This study aimed to examine effect modification of maternal risk factor exposures and congenital heart disease (CHD) by maternal folic acid supplementation (FAS)/non-FAS. We included 8379 CHD cases and 6918 CHD-free controls from 40 clinical centers in Guangdong Province, Southern China, 2004–2016. Controls were randomly chosen from malformation-free fetuses and infants and frequency matched to the echocardiogram-confirmed cases by enrollment hospital and year of birth. We used multiple regression models to evaluate interactions between FAS/non-FAS and risk factors on CHDs and major CHD categories, adjusted for confounding variables. We detected statistically significant additive and multiplicative interactions between maternal FAS/non-FAS and first-trimester fever, viral infection, and threatened abortion on CHDs. An additive interaction on CHDs was also identified between non-FAS and living in a newly renovated home. We observed a statistically significant dose-response relationship between non-FAS and a greater number of maternal risk factors on CHDs. Non-FAS and maternal risk factors interacted additively on multiple critical CHDs, conotruncal defects, and right ventricular outflow tract obstruction. Maternal risk factor exposures may have differential associations with CHD risk in offspring, according to FAS. These findings may inform the design of targeted interventions to prevent CHDs in highly susceptible population groups.

В данном обзоре на основании литературных данных изучен ряд биомаркеров и определена их связь с развитием преэклампсии у женщин с прегестационным сахарным диабетом в I триместре беременности. В связи с тем, что в основе этих состояний лежат схожие механизмы, ранняя диагностика преэклампсии у данной категории пациенток значительно затруднена. Выполнена оценка материнских факторов риска, биофизических и биохимических маркеров, показана их роль в предикции преэклампсии. Установлено, что при наличии сахарного диабета исследование некоторых гормонов, маркеров воспалительной реакции, метаболизма липидов может иметь потенциальную ценность для прогнозирования развития преэклампсии. Таким образом, для активного внедрения биомаркеров в практическую деятельность требуются дальнейшее, более детальное изучение этого направления и оптимизация дизайна исследований. In this review, based on the literature data, a number of biomarkers have been studied and their relationship with the development of preeclampsia in women with pregestational diabetes mellitus in the first trimester of pregnancy has been determined. Due to the fact that these conditions are based on similar mechanisms, early diagnosis of preeclampsia in this category of patients is significantly difficult. The assessment of maternal risk factors, biophysical and biochemical markers was carried out, their role in the prediction of preeclampsia was shown. It has been established that in the presence of diabetes mellitus, the study of certain hormones, markers of the inflammatory response, lipid metabolism may be of potential value for predicting the development of preeclampsia. Thus, for the active introduction of biomarkers into practice, further, more detailed study of this area and optimization of research design are required.

Data from the Polish Registry of Congenital Malformations (PRCM) suggest that the prevalence of limb reduction defects (LRDs) in some Polish regions is significantly higher in comparison to that reported in the European Surveillance of Congenital Anomalies (EUROCAT) registry, but specific risk factors are still unknown. The objectives of this study were two-fold: to detect risk factors linked to isolated LRDs among Polish natives and to search for geospatial clusters of isolated LRDs to identify high-risk areas across the country. Among the 2,939,001 births accounted for in the PRCM, we determined that there were 852 children with distinct LRDs. Our data demonstrate that lower birth weight, prematurity, and maternal smoking history are strongly associated with isolated LRDs. Furthermore, our investigation pointed to various additional risk factors for isolated LRDs, including paternal education, gestational hypertension, upper respiratory tract infections, and exposure to anti-inflammatory drugs in the first trimester of pregnancy. We did not recognize statistically significant spatial or spatiotemporal clusters over the area of Poland using Kulldorff’s scan. Our study strengthens the hypothesis that maternal factors have an integral role in the etiology of isolated LRDs.
