of the digestive tract performed before and after the treatment.Materials and Methods.19 patients (10 males and 9 females) were enrolled in this study; each patient underwent a barium X-ray examination to evaluate the esophageal diameter and the height of the barium column before and after endoscopic or surgical treatment.Results.The mean variation of oesophageal diameter before and after treatment is −2.1 mm for surgery and 1.74 mm for pneumatic dilation (OR 0.167, CI 95% 0.02–1.419, andP: 0.10). The variations of all variables, with the exception of the oesophageal diameter variation, are strongly related to the treatment performed.Conclusions.The barium X-ray study of the digestive tract, performed before and after different treatment approaches, demonstrates that the surgical treatment has to be considered as the treatment of choice of achalasia, reserving endoscopic treatment to patients with high operative risk and refusing surgery.

Electrospun MWCNTs nanofibers (CNF1, CNF2 and CNF3) with different concentrations of MWCNTs (0.3, 1.5, 2 wt%), respectively, were deposited on Aluminum foil substrates.  Also,Zinc AcetatedihydrateZn(CH3COO)2.2H2O (ZNF) and MWCNTs/zinc acetate (CZNF)nanofiberswere deposited on Aluminum foil substratesand annealed in the presence of oxygen at 400 oC. The resultant fibers were characterized using X-ray differaction (XRD), scanning electron microscope with energy dispersive X-Ray spectrophotometry (SEM,EDX), Fourier transform infrared (FTIR). SEM,EDX and FTIR exhibited a total decomposition of the organic precursor after calcination and formation of zinc oxide (ZONF and CZONF). The mean fiber diameter was found to be increased with increasing MWCNTs concentration and ranged 490-767 nm. XRD patterns indicated that ZnO was corundum with the hexagonal wurtzite structure. The crystallite size of ZONF and CZONF were determined by shurrer equation to be26 and  29.7  nm, respectively. The optical analysis indicated that the percentage transmittance increased after calcination.The band gap for the electrospun fibers before and after calcination was calculated. CZONF nanofibers have elec­trical properties similar to those of semiconductors. The testedcompounds CNF2, CNF3, CZNF and CZONF exhibited different activities against the bacteriaand yeast pathogen Candidaalbicans. CZNF compound is the most active against the bacteria and yeast pathogen. So, these compounds can be used as food packaging.

Hallux valgus (HV) violates the musculoskeletal function of the lower limb, and also affect the x-ray anatomical parameters of the foot. There is the study of the most important correlations between age, morphological and functional changes of the forefoot play a big role in the choice of treatment tactics for this pathology. The aim was to analyze the age, functional and radiological results of surgical treatment of Hallux valgus deformity of the first toe using the methods saving the metatarsophalangeal joint. 126 patients had Hallux valgus deformity of the first toe of I, II, III degree. They were examined before surgery, then 3, 6 and 12 months after operation. 126 patients were operated according to the method of Scarf, Austin, Bosch-Magnan osteotomy. X-ray results were assessed by the Hallux valgus angle (HVA), the intermetatarsal angle (IMA), the distal metatarsal articular angle (DMMA) before and after the operations. AOFAS rating scale (Kitaoka) and biomechanical (stabilometric) study were used for the functional assessment of the surgical treatment results. The IMA correction angle after operation increases with the patient's age. HVA becomes the most deformed angle in the pathology of Hallux valgus with increasing age before osteotomy. The best functional result according to the AOFAS scale was obtained with the greatest correction of the IMA angle after osteotomy. The functional index of the foot decreased with increasing age after osteotomies.

Achalasia is an esophageal motility disorder that leads to dysphagia, chest pain, and weight loss. Its diagnosis is clinically suspected and is confirmed with esophageal manometry. Although pneumatic dilation has a role in the treatment of patients with achalasia, laparoscopic Heller myotomy is considered by many experts as the best treatment modality for most patients with newly diagnosed achalasia. This review will focus on the surgical treatment of achalasia, with special emphasis on laparoscopic Heller myotomy. We will also present a brief discussion of the evaluation of patients with persistent or recurrent symptoms after surgical treatment for achalasia and emerging technologies such as LESS, robot-assisted myotomy, and POEM.

Abstract. In a total of 195 children and adolescents of both sex (mean age 12.9, range 5–17 years) with endemic non-toxic goitre the thyroidal iodine concentration (IC) was determined using X-ray fluorescent scanning on admission and during iodine (100 μg daily) and l-thyroxine (3 μg/kg body weight daily) treatment respectively. Additionally the thyroid volume was measured sonographically in a longitudinal study including 46 patients before and after 4–8 months of iodine supplementation (100 μg daily). The IC was 305 ± 144 μg/g. It compared well with that of adult goitre patients (288 ± 109 μg/g) and was significantly inferior to the value of normal controls (389 ± 170 μg/g). Under l-thyroxine therapy the IC further decreased (243 ± 144 μg/g), whereas patients receiving iodide showed an increase of the IC (570 ± 197 μg/g). The mean TSH level fell from 2.3 ± 0.9 μU/ml to 1.4 ± 0.6 μU/ml. The average T4/TBG (thyroxine binding globulin) ratio showed a slight increase which, however, was not significant. The mean goitre volume decreased by 40%. It was evidenced that iodide is useful not only in the prophylaxis of non-toxic goitre but also as a more physiologic treatment than thyroid hormones, at least for young subjects with simple diffuse goitres.

In this study three-dimensional X-ray computed tomography (X-ray CT) is used to investigate the testing results differences of cement paste and mortar before and after drying. It can be found that the mean gray values of paste and mortar before drying are bigger than after drying, and the impact of drying on cement paste is more serious than mortar. In addition, the porosity of non-drying cement paste and mortar is 1.10% and 0.43%, while that of drying cement paste and mortar is 1.55% and 0.70%, respectively. So, the porosity of paste and mortar markedly increases after drying process. The numbers of pores of paste and mortar sharply increase after drying process in particular between 0.01mm3 and 0.1mm3. The impact of drying on smaller pores is more serious than bigger pores.

Background: Benign obstructive pathology of the ureterovesical junction includes congenital and acquired illnesses. The objective of this study was to describe the endoscopic oblique meatotomy technique using scissors and cold cutting to treat benign obstructive pathology of the ureterovesical junction.Methods: From 2007 to 2012, we treated 18 patients with obstructive pathology of the ureterovesical junction (5 megaureters [3 with lithiasis], 4 iatrogenic stenoses, and 9 ureteroceles with lithiasis). In all cases, oblique meatotomy was performed using endoscopic scissors through an 8.5 Ch ureteroscope. The mean follow-up for all patients was 3 years. Pain, grade of hydronephrosis, and occurrence of vesicoureteral reflux were evaluated before and after treatment.Results: The mean endoscopic treatment time was 13.4 minutes. The procedure was performed on an outpatient basis with 6 hours of hospital admission, and a double J stent was inserted for 6 weeks. We achieved treatment success in 94.5% of patients after 3 years of follow-up. Only 1 patient presented with vesicoureteral reflux at 12 months after treatment; however, this condition did not require further treatment. Overall, 100% of patients remained free from lithiasis. There are 2 main limitations: the small number of patients and the lack of another group to compare the results of this technique; however, the aim of this work was to communicate a new technique to treat ureterovesical junction stricture.Interpretation: Oblique ureteral meatotomy is a safe and effective treatment for benign obstructive pathology of the ureterovesical junction and has a low index of complications.

Objective: To clinically and radiographically observe the outcomes of the surgical treatment of moderate and severe hallux valgus using percutaneous distal chevron osteotomy. Methods: A total of 35 feet (33 patients) with moderate and severe hallux valgus were evaluated using the Couhling classification from June 2016 to January 2018. The mean patient age was 53 years, and the mean postoperative follow-up time was 13 months. All patients who underwent chevron osteotomy were evaluated before and after surgery using the American Orthopedic Foot and Ankle Society (AOFAS) scale. We measured the hallux valgus angle (HVA), the intermetatarsal angle (IMA) and the distal metatarsal articular angle (DMAA) using radiographic studies. Results: The AOFAS score increased by a mean of 54 points. The HVA decreased by a mean of 19°, the IMA by 7° and the DMAA by 11°. The median decrease in the length of the first metatarsal bone was 0.35 cm. Conclusion: Chevron osteotomy using the percutaneous surgical technique made it possible to correct moderate and severe hallux valgus deformities, with excellent angular correction and a significant increase in the AOFAS score.

Background. Nonunion of the lateral humeral condyle  are of the complex pathology of the elbow joint, occurring relatively often and resulting in disability of children.The treatment of nonunion of the lateral humeral condyle of humerus with cubitus valgus remains controversial. Purpose of this report was improvement of the results of surgical treatment of the nonunion of the lateral humeral condyle of humerus with cubitus valgus of the lateral humeral condyle of humerus with cubitus valgus with use of differential approach to the surgical strategy.Material and methods. We were observing 28 children (17 boys and 11 girls) with nonunion and long-standing nonunion of lateral condyle of humerus, with various degrees of severity. There were used MRI and X-ray investigations for differential study of the patients divided into 3 groups in relation to stability and non-stability of the nonunion of the lateral humeral condyle of humerus with cubitus valgus of the lateral humeral condyle of humerus with cubitus valgus.Results. All 28 patients lateral humeral condyle nonunions with cubitus valgus achieved union within sixty five days after operative procedure using Ilisarov’s technique. The mean postoperative humerus-ulna angle was 6,0 degrees of cubitus valgus. All of reverse T-osteothomies healed uneventfully, and there was no loss of correction postoperatively.  The mean duration of follow-up was 7 years. The overall results were excellent in 15(53,5%) patients,  good in 11(39,3%) patients, and fair in 2(7,2%) patients. The objective of the study is to evaluate the condition of the alveolar part of the upper jaw and the part of the mandible according to X-ray studies in postmenopausal women before and after surgical treatment of the atrophy of jaw bone tissue using osteoplastic material and an ossein-hydroxyapatite compound. Materials and methods: There were observed 24 women in the postmenopausal period, aged from 51 to 58 years, with atrophy of the alveolar process of the upper jaw and the part of the lower jaw who were surgically treated. Results of the study. On the basis of the obtained results of the X-ray examination, we can assume that the developed by us technique of surgical treatment of atrophy of the alveolar process of the upper jaw and the part of the lower jaw by the use of bone material of animal origin in combination with the ossein-hydroxyapatite compound allows us to intensify bone tissue regeneration processes, which further will contribute to the increase of the volume of bone tissue. Conclusions. The use of modern X-ray methods, in particular orthopantomography and cone-beam computerized tomography in the diagnosis and surgical treatment of atrophy of the alveolar process of the upper jaw and the part of the lower jaw, are highly informative, and also provide the possibility of work with a roentgenographic image in a digital format that allows a more detailed assessment of the area of surgical intervention before and after the treatment.

Inferential studies suggest that circulating insulin concentrations positively regulate leptin secretion by adipocytes. In humans, however, insulin requires prolonged periods of time, and relatively artificial set-ups before a relationship with leptin can be observed. In the present work, serum leptin concentrations were measured in five patients with insulinoma before and one month after surgery and in five control subjects matched by sex and body mass index (BMI). The control subjects presented a mean serum leptin concentration of 6.7+/-1.5 microg/l and a BMI of 24.9+/-1.1. The mean serum leptin concentration in patients with insulinoma was 11.8+/-3.1 microg/l (P < 0.05 vs controls), with a BMI of 26.3+/-1.9. After surgery, there was a non-significant reduction in BMI (25.8+/-1.7), and a clear reduction in serum leptin concentration (5.6+/-2.4 microg/l, P < 0.05 vs pre surgical values and no difference vs control subjects). The fasting area under the curve (AUC) of insulin concentration (in mU/l per 120 min) before surgery was 14421+/-4981 and after surgery was 1306-/+171 (P < 0.05). Before surgery, serum leptin concentrations significantly correlated with BMI (r = 0.71) and AUC of insulin (r = 0.82), a correlation that was lost after surgery. In conclusion, serum leptin concentrations are significantly elevated in patients with chronically high insulin levels due to insulinoma. After surgical treatment and normalization of insulin values, leptin levels return to normal.
