Abstract P394: Impact of Caffeinated Versus Decaffeinated Energy Shots on Blood Pressure

Introduction: Energy drinks are commonly used to boost cognitive performance. However, this is paralleled with a significant increase in systolic and diastolic blood pressures. No studies have evaluated these changes with energy “shots” and whether these effects are driven solely by the caffeine component in energy drinks or a multitude of ingredients remains unknown. The purpose of this study was to determine if the effects of regular (caffeinated) 5-Hour Energy® shot differ from that of decaffeinated 5-Hour Energy® (decaf) shot as assessed by changes in systolic (SBP) and diastolic blood pressure (DBP). Methods: This was a randomized, double-blind, crossover study conducted at a university campus. Healthy subjects, between 18-40 years of age with a blood pressure less than 140/90 and not on any medications were eligible for inclusion. Participants were randomized into either the regular 5-Hour Energy® drink or the decaf 5-Hour Energy® drink study arms with blood pressure recorded at baseline, 1, 3 and 5 hours. Following a washout period of at least 6 days, the same was performed giving the alternate study drink. Additionally, heart rate, adverse events and energy score (1-5 scale) were also assessed. Results: Ten males and 10 females were enrolled in the study with 90% (18 of 20) being of Asian race. Average age was 23.3±2.7 years, weighing 149.1±31.7 pounds with SBP of 114±11.3 mmHg and DBP of 69.5±7.6 mmHg. Baseline coffee consumption was less than 2 cups per week in 11 subjects and at least 1 cup per day in the remainder. The maximum SBP increase from baseline in the regular arm was 7.8±6.5 mmHg versus 2.8±5.6 mmHg in the decaf arm (p = 0.046). Maximum DBP increase was 6.3±3.7mmHg and 1.0±5.4 mmHg (p = 0.003) in the regular and decaf arms, respectively. Significant increases in systolic and diastolic pressures were evident at 1 and 3 hours post consumption (p ≤ 0.027) but not at 5 hours. Maximum heart rate or energy levels between the two groups were not significantly different (p ≥ 0.436). In subgroup analysis, regular energy drinks invoked a significantly higher increase in systolic and diastolic blood pressures (p ≤ 0.003) 1 hour post consumption in caffeine naïve subjects (n=11). Conclusion: Regular energy drinks available in the form of a “shot” increase both systolic and diastolic blood pressures by approximately 5mmHg. This effect appears to be exaggerated in caffeine naïve subjects.

Introduction: An increasing number of healthy adults consume energy drinks to enhance their physical and mental performance. Energy drinks contain caffeine and multiple other “natural” stimulants and their combined effects on cardiac hemodynamics in healthy individuals are unclear. Hypothesis: We hypothesized that drinking a commercially available energy drink, Rockstar , compared to a placebo drink, increases resting blood pressure and heart rate in healthy adults, and these increases would be further accentuated by stress conditions. Methods: We enrolled 25 healthy, normotensive subjects (14 males), aged 29±1 years, with body mass index 24.5±1 kg/m 2 , in a randomized, double-blind, placebo-controlled, crossover study. Each subject consumed a placebo drink and a commercially available Rockstar energy drink (473 ml), in random order on two separate study days. Blood pressure and heart rate responses were recorded and compared before and 30 minutes after drink consumption, both at rest and then in response to physical, mental and cold stressors. Results: The Rockstar energy drink induced a 6±1% increase in resting systolic blood pressure, compared to 3±1% with the placebo drink (P=0.0078). Diastolic blood pressure increased by 7±1% vs 0±1% with the placebo drink (P=0.0007; Figure 1). Heart rate increased similarly in both groups, by 5±2% in the energy drink group vs. 7±2% in the placebo group. Blood pressure and heart rate increases during the stress stimuli (sustained handgrip, mental stress and cold stress) did not differ between energy drink and placebo. Conclusions: Drinking a commercially available Rockstar energy drink significantly increases resting blood pressure in young healthy adults. Blood pressure increases are not further accentuated by exercise, mental or cold pressor stress stimuli. These hemodynamic changes induced by energy drink consumption could predispose to cardiovascular events.

We assessed the effect of 2% lidocaine with 1:80,000 adrenaline (L + AD) and 3% prilocaine with 0.03 IU/mL felypressin (P + FP) on blood pressure and heart rate in older adults with systemic diseases undergoing dental extraction. This double-blind, randomized crossover study included 22 elderly participants, aged over 65 years. The participants were administered L + AD for one dental extraction and P + FP for the other. Blood pressure and heart rate were recorded immediately, 5, and 10 minutes after local anesthetic administration and the data were analyzed. The systolic and diastolic blood pressures in the P + FP group increased at all measurement points (p < .001). In the L + AD group, the diastolic blood pressure decreased at 5 and 10 minutes after local anesthetic administration (p < .05), whereas the heart rate increased at all measured time points (p < .001). There were statistically significant differences in systolic blood pressure at 5 and 10 minutes after local anesthetic administration and in diastolic blood pressure and heart rate at all time points between the 2 groups. In older adults, P + FP administration increased the systolic and diastolic blood pressures. L + AD administration increased the heart rate and decreased the diastolic blood pressure.

Abstract. Caffeine placebo expectation seems to improve vigilance and cognitive performance. This study investigated the effect of caffeine and placebo expectation on vigilance and cognitive performance during 28 h sleep deprivation. Ten healthy males volunteered to take part in the double-blind, cross-over study, which required participants to complete five treatment periods of 28 h separated by 1-week wash-out intervals. The treatments were no substance (Control); caffeine 200 mg at 00:00 (C200); placebo 200 mg at 00:00 (P200); twice caffeine 200 mg at 00:00 and 04:00 (C200-C200); caffeine 200 mg at 00:00 and placebo 200 mg at 04:00 (C200-P200). Participants were told that all capsules were caffeine and given information about the effects of caffeine to increase expectation. Vigilance was assessed by a three-letter cancellation test, cognitive functions by the continuous addition test and Stroop test, and cardiovascular regulation by heart rate and blood pressure. Tests were performed bihourly from 00:00 to 10:00 of the second day. Results indicated that C200-P200 and C200-C200 were more alert (p < .05) than Control and P200. Their cognitive functions were higher (p < .05) than Control and P200. Also, C200-P200 scored higher than C200 in the letter cancellation task (p < .05). No test showed any significant differences between C200-P200 and C200-C200. The results demonstrated that the combination of caffeine 200 mg and placebo 200 mg expectation exerted prolonged positive effects on vigilance and cognitive performance.

Recent studies have revealed evidence for increased pain sensitivity in individuals with chronically low blood pressure. The present trial explored whether pain sensitivity can be reduced by pharmacological elevation of blood pressure. Effects of the sympathomimetic midodrine on threshold and tolerance to heat pain were examined in 52 hypotensive persons (mean blood pressure 96/61 mmHg) based on a randomized, placebo-controlled, double-blind design. Heat stimuli were applied to the forearm via a contact thermode. Confounding of drug effects on pain perception with changes in skin temperature, temperature sensitivity, and mood were statistically controlled for. Compared to placebo, higher pain threshold and tolerance, increased blood pressure, as well as reduced heart rate were observed under the sympathomimetic condition. Increases in systolic blood pressure between points of measurement correlated positively with increases in pain threshold and tolerance, and decreases in heart rate were associated with increases in pain threshold. The findings underline the causal role of hypotension in the augmented pain sensitivity related to this condition. Pain reduction as a function of heart rate decrease suggests involvement of a baroreceptor-related mechanism in the pain attrition. The increased proneness of persons with chronic hypotension toward clinical pain is discussed.

We studied the effect of experimenter expectations and different instructions in a balanced placebo design. 157 subjects were randomized into a 2 × 4 factorial design. Two experimenters were led to expect placebos either to produce physiological effects or not (pro- vs. antiplacebo). All subjects except a control group received a caffeine placebo. They were either made to expect coffee, no coffee, or were in a double-blind condition. Dependent measures were blood pressure, heart rate, well-being, and a cognitive task. There was one main effect on the instruction factor (p = 0.03) with the group “told no caffeine” reporting significantly better well-being. There was one main effect on the experimenter factor with subjects instructed by experimenter “proplacebo” having higher systolic blood pressure (p = 0.008). There was one interaction with subjects instructed by experimenter “proplacebo” to receive coffee doing worse in the cognitive task than the rest. Subjects instructed by experimenter “antiplacebo” were significantly less likely to believe the experimental instruction, and that mostly if they had been instructed to receive coffee. Contrary to the literature we could not show an effect of instruction, but there was an effect of experimenters. It is likely, however, that these experimenter effects were not due to experimental manipulations, but to the difference in personalities.

1. Changes in blood pressure in the dorsal aorta, ventral aorta and subintestinal vein, as well as changes in heart rate and breathing rate during moderate swimming activity in the rainbow trout are reported. 2. Blood pressures both afferent and efferent to the gills increased during swimming and then returned to normal levels within 30 min. after exercise. 3. Venous blood pressure was characterized by periodic increases during swimming. The pressure changes were not in phase with the body movements. 4. Although total venous return to the heart increased during swimming, a decreased blood flow was recorded in the subintestinal vein. 5. Heart rate and breathing rate increased during swimming and then decreased when swimming ceased. 6. Some possible mechanisms regulating heart and breathing rates are discussed.

The laboratory mouse is a facultative daily heterotherm in that it experiences bouts of torpor under caloric restriction. Mice are the most frequently studied laboratory mammal, and often, genetically modified mice are used to investigate many physiological functions related to weight loss and caloric intake. As such, research documenting the cardiovascular changes during fasting-induced torpor in mice is warranted. In the current study, C57BL/6 mice were implanted either with EKG/temperature telemeters or blood pressure telemeters. Upon fasting and exposure to an ambient temperature (Ta) of 19°C, mice entered torpor bouts as assessed by core body temperature (Tb). Core Tb fell from 36.6 ± 0.2°C to a minimum of 25.9 ± 0.9°C during the fast, with a concomitant fall in heart rate from 607 ± 12 beats per minute (bpm) to a minimum of 158 ± 20 bpm. Below a core Tb of 31°C, heart rate fell exponentially with Tb, and the Q10 was 2.61 ± 0.18. Further, mice implanted with blood pressure telemeters exhibited similar heart rate and activity profiles as those implanted with EKG/temperature telemeters, and the fall in heart rate and core Tb during entrance into torpor was paralleled by a fall in blood pressure. The minimum systolic, mean, and diastolic blood pressures of torpid mice were 62.3 ± 10.2, 51.9 ± 9.2, 41.0 ± 7.5 mmHg, respectively. Torpid mice had a significantly lower heart rate (25–35%) than when euthermic at mean arterial pressures from 75 to 100 mmHg, suggesting that total peripheral resistance is elevated during torpor. These data provide new and significant insight into the cardiovascular adjustments that occur in torpid mice.

Objective To identify the cardiovascular effects of long-acting methylphenidate administered for twelve weeks in Indonesian children with ADHD.Methods This was an 18-week, time series study on children with ADHD who were given 20 mg of long-acting methylphenidate for twelve weeks. During the study period we made ten serial observations of the subjects, including before, during and 6 weeks following drug administration. We included drug naive children with ADHD between the ages of 7 – 10 years. Children with mental retardation and chronic physical or mental disorders were excluded. Blood pressure was measured by sphygmomanometer with a child’s cuff at the brachial artery. We also collected data on heart rate, side effects, complaints and other medications used during the study. Repeated analysis was performed on the data with a P level of 0.05.Results Twenty-one subjects were recruited for this study. Mean blood pressure fluctuated insignificantly during the research period, for both mean systolic and mean diastolic blood pressures (P=0.115 and P=0.059). Mean heart rate also fluctuated insignificantly (P=0.091). All fluctuations were within the normal ranges. During the study, there were complaints of dizziness, nausea, and gastrointestinal upset, but they were reportedly mild and disappeared before the second week of observation.Conclusion Administration of 20 mg long-acting methylphenidate for twelve weeks in children with ADHD altered mean blood pressures and heart rates, but within the normal range for children of their age. However, cardiovascular risk observation is still needed when administering methylphenidate to children with ADHD, especially for those using the medication long-term.[Paediatr Indones. 2011;51:282-7].

Background: Dynamic exercises are known to elicit hemodynamic changes such as an increase in arterial blood pressure and heart rate. Zumba and ZOCA are part of a fast growing group of dance fitness programmes designed to provide a cardiovascular dynamic workout. Despite their growing popularity, very few studies have been done to provide knowledge regarding the hemodynamic changes associated with these exercises. Methods: Case study in which 27 females took part in either a Zumba or ZOCA class. Using digital blood pressure monitors, recordings of blood pressure and heart rate were taken, firstly, before commencement of the exercise, secondly, after 30 minutes after exercise and thirdly, at the end of the class.Results: Mean baseline blood pressures were 118 (SD = 14) mmHg and 77 (SD = 7) mmHg, systolic and diastolic blood pressures, respectively. After 30 minutes of dancing, mean systolic blood pressure increased to 130 (SD = 19) mmHg (p˂ 0.05) while diastolic blood pressureonly rose to an average of 80 (SD = 8) mmHg (p˃ 0.05). At the end of the class (after the cool down phase) mean systolic blood pressure reduced to 109 (SD = 13) mmHg (p˂0.05) while diastolic blood pressure reduced to 74(SD = 12) mmHg (p˂ 0.05). Conclusions: Zumba and ZOCA elicited significant hemodynamic changes that can be attributed to these exercises stimulating the cardiovascular regulatory mechanisms (e.g central command and exercise-pressor) sufficiently and hence resulting in autonomic adjustmentsthat were concurrent with effective dynamic exercise. Objective To investigate the effects of Shaolin Ba Duan Jin on patients with hypertension. Methods In the hypertensive patients detected in the national physical fitness test of public officials in Guiyang City, 40 patients with hypertension were selected. The experiment was started after Shaolin Ba Duan Jin has been practiced for seven days and the subjects had learned it. Experimental method: Frequency of subjects practicing Ba Duan Jin is 7 days a week, subjects practice once a day, practicing time is 6:00-7:30 am or 18:00 -19:30 pm, practicing lasts 1.5 hours each time (practicing has 3 groups, 14 minutes in each group;subjects rest 2 minutes between groups,;preparing part is 8 minutes;ending part is 5 minutes), record the blood pressure of the subjects before and after each experiment, and fill in the quality of life scale (WHOQOL-BREF scale) regularly for a period of 12 weeks. Questionnaire method: Fill in the quality of life scale every three weeks and measure heart rate and blood pressure before and after each exercise. Mathematical statistics: The paired sample t test was used to analyze the changes in blood pressure before and after the 12-week experiment. Results 1) The systolic and diastolic blood pressures of the subjects were significantly lower than before the experiment (P <0.01). 2) The WHOQOL-BREF scale after 12 weeks has improved significantly in all areas than before the experiment. Conclusions 1) Martial arts Ba Duan Jin has a good influence on the circulatory system of hypertensive patients. The performance is that the heart rate is slow and the systolic and diastolic blood pressures have a downward trend. 2) Martial arts Ba Duan Jin has significant improvement in the physiology, psychology, social relations, environment and other fields .
