Microscopy of stool samples is a labour-intensive and inaccurate technique for detection of intestinal parasites causing diarrhoea and replacement by PCR is attractive. Almost all cases of diarrhoea induced by parasites over a nine-year period in our laboratory were due toGiardia lamblia,Cryptosporidiumspecies, orEntamoeba histolyticadetected by microscopy. We evaluated and selected in-house singleplex real-time PCR (RT-PCR) assays for these pathogens in 99 stool samples from patients suspected of having intestinal parasitosis tested by microscopy. The strategy included a genus-specific PCR assay forC. parvumandC. hominis, with subsequent identification by a PCR that distinguishes between the two species.G. lambliawas detected in five andC. parvumin one out of 68 microscopy-negative samples. The performance of the in-house RT-PCR assays was compared to three commercially available multiplex test (MT-PCR) kit systems in 81 stool samples, collected in 28 microscopy-positive and 27 microscopy-negative samples from individuals suspected of intestinal parasitosis and in 26 samples from individuals without suspicion of parasitic infection. The in-house assays detected parasites in more samples from patients suspected of having parasitosis than did any of the kits. We conclude that commercial kits are targeting relevant parasites, but their performance may vary.

Background: Intestinal parasitosis still constitutes one of the major causes of public healthproblems in the world, particularly in developing countries. Nepal is a small, impoverished country prevalent to infectious diseases, including intestinal parasitosis. Poverty, lack of awareness, failure to practice proper hand washing after defecation, practice of open defecation, unsafe drinking water and use of improper toilets are some of the reasons causing of parasitic infections.Aims and Objective: To determine the types of intestinal parasites in stool samples of patients attending to hospital. Materials and Methods: Stools specimens collected in standard stool vial were submitted in microbiology (2006-2010) unit at BP Koirala Institute of Health Sciences hospital (BPKIHS) were examined for intestinal parasites by direct wet mount using normal saline (0.9%) and lugol’s iodine (0.5%).Results: A total of 11,791 stool samples (2928 in 2006, 2238 in 2007, 2151 in 2008, 2344 in 2009 and 2130 in 2010) were submitted to the Parasitology section of Department of Microbiology BPKIHS were included in the analysis. Of these, 675 (5.72%) were positive intestinal protozoa and 289 (2.45%) for intestinal helminths. Giardia intestinalis accounted for the most prevalent parasitic infection (3.34%) followed by Entamoeba histolytica/E. dispar (1.96%) and Hookworm (0.97%). A parasitic infection was observed to be highest among 20-50 years of age group and lowest in the less than 5 years group of subjects.Conclusion: Intestinal parasitosis is a common problem. Amoebiasis, giardiasis and the common intestinal helminthes Ascaris lumbricoides, hookworm, and Trichuris trichiura were prevalent in our settings.Asian Journal of Medical Sciences Vol.8(3) 2017 55-59

 Intestinal parasitic infestation reflects a health threat with patients living with HIV & Tu-berculosis (TB). Parasitic infections caused by protozoa and helminths are the most com-mon infections worldwide. The present study was done to find out the frequency of intes-tinal parasitosis among HIV and Tuberculosis (TB) patients of Dharan total of 53 samples (>15 years) from HIV seropositive patients and 28 samples (>20years) from TB patients were collected in a clean, dry and capped fitted container and subjected to macroscopic and microscopic examination for ova, cyst, adult parasites and or segments of parasites. Stool samples were fixed in a 10% formalin-ether solution. Sedimentation technique, with modified acid-fast (Ziehl-Neelsen) staining method, was performed for opportunistic in-testinal parasites in both patients. Multi-parasitic infection was noted in the study. The overall prevalence of intestinal parasites was found to be 54 (66.67%). This result had shown that the participation of male patients was comparatively higher than female pa-tients. Out of 81 stool samples, 15 (27.78%) G. lamblia, 12(22.22%) E. histolytica, 12 (22.22%) Cryptosporidium parvum, 6 (11.11%), Isospora belli 4 (7.40%) Microspridium, 1(1.85%) Blastocystis hominis, 2(3.70%) and Taenia spp, 2(3.70%). To prevent this infec-tion appropriate health education should be given to the patients concerning disease trans-mission, antiparasitic therapy, personal hygiene, and safe drinking water.

Nowadays, many commercial kits allowing the detection of digestive parasites by DNA amplification methods have been developed, including simplex PCR assays (SimpPCRa) allowing the identification of a single parasite, and multiplex PCR assays (MultPCRa) allowing the identification of several parasites at once. Thus, aimed at improving the diagnosis of intestinal protozoal infections, it is essential to evaluate the performances of these new tools. A total of 174 DNA samples collected between 2007 and 2017 were retrospectively included in this study. Performances of four commercial SimpPCRa (i.e., CerTest-VIASURETM) and three MultPCRa (i.e., CerTest-VIASURETM, FAST-TRACK-Diagnostics-FTD-Stool-ParasiteTM and DIAGENODE-Gastroenteritis/Parasite-panel-ITM) were evaluated for the detection of Cryptosporidium spp., Entamoeba spp., and Giardia intestinalis in stool samples compared to our routinely used in-house SimpPCRa. Globally, the SimpPCRa showed better sensitivity/specificity for the detection of G. intestinalis, E. histolytica, E. dispar, and Cryptosporidium spp. (i.e., 96.9/93.6%; 100/100%; 95.5/100%; and 100/99.3%, respectively), compared to the three commercial MultPCRa tested. All in all, we showed that MultPCRa offer an interesting alternative for the detection of protozoans in stool samples depending on the clinical context.

Intestinal parasitosis is highly prevalent among the general population in Nepal. This study aimed to evaluate the distribution of intestinal parasites among patients attending Nobel Medical College Teaching Hospital, Biratnagar, Nepal. A total of 5,524 stool samples examined by direct smear and Formal-Ether concentration technique were reported. The overall prevalence of intestinal parasitic infection was found to be 15.17% (M=15.65% vs F=14.62%). The overall prevalence of intestinal parasitosis was found to be highest among patients aged 5-14 years (20.66%), followed by elderly people aged > 45 years (16.20%) and least among children aged < 5 years (9.09%). Among various parasites detected, the most common was Entamoeba histolytica (44.86%) followed by Giardia lamblia (33.65%), hookworm (10.50%), Ascaris lumbricoides (5.72%), Strongyloides stercoralis (4.77%), Hymenolepis nana (3.34%), Trichuris trichiura (0.95%) and Enterobius vermicularis (0.23%) respectively. The molecular epidemiology of giardiasis in Africa remains unclear. A study was carried out across four hospitals in Algeria. A total of 119 fecal samples from 55 children, 37 adults, and 27 individuals of undetermined age, all scored positive for intestinal parasites by microscopy, and were screened by real-time PCR for Giardia. Molecular characterization of Giardia was performed by assemblage-specific PCR and PCR targeting the triose phosphate isomerase gene (tpi). Of the 119 samples, 80 (67%) were Giardia-positive by real-time PCR. For 48 moderately-highly real-time PCR-positive samples, tpi genotyping assigned 22 samples to Assemblage A and 26 to Assemblage B. Contrary to Assemblage A, Assemblage B exhibited substantial genetic diversity and allelic heterozygosity. Assemblage-specific PCR proved to be specific for discriminating Assemblage A or B but not as sensitive as tpi genotyping. We confirmed that real-time PCR is more sensitive than microscopy for detecting Giardia in stool samples and that robust amplification and sequencing of the tpi gene is feasible when moderate-to-strongly real-time PCR-positive samples are used. This study is one of the few performed in Africa providing genotyping data on Giardia infections in humans. Both assemblages A and B were commonly seen and not associated with specific sociodemographic data.

Background. Neglected tropical diseases continue to be one of the leading causes of morbidity and mortality in the developing world. Psychiatric patients are among groups at risk for parasitic infection although control and monitoring programs largely overlook this population. This study aimed at determining prevalence and factors associated with intestinal parasitic infection among patients admitted to a psychiatric facility.Method. The study followed cross-sectional design; all the residing patients that met the inclusion criteria were included in the survey. Stool samples were collected and examined by direct wet preparation and formol-ether concentration. Data were analyzed with STATA version 12.1; Chi-square test was computed to determine the level of significance atpvalue < 0.05.Results.Of all 233 patients who returned the stool samples, 29 (12.45%) screened were positive for an intestinal parasite. There was no significant association between parasite carriage and age, sex, or duration of hospital stay.Conclusion. The study shows that intestinal parasitic infection is common among patients in a psychiatric facility and highlights that parasitic infections that enter through skin penetration may be a more common mode of transmission than the oral route. Furthermore, the study underscores the need for surveillance and intervention programs to control and manage these infections.

Introduction: Nowadays in our state, intestinal parasitic diseases are still endemic, with a greater frequency in rural and mountainous urban areas. After the years have elapsed since the last national survey, it was necessary to carry out a new survey to know the prevalence rates of intestinal parasitosis and to compare the results obtained between both studies. Such knowledge would be of great value in developing health strategies and designing intestinal parasitic infection control programs. Objective: to determine the current prevalence rates of intestinal parasites, to compare the results with those of the survey obtained. Methods: a random cross-sectional descriptive study was carried out during the months of December 2018 to March 2019 in a random sample of both sexes from the population of urban communities and the province of Zacatecas, Zac. Each one had a stool sample collected that was analyzed by the direct examination method, the Willis concentration technique, and the Kato-Katz examination; A questionnaire was also applied to them. Results: when comparing the results of the national and state surveys of intestinal parasitic infections carried out in 2009 and 2018, it was found that, in general, infections by parasites decreased, both helminths and pathogenic protozoa, although those infected with commensals increased their frequency in that of 2019. There was also a decrease in the frequency of infections by all species of soil-transmitted helminths, Trichuris trichiura, Ascaris lumbricoides, ancylostomideos, and Strongyloides stercoralis, as well as by the pathogenic protozoa Giardia lamblia and Entamoeba histolitica. Conclusions: when comparing the results of national and state surveys of intestinal parasite infections carried out in 2018 and 2019, it was found that in general the frequencies of infections with intestinal parasites decreased. The finding in both studies of a higher frequency of infection with parasites or commensals in the group aged 5 to 14 years (school age), supports the recommendation to place emphasis on control programs for intestinal parasites in this age group.

ABSTRACT Although the practice of composting animal wastes for use as biofertilizers has increased in recent years, little is known about the microorganisms responsible for the nitrogen transformations which occur in compost and during the composting process. Ammonia is the principle available nitrogenous compound in composting material, and the conversion of this compound to nitrite in the environment by chemolithotrophic ammonia-oxidizing bacteria is an essential step in nitrogen cycling. Therefore, the distribution of ammonia-oxidizing members of the β subdivision of the class Proteobacteriain a variety of composting materials was assessed by amplifying 16S ribosomal DNA (rDNA) and 16S rRNA by PCR and reverse transcriptase PCR (RT-PCR), respectively. The PCR and RT-PCR products were separated by denaturing gradient gel electrophoresis (DGGE) and were identified by hybridization with a hierarchical set of oligonucleotide probes designed to detect ammonia oxidizer-like sequence clusters in the genera Nitrosospira and Nitrosomonas. Ammonia oxidizer-like 16S rDNA was detected in almost all of the materials tested, including industrial and experimental composts, manure, and commercial biofertilizers. A comparison of the DGGE and hybridization results after specific PCR and RT-PCR suggested that not all of the different ammonia oxidizer groups detected in compost are equally active. amoA, the gene encoding the active-site-containing subunit of ammonia monooxygenase, was also targeted by PCR, and template concentrations were estimated by competitive PCR. Detection of ammonia-oxidizing bacteria in the composts tested suggested that such materials may not be biologically inert with respect to nitrification and that the fate of nitrogen during composting and compost storage may be affected by the presence of these organisms.

Background This study was carried out to estimate the prevalence of intestinal parasites among school-going children of the Baglung municipality from December 2010 to January 2011. Objective To find out prevalence of parasitosis among school aged children and to make necessary recommendations for preventive measures. Method A total of 260 stool samples were collected. A structured questionnaire was used to collect data on predisposing factors. Nails were observed without prior information to the subjects so as to find their hygienic practice. The stool samples were examined by direct wet mount and formal ether concentration technique. Results The total prevalence of the intestinal parasitosis was found to be 21.05%. The prevalence for individual parasites was as follows: Entamoeba histolytica (9.23%), Giardia lamblia (5.76%), Trichuris trichuria (5%), Ancylostoma duodenale (2.65%) and Ascaris lumbricoides (2.3%). Nail hygiene and level of education were significantly associated with intestinal parasitosis. The gender and age of the children, sanitary habits including toilet use, hand washing practice, and the use of the antihelminthic drug (albendazole) were not significantly associated with intestinal parasitosis. Higher prevalence was seen in boys, children belonging to age group 10-14 years, lower secondary students, among those who reported gastrointestinal problems within last six months, children from agriculture-based families and children with untrimmed nail. Conclusion Major contributors for the prevalence of parasites were found to be poor personal hygiene and educational level of the children. Health education and mass treatment are recommended as a preventive measures. Data concerning the burden of parasites among populations living in overcrowded circumstances have paramount importance for the success of interventions aimed at eliminating such disease. A cross-sectional study was therefore conducted from February to May 2019 among 217 children at child centres in Bahir Dar city. Data were collected using a structured questionnaire and stool samples were processed for parasitological investigation. A total of 53 (24.4%) children were infected with at least a single species of intestinal parasite. Giardia lamblia (7.8%) was the most frequent, followed by Hymenolepis nana (6%). Children with untrimmed fingernails (adjusted odds ratio [AOR] = 2.422, P = 0.027) were significantly more prone to infection. Similarly, children with no habit of hand washing after soil contact (AOR = 2.752, P = 0.014) or after defecation (AOR = 3.087, P = 0.026) were significantly associated with parasitic infection. The prevalence of intestinal parasites at child centres in Bahir Dar is of public health importance; therefore, control programmes should target these children.
