Background/Aims: We aimed to determine whether the concentration of serum C-reactive protein (CRP) is associated with cognitive function in an adult Japanese population. Methods: Participants of this cross-sectional study were from a subgroup of the Project in Sado for Total Health (PROST; n = 454; mean age, 70.5 years). The cognitive state was evaluated using the Mini-Mental State Examination (MMSE), and those with an MMSE score <24 were considered ‘cognitively declined'. Concentrations of serum high-sensitivity CRP were measured. Multiple logistic regression analysis was used to calculate odds ratios (ORs) for cognitive decline, adjusting for the covariates of age, sex, BMI, disease history, and APOE allele. Results: Of the 454 participants, 94 (20.7%) were cognitively declined. Relative to the lowest (first) quartile of CRP concentration, adjusted ORs were 1.29 (95% CI 0.61-2.75) for the second, 1.78 (95% CI 0.82-3.86) for the third, and 3.05 (95% CI 1.45-6.42) for the highest (fourth) quartiles (p for trend = 0.018). When data were stratified by sex, the association between CRP concentration and cognitive decline was observed only in women. Conclusion: Our findings suggest an association between higher CRP concentration and lower cognitive function. Chronic inflammation may affect cognitive function in adults, in particular women.

Human Immunodeficiency Virus (HIV) infection was one of the most serious health challenges in the world. The Ministry of Health of the Republic of Indonesia reports the number of HIV cases in Indonesia as of June 2019 as many as 349,882. At present, although antiretroviral combination therapy has been found, the prevalence of neurocognitive disorders in the form of HIV-associated neurocognitive disorders (HAND) reaches 50% of HIV patients. This study aimed to determine the relationship between high sensitivity-C Reactive Protein (hs-CRP) level and cognitive impairment assessed using MoCA-INA score in HIV patients at the UPIPI Dr. Soetomo Academic Hospital Surabaya. This cross-sectional study used consecutive sampling that fulfilled inclusion and exclusion criteria. The cognitive function of the subjects was examined by MoCA-INA score and blood samples were collected for measuring hs-CRP level. Of 100 subjects, 41 had abnormal MoCA-INA score and 59 had normal score. The number of subjects with high level of hs-CRP (>= 5) who had abnormal and normal MoCA-INA score were 22 (53,7%) and 6 (10,2%) respectively. This result was significantly difference with p = 0,0001, RO = 28,072 (95% CI, 5,470-144,052). Therefore, there was a significant relationship between hs-CRP level and cognitive function, where a subject with a high hs-CRP level was more likely to have impaired cognitive function.

INTRODUCTION:Cerebrovascular accident (commonly called stroke) is dened as an abrupt onset of a neurologic decit that is attributable to a focal vascular cause. Thus, the diagnosis of stroke is clinical and laboratory studies including brain imaging are used to support the diagnosis AIMS AND OBJECTIVES: To evaluate the serum levels of high sensitivity C-reactive protein (hsCRP) in different types of cerebrovascular accidents on admission. MATERIALS AND METHODS: This study was an institution based cross-sectional study designed to investigate the association of hs-CRP levels with stroke and its types in Indian patients. The study was done in the Department of Medicine, Darbhanga Medical College and Hospital, Laheriasarai, Bihar from march 2019 to March 2020. 50 patients of either sex above 12 years of age admitted in Medicine Department of DMCH, Laheriasarai, Bihar with clinically or radiologically diagnosed stroke. RESULT: The mean level of hsCRP (mean ± s.d.) of the cases in our study was 7.65±7.01 with range 1.4 – 26 and the median was 3.7 and 58% of the cases were having level of hsCRP≥3 (p=0.023).Mean level of hsCRPof cases was signicantly higher than that of control (t98=7.25;p=0.001). CONCLUSION:We concluded that hs-CRPlevel is increased in cases of cerebrovascular accident- ischemic as well as haemorrhagic, suggesting an inammatory response in acute cerebrovascular accident. hs-CRP level is increased in patients with ischemic cerebrovascular accident dramatically but not in haemorrhagic cerebrovascular accident which might be considered as useful adjunct method for determining type of stroke in patients with cerebrovascular problems.

Background: Diabetic retinopathy (DR) is one of the major visual morbidities associated with diabetes. This study determined the levels of serum high-sensitivity C-reactive protein (hs-CRP) in patients with DR and to correlate the estimated levels of serum hs-CRP with the severity of DR and other coexisting factors.Methods: This was a cross-sectional study conducted between March 2009 and August 2010 and included patients with type 2 diabetes mellitus (T2DM) with or without DR. A detailed fundus evaluation was performed using direct and indirect ophthalmoscopy. The retinopathies were observed and documented in accordance with the Kanski's system of classification as background DR (BDR), pre-proliferative DR (PPDR), and proliferative DR (PDR). Laboratory investigations determined the levels of fasting blood glucose (FBG), postprandial blood glucose (PPBG), glycosylated hemoglobin, urine albumin excretion, and serum hs-CRP levels.Results: In total, 80 patients with T2DM were included (DR group, n=40 [BDR, n=22; PDR, n=11; PPDR, n=7]; control group, n=40). Highest serum hs-CRP levels were found in PDR group (6.68 mg/L), followed by PPDR and BDR group (3.2 mg/L and 1.56 mg/L, respectively). The PDR group showed the longest duration of diabetes (16 years), highest FBG (221.8 mg/dL) and HbA1c (6.68 mg/L). The incidence of albuminuria and maculopathy was higher in PDR group (72.7% and 54.54%, respectively). A significant association of hs-CRP levels with DR in patients with T2DM was observed. A significantly (<0.005) positive correlation of hs-CRP was also observed with age, duration of disease, FBG, PPBG, and HbA1c.Conclusions: Patients with severe grades of retinopathy had significantly higher hs-CRP levels than patients with the milder grades.

THE RELATIONSHIP BETWEEN ELEVATED C-REACTIVE PROTEIN AND COGNITIVE FUNCTION DETERIORATION OF GERIATRIC OUTPATIENTSABSTRACTIntroduction: Geriatric patients have different characteristics compared to other patients in general, in which they potentially experiencing cognitive impairment, decreasing of physiological and functional status, also the immunological system. This situation causes various inflammatory reactions that play a role in the development of neurodegenerative diseases, including the increase production of acute phase protein called C-reactive protein (CRP).Aims: To determine the association  between the CRP serum concentration  elevation and the deterioration  of geriatric outpatients’s cognitive function at Neurology Clinic Sebelas Maret University Hospital.Methods: A cross-sectional study of geriatric patients at the Neurology Clinic of Sebelas Maret University Hospital, Surakarta.   The cognitive state was evaluated using Mini-Mental State Examination (MMSE) and those with MMSE score<24 were considered cognitively declined. Concentrations of serum CRP were measured. Multiple logistic regression analysis was used to calculate Odds ratios (ORs) for cognitive decline, adjusted for the covariates of age, sex, body mass index, and disease history.Results: There were 73 participants with mean age was 65.6 years old. There were 56,2% who were experiencing a cognitive decline. Relative to the lowest (first) quartile of CRP concentration, adjusted ORs were 1,44 for the second, 1,97 for the third, and 2,33 for the highest quartiles (p=0,035). The association between CRP levels and decreased cognitive function was found to be significant after adjusting for covariates. When data were stratified by sex, the association between CRP concentration and cognitive decline was observed in women.Discussions: This research  suggested an association  between higher CRP concentration  and lower cognitive function. Keadaan ini menimbulkan berbagai reaksi inflamasi yang berperan dalam perkembangan penyakit neurodegeneratif, antara lain peningkatan produksi protein fase akut bernama C-reactive protein (CRP) oleh hepar.Tujuan: Mengetahui hubungan antara peningkatan kadar CRP dengan penurunan fungsi kognitif pasien geriatri rawat jalan di Poliklinik Saraf RS Universitas Sebelas Maret.Metode: Penelitian potong lintang terhadap pasien geriatri di Poli Saraf RS Universitas Sebelas Maret, Surakarta. Evaluasi fungsi kognitif menggunakan Mini-Mental State Examination (MMSE); dengan skor <24 dikategorikan mengalami penurunan fungsi kognitif. Kadar serum CRP masing-masing subjek juga diukur. Analisis regresi logistik berganda digunakan dalam menghitung rasio Odds (RO) untuk penurunan fungsi kognitif, dengan pengaturan kovariat usia, jenis kelamin, indeks massa tubuh, dan riwayat penyakit sebelumnya.Hasil: Didapatkan 73 subjek dengan rerata usia 65,6 tahun. Sebanyak 56,2% subjek mengalami penurunan fungsi kognitif. Dengan perbandingan relatif terhadap kuartil konsentrasi CRP terendah (pertama), RO yang telah diatur adalah 1,44 untuk kuartil kedua; 1,97 untuk kuartil ketiga; serta 2,33 untuk kuartil keempat (p=0,035). Hubungan kadar CRP dan penurunan fungsi kognitif ditemukan signifikan setelah disesuaikan dengan kovariat. Setelah dilakukan sub-analisis berdasarkan jenis kelamin, peningkatan kadar CRP terhadap penurunan kognitif secara signifikan terjadi pada subjek perempuan.Diskusi: Penelitian ini menunjukkan adanya hubungan antara peningkatan kadar CRP dengan penurunan fungsi kognitif. BACKGROUND: Many studies have shown that obesity was closely related to insulin resistance via several pathways such as inflammation, oxidative stress, lipolysis, and endothelial dysfunction. This study was carried out to observe the correlation between inflammation (IL-6 and hsCRP), lipolysis process (ET-1), and endothelial dysfunction (ADMA) and insulin resistance (HOMA-IR) in centrally obese men.METHODS: This was a cross sectional study on 62 male subjects aged 30–60 years old with waist circumference (WC) >90 cm. IL-6, ET-1 and ADMA levels were measured using ELISA method, while hsCRP and insulin were measured using chemiluminescence method. All blood testings were conducted in Prodia Clinical Laboratory.RESULTS: The results showed that WC was significantly correlated with hsCRP (r=0.294, p=0.022 ), ET-1 (r=0.257, p=0.047 ) and ADMA (r=0.338, p=0.009). We also found a significant correlation between hsCRP with HOMA-IR (r=0.324, p=0.021), ADMA with HOMA-IR (r=0.280, p=0.045), and IL-6 with hsCRP (r=0.437, p=0.003).CONCLUSIONS: hsCRP and ADMA have significant correlation with HOMA-IR in centrally obese men. HOMA-IR significantly increases in subjects with ADMA above median and either IL-6 or hsCRP above median, as compared to those in the other groups. Inflammation and endothelial dysfunction are important causal pathways of insulin resistance state in centrally obese men.KEYWORDS: obesity, IL-6, hsCRP, ET-1, ADMA, HOMA-IR
