Gastrointestinal Decompression on Computed Tomography Examination for Patients with Craniocerebral Injury

Objective: Patients with craniocerebral injury usually accompanied by intracranial hypertension and vomiting, which affect the effect of CT scanning. The aim of this study was to discuss the application effects of gastrointestinal decompression on computed tomography (CT) examination for patients with craniocerebral injury. Methods: A total of 80 patients with craniocerebral injury in our hospital were randomly selected and divided into control and observation groups, 40 cases in each group. The observation group was given gastrointestinal decompression before the CT examination, while the control group received routine examination. The success rate of CT examination, CT scanning effect, examination, and complication during scanning were recorded and compared in two groups. Results: The one-time success rate (100.0%) of CT examination was 100.0% (40/40) in the observation group was significantly higher than the rate in the control group (82.5%, P = 0.034). The CT examination clearly showed in the types of craniocerebral injury, the brain tissue injury, the ventricular compression and the midline displacement. The CT examination time of observation group was shorter than that of control group (P < 0.001), while there was no significant difference in the whole examination time spent on both gastrointestinal decompression operation and CT examination between the two groups (P = 0.301). In the observation group, there was only 1 case of retching, and no cases of vomiting or aspiration during scanning. The incidence of various complications in observation group was significantly lesser than the incidence in control group (P = 0.034). Conclusions: Continue gastrointestinal decompression can significantly reduce the incidence of vomiting and other complication in patients with craniocerebral injury during CT examination, reduce the CT examination time, and improve the success rate of one-time scanning.

Abstract Objective To report a method of PICC catheterization with the aid of a new medical guide wire, and to discuss the curative effect. Methods Five hundred and thirteen patients who visited our hospital from January 2013 to December 2019 for PICC catheterization were selected as study subjects. Random number method was used to group patients. General information was recorded for both groups. Patients in the observation group received catheterization with the aid of a new medical guide wire. The control group received catheterization via conventional guide wire. The success rate of the first catheterization, the success rate of the catheterization, the timing of the catheterization, the complications and the position of the catheter end were compared between the two groups. Results There was no significant difference in general information between the two groups. After analyzing the puncture situation of the two groups, it was found that the average catheterization time of the observation group was shorter than that of the control group, and the difference was statistically significant. Patients in the observation group had higher success rate of one-time catheterization and catheterization success rate, and the difference was statistically significant. The incidences of occult thrombosis, phlebitis and catheter blockage in the observation group were lower than those in the control group, and the difference was statistically significant. The incidence of dominant thrombosis and bleeding at puncture point in the observation group was also lower than that in the control group, but the difference was not statistically significant. Conclusion The new type of medical guide wire component is effective for PICC catheterization and worthy of further promotion.

Objective: To explore the application effect and nursing strategy of comprehensive safety management process in CT examination of ICU patients with tracheal incubation and general tuberculosis. Methods: A total of 52 patients with tracheal incubation and general pulmonary tuberculosis who underwent CT examination in our hospital were selected as the research objects, and they were divided into control group and observation group according to the random number table method, with 26 cases in each group. The control group adopted the traditional nursing mode, and the observation group applied the integrated safety management process based on the control group. The general conditions, nursing satisfaction, comfort, compliance behavior, and anxiety scores of the two groups during the CT examination were compared. Results: The change rate of CT vital signs in the observation group (19.23%) was significantly lower than that in the control group (46.15%), and the scan completion rate (100.00%) was significantly higher than that in the control group (84.62%). The satisfaction score and comfort score of the observation group after inspection were significantly higher than those of the control group, and the differences were statistically significant (P < 0.05). There was no significant difference in the anxiety scores between the two groups of patients before the examination (P > 0.05); the anxiety scores of the two groups after the examination were significantly lower than those before the examination; the anxiety score of the observation group after the examination was significantly lower than that of the control group. There was statistical significance (P < 0.05). The Grade 1 coordination rate in the observation group (69.23%) was significantly higher than that in the control group (57.65%), and the difference was statistically significant (P < 0.05). Conclusions: The combined application of comprehensive safety management procedures in CT examination of ICU tracheal intubation patients can effectively improve the patient's nursing satisfaction, comfort, safety and compliance behavior, and improve patient anxiety, which is worthy of clinical promotion and application.

Objective: To compare the effects of blunt separation combined with early sheath removal and conventional catheterization on the incidence of bleeding, exudation and subcutaneous congestion after peripherally inserted central catheter (PICC). Methods: 250 patients with PICC catheterization in our hospital were selected, including 125 in the control group and 125 in the observation group. The conventional catheterization method was used in the control group: The sheath was removed by scalpel skin expansion and complete tube delivery in place. The observation group used blunt separation combined with early sheath removal. The success rate of one-time sheath delivery, immediate bleeding, 24-hour bleeding and exudation were compared. Results: The success rate of sheath delivery in the observation group was 100%; The amount of immediate blood loss and blood loss 24h after catheterization in observation group was obviously lower than that in the control group, with statistical significance (P < 0.05). There were 6 cases of exudation in the control group and no exudation in the observation group. There was significant difference in the incidence between the two groups (P < 0.05); There were 10 cases of subcutaneous congestion in the control group and 2 cases of subcutaneous congestion in the observation group. There was significant difference in the incidence between the two groups (P < 0.05); Conclusion: Blunt separation combined with early sheath removal can reduce the occurrence of local blood and fluid leakage after PICC catheterization.

Objective: To compare the success rate of different obturation procedures in primary mandibular second molars clinically and also by digital radiovisiography. Methods: A total of 40 children aged between 4-8 years with deeply carious mandibular second primary molars indicated for single session pulpectomy were selected. Canals were obturated with Metapex. The 3 study groups (Endodontic plugger, Handheld lentulospiral, Navi Tip syringe) were compared with the control group (reamer) both clinically and radiovisiographically. The data collected were statistically analyzed using Pearson’s Chi-square and Fisher’s exact test. Results: The use of Navi tip syringe led to the least number of voids followed by Endodontic plugger and Reamer and the highest number of voids was reported with Lentulospiral. Navitip presented maximum number of optimally filled cases followed by Endodontic plugger and Lentulospiral and least number of optimally filled cases with reamer. However, there was no statistically significant difference (p>0.05) in any of the groups with clinical (pain and tenderness to percussion) and radiographic parameters (presence or absence of voids and length of obturation). Conclusion: Within the limitations of the present study, though the clinical outcome was statistically insignificant, Navitip syringe exhibited encouraging results and is a promising option for obturation in primary teeth.

PURPOSE: The pancreatic capillary blood flow (PCBF) was studied to determine its alterations during caerulein-induced pancreatitis in rats. METHODS: Twenty rats were divided in groups: control and caerulein. A laser-Doppler flowmeter to measure PCBF continuously was used. Blood pressure (BP) and heart rate (HR) were monitored. Serum biochemistry analyses were determined. Histopathological study was performed. RESULTS: The PCBF measured a mean of 109.08 ± 14.54% and 68.24 ± 10.47% in control group and caerulein group, respectively. Caerulein group had a mean decrease of 31.75 ± 16.79%. The serum amylase was 1323.70 ± 239.10U.I-1 and 2184.60 ± 700.46U.I-1 in control and caerulein groups, respectively. There was a significant difference in the PCBF (p<0.05) and serum amylase (p<0.05) when compared to control and caerulein groups. Although micro and microvacuolization were seen in 30% in caerulein group, no significant difference was seen between the groups. CONCLUSION: A decrease in the PCBF may be one of the leading events and it is present before histopathological tissue injury had been established in this model of acute pancreatitis.

Objective: To observe the clinical efficacy of sequential butylphthalide therapy combined with dual antiplatelet therapy in the treatment of elderly patients with acute cerebral infarction (ACI). Methods: One hundred and twenty-two elderly patients with ACI who were admitted to the department of neurology of our hospital at May 2016-August 2018 were selected grouped into a control group and an observation group by random number table method, 61 in each group. On the basis of conventional treatment, the patients in the control group were given dual antiplatelet therapy (aspirin enteric-coated tablets + clopidogrel bisulfate tablets), while the patients in the observation group were given sequential butylphthalide therapy on the basis of the control group. The clinical effects of the two groups were compared after four weeks of treatment, and the changes of National Institutes of Health Stroke Scale (NIHSS), ADL score, plasma 3-mercaptopyruvate sulphurtransferase (3-MST) and Amyloid β42 (Aβ42) levels and the occurrence of adverse reactions during treatment were recorded. Results: The clinical efficacy of the observation group was better than that of the control group (P<0.05). There was no significant difference in NIHSS and ADL scores between the two groups before treatment (P>0.05). After treatment, the NIHSS and ADL scores of the observation group were better than those of the control group (P<0.05). There was no significant difference in plasma levels of 3-MST and AB42 between the two groups before treatment (P>0.05). The level of plasma 3-MST in the observation group was higher than that in the control group, and the level of plasma Aβ42 was lower than that in the control group (P<0.05). No serious adverse reactions occurred during the treatment period in both groups. Conclusion: Butylphthalide sequential therapy combined with dual antiplatelet therapy is effective in the treatment of elderly ACI.  Objective: Investigate the effectiveness of nursing risk management in the care of critically ill patients in the respiratory unit. Methods: Among the critically ill respiratory patients admitted to our hospital between May 2019 and April 2020, 78 patients were randomly selected and divided into an observation group and a control group, each consisting of 39 patients. In the observation group, a nursing risk management model was implemented, i.e., patients' clinical symptoms were observed at any time to monitor their treatment satisfaction and the effectiveness of their care and routine care was implemented for the control group. Results: The heart rate, respiratory rate, and pH of patients in the observation group were more stable than those in the control group, and their respiratory status was better, with differences in data. There was also significant statistical significance (P<0.05). The incidence of patient-provider disputes, unplanned extubation, and unplanned events were lower in the observation group compared to the control group, and their data difference was statistically significant (P<0.05). The treatment satisfaction as well as the total effective rate of patients in the observation group was also much higher than that of the control group, and there was also a statistically significant difference in the data (P<0.05). Conclusion: The nursing risk management model has a significant therapeutic effect in the care of critically ill respiratory patients. Therefore, it is worth popularizing to use in the clinical nursing of respiratory critical patients.

【Abstract】Objective: To investigate the clinical efficacy of carboprost tromethamine combined with mifepristone in the treatment of uterine fibroids. METHODS: A total of 66 patients with uterine fibroids admitted to our hospital between April 2018 and January 2019 were selected as subjects. According to the two different treatment methods, patients were divided equally. The observation group and the control group, each group of 33 people. The oxytocin drug treatment was medicated to the control group, and the prostaglandin tromethamine combined with mifepristone was medicated to the observation group. The treatment effect, adverse reaction, operation, and uterine muscle before and after surgery were observed in these two groups. Tumor tissue progesterone receptor and estrogen receptor levels. Results: The clinical treatment effect of the observation group was 93.94%, and the clinical treatment effect of the control group was 60.61%. The clinical treatment effect of the observation group was significantly higher than that of the control group; also the incidence of adverse reactions in the observation group in terms of rash, fatigue, nausea and vomiting was much lower than the control group, and could observe a significant difference. Finally in observation group, the amount of intraoperative blood loss, operation time and hospitalization time were better than those of the control group. The progesterone receptor and estrogen receptor levels in the uterine fibroid tissue after surgery should also be better than the control group, it is worth to make further comparison. Conclusion: Carprostol tromethamine combined with mifepristone is effective in the treatment of uterine fibroids and can be further developed.

Objective: To explore the clinical effect of iRoot BP Plus pulpotomy for immediate repair in children with young permanent teeth crown fracture. Methods: From September 2017 to October 2018, 80 children (80 affected teeth) with young permanent teeth crown fracture who treated in the hospital were selected as the research objects. Random number table method was used to divide them into observation and control groups with each group of forty patients (40 affected teeth). The children in the control group were treated with calcium hydroxide resin, while the observation group were treated with iRoot BP Plus for immediate repair. The surgical success rate was compared between the two groups, and the pulp vitality before and after treatment were compared. Results: At 3 months and 6 months after surgery, the success rate of children in the observation group was higher than that in the control group, and the difference was statistically significant (P<0.05). The percentage of children with negative pulp activity in the observation group was higher than that in the control group, and the difference was statistically significant (P<0.05). Conclusion: Immediate repair with iRoot BP Plus pulpotomy in children with young permanent teeth crown fracture has significant clinical effect.

Objective: The mortality of rebleeding after ruptured intracranial aneurysms was more than 40%. It is essential to identify the intracranial artery in sectional anatomy. However, it is difficult for students to understand the intracranial artery in the section. Therefore, the purpose of this study is to explore the application effect of three-dimensional (3D) reconstruction and numerical simulation of intracranial aneurysm for sectional anatomy based on computed tomography angiography (CTA). Method: Sixty students in medical imaging specialty of our university were divided into two groups. The control group was taught with conventional sectional anatomy and CT images, while the observation group was taught with 3D reconstruction and numerical simulation of intracranial aneurysm. The teaching characteristics and teaching effects were analyzed and compared between the two groups. Result: The 3D reconstruction can accurately express the size, direction, and adjacent relationship of aneurysms. Through rotation of the 3D image, students can easily understand the name and location of cerebral arteries. Combined with the function of 3D positioning, each blood vessel can automatically and accurately locate in the transverse, coronal, and sagittal plane. Abnormal wall shear stress was easily found in the intersection of cerebral artery circle, which was the physiological basis for the occurrence of aneurysms. There was a high shear zone in the root of an aneurysm, which was the physiological factor of rupture. The scores of sectional specimen identification, drawing examination, and theoretical assessment in the observation group were significantly higher than those in the control group (P < 0.05). Conclusion: The 3D reconstruction and numerical simulation can directly display the 3D morphological and physiological characteristics of intracranial aneurysms, which is convenient for students to understand and memorize. It can reach a good teaching effect in sectional anatomy.
