Deployment of a New CRRT/PIRRT Device during the COVID-19 Pandemic Emergency: Organizational Challenges and Implementation Results

<b><i>Introduction:</i></b> The coronavirus disease 2019 (COVID-19) pandemic led to increased demand nationwide for dialysis equipment, including supplies and machines. To meet the demand in our institution, our surge plan included rapid mobilization of a novel continuous renal replacement treatment (CRRT) machine named SAMI. The SAMI is a push-pull filtration enhanced dialysis machine that can conjugate extremely high single-pass solute removal efficiency with very precise fluid balance control. <b><i>Material and Methods:</i></b> Machine assembly was conducted on-site by local biomedical engineers with remote assistance by the vendor. One 3-h virtual training session of 3 dialysis nurses was conducted before SAMI deployment. The SAMI was deployed in prolonged intermittent replacement therapy (PIRRT) mode to maximize patients covered per machine per day. Live on-demand vendor support was provided to troubleshoot any issues for the first few cases. After 4 weeks of the SAMI implementation, data on treatments with the SAMI were collected, and a questionnaire was provided to the nurse trainees to assess device usability. <b><i>Results:</i></b> On-site installation of the SAMI was accomplished with remote assistance. Delivery of remote training was successfully achieved. 23 PIRRT treatments were conducted in 10 patients. 7/10 of patients had CO­VID-19. The median PIRRT dose was 50 mL/kg/h (IQR [interquartile range] 44 – 62 mL/kg/h), and duration of the treatment was 8 h (IQR 6.3 – 8 h). Solute control was adequate. The user response was favorable to the set of usability questions involving user interface, on-screen instructions, machine setup, troubleshooting, and the ease of moving the machine. <b><i>Conclusion:</i></b> Assembly of the SAMI and training of nurses remotely are possible when access to vendor employees is restricted during states of emergency. The successful deployment of the SAMI in our institution during the pandemic with only 3-h virtual training supports that operating the SAMI is simple and safe.

Abstract Background Healthcare workers (HCWs) are vulnerable to developing mental ill-health. Prior research has shown those in managerial or supervisory positions have pivotal roles in creating a healthy work environment, as well as recognizing and supporting HCWs with mental health symptoms to improve occupational outcomes and reduce sickness absence. Aims To evaluate the effectiveness of active listening skills training (REACTMH) for UK healthcare managers. Methods Managers of HCWs were voluntarily enrolled on a REACTMH active listening skills training package. Attendees anonymously completed questionnaires before and immediately after attending the 1-h remote training session and again 1 month later. The questionnaire asked about confidence in recognizing, conversing with and supporting distressed colleagues as well as perceived usefulness and importance of the training and how often they had used the taught active listening skills. Results Fifty-eight healthcare managers enrolled onto the REACTMH programme in July 2020 and 32 (55%) completed the 1-month follow-up. Just less than half reported feeling confident in being able to identify, speak to and support potentially distressed staff before the training; significantly more (&gt;80%) reported feeling confident 1 month afterwards (P &lt; 0.001). Around three-quarters of attendees reported the REACTMH programme as highly useful, highly important and said they had frequently used the learned skills. Conclusions A brief active listening skills training package was associated with significant improvements healthcare manager’s confidence in recognizing, speaking with and supporting HCWs with suspected mental health conditions. Further research is needed to investigate the longer term effectiveness and acceptability of REACTMH training.

Abstract Developing livestock and equine trainings to empower county Extension agents is challenging, especially when spanning in-person and online delivery modules. Real life application of training concepts is difficult, particularly when participants have varied backgrounds and experience. Thus, we assessed if scenario-based training modules were an effective training method across in-person and virtual formats. The same scenario-based training was delivered at three, regional in-person trainings (n = 42), and one virtual training (n = 32). Training format consisted of four, species-specific lectures addressing various production topics. Small groups then developed recommendations for a specific scenario, followed by a debriefing session consisting of group reactions and specialist recommendations. Topic-area application to county programs, instructor effectiveness, and overall benefit of the training session were evaluated (Qualtrics, in-person n = 26, 62% completion; virtual n = 17, 53% completion). Data were assessed using analysis of variance and mean comparisons (α=0.05), with Tukey’s pairwise post hoc analysis where appropriate (STATA 16). Across all sessions, likert scale responses (1=poor and 5=excellent, n = 43) indicated lecture sessions were applicable to county areas of need across material content (mean±SD, cattle=4.71±0.57, equine=4.64±0.50), teaching effectiveness (cattle=4.77±0.42, equine=4.75±0.43), and overall quality (cattle=4.68±0.57, equine=4.67±0.51), respectively. Scenario-based training benefit was not influenced by the number of times an agent had attended in-service training on livestock species, agent appointment (youth vs. adult educator), or training location (p &gt;0.05). Attendance at previous in-service trainings (cattle P = 0.005; equine P = 0.013) and agent appointment (cattle P = 0.0006; equine P = 0.05) had a significant impact on the number of questions agents reported to have received on scenario topics in the last 12 months. More topic area questions were reported by agents with adult education responsibilities and previous training attendance. Based upon these results, scenario-based training is an effective in-person and virtual training tool for 4-H and adult Extension agents of varying experience.

Purpose The purpose of this pilot study was to demonstrate the effect of augmented visual feedback on acquisition and short-term retention of a relatively simple instruction to increase movement amplitude during speaking tasks in patients with dysarthria due to Parkinson's disease (PD). Method Nine patients diagnosed with PD, hypokinetic dysarthria, and impaired speech intelligibility participated in a training program aimed at increasing the size of their articulatory (tongue) movements during sentences. Two sessions were conducted: a baseline and training session, followed by a retention session 48 hr later. At baseline, sentences were produced at normal, loud, and clear speaking conditions. Game-based visual feedback regarding the size of the articulatory working space (AWS) was presented during training. Results Eight of nine participants benefited from training, increasing their sentence AWS to a greater degree following feedback as compared with the baseline loud and clear conditions. The majority of participants were able to demonstrate the learned skill at the retention session. Conclusions This study demonstrated the feasibility of augmented visual feedback via articulatory kinematics for training movement enlargement in patients with hypokinesia due to PD. Abstract INTRODUCTION Telemedicine applied to surgery is called telementorization (TM). It allows remote assistance by expert surgeons to others with less experience in certain techniques, thus guaranteeing learning that overcomes the barriers of time and location without being associated with a higher rate of complications. The objective is to publicize the use of this type of technology in our surgical department, particularly needed with the current restrictions. MATERIAL AND METHODS In our center, the Proximie platform (www.proximie.com) is used as a tool for TM. It allows to remotely broadcast live procedures performed by expert surgeons, as well as to create a virtual library of our own procedures that can be used as study and teaching tool by residents. This platform transmits not only bidirectional image and sound, but it also allows the active participation of a remote assistant with interactive features that are projected directly on the screen of the local surgeon. RESULTS The Proximie platform is being used frequently (and more intensively during this time of greatest restrictions) in advanced colorectal surgeries, with TM by remote experts and in our department for the continuing training program for residents. It also offers the possibility of conducting multidisciplinary sessions and webinars with subsequent availability control online. CONCLUSIONS TM and digital platforms such as Proximie provide important benefits in the practical learning and training of surgeons, especially with the current restrictions, being a safe technique and a teaching tool to be exploited.

An inquiry science-based education is commonly followed in a variety of educational contexts around the world and is a key parameter in various national curriculum guidelines. The impetus of this chapter is to record the initial and final reactions of science teachers participating in a series of one-year action research and training program that took place in the University of Crete (UoC) in 2013-2016, identify their perception of the first training course, and explore the impact this data had on the program's redesign for the following training session by the technical board. Teacher reactions and responses regarding what they thought had, and had not, worked well in their classes were taken into account prior to re-designing the training program that the new teachers were going to join the following year. Looking into the general benefits as well as challenges, the authors also examined the overall effect of the UoC IBSE training program to participants as reported by both students and teachers.

Purpose:To assess the weekly fluctuations in workload and differences in workload according to playing time in elite female basketball players.Methods:A total of 29 female basketball players (mean [SD] age 21 [5] y, stature 181 [7] cm, body mass 71 [7] kg, playing experience 12 [5] y) belonging to the 7 women’s basketball teams competing in the first-division Lithuanian Women’s Basketball League were recruited. Individualized training loads (TLs) and game loads (GLs) were assessed using the session rating of perceived exertion after each training session and game during the entire in-season phase (24 wk). Percentage changes in total weekly TL (weekly TL + GL), weekly TL, weekly GL, chronic workload, acute:chronic workload ratio, training monotony, and training strain were calculated. Mixed linear models were used to assess differences for each dependent variable, with playing time (low vs high) used as fixed factor and subject, week, and team as random factors.Results:The highest changes in total weekly TL, weekly TL, and acute:chronic workload ratio were evident in week 13 (47%, 120%, and 49%, respectively). Chronic workload showed weekly changes ≤10%, whereas monotony and training strain registered highest fluctuations in weeks 17 (34%) and 15 (59%), respectively. A statistically significant difference in GL was evident between players completing low and high playing times (P = .026, moderate), whereas no significant differences (P > .05) were found for all other dependent variables.Conclusions:Coaches of elite women’s basketball teams should monitor weekly changes in workload during the in-season phase to identify weeks that may predispose players to unwanted spikes and adjust player workload according to playing time.

Purpose:Investigating adolescent training loads might help us understand optimal training adaptations. GPS tracking devices and training diaries were used to quantify weekly sport and other physical activity demands placed on adolescent rugby union players and profile typical rugby training sessions.Methods:Participants were 75 males age 14 to 18 y who were recruited from rugby teams representing 3 levels of participation: schoolboy, national representative, and a selective sports school talent squad.Results:Schoolboy players covered a distance of (mean ± SD) 3511 ± 836 m, representative-squad players 3576 ± 956 m, and talent-squad players 2208 ± 637 m per rugby training session. The representative squad recorded the highest weekly duration of sport and physical activity (515 ± 222 min/wk), followed by the talent squad (421 ± 211 min/week) and schoolboy group (370 ± 135 min/wk). Profiles of individual players identified as group outliers showed participation in up to 3 games and up to 11 training sessions per week, with twice the weekly load of the team averages.Conclusion:Optimal participation and performance of adolescent rugby union players might be compromised by many high-load, high-impact training sessions and games and commitments to other sports and physical activities. An improved understanding of monitoring and quantifying load in adolescent athletes is needed to facilitate best-practice advice for player management and training prescription.

This single-subject experiment explored the effect of oral instructions and training on the expansion of written language using a reversal design. The subject, a twelve-year-old boy who attended a tutorial clinic, wrote about a stimulus picture four times a week during a twelve-week period. Productivity was based on the number of words and sentences used in the composition; the type-token ratio for vocabulary diversity was also charted. During the intervention phases of the experiment, the tutor added oral instructions to write many words and ideas. These additional instructions increased the subject's productivity. As the oral instructions were changed, the number of total words and sentences also changed. When a simple training session was provided prior to the writing of a composition, the subject wrote more adjectives.
