Abnormal Functional Activation During a Simple Word Repetition Task: A PET Study of Adult Dyslexics

Eight dyslexic subjects, impaired on a range of tasks requiring phonological processing, were matched for age and general ability with six control subjects. Participants were scanned using positron emission tomography (PET) during three conditions: repeating real words, repeating pseudowords, and rest. In both groups, speech repetition relative to rest elicited widespread bilateral activation in areas associated with auditory processing of speech; there were no significant differences between words and pseudowords. However, irrespective of word type, the dyslexic group showed less activation than the control group in the right superior temporal and right post-central gyri and also in the left cerebellum. Notably, the right anterior superior temporal cortex (Brodmann's area 22 [BA 22]) was less activated in each of the eight dyslexic subjects, compared to each of the six control subjects. This deficit appears to be specific to auditory repetition as it was not detected in a previous study of reading which used the same sets of stimuli (Brunswick, N., McCrory, E., Price, C., Frith, C.D., & Frith, U. [1999]. Explicit and implicit processing of words and pseudowords by adult developmental dyslexics: A search for Wernicke's Wortschatz? Brain, 122, 1901-1917). This implies that the observed neural manifestation of developmental dyslexia is task-specific (i.e., functional rather than structural). Other studies of normal subjects indicate that attending to the phonetic structure of speech leads to a decrease in right-hemisphere processing. Lower right hemisphere activation in the dyslexic group may therefore indicate less processing of non-phonetic aspects of speech, allowing greater salience to be accorded to phonological aspects of attended speech.

ABSTRACTWe have compared the verbal comprehension abilities of 34 right hemisphere damaged patients to 16 hospitalized control subjects of comparable age and educational attainment. The right hemisphere damaged patients performed as well as the control subjects on a vocabulary test, but were impaired in their ability to interpret proverbs and comprehend logico-grammatical sentences. Impairment on the proverbs test was the result of a decrease in the number of abstract interpretations, whereas impairment on the logico-grammatical sentence comprehension test was related to difficultes in grasping spatial and passive relationships. These comprehension impairments tended to correlate with visuospatial deficits and hemianopia, but not with the degree of hemiparesis or the presence of sensory extinction. Patients with anterior right hemisphere damage performed better on the logico-grammatical sentence conprehension test than patients with posterior damage. A variety of factors probably contribute to these verbal deficits including impaired intellect, inattention, an inability to grasp spatial relationships, and difficulties in manipulating the inner schemata of language.

Fluency tests are frequently used in clinical practice to asses executive functions. The literature data are not unequivocal although in a great number of papers is pointed out the importance of the left hemisphere, specially of the left frontal lobes in the mediation of phonological fluency and the right hemisphere in the mediation of nonverbal fluency. This paper considers the suitability of fluency tests for the detection of left versus right seizure laterality. The sample consisted of thirty-two epilepsy patients divided into two groups: LHF-participants with the seizure focus in the left hemisphere (n=16), and DHF-participants with the seizure focus in the right hemisphere (n=16), and K-the control group of t age-matched healthy children (n=50) aged 7-11 years. The qualitative and quantitative comparison of the phonological and nonverbal fluency performance was carried out in consideration of the seizure laterality as well as compared to the healthy controls. The results of phonological fluency performance revealed that the performance of the LHF group was significantly reduced as compared to both DHF and K group. The analysis of nonverbal fluency performance revealed that the performance of the DHF group was significantly reduced as compared to both LHF and K group The qualitative analysis obtained valuable data, which could additionally contribute to the neuropsychological evaluation of the left versus right seizure laterality.

AbstractRhythmic auditory stimuli are known to elicit matching activity patterns in neural populations. Furthermore, recent research has established the particular importance of high-gamma brain activity in auditory processing by showing its involvement in auditory phrase segmentation and envelope-tracking. Here, we use electrocorticographic (ECoG) recordings from eight human listeners, to see whether periodicities in high-gamma activity track the periodicities in the envelope of musical rhythms during rhythm perception and imagination. Rhythm imagination was elicited by instructing participants to imagine the rhythm to continue during pauses of several repetitions. To identify electrodes whose periodicities in high-gamma activity track the periodicities in the musical rhythms, we compute the correlation between the autocorrelations (ACC) of both the musical rhythms and the neural signals. A condition in which participants listened to white noise was used to establish a baseline. High-gamma autocorrelations in auditory areas in the superior temporal gyrus and in frontal areas on both hemispheres significantly matched the autocorrelation of the musical rhythms. Overall, numerous significant electrodes are observed on the right hemisphere. Of particular interest is a large cluster of electrodes in the right prefrontal cortex that is active during both rhythm perception and imagination. This indicates conscious processing of the rhythms’ structure as opposed to mere auditory phenomena. The ACC approach clearly highlights that high-gamma activity measured from cortical electrodes tracks both attended and imagined rhythms.

Electrophysiological methods have been used to study the temporal sequence of syntactic and semantic processing during sentence comprehension. Two responses associated with syntactic violations are the left anterior negativity (LAN) and the P600. A response to semantic violation is the N400. Although the sources of the N400 response have been identified in the left (and right) temporal lobe, the neural signatures of the LAN and P600 have not been revealed. The present study used magnetoencephalography to localize sources of syntactic and semantic activation in Finnish sentence reading. Participants were presented with sentences that ended in normally inf lected nouns, nouns in an unacceptable case, verbs instead of nouns, or nouns that were correctly inflected but made no sense in the context. Around 400 msec, semantically anomalous last words evoked strong activation in the left superior temporal lobe with significant activation also for word class errors (N400). Weaker activation was seen for the semantic errors in the right hemisphere. Later, 600-800 msec after word onset, the strongest activation was seen to word class and morphosyntactic errors (P600). Activation was significantly weaker to semantically anomalous and correct words. The P600 syntactic activation was localized to bilateral sources in the temporal lobe, posterior to the N400 sources. The results suggest that the same general region of the superior temporal cortex gives rise to both LAN and N400 with bilateral reactivity to semantic manipulation and a left hemisphere effect to syntactic manipulation. The bilateral P600 response was sensitive to syntactic but not semantic factors.

The visual span of apprehension for random letter and digit sequences, approximations to English sequences and non-symbolic line stimuli was measured in patients with unilateral cerebral lesions. The left hemisphere group was significantly impaired relative to the right hemisphere group and a control group on all three types of visual span task. The deficit was most marked in patients with left posterior lesions. The visual span deficits were not related to other language deficits. The findings are discussed in terms of a modality-specific defect of visual short-term memory.

The current study investigated the capacity of the right hemisphere to process verbs using a paradigm proven reliable for predicting differential, minor hemisphere lexical analysis in the normal, intact brain. Vocal reaction times of normal subjects were measured to unilaterally presented verbs of high and of low frequency. A significant interaction was noted between the stimulus items and visual fields. Post hoc tests showed that vocal reaction times to verbs of high frequency were significantly faster following right visual-field presentations (right hemisphere). No significant differences in vocal reaction time occurred between the two visual fields for the verbs of low frequency. Also, significant differences were observed between the two types of verbs following left visual-field presentation but not the right. These results were interpreted to suggest that right-hemispheric analysis was restricted to the verbs of high frequency in the presence of a dominant left hemisphere.

Objective: To study the effect of check width size of the stimuli on the amplitude and latency of the P100 component of visual evoked potentials recorded in patients with retinitis pigmentosa (RP). Methods: Pattern reversal visual evoked potentials (PVEPs) were recorded in 16 RP patients and 20 visually normal subjects. Pattern reversal stimuli with five different check widths and 100% of contrast were projected in the right eye of both patients and control subjects. PVEPs induced by stimuli with 78%, 16%, and 6% of contrast were also recorded in 10 of the control subjects. Results: In RP patients, the amplitude of P100 was smaller than controls in all check sized used and the peak P100 amplitude was obtained with a larger check width than in controls. P100 was also delayed in RP patients in all check sizes studied. The P100 amplitude- and latency-check size functions of RP patients were like those found in control subjects with low contrast stimuli of 16% and 6%. Conclusion: The PVEPs spatial functions of RP patients show quantitative and qualitative changes, suggesting disease induced alteration in the neural processing of stimulus contrast.

Deep dyslexia is a striking reading disorder that results from left-hemisphere brain damage and is characterized by semantic errors in reading single words aloud (e.g., reading spirit as whisky). Two types of explanation for this syndrome have been advanced. One is that deep dyslexia results from a residual left-hemisphere reading system that has lost the ability to pronounce a printed word without reference to meaning. The second is that deep dyslexia reflects right-hemisphere word processing. Although previous attempts to adjudicate between these hypotheses have been inconclusive, the controversy can now be addressed by mapping functional anatomy. In this study, we demonstrate that reading by two deep dyslexic patients (CJ and JG) involves normal or enhanced activity in spared left-hemisphere regions associated with naming (Broca's area and the left posterior inferior temporal cortex) and with the meanings of words (the left posterior temporo-parietal cortex and the left anterior temporal cortex). In the right-hemisphere homologues of these regions, there was inconsistent activation within the normal group and between the deep dyslexic patients. One (CJ) showed enhanced activity (relative to the normals) in the right anterior inferior temporal cortex, the other (JG) in the right Broca's area, and both in the right frontal operculum. Although these differential right-hemisphere activations may have influenced the reading behavior of the patients, their activation patterns primarily reflect semantic and phonological systems in spared regions of the left hemisphere. These results preclude an explanation of deep dyslexia in terms of purely right-hemisphere word processing.

ABSTRACT:Background:Constructional impairment following left vs. right hemisphere damage has been extensively studied using drawing tasks. A confounding factor in these studies is that right-handed patients with left hemisphere damage (LHD) are often forced by weakness to use their non-dominant (left) hand or hemiparetic dominant hand. Qualitative differences in the drawing characteristics of left and right hand drawings by normal subjects have not previously been characterized. The present study was undertaken to determine the qualitative differences between left and right hand drawings of normal subjects.Methods:Thirty right-handed, elderly subjects without a history of neurological disease were asked to draw, from memory, seven objects using the right and left hand. Half of the subjects were randomly assigned to draw with the left hand first, and half the right hand first. Right and left hand drawings were compared using a standardized scoring system utilized in several previous studies of drawing in focal and diffuse neurological disease. Each drawing was scored on eighteen criteria. Right and left hand drawing scores were then compared using the t-test for paired samples or the Wilcoxon matched-pairs testResults:Drawings made using the left hand were found to be significantly simpler, more tremulous and of poorer overall quality than drawings made by the same subjects using the right hand.Conclusions:The deficits found in left versus right hand drawings of normals are similar to those found in patients with LHD, suggesting that much of the drawing impairment seen following LHD is due to an elementary motor disturbance related to use of the non-dominant hand.
