Non-Communicable Diseases (NCD) is one of the causes of death in the world. Indonesia is a developing country that is facing a double burden of diseases, namely infectious diseases and non-communicable diseases. NCD can appear without symptoms and show no clinical signs, so many people are not aware of the dangers of NCD disease. The purpose of this comunity service is to provide counseling about NCD and measure blood pressure in an effort to detect NCD Early.This community service uses counseling as a method to increases partner knowledge in early detection of NCD. The average of partners knowledge increase by 42.6%. This is calculated by comparing the pre test average value of 5.70 and the post test average value of 8.20. Implementation of community service can increase the knowledge of the elderly posyandu group of Sokaraja Kulon Village so that the participants have the desire to prevent NCD and find out blood pressure from blood pressure measurement results in order to reduce the causes of non-communicable diseases (NCD) and have the intention to inform knowledge about Non-Communicable Diseases (NCD) to Family, Relatives and Communities.Keywords :Early Detection,Non-Communicable Diseases

Blood pressure is the most important, modifiable risk factor for cardiovascular diseases. Lifestyle factors and also workload are the main, potential risk factors for the development of hypertension. This study focused on the early detection of unknown hypertension by screening employees in the hotel and restaurant industry (HRI). 148 HRI employees without hypertension (mean age: 34 years, men: 45%) self-measured their blood pressure during rest and for 24 hours of a normal workday. Individuals with a resting blood pressure ≥ 135/85 mmHg were classified as hypertensive. A further analysis investigated whether the currently applicable thresholds for hypertension during work, leisure, and sleep were exceeded on a working day. At rest, 36% of the study participants suffered from hypertension, which increased to 70% under workload and 46% during leisure time and dropped to 8% during sleep. Normal nocturnal dipping (10–20%) occurred only in 18% of cases; 78% were extreme dippers (>20%). Occupational hypertension screening is a suitable component of preventive healthcare. Resting blood pressure measurement alone is insufficient for the early detection of risk individuals and should be supplemented by 24-hour ambulatory blood pressure monitoring under working conditions. The impact of workload on blood pressure needs to be given more attention in the guidelines.

The review shows the significance of blood pressure regulation studies conducted during several decades at the Department of Physiology, Faculty of Medicine, Masaryk University, Brno. Continuous non-invasive blood pressure measurement was first introduced and patented here and, with the obtained data, the first spectral analysis of blood pressure was performed. This method was used in many different physiological studies on the relationship of blood pressure regulation to circulatory parameters, breathing, and baroreflex sensitivity. The article deals with studies on risk stratification of sudden cardiac death according to decreased baroreflex sensitivity, 24-hour heart rate variability, the amount of extrasystoles and late potentials. Importance of the new method of determination of one summation risk index is described here. A summary of the new conception of the relationship between low baroreflex sensitivity and hypertension is presented. Here, not only pathological changes of the vessel wall but also increased sympathetic activity and genetic predisposition play a role. Importance of studies conducted in young adults is highlighted, as inherited BRS decrease contributes to earlier blood pressure increase in the young. This research is highly topical, since prevention of hypertension in childhood is possible. Recent studies are dedicated to blood pressure regulation in young diabetics.

Aim: This study investigates the topology of convolutional neural networks and proposes an information technology for the early detection of pneumonia in X-rays. Background: For the past decade, pneumonia has been one of the most widespread respiratory diseases. Every year, a significant part of the world's population suffers from pneumonia, which leads to millions of deaths worldwide. Inflammation occurs rapidly and usually proceeds in severe forms. Thus, early detection of the disease plays a critical role in its successful treatment. Objective: The most operating means of diagnosing pneumonia is the chest X-ray, which produces radiographs. Automated diagnostics using computing devices and computer vision techniques have become beneficial in X-ray image analysis, serving as an ancillary decision-making system. Nonetheless, such systems require continuous improvement for individual patient adjustment to ensure a successful, timely diagnosis. Methods: Nowadays, artificial neural networks serve as a promising solution for identifying pneumonia in radiographs. Despite the high level of recognition accuracy, neural networks have been perceived as black boxes because of the unclear interpretation of their performance results. Altogether, an insufficient explanation for the early diagnosis can be perceived as a severe negative feature of automated decision-making systems, as the lack of interpretation results may negatively affect the final clinical decision. To address this issue, we propose an approach to the automated diagnosis of early pneumonia, based on the classification of radiographs with weakly expressed disease features. Results: An effective spatial convolution operation with several dilated rates, combining various receptive feature fields, was used in convolutional layers to detect and analyze visual deviations in the X-ray image. Due to applying the dilated convolution operation, the network avoids significant losses of objects' spatial information providing relatively low computational costs. We also used transfer training to overcome the lack of data in the early diagnosis of pneumonia. An image analysis strategy based on class activation maps was used to interpret the classification results, critical for clinical decision making. Conclusion: According to the computational results, the proposed convolutional architecture may be an excellent solution for instant diagnosis in case of the first suspicion of early pneumonia.
